Megaureter in adults: a review of the current literature.

Megaureter (MU) is an uncommon condition in adults. The aim is to present a review of the current literature. A literature search was conducted to explore the current literature including case reports on MU in adults in the period 2003-2023. Each case was characterised in terms of the eight categories: sex, age, etiology, location, symptoms, diagnostic tool used, type of treatment and follow-up. There was no meta-analysis but two reviews. Total, 39 case reports were included based on 35 articles. The most common observations for the first four categories were male (sex), age below 40 (age), obstructed MU (etiology) and left-sided MU (location). Almost every patient appeared with symptoms, most frequently flank pain, abdominal pain and recurrent urinary tract infections. CT scan and transabdominal ultrasonography are the most commonly used diagnostic tools. Most cases underwent either laparoscopic or open surgical treatment with ureteral reimplantation as the most performed surgical procedure. A majority of the patients had an uneventful follow-up. MU in adults is a rare condition presenting with heterogeneous symptoms. CT scan and transabdominal ultrasonography should be used as diagnostic tools. Nearly all patients underwent surgical treatment with an uneventful follow-up. Complications such as stone formation and altered function of the affected kidney are common. Surgical treatment is recommended and is determined by consideration of age, symptoms, ureteral extension and progressive loss of renal function.

Transvaginal radiofrequency ablation of a myoma in ten key steps.

To describe transvaginal radiofrequency ablation of myomas (TRAM) in ten keys steps. Video article STATEMENT OF CONSENT: The patient included in this video gave her consent for its publication online, including social media, the journal website, scientific literature websites (such as PubMed, ScienceDirect, Scopus, etc…) and other applicable sites. A 38-year-old woman presenting with menometrorrhagia and consequent chronic anemia (hemoglobin 8.5 g/dl) caused by a cervical myoma measuring 51 cm3. Initially, a cervico-vaginal embolization was performed but without any improvement in symptoms. Given the complex position of the myoma (close proximity to the uterine arteries), as well as the patient's desire for future pregnancies, a TRAM was proposed. Not applicable. Volume of myoma and menometrorrhagia 6 months following treatment. Not applicable CONCLUSION(S): TRAM is a quick, simple, minimally invasive and easily reproducible technique. It could be an interesting alternative to the conventional laparoscopic and open surgical treatments of myomas. There is lack of high-quality data establishing TRAM safety in infertility and pregnancy. The only available radiofrequency ablation device in the United States has specific language stating that this procedure is not recommended for women considering future pregnancy.

A GIANT HEPATIC HYDATID CYST: CASE REPORT

Hydatid cyst is a general health problem that can be seen in every country, with a higher prevalence in some certain areas. Liver is the most involved organ. The disease is usually asymptomatic. The cyst may enlarge and lead to compressive symptoms or complications through opening to THE biliary system. Although medical agents and surgical interventions have long been used in the treatment of hydatid cyst, minimally invasive percutaneous interventions and laparoscopic surgical interventions are also safely applied in selected patients. In this paper, we presented a case of a giant hepatic hydatid cyst for which we preferred open surgery.

The efficacy and safety of laparoscopic radical gastrectomy after neoadjuvant chemotherapy combined with immunotherapy and targeted therapy

Objectives: To explore the efficacy and safety of laparoscopic radical gastrectomy after neoadjuvant chemotherapy combined with immunotherapy and targeted therapy in patients with gastric cancer. Methods: A retrospective analysis of clinical and pathological data of 20 patients with locally advanced gastric cancer (clinical TNM stage T3-4aN+M0) admitted to the Cancer Hospital, Chinese Academy of Medical Sciences from July 2021 to July 2023. All patients received 3 cycles of SOX (Oxaliplatin+S-1) regimen combined with immunotherapy (Trastuzumab) and targeted therapy (Apatinib) as neoadjuvant treatment followed by laparoscopic radical gastrectomy for gastric cancer. Surgical outcomes, postoperative pathological response, and postoperative recovery were observed. Quantitative data, except for age and operation time, were expressed using Median (range). Results: Among the 20 patients, there were 18 males and 2 females, aged 41 to 73 years [(60.6±9.7) years]. All 20 patients underwent laparoscopic surgical treatment after neoadjuvant therapy, with one patient undergoing laparoscopic conversion to open total gastrectomy with partial transverse colon resection due to tumor invasion into the transverse mesocolon. Eight patients underwent totally laparoscopic radical gastrectomy, all with Billroth Ⅱ+Braun anastomosis at the distal stomach. Eleven patients underwent laparoscopic-assisted radical gastrectomy, among which total gastrectomy with Roux-en-Y anastomosis was performed in ten cases, and proximal gastrectomy with esophagogastrostomy overlap anastomosis was performed in one case. The mean operation time for the 20 patients was (165.0±34.1) minutes; intraoperative blood loss was 80 (20-100) ml; and the number of lymph nodes retrieved was 68 (21-89). Postoperative pathological TNM staging revealed stage T0N0M0 in six cases, stage Ⅰ in two cases, stage Ⅱ in three cases, and stage Ⅲ in nine cases. Six patients (30.0%) achieved pathological complete response, and nine patients (45.0%) achieved significant pathological response. The median postoperative time to flatus was 4 (1-5) days; oral intake resumed after 3 (2-5) days; and the median length of hospital stay was 13 (6-19) days. One patient developed colonic anastomotic leakage with intra-abdominal infection, and one patient developed duodenal stump leakage with intra-abdominal infection, both classified as Clavien-Dindo grade 3A complications, and improved after treatment and discharged. One patient developed gastric paresis, and two patients developed pleural effusion, classified as Clavien-Dindo grade 2 complications, and improved after treatment and discharged. There were no deaths within 30 days after discharge. Conclusions: Laparoscopic radical gastrectomy for gastric cancer after neoadjuvant treatment with the SOX regimen combined with immunotherapy and targeted therapy is safe and feasible, with satisfactory short-term efficacy. However, there is an increase in overall surgical risk and difficulty, and it is recommended to be performed in experienced gastric cancer centers.

Comparative evaluation of robot-assisted, laparoscopic and open surgical interventions in the combined treatment of stomach cancer

Introduction. Gastric cancer ranks fifth in incidence and fourth in mortality among all oncological diseases worldwide. According to recent clinical data, for stage cT2N0 and beyond, a combined approach using various schemes and regimens of anticancer drug therapy is recommended. Currently, indications for the use of minimally invasive techniques in the surgical treatment of locally advanced gastric cancer are not defined. Research objective. Comparative assessment of the immediate outcomes of robot-assisted, laparoscopic, and open surgical interventions in patients with gastric cancer of different localizations who underwent neoadjuvant polychemotherapy. Materials and methods. The study is based on a retrospective analysis of 81 medical histories of patients with locally advanced gastric cancer of different localizations operated on from 2018 to 2022 in the Department of Abdominal Oncology of City Hospital №40 in St. Petersburg. All patients underwent preoperative cycles of anticancer drug treatment within perioperative polychemotherapy schemes: FLOT – 64 (79%) patients, mFOLFOX6 – 9 (11.1%) patients, and FLOT+Тrastuzumab – 8 (9.9%) patients. Laparoscopic, traditional open, and robot-assisted surgical interventions were performed using the da Vinci surgical system. Results. Thirty-seven (45.7%) robot-assisted operations were performed, (22/27.2% – distal subtotal resections of the stomach (DSRS), 15/18.5% – gastrectomies (GE), 19 (23.5%) laparoscopic (11/13.6% – LDSRS, 8/9.9% – LGE), and 25 (30.9%) operations via laparotomy (9/11.1% – DSRS, 16/19.7% – GE). All patients underwent D2 and D2+ lymphadenectomy. Statistically significant differences were noted in groups for parameters such as operative intervention time, intraoperative blood loss, and length of hospital stay. Open surgical intervention group had significantly higher blood loss volume – median 275 (90.00; 300) ml (p<0.001) and postoperative hospital stay duration – median 10.00 (9.00–14.00) days (p=0.017). Operation time was significantly shorter compared to minimally invasive interventions – median 150.00 (97.50–210.00) minutes for distal resections and 150.00 (96.25–207.50) minutes for GE (p<0.005). There were no statistically significant differences in complication rates (p>0.05) and the number of removed lymph nodes (p=0.334). However, the percentage of early postoperative complications, including lethal outcomes, was higher in the open intervention group (n=4/16%) compared to laparoscopic (n=2/10.5%) and robot-assisted (n=2/5.4%). The overall frequency of postoperative complications IIIA–V according to the Clavien-Dindo classification was 9.9%. One (1.2%) case had a lethal outcome after open gastrectomy. Histological examination showed no tumor growth in the resection margins (R0) for all patients. Complete regression of the gastric tumor (CAP-0) was noted in one case (1.23%) after combined treatment. Nearly complete response to therapy was observed in 13 cases (16%) (CAP-1), partial response in 28 cases (34.6%) (CAP-2), and no response to neoadjuvant therapy in 39 cases (48.1%) (CAP-3). Conclusions. Minimally invasive surgical interventions in the combined treatment of locally advanced gastric cancer are feasible, safe, and oncologically justified. Robot-assisted surgery demonstrates advantages over open and laparoscopic surgery in parameters such as intraoperative blood loss, number of removed lymph nodes, activation and hospitalization periods.

Nutcracker syndrome in childhood

Nutcracker phenomenon (NP) is defined as the compression of the left renal vein, often occurring between the aorta and the superior mesenteric artery (SMA). Patients with symptoms associated with the Nutcracker anatomy are called “Nutcracker syndrome” (NCS). Renal vein compression results in venous congestion, outlet obstruction, and increased pressure in the left renal vein. The clinical manifestations of NCS in children vary widely depending on the severity of compression. It can range from being asymptomatic to presenting with intermittent or persistent micro or macrohematuria, orthostatic proteinuria, renovascular hypertension, abdominal pain, left-sided flank pain, dysmenorrhea, pain in the testicles or scrotum, and left varicocele. Hematuria, proteinuria, and flank pain are prevalent symptoms. The anatomical and physiological degree of compression of the left renal vein can be diagnosed through Doppler ultrasound (DUS), computer tomography (CT) scan, or magnetic resonance imaging (MRI). In cases with mild symptoms, conservative treatment is an appropriate option, and ACE inhibitors can be used for patients with proteinuria. In more severe cases where conservative approaches and medical treatment fail to yield satisfactory results, endovascular, laparoscopic, or open surgical interventions are employed.

Laparoscopic versus laparotomic surgical treatment in apparent stage I ovarian cancer: a multi-center retrospective cohort study

BackgroundLaparoscopic treatment shows non-inferior survival outcomes and better surgical outcomes in apparent stage I ovarian cancer (OC) in some studies but has not been well defined.MethodsWe conducted a retrospective study of patients with apparent stage I OC treated in two hospitals between 2012 and 2022. The surgical and oncologic outcomes were evaluated between patients receiving laparoscopic and laparotomic surgery.ResultsWe identified 37 patients with apparent stage I OC, including 15 (40.5%) serous carcinomas, 9 (24.3%) mucinous cancers, 3 (8.1%) endometroid cancers, 2 clear cell carcinomas, and 8 (21.6%) non-epithelial cancers. Sixteen patients received laparoscopic surgery and the other 21 patients underwent laparotomic surgery. The median age (44.5 vs. 49.0 years), mean mass size (10.5 vs. 11.3 cm), and median follow-up time (43.5 vs. 75.0 months) showed no statistically significant differences between patients in laparoscopic and laparotomic groups (all P > 0.05). All the patients underwent comprehensive surgical staging surgery, and the mean surgical time (213.5 vs. 203.3 min, P = 0.507), number of lymph nodes sampling (18.6 vs. 17.5, P = 0.359), proportion of upstaging (12.5% vs. 19.0%, P = 0.680), and postoperative complications (no Accordion Severity Grading System grade ≥ 3) were comparable between two surgical groups. Moreover, patients in the laparoscopic group had significantly less intraoperative blood loss (231.3 vs. 352.4 mL, P = 0.018), shorter interval between surgery and postoperative adjuvant chemotherapy (7.4 vs. 9.5 days, P = 0.004), shorter length of hospital stay (9.9 vs. 13.8 days, P < 0.001) than those treated with laparotomic surgery. During a median follow-up of 54.0 months, 9 (24.3%) relapsed and 1 (2.7%) died, with a 5-year recurrence-free survival (RFS) and disease-specific survival (DSS) rate of 70.6% and 100%, respectively. However, the 5-year RFS (93.3% vs. 58.8%, P = 0.084) and DSS (100% vs. 100%, P = 0.637) rates did not significantly differ between the two groups.ConclusionLaparoscopic surgical treatment had less intraoperative blood loss, earlier postoperative adjuvant chemotherapy administration, shorter hospitalization time, and non-inferior survival outcomes in apparent stage I OC when compared with laparotomic surgery.

Ultrasonographic characterization of parametrial endometriosis: a prospective study

ObjectiveTo study the ultrasonographic diagnostic accuracy and characteristics of parametrial endometriosis comprehensively. DesignThis prospective study enrolled patients with suspected deep endometriosis scheduled for laparoscopic surgical treatment. Preoperative ultrasonographic examinations were performed following the International Deep Endometriosis Analysis criteria. The study aimed to evaluate the presence of parametrial endometriosis and its ultrasonographic characteristics, using surgical diagnosis as the reference standard. Additionally, indirect signs of deep endometriosis and concomitant deep endometriosis nodules associated with parametrial involvement were identified, assessing their predictive significance in anterior, lateral, and posterior parametrial areas. SubjectsPatients with suspected deep endometriosis scheduled for surgical treatment. Intervention/ExposureStandardized preoperative ultrasonographic examination. Main Outcome MeasuresThe diagnostic accuracy of transvaginal ultrasound in identifying parametrial endometriosis, including sensitivity and specificity. Ultrasonographic characteristics of parametrial nodules, prevalence in distinct parametrial areas, and associations with indirect deep endometriosis signs and concomitant nodules. ResultsSurgical confirmation of parametrial nodules was observed in 105 out of 545 patients (18.5% left, 17.0% right). Transvaginal ultrasound demonstrated a sensitivity of 77.1% (95% CI 68.0–84.8%) and specificity of 99.1% (95% CI 67.7–99.8%). Parametrial nodules typically exhibited characteristics such as a mild hypoechoic appearance (83.6%), starry morphology (74.7%), irregular margins (70.2%), and low vascularization. The posterior parametrium was the most common location (52.2%), followed by the lateral (41.0%) and anterior parametrium (6.8%). Concomitant deep nodules in the rectum (63.5%) and infiltrating the rectovaginal septum (56.5%) were significantly more prevalent in patients with parametrial involvement (both p<0.001). Indirect signs of deep endometriosis, such as ovaries fixed to the uterine wall (71.8%) and the absence of a posterior sliding sign (51.8%), were also more common in women with parametrial DE nodules (both p<0.001). Hydronephrosis, though relatively uncommon in patients with parametrial involvement (8.2%), was largely detected in lateral parametrial nodules (70.0%). ConclusionThis study represents a systematic ultrasonographic characterization of parametrial endometriosis, comprehensively assessing the diagnostic accuracy of transvaginal ultrasound in identifying parametrial involvement within a sizable cohort of patients with preoperative suspicion of deep endometriosis.

Curative surgical treatment of common bile duct stones: Retrospective cohort study.

In the event of symptomatic common bile duct (CBD) stones with dilated CBD, one possible curative treatment option is stone extraction through choledocotomy associated with cholecystectomy. Endoscopic treatment is only reserved for residual stones at 6weeks. The aim of this study was to evaluate the results from laparoscopic curative surgical treatment of CBD stones with dilated CBD. This is a retrospective single-centered cohort study. All consecutive patients admitted for laparoscopic cholecystectomy with evidence of CBD stones with dilated CBD from January 2010 to December 2020 at our center were included. Success was defined by CBD clearance at 6weeks. Need for additional procedures, such as endoscopic sphincterotomy, immediate, and end-of-procedure morbi-mortality as well as factors associated with procedure failure, were also studied. A total of 246 patients who received curative treatment were included in the study. The success rate for the curative treatment was 93.1% (229 patients). Immediate postoperative morbidity was 24.4% with a 5.3% reintervention rate. Immediate and 6-week postoperative mortality rates were zero and 0.4%, respectively. The mean length of stay was 11.3days. Factors associated with procedure failure appeared to be the occurrence of an early postoperative complication and the need for readmission during the period between surgery and drain removal. This study indicates that laparoscopic curative surgical treatment for symptomatic CBD stones may be performed with acceptable results without routine need for additional procedures.

Evaluation of the results of laparoscopic surgical treatment for acute appendicitis in elderly patients

Objectives: The aim of this study was to evaluate the clinical and paraclinical characteristics and results of treatment of acute appendicitis in elderly patients. Methods: A prospective study was conducted on 189 patients, including 42 of them over the age of 60, diagnosed with acute appendicitis or peritonitis due to appendicitis based on clinical and paraclinical symptoms and treated with laparoscopic surgery from April 2022 to June 2023 at the Department of Digestive Surgery, Hue University of Medicine and Pharmacy Hospital. Results: Elderly patients with acute appendicitis admitted to the hospital had an average age of 69.3 years. The time from symptom onset to hospital admission in elderly patients was 30.1 hours compared to 19.1 hours in the non-elderly group. The elderly group had a rate of peritonitis of 40,5% compared to 13.6% in the non-elderly group (p &lt; 0.001). The mean time for passing initial gas in elderly patients was 18,1 hours compared to 13.3 hours in the non-elderly group (p = 0.004). Postoperative hospital stay in elderly patients was 4.7 days compared to 3.5 days in the non-elderly group (p &lt; 0.001). Postoperative complications in the elderly patient group were 2.4% compared to 0% in the non-elderly group (p = 0.222). There were no complications during surgery or 30-day mortality. Conclusions: Elderly patients with acute appendicitis were hospitalized later, had a higher rate of peritonitis and had a longer postoperative hospital stay than non-elderly patients. Laparoscopic surgery to treat acute appendicitis in the elderly is a safe method. Key words: acute appendicitis, elderly, laparoscopic appendectomy, appendicitis peritonitis.

Despite the achievements of modern pediatric surgery, to date there is no single comprehensive approach to the treatment and prevention of adhesive intestinal obstruction (AIO), both primary (prevention of adhesions formation after primary surgery) and secondary (prevention of recurrence). Purpose - to study the clinical effectiveness of treatment, primary and secondary prevention of postoperative intestinal adhesive obstruction in children. Materials and methods. An examination and analysis of medical records of 119 children aged 5 months to 17 years who were treated at the Pediatric Surgery Clinic of Ivano-Frankivsk National Medical University over the past 10 years for post-operative AIO was carried out. In 35 (29.41%) children a positive effect was achieved from conservative treatment. The remaining 84 (70.59%) children were operated on. Two groups of patients were formed: the comparison group - 38 (45.24%) children, who received traditional treatment, and the main group - 46 (54.76%) children, who were intraoperatively applied anti-adhesive gel to prevent recurrence of postoperative AIO. Results. It was established that postoperative AIO occurred in only 1 (1.19%) child after laparoscopic intervention, in the rest of the children primary surgical interventions were performed by an open method. In the comparative assessment of the postoperative course in two groups of patients, we drew attention to a more favorable course in the children of the main group, which was manifested by a faster recovery of peristalsis, a decrease in the duration of gastric stasis, a faster appearance of independent defecation, and a shorter period of inpatient treatment. When comparing the results of treatment between groups of patients, it was established that the frequency of recurrences of AIO in the main group of patients was 5.44 times lower than in children of the comparison group, and the frequency of relaparotomy for recurrent AIO was 7.28 times lower than in patients of comparison group. We did not observe complications associated with the use of anti-adhesive gel. We did not notice a significant difference in the results of treatment between patients who were treated with anti-adhesive gel of different composition (based on carboxymethylcellulose and sodium hyaluronate). Conclusions. The most effective method of primary prevention of postoperative adhesive intestinal obstruction is the use of laparoscopic primary surgical interventions. Intraoperative application of anti-adhesive gel is a highly effective and safe recurrence prevention method of adhesive intestinal obstruction in children. The research was carried out in accordance with the principles of the Helsinki Declaration. The study protocol was approved by the Local Ethics Committee of the participating institution. The informed consent of the patient was obtained for conducting the studies. No conflict of interests was declared by the authors.

Shwannoma of the stomach and synchronous cancer of the transverse colon: a clinical case report

With an increase in the frequency of publication of clinical observations with synchronous neoplasms of different histological types, experience is accumulating in the diagnosis and treatment of this cohort of patients. new clinical guidelines and strategies for the treatment of patients with primary multiple neoplasms of the gastrointestinal tract are being developed. A significant number of identified cases of the development of minimally invasive laparoscopic and robotic surgical interventions for primary multiple neoplasms of the gastrointestinal tract, as well as an increasing number of studies demonstrating the integrity and safety of performing minimally invasive endoscopic intraluminal aids in small mesenchymal tumors. The clinical observation presents the data of patient 82 years old with benign schwannoma and adenocarcinoma of the transverse colon.

Laparoscopic surgical treatment for sliding hiatal hernia is associated with an owner-perceived improvement in clinical outcome in dogs

Abstract OBJECTIVE To report perioperative complications and client-perceived outcome following laparoscopic surgical treatment for sliding hiatal hernia (SHH) in dogs. ANIMALS Client-owned dogs (n = 9). METHODS Medical records were reviewed and perioperative data collected including preoperative diagnostic imaging, operative details, complications, and need for conversion to open celiotomy. A single-incision, multicannulated port was inserted in the subumbilical region followed by placement of an additional 2 or 3 instrument portals. Hiatal plication, esophagopexy, and left-sided gastropexy were performed laparoscopically. Follow-up information was collected with telephone interview with the owners and/or referring veterinarian. A standardized questionnaire was completed by dog owners postoperatively. RESULTS Intraoperative pneumothorax occurred in 5 of 9 (55.6%) dogs and resulted in conversion to open celiotomy in 2 (22.2%) dogs. In 4 dogs, pneumothorax was suspected to be the result of progressive leakage of capnoperitoneum through the suture bite holes of the esophageal hiatal plication sutures. Hiatal plication was performed using intracorporeal simple interrupted sutures (n = 4) or a simple continuous pattern with barbed suture (4). Esophagopexy was performed using barbed suture in all dogs. Gastropexy was performed using a total laparoscopic technique (n = 4) or laparoscopic-assisted technique (3). Using a standardized questionnaire, dog owners perceived a postoperative improvement in regurgitation after eating and regurgitation after excitement/increased activity. CLINICAL RELEVANCE Laparoscopic treatment of SHH resulted in owner-perceived improvement in clinical signs. Intraoperative pneumothorax occurred in a high proportion of dogs but did not result in long-term sequelae.

INTRODUCTION: Despite the significant advances in the prevention and treatment of venous thromboembolic complications (VTEC), pulmonary embolism (PE) remains the most common preventable cause of hospital postoperative mortality. One of the causes of thrombosis is surgical operations, especially laparoscopic, associated with the activation of the hemostasis system. Aim: The choice of a prevention method of thrombotic complications in comorbid patients with signs of varicose veins of the lower extremities requiring laparoscopic surgical treatment of benign gastric diseases (BGD). MATERIALS AND METHODS: The study is based on a treatment analysis of 128 patients with BGD in combination with a high comorbid background. The treatment of these patients was divided into several stages, which included both vascular interventions for the prevention of VTEC and a laparoscopic stage aimed at treating the underlying disease. The patients were divided into 3 groups comparable in gender and age. The first group, consisting of 45 people, VTEC prevention was carried out using a standard conservative technique (anticoagulant therapy in combination with mechanical prophylaxis). The second group included 43 patients who underwent both standard conservative therapy and open surgery aimed at correcting venous insufficiency in the great saphenous vein system. The third group is represented by 40 patients who, in addition to the standard conservative prevention of VTEC, underwent a combined surgical treatment (crossectomy and endovenous laser coagulation (EVLC) of the trunk of the great saphenous vein by retrograde method). All patients underwent coagulogram analysis and ultrasound triplex scanning of the veins of the lower extremities at each stage of the treatment. RESULTS: In the first group of patients, the largest number of thrombotic complications was noted — 24.4%, in the second group their number was 14%, while in the third group, thrombotic complications were detected in only 7.5% of patients. In the second and third groups, there was no development of PE, both after the first operation and after the main laparoscopic stage of treatment. In the first group, on the contrary, 2 episodes of PE (4.4%) were noted after the main surgical treatment. In turn, the standard anticoagulant therapy carried out in all three groups led to the development of both small and large hemorrhagic complications. At the same time, it should be noted that their frequency was comparable in all three groups, and their number did not exceed the number of such complications based on literature data. CONCLUSION: Carrying out active operative prevention of thrombotic complications significantly reduces their frequency in comorbid patients with signs of varicose veins of the lower extremities, thereby allowing to improve the result of the treatment. Considering that laparoscopic surgical interventions on the stomach increase the risk of developing VTEC in comorbid patients, according to our data, performing EVLC in combination with crossectomy, as the first stage of prophylaxis, prevents the development of PE and reduces the risk of venous thrombotic complications by 16.9%.

Migration of foreign bodies of firearms origin

Objective. To analyse the results of diagnosis and treatment of wounded with gunshot combat trauma, in which foreign body migration was recorded, and to determine their optimal algorithm.&#x0D; Materials and methods. The data of anamnesis, objective clinical and general clinical and laboratory studies, as well as the results of instrumental diagnostics (radiological, endoscopic, ultrasound examinations) of 67 wounded with blind gunshot wounds of various localisations were analysed. Thoracic, laparoscopic and arthroscopic surgical interventions were performed on a video endoscopic stand, and surgical magnetic instruments were used to diagnose and remove ferromagnetic foreign bodies.&#x0D; Results. The main directions of foreign body migration are the respiratory tract, gastrointestinal tract, vascular bed, cavities (pleural, abdominal, joints) and soft tissues in case of suppuration. Foreign body fixation occurs in the place of narrowing of an artery or vein, distal parts of the respiratory tract, gentle parts of the pleural or abdominal cavity, in organs with structural features (heart, intestine, joint).&#x0D; Conclusions. Although the migration of foreign bodies of gunshot origin is rare, it requires appropriate attention and response.&#x0D; In case of gunshot penetrating wounds of cavities (thoracic, abdominal, large joints), to which foreign bodies may migrate, preference should be given to minimally invasive endoscopic methods of their removal using modern magnetic instruments.

Organ-sparing resection techniques in the treatment of liver echinococcosis

This study aims to present and evaluate the results of treating surgery patients using organsparing resection interventions for liver echinococcosis.Material and methods. The study presents the results of treating 103 patients (51/49.6 % males, 52/50.6 % females) who have undergone total radical pericystectomy/atypical liver resection for liver echinococcosis. Patients were divided into two groups: Group 1, with interventions performed through laparotomic access (n = 84), and Group 2, with interventions using laparoscopic techniques (n = 19).Results. In the abdominal intervention group, there was no statistically significant advantage in the rate of parenchyma dissection between different methods (p &gt; 0.05). The average time of haemostasis without the Tachocomb haemostatic substance applied showed a lower value (p = 0.17). The average blood loss during the Pringle manoeuvre was significantly lower (p = 0.043).There was no statistically significant advantage in the laparoscopic intervention group as regards the rate of parenchyma dissection (p = 0.74). The average haemostasis time was significantly shorter in patients using the Floseal haemostatic matrix than those treated otherwise (p = 0.001). In patients with the improved laparoscopic Pringle manoeuvre, blood loss was significantly less than in those who had not undergone the technique (p = 0.00008).No statistically significant differences were observed (p = 0.76) when comparing the percentages of nonspecific complications in the two groups. The average value of postoperative bed-day was significantly lower in the laparoscopic surgical treatment group (p = 0.00001).Conclusion. Using the Pringle maneuver when performing abdominal and laparoscopic interventions statistically proved its effectiveness. The use of the Floseal haemostatic matrix during laparoscopic operations allowed a statistically significant reduction in the time of haemostasis and intraoperative bleeding. The duration of the operation and the time of dissection of the parenchyma in the laparoscopic surgery were significantly higher (p &lt; 0.05). The immediate results of the laparoscopic and abdominal interventions proved comparable.

Laparoscopic Treatment of Morgagni-Larrey Diaphragmatic Hernia with Reposition of the Transverse Colon in Patients with Cardiac Comorbidity. Clinical Observation

Morgagni-Larrey hernia is a rare form of diaphragmatic hernia (2-5% of cases) usually diagnosed in children; in adults, it is often detected in emergency cases or accidentally, during examinations or during surgery. The symptoms of the disease are non-specific, so the diagnosis is made using imaging methods (computed tomography is the gold standard). Currently, there is no single method of treating Morgagni-Larrey hernias. Closure of the defect is performed with or without the use of various mesh prostheses through the abdominal or thoracic route; open or minimally invasive access.&#x0D; The aim. To conduct a retrospective analysis of the treatment of adult patients with a Morgagni-Larrey diaphragmatic hernia, including those with cardiovascular comorbidity, with a comparison of symptoms, contents of the hernial sac, with the use of laparoscopic techniques and composite mesh prostheses.&#x0D; Materials and methods. The results of the treatment of three patients with a Morgagni-Larrey hernia in the period from 2020 to 2022 at the Shalimov National Institute of Surgery and Transplantation are shown. In two patients, the content of the hernial sac was a loop of the transverse colon, in one, the content of the hernial sac was a greater omentum. Taking into account the accompanying cardiovascular pathology, an interdisciplinary council consisting of a surgeon, anesthesiologist, and cardiologist recommended laparoscopic surgical intervention. Laparoscopic allogeneoplasty with composite prosthesis implantation was performed in three patients.&#x0D; Conclusions. The laparoscopic method of treating diaphragmatic hernias is safe and effective. The use of composite prostheses in combination with tension closure of the defect allows to reduce the risk of recurrence of the disease in the distant postoperative period.

#4691 LAPAROSCOPIC NEPHRECTOMY IN PATIENTS WITH AUTOSOMAL DOMINANT POLYCYSTIC KIDNEY DISEASE

Abstract Background and Aims At the present, discussions continue on the indications and timing of performing kidney nephrectomy in patients with autosomal dominant polycystic kidney (ADPK) disease who are on the waiting list for kidney transplantation. Large sizes of kidneys, traumatic access, accompanying these operations cause a high incidence of postoperative complications, mortality and aggravate the severity of patients. The aim of the presented study was to find the preferred algorithm for preparing patients for kidney transplantation. Method The results of the nephrectomy of polycystic kidneys were analyzed in 28 patients (mean age 49±1.5 years, 16 men, 12 women), which were conditionally divided into two groups. The first group included 13 patients (46.4%) with open surgery using median laparotomy and lumbotomy, in the second group - 15 (53.6%) - laparoscopic nephrectomy. Surgical interventions for patients in both groups receiving renal replacement therapy dialysis, performed according to emergency and planned indications. Results The average duration of laparoscopic and open surgical interventions was 149±14 and 132±15 min, respectively. The maximum size of deleted polycystic-altered kidneys in the first group was 21.3±4.21 cm, in the second group it was 20.5±3.3 cm. The incidence of postoperative complications in the 1st and 2nd groups was 6 cases (46.1%) and 2 cases (13.3%), respectively. There was one fatal outcome (7.69%) in the 1st group as a result of septic complications. the average postoperative bed-day in the first group was 12-13 (12.7 ± 1.1), in the second - 8-9 (8.6±0.5). Patients after laparoscopic interventions are activated on the 2nd-3rd day (2.8±0.14), after open operations - on the 4th-5th (4.43±0.32). Conclusion The incidence of postoperative complications after laparoscopic nephrectomy in patients with ADPK does not exceed 13.3%. The use of laparoscopic technologies makes it possible to expand the possibilities of using nephrectomy for the treatment and preparation of patients with polycystic kidney transplantation.

Laparoscopic surgical technique for left intrahepatic cholangiocarcinoma with lymph node metastasis after conversion therapy (with video).

Intrahepatic cholangiocarcinoma (ICC) with lymph node metastasis has a poor clinical prognosis. Comprehensive surgical treatment based on surgery is critical for improving the prognosis. Conversion therapy provides an opportunity for radical surgery in such patients but also increases the difficulty of surgery. The technical barrier to laparoscopic lymph node dissection is determining the extent of regional lymph node dissection after conversion therapy and formulating a suitable procedure to ensure the quality of lymph node dissection and oncological safety. One patient with initially unresectable left ICC underwent successful conversion therapy at another hospital. Then, we performed laparoscopic left hemihepatectomy with middle hepatic vein resection and regional lymph node dissection. Specific surgical techniques are used to reduce injury and bleeding, ultimately reducing the incidence of complications and promoting rapid recovery of patients. No postoperative complications were noted. The patient recovered well; no tumor recurrence was observed during the follow-up. Preoperatively planned regional lymph node dissection provides a reference for exploring the standard laparoscopic surgical treatment of ICC. Procedural regional lymph node dissection and artery protection techniques ensure quality and oncological safety in lymph node dissection. When selecting appropriate cases, as long as the laparoscopic surgical technique is mastered, laparoscopic surgery is safe and feasible with faster postoperative recovery and less trauma for left ICC.
