Paracentral Acute Middle Maculopathy After COVID-19 Disease: Multimodal Evaluation.

To report the case and multimodal imaging findings of a healthy young woman who developed paracentral acute middle maculopathy (PAMM) 9 weeks after COVID-19 disease. Case report. Ultra-widefield fundus photography, macular spectral-domain optical coherence tomography (SD-OCT), fluorescein angiography (FA), and OCT-angiography (OCT-A) were performed. A 36-year-old woman who developed PAMM 9 weeks after SARS-CoV-2 infection. A 36-year-old woman went to the emergency department (ED) with sudden, painless, left eye (LE) vision loss. The only relevant past medical history was COVID-19 disease 9 weeks before. Best corrected visual acuity (BCVA) was 20/200, a LE relative afferent pupillary defect (RAPD) was present and superficial hemorrhages throughout the macular area and peripheral retina were found. Nearly four hours after admission, LE BCVA recovered to 20/20 without RAPD. Five days after presentation in the ED, the patient returned with recurrent LE vision loss, with spontaneous recovery within 12 hours. Macular SD-OCT revealed hyperreflectivity of the inner plexiform and inner nuclear layers and the diagnosis of PAMM was established. The patient started oral acetylsalicylic acid and oral prednisolone. The patient did not report any new episodes of vision loss and there was a progressive resolution of abnormal fundus findings. SARS-CoV-2 infection increases the risk of vascular thrombotic events with possible involvement of the retinal circulation, and PAMM may present as a possible complication. Ophthalmologists should be able to recognize it promptly through multimodal imaging findings.

<p>FLACS in Congenital Iris Coloboma: A Useful Technique</p>

This paper reports the technique and outcome of femtosecond laser-assisted cataract surgery (FLACS) in an iris coloboma patient (59-year-old male patient) with bilateral grade 3 cataractous lens and a shallow anterior chamber. Best-corrected visual acuity (BCVA) was 0 on logMAR with spherical equivalent (SE) 9 D in the right eye, and BCVA of 0.3 logMAR with a SE 8.75 D in the left. FLACS was performed on the left then the right eye using the Catalys platform of Johnson & Johnson (intraocular lens implantation of AMO Tecnis 1 ZCB00). Customized parameters were constructed to create capsulotomies of 4.54.7 mm for the left and right eyes, respectively, with manual centration of the distorted pupils. On the second day of follow-up, left-eye BCVA was 0.5 with SE 1 D, and right-eye BCVA was 1 with SE −0.75 D. At the 1-year follow-up, eyes showed stable bilateral Tecnis 1 IOL and refraction. FLACS is a safe surgical option with stable outcomes in cases of cataract with iris coloboma.

Idiopathic Macular Telangiectasia Type 1 treated with Intravitreal Ranibizumab and Laser Photocoagulation

Introduction: Idiopatic Macular Telangiectasia (IMT) are a rare group of various entities presenting with incompetence, ectasia and/or irregular dilations of the juxtafoveolar capillaries of one or both eyes. Case Report: A 62 year old female with Glaucoma was sent to the Retina Department with a suspected diagnosis of retinal vein occlusion on the left eye (LE). Her best corrected visual acuity (BCVA) was 20/20 in the right eye (RE) and 20/25 in the LE. Slit lamp examination was normal; fundoscopy showed a cup-to-disc relation of 0.6 in the RE with normal macula and a cup-to-disc relation of 0.7 in the LE with multiple perifoveal microaneurysms, inferior temporal hard exudates and edema comproved with macular optical coherence tomography; fluorescein angiography revealed an area of hyperfluorescence with juxtafoveolar telangiectatic vessels located inferiorly and temporally to the fovea, with progressive filling and late active leakage in the LE. The diagnosis of IMT type 1 was made and she was treated with a loading dose of intravitreal injections of ranibizumab followed by a fourth injection because of persistent macular edema. One month after, her BCVA was 20/20 in the LE with improvement of macular edema and macular argon laser photocoagulation was performed. Three months after the laser treatment her LE BCVA was 20/20 with fundoscopy showing small macular hemorrhages and microaneurysms. Conclusion: Macular edema and exudation are the main cause of visual loss in these patients. This case suggests that ranibizumab and laser photocoagulation can be effective in the treatment of type 1 IMT.

Visual Outcome versus Vision Satisfaction after Intravitreal Bevasizumb Injection in Diabetic Retinopathy

Introduction: Diabetic retinopathy as an important complication of diabetes mellitus is a common cause of blindness in diabetic people. A cure or treatment is not available yet. This study was designed to investigate the effectiveness of the intravitreal injection of Avastin on patient’s visual satisfaction with diabetic retinopathy. Materials and Methods: This study included 30 eyes of thirty diabetic retinopathy patients (mean age: 60.47±8.94 years) showing no recovery with common treatments. After the intravitreal injection of 1.25mg/0.05ml of Avastin, the examination was performed after injection. In this examination, the resolution of neovascularization, clearance of vitreous and visual acuity were evaluated versus visual satisfaction. Results: 60% of patients were women (n=18) and 40% were men (n=12). All patients had type 2 diabetes for a period of 8. 47±5.28 years (range 2-25 years). OR.2014.6.009 353 was 0.22±0.63 log MAR in the best corrected visual acuity (BCVA) in right eye which was statistically significant (p=0.010). The BCVA in left eye was 0.31±0.74 log mar, also showing significant relation (p=0.020). BCVA Changes did not have a meaningful relationship with age and sex of patients, but these changes were negatively related to duration of diabetes retinopathy in both right and left eyes (p=0/048, p=0/006 respectively). Conclusion: Avastin showed short-term statistically significant visual benefits and also, improvements in ophthalmic pathology in clinical examination versus visual satisfaction, however it was not compatible to ask all of the patient's visual satisfaction. Therefore, further studies will be needed to determine the therapeutic effects of this treatment option.

Acute corneal hydrops with perforation after LASIK‐associated keratectasia

Herein a case of acute corneal hydrops with perforation in keratectasia 9 years after bilateral laser in situ keratomileusis is presented. A 27-year-old man presented with right eye sudden loss of vision. Right eye best-corrected visual acuity was counting fingers. Left eye best-corrected visual acuity was 6/9. Right eye slit-lamp examination revealed diffuse corneal oedema with central epithelial cystic change. The interface between the flap and residual stroma was open and filled with fluid. This separation extended nearly to the flap margin. Shallow anterior chamber and aqueous leakage were noted the following day. An emergency penetrating keratoplasty was performed to the right eye. Histopathology of corneal button revealed reactive keratocytes with irregular, wrinkled collagen formation over ablated stroma. Marked stromal oedema was observed. Twelve months after corneal transplantation, vision improved to 6/6 with correction in the right eye.
