Mediastinal mature teratoma with complete gastrointestinal and bronchial walls

An extremely rare case of anterior mediastinal mature teratoma with almost complete gastrointestinal and bronchial walls is described. A 65-year-old woman presented with left precordial pain. Chest computed tomography showed a huge anterior mediastinal tumor, 15 cm × 21 cm, occupying the left thoracic cavity. Post-resection histopathological examination confirmed the diagnosis of mature teratoma and demonstrated almost complete gastrointestinal and bronchial walls. Although mature teratomas of the ovary and sacrococcygeal area are known to rarely contain organoid structures with various degrees of differentiation, this is the first case of an anterior mediastinal mature teratoma that contained well-developed organoid structures.

Congenital Partial Absence of the Left Pericardium Associated With Tricuspid Regurgitation

We report the case of a 47-year-old man who presented with several episodes of left precordial pain, one of which had been severe, but was unrelated to exertion or posture. Transthoracic echocardiography and cardiovascular magnetic resonance showed evidence of congenital partial absence of the left pericardium and severe tricuspid regurgitation. Both diagnoses were confirmed at surgery when the pericardial defect was repaired and the tricuspid valve was replaced at the same operation. He went on to make a good recovery.

Unsuspected Esophageal Perforation

ESOPHAGEAL perforation has been seen more frequently in recent years because of the increased use of instrumentation as well as the increase in the number of surgical procedures performed on the esophagus. 1 Despite aggressive therapy, including surgical drainage and antibiotics, mortality remains high. If therapy is delayed, mortality is reportedly as high as 86%. 2 The following cases were recently seen at the University of California at Los Angeles (UCLA) and represent unsuspected perforations of an unusual cause with a fatal outcome. Report of Cases Case 1.— An 81-year-old man was admitted to the UCLA Medical Center with a three-day history of left precordial pain associated with dyspnea. The pain was increased with deep inspiration and by lying on his left side. A sore lump in the throat was noted about one week before admission. Chest roentgenograms demonstrated an elevated left hemidiaphragm, with a left pleural effusion and cardiomegaly.

Massive pericardial effusion in subacute bacterial endocarditis.

MINOR pericardial effusion has been reported to occur in a small percentage of cases of subacute bacterial endocarditis. However, a review of the literature fails to reveal any case of massive pericardial effusion due to subacute bacterial endocarditis alone. It is the purpose of this paper to present such a case. Report of a Case A 39-year-old male was admitted to Brooklyn Hospital on May 10, 1962, because of severe left precordial pain and dyspnea. Eight months prior to admission he noticed a painful blood blister in the pad of the right great toe. A similar lesion was subsequently seen in the pad of the right fourth finger 1 month prior to admission, at which time he developed bilateral pedal edema. Two weeks prior to admission he noticed increasing dyspnea on exertion and orthopnea. Night sweats began 4 days prior to admission. There was no history of fever or shaking
