THE LEFT ATRIAL APPENDAGE MORPHOLOGY IN PATIENTS WITH ATRIAL FIBRILLATION BY CARDIAC COMPUTED TOMOGRAPHY

Purpose: To describe the morphological characteristics of left atrial appendage (LAA) on multislice computed tomography in patients with atrial fibrillation.
 Materials and methods: A prospective descriptive study on 44 patients diagnosed with atrial fibrillation (AF) and underwent multislice computed tomography at Bach Mai Hospital from August 2022 to July 2023. Results: Mean age 64.45 ± 10.50. The most common morphology of LAA is “windsock” with 40.91%, followed by “chicken wing” with 34.09%, then “cauliflower” with 13.64%, and finally “cactus” with 11.36%. The most common LAA orifice position is in the middle (63.64%), the average length of the LAA is 47.03 ± 11.69 mm, and the mean volume of the LAA is 14.39 ± 8.00 ml. There is little relationship between gender and morphological characteristics of the LAA.
 Conclusion: The most common LAA morphology in patients with atrial fibrillation is “windsock”. Gender has little association with left atrial appendage morphology.

Left atrial appendage structure predicts thromboembolic risk in 185 patients with crytogenic stroke/TIA without history of AF

Main funding source(s): Akerhus University Hospital Dept. of Cardiology Background Cryptogenic stroke/TIA (CS) is supposed to be associated with intracardiac thrombus formation and left atrial (LA) structure, in particular thrombus formation in left atrial appendage (LAA). However, there is little evidence if thromboembolic risk is associated with LAA morphology in patients with CS. Purpose We aimed to investigate, if different LAA morphology types may predict thromboembolic risk in patients with CS. Methods In this prospective study (mean follow-up 859 ± 226 days), 185 patients with CS (mean age 68 ±13 years, 33% female) without history of AF were included. All patients underwent complete transesophageal echocardiography in sinus rhythm after index CS (mean 5 ± 3days). Occurrence of LAA thrombus, sludge or spontaneous echo contrast (SEC) were defined as thromboembolic risk and LAA structure was evaluated by 2D and 3D transesophageal echocardiography. Multilobate LAA was defined as LAA with > 1 lobes. Results Of the 185 included patients, LAA type chicken wing was found in 79 (43%), type windsock in 64 (35%), type cactus in 35 (19%) and type cauliflower in 7 (4%). Thrombus formation occurred in 29/185 (16%), LAA sludge in 112/185 (61%), and LAA SEC in 74/185 (40%), which is increased compared to data in the general population. We found SEC in sludge in 65/112 (58%), SEC in thrombus in 18/29 (62%) and sludge in thrombus in 27/29 (93%). SEC and sludge were significantly associated with thrombus formation, (p < 0.001 and p < 0.01, respectively). Finally, thromboembolic risk occurred in 123 (67%) of all patients with CS. Moreover, LAA type chicken wing was significantly associated with thromboembolic risk (p < 0.01), while all other LAA types were not. Multilobate LAA occurred in 115 (62%) of all patients and predicted thromboembolic risk vs. non-lobated LAA (p= 0.04). In a multivariate analysis LAA type chicken wing and multilobate LAA were independently predictors of thromboembolic risk, figure 1. Conclusions Patients with cryptogenic stroke/TIA without history of AF showed increased thromboembolic risk, and SEC and sludge were significantly associated with LAA thrombus formation. Left appendage morphology as risk factors for cardio-embolism in patients with stroke of undetermined source

Abstract Background Left atrium (LA) volume and function and certain left atrial appendage (LAA) morphologies are known to be strongly associated with the risk of thrombus formation and thromboembolism in patients with atrial fibrillation (AF). Little is known about the role of LA parameters and LAA morphology in patients with Embolic Stroke of Undetermined Source (ESUS). Purpose To assess LA volume and function and LAA morphologies in ESUS patients and the potential association with cardioembolic risk. Methods Twenty consecutive patients with ESUS who underwent contrast enhanced electrocardiogram-gated computed tomography (CT) as part of stroke etiology evaluation were matched by age and gender to patients with and without history of AF (twenty in each group) who had CT as part of standard evaluation before transcatheter aortic valve implantation. LA maximum (LAVmax) and minimum (LAVmin) volumes and function, measured as total emptying volume (LAEV) and total emptying fraction (LAEF) were assessed, and three- dimensional structures of the LA and LAA were constructed using the volume-rendered post-processing technique. Results The mean age of patients was 75.9±8.61, 76.1±8.52, 77.8±7.72) in the ESUS, AF and non-AF groups, respectively, with 11 (55%) females in each group. Compared to patients with ESUS, patients with AF showed significantly greater LAVmax (100.59±31.59 ml vs.155.78±63.15 ml, respectively, p=0.001) and LAVmin (71.38±30.69 ml vs. 140.73±64.60 ml; respectively, p<0.001), while LAV max and LAVmin were similar, in patients with ESUS and non-AF. The distribution of LAA morphologies significantly differed between patients with ESUS and patients with AF and non-AF: 1 (5%) vs. 9 (45%) chicken-wing; 14 (70%) vs. 8 (40%) cauliflower; and 5 (25%) vs. 3 (15%) Cactus; respectively. Conclusion LA appendage morphology, but not LA volume, seems to be associated with ESUS. LAA morphologies with multiple lobes, cauliflower and cactus, seems to be associated with ESUS. Left atrial appendage morphology predicts the formation of left atrial appendage thrombus.

Nonchicken wing left atrial appendage (LAA) morphology is associated with higher risk for stroke in patients with atrial fibrillation (AF) than chicken wing (CW) morphology. Assess whether LAA morphology predicts the formation of LAA thrombus independent of age, sex, presenting rhythm, left ventricular ejection fraction (LVEF), or anticoagulant use. A cross-sectional analysis was performed on patients prospectively enrolled in the Vanderbilt LAA Registry or presenting for transesophageal echocardiogram (TEE) between January 1, 2015, and November 1, 2017 (n = 306). Two physicians independently reviewed TEEs interpreted as having LAA thrombus. Determination of LAA morphology, ejection velocity, and presence of thrombus (n = 102) were based on 0°, 45°, 90°, and 135° TEE views. The control cohort (n = 204) included consecutive AF patients undergoing TEE without LAA thrombus. LAA morphology in patients with LAA thrombus was: 35% windsock, 47% broccoli, and 12% CW. Windsock (odds ratio [OR], 4.0; 95% confidence interval [CI]: 1.7-9.3, p = .001) and broccoli (OR, 6.6; 95% CI: 2.6-16.6; p < .001) morphology were higher risk for thrombus compared to CW. Female sex predicted higher-odds for LAA thrombus (OR, 2.6; 95% CI: 1.4-4.8; p = .002) as did LAA-EV < 20 cm/s (OR, 11.12; 95% CI: 5.6-22.1). Anticoagulation use (OR, 0.5; 95% CI: 0.3-0.9; p = .03) and higher LVEF (OR, 0.95; 95% CI: 0.93-0.98; p < .001) were associated with lower risk. In patients with a CW morphology who had LAA thrombus, 4 of the 7 had an LAA-EV < 20 cm/s and acute systolic heart failure with LVEF < 30% or active malignancy. In multivariable linear regression analysis controlling for presenting rhythm, anticoagulant use, age, sex, and LVEF, CW morphology appears relatively protective from LAA thrombus (p = .001). CW LAA morphology appears relatively protective against the formation of LAA thrombus.

Abstract 13077: Impact of Age on the Morphology of Left Atrial Appendage in Atrial Fibrillation

Background: Left atrial appendage (LAA) is the main location of thrombus formation in patients with atrial fibrillation (AF), which has a variety of structure. Recent studies reported that “non-chicken wing” LAA morphology had a significantly higher risk of stroke compared with “chicken wing” morphology. However, LAA morphology might be affected by aging or AF. Thus, we evaluated the relationship between age and frequency of LAA morphology in patients with AF and sinus rhythm (SR). Methods: We enrolled 145 AF patients and 199 SR patients without any cardiovascular disease. LAA volume and morphology were assessed by contrast-enhanced multi-detector computed tomography. The morphology was visually classified “chicken wing” or “non-chicken wing” according to the previous report. Results: LAA volume index was significantly higher in the AF group than in the SR group (6.8±2.8 ml/m 2 vs. 4.4±1.8 ml/m 2 , p&lt;0.001).There was no significant trend in frequency of non-chicken wing among ages in SR group (p=0.36 for trend), whereas it was negatively related to age in AF group (p=0.002 for trend). In multivariable logistic regression analysis, age (OR 0.95, p=0.021) and duration of AF (OR 0.97, p=0.002) were independent factors of non-chicken wing LAA morphology. Conclusions: LAA morphology is affected by age, especially in patients with AF. When we utilize LAA morphology (non-chicken wing type) as a stroke risk factor in patients with AF, we may have to pay attention to their age.

Morphologic remodeling of left atrial appendage in patients with atrial fibrillation

Left atrial appendage (LAA) has recently been of significant focus because of the development of the LAA closure device. The purpose of this study was to test the hypothesis that atrial fibrillation (AF) leads to a morphologic change in the LAA and to investigate the characteristics of LAA morphology in patients with and without AF. This retrospective study included 225 patients (persistent AF [PeAF], n = 76; paroxysmal AF [PAF], n = 70; control, n = 79] who underwent echocardiography and computed tomography (CT). All patients were classified into 2 types (chicken wing [CW] or non-chicken wing [non-CW]) using CT. The prevalence of non-CW-type LAA was 39.5%, 15.7%, and 8.9% in the PeAF, PAF, and control groups, respectively. Patients in the PeAF group had a higher prevalence of non-CW-type LAA than did those in the PAF and control groups (P = .0014 and P <.0001, respectively). In addition, all patients were divided into 5 groups based on the type of cardiac rhythm (PeAF, PAF, or sinus rhythm) and left atrial volume index (cutoff value; 34 mL/m(2)): group A (Control), group B (PAF/Small-LA), group C (PAF/Large-LA), group D (PeAF/Small-LA), group E (PeAF/Large-LA). The prevalence of non-CW-type LAA was 9%, 14%, 17%, 29%, and 41% in groups A, B, C, D, and E, respectively. Presence of persistent AF was associated with a higher prevalence of non-chicken wing-type LAA. Our analysis suggests that remodeling in patients with persistent AF can lead to a change in LAA morphology.

Left atrial appendage morphology assessment for risk stratification of embolic stroke in patients with atrial fibrillation: A meta-analysis

Thromboembolic event (TE) risk stratification is performed by using CHA2DS2VASc score. It has been suggested that left atrial appendage (LAA) morphology independently influences TE risk in patients with nonvalvular atrial fibrillation. LAA morphology has been classified into 4 types: chicken wing, cauliflower, windsock, and cactus. The purpose of this study was to determine TE risk for each LAA morphology in patients with atrial fibrillation with low to intermediate TE risk. A systematic review of MEDLINE, Cochrane Library, and Embase for studies that used computed tomography, tridimensional transesophageal echocardiography, and cardiac magnetic resonance imaging to categorize the LAA morphologies with assessment of TE prevalence. Odds ratio (OR) and 95% confidence intervals (CIs) were measured using the Mantel-Haenszel method. The fixed effects model was used, and if heterogeneity (I2) was >25%, effects were analyzed using a random model. Eight studies with 2596 patients were included. Eighty-four percent (n=1872) of patients had a CHADS2 score of <2. TE risk was lower in chicken wing morphology than in non-chicken wing morphology (OR 0.46; 95% CI 0.36-0.58). Likewise, chicken wing morphology had lower TE risk than did other morphologies (chicken wing vs cauliflower: OR 0.38; 95% CI 0.26-0.56; chicken wing vs windsock: OR 0.48; 95% CI 0.31-0.73; chicken wing vs cactus: OR 0.49; 95% CI 0.36-0.66). Patients with chicken wing LAA morphology are less likely to develop TE than patients with non-chicken wing morphology. LAA morphology may be a valuable criterion in predicting TE and could affect the stratification and anticoagulation management of patients with low to intermediate TE risk.

Morphological Changes of the Left Atrial Appendage After Catheter Ablation of Atrial Fibrillation

The left atrial appendage (LAA) has been proven to be the most important site of thrombus formation in patients with atrial fibrillation (AF). However, the information regarding the morphometric alteration of the LAA related to the outcome of AF ablation is still lacking. Thus, we evaluated the long-term changes of the LAA morphology in patients undergoing catheter ablation of AF using magnetic resonance angiography (MRA). Group 1 included 15 controls without any AF history. Group 2 included 40 patients with drug-refractory paroxysmal AF. They were divided into two subgroups: group 2a included 30 patients without AF recurrence after pulmonary vein (PV) ablation. Group 2b included 10 patients with late recurrence of AF. The LAA morphology before and after (20 +/- 11 months) ablation was evaluated by three-dimensional MRA. The group 2 patients had a larger baseline LAA size (including the LAA orifice, neck, and length) and less eccentric LAA orifice and neck. After the AF ablation, there was a significant reduction in the LAA size in the group 2a patients, and the morphology of the LAA neck became more eccentric during the follow-up period. In group 2b, the LAA size increased and no significant change in the eccentricity of the orifice and neck could be noted. The morphometric remodeling of the LAA in the AF patients could be reversed after a successful ablation of the AF. Progressive dilation of the LAA was noted in the patients with AF recurrence. These structural changes in the LAA may play a role in reducing the potential risk of cerebrovascular accidents.
