Linking leptin in GCF with tooth movement among Indians.

It is of interest to evaluate the association of orthodontic tooth movement with concentration of leptin in Gingival Crevicular Fluid (GCF).Thirty orthodontic patients of both genders with equal sample size were included for the present study. Concentration of leptin was assessed at baseline (T0), 1 hr after application of force (T1), 24 hours later (T2), 7 days after application of force (T3), and 1 month after application of orthodontic force (T4). Using strips of filter paper, GCF was collected from the gingival sulcus on distal aspect of the right maxillary canine. Distalized tooth movement was evaluated by measuring the difference on dental casts, at baseline and one month after force application. One-way ANOVA with Bonferroni correction and Pearson's correlation test were used to analyze the data. Average concentration of leptin in GCF raises from baseline (T0) to 1 hours after application of force (T1), then increased to peak after 24 hours (T2), and declined to a minimum value after 7 days (T3) and again raises after 1 month (T4), closer to the base line value (T0), and this was statistically significant (P < 0.05). There was significant association of the overall average concentration of leptin to degree of tooth movement (correlation coefficient = 0.625). There was a biphasic change in GCF leptin concentration during one cycle of orthodontic force application, thus, a significant association between rates of tooth movement with GCF leptin concentration is noted.

Leptin Levels in Gingival Crevicular Fluid during Orthodontic Tooth Movement.

Aim:To evaluate the levels of leptin in gingival crevicular fluid (GCF) during orthodontic tooth movement and to find the role of leptin as a mediator for tooth movement.Materials and Methods:GCF was collected from 25 subjects and the levels of leptin in it were measured. Sample (A) pretreatment, (B) 6 h after applying retractive orthodontic force from right side canine, (C) without retractive force from left side canine, and after 21 days from right canine (D), and left canine (E).Results:GCF Leptin concentration in pretreatment, 6 h control and 21st day control were all similar. GCF leptin concentration at 6 h test tooth site was increased which was statistically significant. GCF leptin concentration in 21st day test tooth was decreased only a little than the control tooth which shows statistically no difference.Conclusion:Leptin is one of the mediators of orthodontic tooth movement.

Effects of exercise on periodontal parameters in obese women.

Several studies have demonstrated an association between obesity, periodontitis, and exercise. This study aimed to investigate the effects of regular exercise on obese women with periodontal disease, using serum, saliva, and gingival crevicular fluid (GCF) samples. A before-after study design was adopted to evaluate the effects of 12 weeks of regular exercise on obese women grouped according to periodontal status, without a control group (no exercise). The study sample comprised of 15 patients without periodontitis (NP group) and 10 patients with chronic periodontitis (CP group), from whom periodontal parameters were measured and serum, saliva, and GCF samples were collected. Body mass index (BMI), anthropometric measurements, somatotype-motoric tests, and maximal oxygen consumption (VO2max) were recorded at baseline and after exercise. Med Calc was used for statistical analysis. After exercise, a significant decrease in BMI and a significant increase in VO2max were observed in both groups. A significant decrease in probing depth and clinical attachment loss, serum leptin, GCF tumor necrosis factor-α(TNF-α) and leptin, and a significant increase in GCF resistin were observed in the CP group. A significant decrease in serum TNF-α and leptin levels and a significant increase in serum resistin and GCF TNF-α, leptin, resistin, and adiponectin levels were observed in the NP group. Significant correlations between bleeding on probing and levels of interleukin-1β and leptin in GCF were observed in the CP group. This study showed that regular exercise exerts different impacts with respect to clinical and biochemical aspects of periodontal and systemic conditions in obese women.

Evaluation of leptin concentration in Gingival Crevicular Fluid (GCF) during orthodontic tooth movement and its correlation to the rate of tooth movement.

OBJECTIVES:Leptin, a polypeptide which is related to body fat regulation, is also found to have a role in the inflammatory reaction. The aim of this study is to assess the concentration of leptin in Gingival Crevicular Fluid (GCF) during orthodontic force application and to correlate its concentration to rate of tooth movement.METHODS:Twenty orthodontic patients (10 males and 10 females) were selected for the study. Leptin concentration was measured at T0, before force application; T1, one hour after force application; T2, one day after force application; T3, one week after force application; T4, one month after force application. GCF was collected using filter paper strips from the distal aspect of gingival sulcus of the right maxillary canine distalized by an active lace-backs of tooth movement was measured on dental casts, before and one month after force application. One-way ANOVA with Bonferroni correction and Pearson's correlation test were used to analyze the data.RESULTS:The mean GCF leptin concentration increased from T0 to T1, rose to a peak at T2, then declined to a minimum value at T3 and then increased to a value at T4, closer to the base line value (T0), and it was statistically significant (P < 0.05). There was positive correlation of the overall mean leptin concentration to rate of tooth movement (correlation coefficient = 0.634).CONCLUSION:There was a biphasic change in GCF leptin concentration during one cycle of orthodontic force application. There was a positive correlation between the GCF leptin concentration and rate of tooth movement.

Evaluation of leptin levels in the serum and gingival cevicular fluid of chronic periodontitis patients

Leptin is pro-inflammatory mediator associated with the pathogenesis of periodontitis. The objective of the current study was to assess the levels of leptin in serum and gingival crevicular fluid (GCF) in chronic periodontitis (CP) patients. This cross-sectional study was conducted in Dental Teaching Hospital, Umm Al-Qura University, Makkah, Saudi Arabia. Forty-five individuals participated in this study: 25 with chronic periodontitis patients and 20 periodontally healthy controls. The patients were selected based on the criteria of American Academy of Periodontology using probing depths, bleeding on probing, clinical attachment loss and radiographs. GCF and serum samples were collected to estimate the leptin concentrations using enzyme-linked immunosorbent assay kits. Serum leptin levels were markedly higher in chronic periodontitis patients than in the healthy group (P < 0.00001). Although GCF leptin levels of chronic periodontitis patients were higher than the healthy group but this difference was not statistically significant (P Value = 0.5198). In conclusion, these results indicate positive association between serum leptin concentrations and chronic periodontitis. However, there was no significant correlation between GCF leptin level and periodontitis. Further studies are required to confirm these finding.

Correlation of crevicular fluid and serum levels of retinol-binding protein 4 and leptin in chronic periodontitis and obesity.

Retinol-binding protein 4 (RBP4) and leptin are both adipokines and involved in the pathophysiology of different vascular and inflammatory diseases and selectively elevated in patients with obesity. The aim of the present study was to determine and correlate the levels of RBP4 and leptin in gingival crevicular fluid (GCF) and serum in patients with chronic periodontitis (CP) and obesity. A total of 70 patients with age group 25 to 45years were divided into four groups based on gingival index (GI), probing depth (PD), clinical attachment level (CAL), body mass index (BMI) and radiographic evidence of bone loss. The groups were (1) group I (non-obese periodontally healthy), (2) group II (obese periodontally healthy), (3) group III (non-obese with chronic periodontitis) and (4) group IV (obese with chronic periodontitis). The GCF and serum levels of human RBP4 and leptin were quantified using ELISA. An increase in RBP4 levels from group I to group IV was found in both GCF and serum. However, GCF leptin levels was found to be greatest in group II, then group I, group IV and group III showing the least while an increase in serum levels from group I to group IV was found. The GCF and serum values of the inflammatory mediator correlated with the evaluated periodontal parameters and with each other (p<0.05). RBP4 and leptin can be considered as possible GCF and serum markers of inflammatory activity in CP and obesity, which further longitudinal studies are needed.

Association Between Gingival Crevicular Fluid Leptin Levels and Periodontal Status - A Biochemical Study on Indian Patients.

The present study was carried out to evaluate and compare the gingival crevicular fluid (GCF) concentrations of leptin in periodontally healthy and diseased subjects, to compare GCF leptin levels with respect to Body Mass Index (BMI) among the study groups and essentially to obtain an insight into leptin's possible role in the initiation and progression of periodontal disease. A total of 60 subjects; both males and females, based on their body mass index were selected for the study. They were categorized into three groups of 20 subjects each, based on their periodontal status, as follows: Group I (clinically healthy periodontium); Group II (gingivitis); Group III (chronic periodontitis). GCF samples of 1 μl were collected extra-crevicularly using black color-coded 1-5 μl calibrated volumetric microcapillary pipettes from one site in each person, and samples were analyzed for leptin using a commercially available ELISA kit. The concentration of leptin in GCF of patients in Group I (2272.25 pg/mL) was statistically higher (p < 0.001) than in those of Group II (1421.86 pg/mL) and Group III (1160.67 pg/mL). No statistically significant difference was found when comparing the mean values of leptin between BMI groups in each study group. As periodontal disease progressed, there was a substantial decrease in the gingival crevicular fluid leptin concentration. This suggests a protective role of leptin with regard to periodontal health.

Association between gingival crevicular fluid leptin level and periodontal parameters in patients with aggressive periodontitis

To determine the level of leptin in gingival crevicular fluid (GCF) of patients with aggressive periodontitis (AgP) and to analyze the relationship between leptin and periodontal cilinical parameters. Fifty-four patients with AgP and 30 healthy controls were recruited. Detailed clinical examinations were conducted, and clinical parameters such as bleeding index (BI), probing depth (PD), attachment loss (AL) were recorded. Two teeth were selected as test teeth in each subject, one from posterior area and the other from anterior region. The level of GCF leptin was measured by enzyme linked immunosorbent assay. The level of GCF leptin in AgP patients was significantly lower than that of control subjects [(16.5±4.6) ng vs.(26.0±6.0) ng, P < 0.05]. The level of GCF leptin was negatively related to BI (-0.306, P < 0.01), PD (-0.346, P < 0.01) and AL (-0.250, P < 0.01). AgP patients have significantly decreased level of GCF leptin and the level of leptin was negatively related to BI, PD and AL.

Effects of scaling and root planing with or without a local drug delivery system on the gingival crevicular fluid leptin level in chronic periodontitis patients: a clinico-biochemical study.

The present split mouth study evaluates the effect of nonsurgical periodontal treatment on the gingival crevicular fluid (GCF) leptin level in chronic periodontitis. Ninety sites from 30 nonobese chronic periodontitis patients were selected and divided as follows: group I, 30 healthy sites receiving no treatment; group II, 30 periodontitis sites receiving scaling and root planing (SRP); and group III, 30 periodontitis sites receiving SRP with tetracycline local drug delivery. At baseline, after GCF sampling and clinical parameter recording, the assigned treatment was performed for the study groups. During recall visits, GCF sampling followed by clinical parameter recording was done for groups II and III. Reductions in the probing depth and the clinical attachment level (CAL) were highly significant at different time intervals (except between day 0 and 45) in both groups II and III. Upon comparison, group III showed significant gain in CAL between day 0 and 15 and between day 0 and 45. After treatment, the reduction in the GCF leptin level was more significant in group III than in group II at day 15 but re-elevated almost to the pretreatment levels at day 45. Nonsurgical periodontal therapies were not effective in maintaining stable reduction in the GCF leptin level during the study period.

An Evaluation and Correlation of Leptin in Gingival Crevicular Fluid and Serum in Health, Gingivitis and Periodontitis

ABSTRACT Background and objective Plasma leptin is associated in patients with inflammatory diseases. A high concentration of leptin is associated with healthy gingival tissue. The purpose of this study was to assess the concentration of human leptin in gingival crevicular fluid (GCF) and serum within healthy and diseased gingiva, further to explore the possibility of using the levels of leptin in GCF and serum as a biochemical marker of periodontal disease progression. Materials and methods Ninety subjects were selected with age (30-39 years) and sex (15 males and 15 females) matched, to eliminate age and sex as confounders. The subjects were divided into three groups consisting of 30 subjects in each group based on the clinical and radiological parameters; healthy (group I), gingivitis (group II), periodontitis (group III), from whom the GCF samples were collected with Periopaper GCF collection strips (Proflow, Amityville, NY, USA) for 30 seconds and blood samples with 20-gauge needle syringe respectively. Leptin concentration was determined from individual GCF and serum samples by enzyme-linked immunosorbent assay (ELISA). Results The highest mean leptin concentration in GCF was observed in group I (2,664.30 pg/ml ± 324.73) and least mean leptin concentration was obtained in group III (1,309.43 pg/ml ± 202.45). The mean concentration of group II (1,639.43 pg/ml ± 344.46) was intermediate between the highest and lowest values. In contrast, the highest mean leptin concentration in serum was obtained for group III (12,086.57 pg/ml ± 1,698.23) and least mean leptin concentration was obtained for group I (8,715.09 pg/ml ± 1,649.19). The mean concentration of the group II (10,694.01 pg/ml ± 1,777.72) were intermediate between the highest and lowest values. Conclusion The results indicated a statistically significant decrease in the GCF leptin concentration and increase in serum leptin concentration as the periodontal disease progressed. Leptin Levels in Gingival Crevicular Fluid During Orthodontic Tooth Movement

To test if leptin can be detected in the gingival crevicular fluid (GCF) around moving teeth, and to determine whether any changes occur during orthodontic tooth movement. An upper canine requiring distal movement served as the test tooth; the contralateral canine was used as a control tooth. The control tooth was included in the orthodontic appliance, but was not subjected to the orthodontic force. GCF sampling from the distal sites of the test and control teeth was done at baseline, 1 hour, 24 hours, and 168 hours. Leptin concentrations of the test teeth decreased in a time-dependent manner. When compared with the baseline measurement, the decrease was significant at 168 hours (P < .05). The concentration of leptin in GCF is decreased by orthodontic tooth movement; the results of the present study also suggest that leptin may have been one of the mediators responsible for orthodontic tooth movement.

Gingival crevicular fluid and serum leptin: their relationship to periodontal health and disease

Leptin is a pleiotrophic hormone produced by adipose tissue and it plays an important role in protection of the host from inflammation and infection. The purpose of this study is to determine the presence of leptin in gingival crevicular fluid (GCF) and serum samples and to find out their association, if any. Forty two subjects were selected based on their body mass index and were divided into three groups of 14 each; healthy (Group I), chronic gingivitis (Group II) and chronic periodontitis (Group III). GCF samples (by microcapillary pipettes) and serum samples (by venipuncture) were collected to estimate the levels of leptin using enzyme linked immunosorbent assay kit. The highest mean leptin concentration in GCF was obtained for Group I (2658 pg/ml) and the least for Group III (1312 pg/ml). In contrast, the lowest serum leptin concentration was obtained for the Group I (8783 pg/ml), and the highest for Group III (12082 pg/ml). This suggests a negative correlation of GCF leptin concentration and a positive correlation of serum leptin concentration as the clinical attachment level progresses (p<0.05). These results suggest that greater the periodontal destruction, lesser is the GCF leptin concentration and greater the serum leptin concentration.

Leptin levels in gingival crevicular fluid in periodontal health and disease

A high concentration of leptin is associated with healthy gingival tissue, and the concentration of leptin decreases as periodontal disease progresses. However, to date, the leptin concentration in gingival crevicular fluid has not been documented. Hence, the present study was carried out to explore the presence of leptin in gingival crevicular fluid in periodontal health and disease, and to probe further into its possible role in periodontal disease progression. A total of 45 adult patients were selected, based on their body mass index, for the study. They were categorized into three groups of 15 patients each, based on their periodontal tissue status, as follows: group I (clinically healthy gingiva with no loss of attachment); group II (chronic gingivitis with no loss of attachment); and group III (chronic periodontitis). Gingival crevicular fluid samples of 1 microL were collected extracrevicularly using white color-coded 1-5 microL calibrated volumetric microcapillary pipettes from one site in each person, and samples were analyzed for leptin using a commercially available enzyme-linked immunosorbent assay kit. The concentration of leptin in gingival crevicular fluid of patients in group I (2292.69 pg/mL) was statistically higher (p < 0.05) than in those of groups II (1409.95 pg/mL) and III (1071.89 pg/mL). This suggests a negative correlation of gingival crevicular fluid leptin concentration with clinical attachment loss (p < 0.05). As periodontal tissue destruction increased, there was a substantial decrease in gingival crevicular fluid leptin concentration. This observation extends our knowledge of the protective role of leptin in periodontal health.

Gingival Crevicular Fluid Leptin Levels in Periodontitis Patients With Long‐Term and Heavy Smoking

The aim of the present study was to evaluate gingival crevicular fluid (GCF) leptin levels and the influence of long-term and heavy smoking on GCF leptin levels in patients with chronic periodontitis. In this study, 143 individuals were divided into three groups: non-smokers (NS), smokers (S), and control (C). Three subgroups of NS and S were grouped as follows: a) probing depth (PD) <or=3 mm; b) PD = 4 to 5 mm; and c) PD >5 mm. For each patient, PD, gingival index (GI), plaque index (PI), gingival bleeding time index (GBTI), and clinical attachment level (CAL) values were recorded. The GCF leptin levels obtained from sampling sites were determined by using enzyme-linked immunosorbent assay kits. The GCF leptin levels were found significantly lower in the a and b subgroups in the S group than those in the NS group (P <0.05). The inflammatory markers GI and GBTI showed significant correlations with leptin in NS (P <0.05). Our results suggest that higher leptin GCF levels in healthy sites in periodontitis patients may play a protective role in periodontal disease. Further studies are needed to determine the cellular origin of the leptin in the gingiva and the effect of plasma leptin levels on GCF leptin concentrations.
