Referenceless magnetic resonance temperature imaging using Gaussian process modeling

During magnetic resonance (MR)-guided thermal therapies, water proton resonance frequency shift (PRFS)-based MR temperature imaging can quantitatively monitor tissue temperature changes. It is widely known that the PRFS technique is easily perturbed by tissue motion, tissue susceptibility changes, magnetic field drift, and modality-dependent applicator-induced artifacts. Here, a referenceless Gaussian process modeling (GPM)-based estimation of the PRFS is investigated as a methodology to mitigate unwanted background field changes. The GPM offers a complementary trade-off between data fitting and smoothing and allows prior information to be used. The end result being the GPM provides a full probabilistic prediction and an estimate of the uncertainty. GPM was employed to estimate the covariance between the spatial position and MR phase measurements. The mean and variance provided by the statistical model extrapolated background phase values from nonheated neighboring voxels used to train the model. MR phase predictions in the heating ROI are computed using the spatial coordinates as the test input. The method is demonstrated in ex vivo rabbit liver tissue during focused ultrasound heating with manually introduced perturbations (n = 6) and in vivo during laser-induced interstitial thermal therapy to treat the human brain (n = 1) and liver (n = 1). Temperature maps estimated using the GPM referenceless method demonstrated a RMS error of <0.8°C with artifact-induced reference-based MR thermometry during ex vivo heating using focused ultrasound. Nonheated surrounding areas were <0.5°C from the artifact-free MR measurements. The GPM referenceless MR temperature values and thermally damaged regions were within the 95% confidence interval during in vivo laser ablations. A new approach to estimation for referenceless PRFS temperature imaging is introduced that allows for an accurate probabilistic extrapolation of the background phase. The technique demonstrated reliable temperature estimates in the presence of the background phase changes and was demonstrated useful in the in vivo brain and liver ablation scenarios presented.

A Learning Based Fiducial-driven Registration Scheme for Evaluating Laser Ablation Changes in Neurological Disorders.

In this work, we present a novel learning based fiducial driven registration (LeFiR) scheme which utilizes a point matching technique to identify the optimal configuration of landmarks to better recover deformation between a target and a moving image. Moreover, we employ the LeFiR scheme to model the localized nature of deformation introduced by a new treatment modality - laser induced interstitial thermal therapy (LITT) for treating neurological disorders. Magnetic resonance (MR) guided LITT has recently emerged as a minimally invasive alternative to craniotomy for local treatment of brain diseases (such as glioblastoma multiforme (GBM), epilepsy). However, LITT is currently only practised as an investigational procedure world-wide due to lack of data on longer term patient outcome following LITT. There is thus a need to quantitatively evaluate treatment related changes between post- and pre-LITT in terms of MR imaging markers. In order to validate LeFiR, we tested the scheme on a synthetic brain dataset (SBD) and in two real clinical scenarios for treating GBM and epilepsy with LITT. Four experiments under different deformation profiles simulating localized ablation effects of LITT on MRI were conducted on 286 pairs of SBD images. The training landmark configurations were obtained through 2000 iterations of registration where the points with consistently best registration performance were selected. The estimated landmarks greatly improved the quality metrics compared to a uniform grid (UniG) placement scheme, a speeded-up robust features (SURF) based method, and a scale-invariant feature transform (SIFT) based method as well as a generic free-form deformation (FFD) approach. The LeFiR method achieved average 90% improvement in recovering the local deformation compared to 82% for the uniform grid placement, 62% for the SURF based approach, and 16% for the generic FFD approach. On the real GBM and epilepsy data, the quantitative results showed that LeFiR outperformed UniG by 28% improvement in average.

Quantitative evaluation of multi-parametric MR imaging marker changes post-laser interstitial ablation therapy (LITT) for epilepsy.

Laser-induced interstitial thermal therapy (LITT) has recently emerged as a new, less invasive alternative to craniotomy for treating epilepsy; which allows for focussed delivery of laser energy monitored in real time by MRI, for precise removal of the epileptogenic foci. Despite being minimally invasive, the effects of laser ablation on the epileptogenic foci (reflected by changes in MR imaging markers post-LITT) are currently unknown. In this work, we present a quantitative framework for evaluating LITT-related changes by quantifying per-voxel changes in MR imaging markers which may be more reflective of local treatment related changes (TRC) that occur post-LITT, as compared to the standard volumetric analysis which involves monitoring a more global volume change across pre-, and post-LITT MRI. Our framework focuses on three objectives: (a) development of temporal MRI signatures that characterize TRC corresponding to patients with seizure freedom by comparing differences in MR imaging markers and monitoring them over time, (b) identification of the optimal time point when early LITT induced effects (such as edema and mass effect) subside by monitoring TRC at subsequent time-points post-LITT, and (c) identification of contributions of individual MRI protocols towards characterizing LITT-TRC for epilepsy by identifying MR markers that change most dramatically over time and employ individual contributions to create a more optimal weighted MP-MRI temporal profile that can better characterize TRC compared to any individual imaging marker. A cohort of patients were monitored at different time points post-LITT via MP-MRI involving T1-w, T2-w, T2-GRE, T2-FLAIR, and apparent diffusion coefficient (ADC) protocols. Post affine registration of individual MRI protocols to a reference MRI protocol pre-LITT, differences in individual MR markers are computed on a per-voxel basis, at different time-points with respect to baseline (pre-LITT) MRI as well as across subsequent time-points. A time-dependent MRI profile corresponding to successful (seizure-free) is then created that captures changes in individual MR imaging markers over time. Our preliminary analysis on two patient studies suggests that (a) LITT related changes (attributed to swelling and edema) appear to subside within 4-weeks post-LITT, (b) ADC may be more sensitive for evaluating early TRC (upto 3-months), and T1-w may be more sensitive in evaluating early delayed TRC (1-month, 3-months), while T2-w and T2-FLAIR appeared to be more sensitive in identifying late TRC (around 6-months post-LITT) compared to the other MRI protocols under evaluation. T2-GRE was found to be only nominally sensitive in identifying TRC at any follow-up time-point post-LITT. The framework presented in this work thus serves as an important precursor to a comprehensive treatment evaluation framework that can be used to identify sensitive MR markers corresponding to patient response (seizure-freedom or seizure recurrence), with an ultimate objective of making prognostic predictions about patient outcome post-LITT.

Abstract No. 359: MR guided laser ablation in the chest, abdomen, and pelvis

Understand the indications and versatility of MR guided laser thermal therapy and the advantages this technique has over other ablation methods. Understand the complications that can occur using this technique and the necessary patient follow up that is required. In laser-induced interstitial thermal therapy (LITT), fiber optics are placed directly into the tumor to deliver laser light energy. This results in volumetric heating, which ultimately leads to thermal destruction of the tumor. Laser applicators for LITT may be inserted into target tissue using several different image-guided techniques. The advantage of using MRI includes the ability to ablate lesions which are only visible on particular MR sequences. Dynamic MRI can be used to infer temperature and/or tissue changes as a potential means of feedback during the procedure, providing precise control of ablation zone. Here we evaluate the use of MR-guidance and thermometry to ablate solid lesions using a diode laser. From 5/2010 to 8/2011 we treated 21 patients (7 kidney, 12 liver, 1 lung, and 1 prostate) with solid mass lesions using the Visualase laser system (Visualase, Inc. Houston, TX) with MR-guidance and temperature monitoring. Here we highlight the procedural details and outcomes. Clinical and imaging follow up was performed over a mean of 135 days (range 22–270) in 14 patients. The remaining patients have not yet reached their 3 month post procedure follow up. Complications included short term bladder catheterization for blood clots after renal ablation, thermal ureteral stricture, hematoma in ablation bed, pain requiring oral narcotics, and asymptomatic pneumothorax requiring chest tube placement. Three months after ablation, there is no recurrent or residual tumor by MR examination in 9 patients. Five patients (three liver and two renal) have residual tumor requiring retreatment. Laser ablation with MR guidance and temperature mapping allows precise thermal destruction of solid tumors. Follow up imaging is mandatory to assess for residual disease. Complications encountered are similar to those seen after ablation procedures utilizing other techniques.

Non-invasive 3d magnetic resonance thermal mapping: determination of the lesion size during laser-therapy in ex vivo tissues

Object Developments in open magnetic resonance imaging (MRI) magnets have made possible the use of reproducible thermosensitive sequences to determine temperature distribution inside biological tissue. This study aimed to compare MR thermal mapping during laser-induced interstitial thermal therapy (LITT) with macroscopically observed thermal lesions in order to estimate the 3D size of the coagulative necrosis.

Computer-Assisted Intra-Operative Magnetic Resonance Imaging Monitoring of Interstitial Laser Therapy in the Brain: A Case Report

Hardware and software for a customized system to use magnetic resonance imaging (MRI) to noninvasively monitor laser-induced interstitial thermal therapy of brain tumors are reported. An open-configuration interventional MRI unit was used to guide optical fiber placement and monitor the deposition of laser energy into the targeted lesion. T1-weighted fast spin echo and gradient echo images were used to monitor the laser tissue interaction. The images were transferred from the MRI scanner to a customized research workstation and were processed intraoperatively. Newly developed software enabled rapid (27-221 ms) availability of calculated images. A case report is given showing images which reveal the laser-tissue interaction. The system design is feasible for on-line monitoring of interstitial laser therapy. © 1998 Society of Photo-Optical Instrumentation Engineers.

MRI of laser-induced interstitial thermal injury in an in vivo animal liver model with histologic correlation.

Laser-induced interstitial thermal therapy (LITT) is a preferred method of minimally invasive therapy. MRI is a noninvasive method by which to monitor the thermal effects of LITT. To properly control such effects, changes in MRI parameters during and after LITT should be correlated with changes in the tissue. T1-weighted fast spin echo (FSE) MRI (1 image/10 seconds) at 1.5 T monitored LITT in vivo in rabbit liver (n = 6) using an interstitial bare delivery fiber (600-microm diameter; 3.0 W; 1,064 nm; 150 seconds). During laser irradiation, MRI signal intensity decreased around the fiber tip; after irradiation, this hypointensity proved reversible and permanent lesions were evident. The lesions had hyperintense margins that were brighter than surrounding normal tissue (P < .001); the tissue in these bright regions was mapped to tissue necrosis characterized by the presence of thermally damaged ghost red blood cells amid generally normal hepatocytes. T1-FSE identified the spatial extent of the LITT lesions.
