Epidemiological features papillomatosis of the larynx in adults: a literature review

The literature review presents current data on the epidemiology, drug, and surgical treatment of laryngeal papillomatosis in adults. Possible prospects for further study of the prevalence and incidence of the disease and provoking factors of recurrence of the disease for the development of possible preventive measures are considered.

Effects of oral screen exercise on orofacial and pharyngeal activity: An exploratory study using videofluoroscopy and surface electromyography in healthy adults.

ObjectiveThe oral screen is a device commonly used for treatment of orofacial disorders. The objective of this exploratory study was to examine the effect of oral screen exercise on the muscle activity in the lips, submental complex, masseter muscle, and kinematic activity of the tongue base, soft palate, pharynx, and larynx in healthy adults. This was compared with the kinematic activity during a dry swallow. It was hypothesized that not only the lip musculature but also other structures in the oral and pharyngeal cavities are activated while using an oral screen device.MethodTen healthy subjects used an oral screen during examination with videofluoroscopy and surface electromyography (EMG). Three different instructions for oral screen application and a dry swallow were examined.ResultsThe lip muscles showed the highest activity during oral screen exercise. The other muscle groups were activated to a lesser degree. The pattern of activation differed between individuals. Compared with a dry swallow, the range of motion of the tongue base, posterior pharyngeal wall, and the larynx was significantly smaller during oral screen activation. No major differences were found between three different instructions.ConclusionThis study indicates that the lips and submental complex and, to a lesser degree, oral, pharyngeal, and laryngeal structures are activated with the oral screen, but the pattern of activation varied between individuals. In comparison to the activity during a dry swallow, range of motion during oral screen exercise is small.

Laryngeal Papillomatosis in Adults: Assessment for Ten Years at the ENT Department of the National University Hospital of Fann (Dakar, Senegal).

Objectives The aim of this study was to describe the epidemiological, diagnostic, and therapeutic aspects of adult laryngeal papillomatosis in Senegal. Patients and Methods. This is a retrospective descriptive study of patients aged above 18 years with laryngeal papillomatosis and followed at the ENT department of the NUH of Fann between 01 January 2009 and 31 December 2018. Results The mean age at diagnosis was 37.74 years and a sex ratio of 0.93. The 20–29 age group was the most represented (45.2%). The average consultation delay was 8.34 years. All patients had dysphonia at the moment of the diagnostic and in 35.5% of cases, and it was associated with laryngeal dyspnea. Glottis localization was present in all our patients, i.e., 100% of the cases. A tracheotomy was performed in 9.67% of cases. All of our patients have had their papilloma peeled per endoscopic with tweezers. No cases of malignant degeneration were found in our study. Conclusion Laryngeal papillomatosis is the most common benign tumor of the larynx in both children and adults. Despite the progress of endoscopy and antiviral treatments, its treatment poses many problems in our undermedicalized countries.

Innovative endoscopic technologies in the diagnosis of laryngeal diseases in adults and children

Diseases diagnosis of the larynx in children and adults remains a difficult and urgent task of otorhinolaryngology. Modern diseases diagnostics of the larynx is based on endoscopic examination methods, which are the most informative and allow assessing the status of the larynx at a very advanced level. In 397 patients (329 adults and 68 children) with dysphonia, video rhinopharyngolaryngoscopy was performed using the PENTAX Medical VIVIDEO system. In addition, in 36 adults and 8 children, the status of the larynx was assessed by video laryngoscopy and video laryngostroboscopy using a rigid endoscope. Comparative analysis of video rhinopharyngolaryngoscopy and video endolaryngoscopy usage showed that the quality and size of the video image of the larynx in adults and children when using a video rhinopharyngolaryngoscope are comparable to the picture obtained using a rigid endoscope. The use of a video rhinopharyngolaryngoscope increases the efficiency of the examination, since the small diameter of the inserted working part allows carrying out the procedure as fast and comfortably as possible for the patient.

A Case of Multifocal Giant Rhabdomyoma in Head and Neck Region

Rhabdomyoma is a rare benign mesenchymal tumor originating from striated muscle. It is usually located in heart among children. In adulthood, it is reported most commonly between the ages of 25-40 years, especially in males, and located in extracardiac regions. It is frequently located in head and neck region, mainly on tongue, base of mouth, soft palate, pharynx and larynx in adults. Since rhabdomyomas are slow growing, painless masses, the clinical picture may be asymptomatic or may change depending on the affected region. Sixty eight years old male patient was admitted to our clinic with the complaints of progressive swelling on neck and swallowing difficulty accompanying unable to close mouth properly and speech defects of 10 years. Since the mass completely fulfilling mouth base, submandibular spaces and submental areas bilaterally was determined after the investigations, total mass excision was performed under general anesthesia. The clinical picture and treatment of the patient whose pathological diagnosis was rhabdomyoma is reported here with the review of the literature.

Benign tumors of larynx in adults are common and are often presents with hoarseness of voice or respiratory difficulty with stridor. Papilloma constitutes the major benign tumor of larynx in adults and usually presents with hoarseness of voice or stridor. A unique case of inflammatory myofibroblastic tumour of left aryepiglottic fold in a 19 year old female presenting with lateral neck swelling is presented. MRI revealed a polypoidal left aryepiglottic fold mass with caudal extension into the glottis and changing its position into the left PFS on e-phonation. The diagnosis of inflammatory myofibroblastic tumour remained elusive till it was proved histopathologically, after excision by laryngofissure approach. We present this rare case so that this condition and its surgical approach can be better recognized and understood.

PREVENTIVE MEASURES AGAINST RELAPSES OF PAPILLOMATOSIS OF LARYNX IN ADULTS

Papillomatosis of larynx is the most prevalent non-malignant neoplasm of larynx in adults. The main treatment method of papillomatosis of larynx remains an endolaryngial surgical operation with the application of microinstruments, electric coagulation and laser photodestruction. But an isolated surgical operation does not prevent a second rise of the disease, therefore the complex treatment should include the application of anti-recurrent therapy. We have been widely using cytostatics, interferons, interferon inductors and physiotherapy treatment during the post operation period on the basis of the Republican Scientific Research Centre of Otorhinolaryngology. The carried out research revealed, that the prescribtion of conservative therapy in the post operation period allows to prolong the interrecurrent period and decrease the probability of arising growth of papillomas.

Measurements of pre- and postpubertal human larynx: a cadaver study

This study investigates the differences in proportion and structure of the larynx in both children and adults, as well as changes which the larynx undergoes during puberty and adolescence. Furthermore, we have tried to answer the question on the existence of laryngeal sexual dismorphism in pre-pubertal (or pubertal) children. Larynges of 20 adults (10 men and 10 women) and 34 larynges of children not more than 12 years of age were acquired from the Forensic Medicine Department of the Medical University of Warsaw, Poland. Methods used are based on (1) anatomical preparation, (2) anatomical description and (3) measurements performed with the use of a digital camera and a computer-aided system MULTISCAN. Statistics, the Student's test and Pearson's linear correlation coefficient were calculated. Results show that proportions of the larynx depend on the age of the individual. The younger the individual is the more slender the larynx would be. However, proportion is not gender-related and hence is about the same in both males and females of any given age-group. Nevertheless, basic parameters were larger in males.

Squamous cell carcinoma of the oral cavity, pharynx, and larynx in young adults.

Thirty-nine adults (34 male, 5 female) age 40 or under with squamous cell carcinoma of the head and neck (excluding nasopharyngeal carcinoma) were seen at the Baylor College of Medicine affiliated hospitals from 1964 to 1983. The average age of the patients was 36.3 years and they were nearly uniformly heavy smokers and drinkers. Twenty-six of the 39 patients were black. Lesions of the oral cavity and oropharynx were most frequent, followed by laryngeal lesions. Four patients presented with second primary tumors, and one patient had three separate lesions. Of the 39 patients, only 8 had cancers small enough to be considered curable by radiotherapy or organ-sparing surgery. The remainder of the patients required radical surgery (26), were considered incurable at the time of presentation (8), or refused treatment (3). Also noted was a relatively high incidence of prior trauma (gunshot wounds, laparotomies for trauma). Of 30 patients available for follow-up more than one year after treatment, 19 have died of their tumors. Only one patient had a documented immune disorder (systemic lupus). In contrast to previous authors, we conclude that the development of squamous cell carcinoma at a young age can be related to heavy smoking and drinking and that the poor survival in many patients is due to self-neglect and failure to seek medical care early in the course of the disease.

Squamous papillomas of the larynx in adults. A review of 63 cases.

Sixty-three cases of squamous papillomas of the larynx occurring in adults are presented. Twelve cases started as juvenile papillomas and continued into adult life. Twice as many males as females were in the series. The papillomas were always present on the true vocal cords at presentation, although other parts of the larynx were involved to a variable extent. The symptom of hoarseness gave a good indication as to the state of the larynx during treatment. Malignancy did not develop in patients with papillomas which fulfilled the histological criteria. Papillomas are clusters of thin, cylindrical projections of squamous epithelial covered mucosa with second and even third order branching. Biopsies in 5 cases show papillomatosis of respiratory epithelium, a process which is here described in the larynx for the first time. The prognosis as regards recurrence for 20 patients with solitary lesions was good, for 38 patients with multiple lesions reasonable, but for 5 patients with extensive florid lesions was exceptionally poor.

Orientation in therapeutics of papilloma of larynx.

PAPILLOMA of the larynx in adults is a problem very difficult of solution. In the child it commonly disappears when puberty arrives. Several treatments, medical (as the use of calcined magnesia), physical, and surgical are recognized. Disappearance of papilloma at puberty makes us think of the possibility of a local action of estrogens. However, all therapeutic attempts using estrogens have failed. My colleagues and I also have had opportunity to apply this method, without satisfactory results. We commonly recommend vocal rest and the use of calcined magnesia when the patient remains under our control. Removal of the vilosities is done only to facilitate respiration. We do not agree with the use of radiotherapy because of its inefficacy, noticed on several occasions.
