Juvenile breast hypertrophy: report of two cases presenting with massive asymmetrical breast enlargement and palpable breast masses

Background: Juvenile breast hypertrophy is a rare benign condition leading to rapid and massive enlargement of one or both breasts in young girls during peri-pubertal period. Patients encounter physical and psycho-social problems. Therefore, the condition warrants early appropriate management. Breast imaging studies (ultrasound and magnetic resonance imaging) are particularly important in order to exclude tumors and occult pathologies. First line treatment modality is reduction mammoplasty that significantly improves quality of life with lesser chances of recurrence. Case Presentation: We report two post-menarchal girls with juvenile breast hypertrophy who presented as different clinical cases facing psychological and social embarrassment. Breast ultrasound showed right breast mass in both patients that were biopsied. The lesions turned out to be fibroadenomas on histopathological correlation. Patients were then referred to plastic surgery and endocrinology department. Conclusion: Juvenile breast hypertrophy is definitely a disturbing cosmetic problem. Early investigations and appropriate management will surely leave a positive impact on the quality of life.

Giant Breast Hypertrophy in Juvenile: A Rare Case

Introduction: Juvenile hypertrophy of the breast (JHB) is a benign condition that can lead to gigantomastia in adolescence. The development of breast enlargement in adolescence can cause both physical and psychosocial problems. Therefore, appropriate investigation and proper management at an early stage of the disease are very important. One of the treatment modalities is breast reduction surgery which ideal and offers an improvement in the quality of life. Case Presentation: A 13-year-old female with massive bilateral breast enlargement for a period of 8 months. The patient also complained of severe back pain and neck discomfort since her breast progressively enlarged. Psychosocial problems were recorded. Right breast reduction was performed and closed with inverted-T mammoplasty. The post-operative period was uneventful, and the patient was discharged on day 5 after the operation. Conclusions: Reduction mammoplasty is well accepted by adolescents with juvenile breast hypertrophy and can improve physical and psychological outcomes.

Body size measurements, digit ratio (2D:4D) and oestrogen and progesterone receptors' expressions in juvenile gigantomastia.

Background Juvenile breast hypertrophy is characterised by massive enlargement of the breast in the peri-pubertal period. We aimed to analyse body size measurements (body mass index [BMI], waist-to-hip circumference ratio [WHR]), digit ratio (ratio of II and IV digits' length [2D:4D]) and oestrogen receptor (ER) alpha (ERα) and progesterone receptors (PRs) in the breast gland in women with juvenile gigantomastia. Methods The study involved 30 women (mean age 25.7 years) (mean age of onset - 14.8 years). ERα and PR expressions were detected immunohistochemically in breast gland samples. For comparison, 100 controls (50 women and 50 men) were included. Results BMI and WHR in women with gigantomastia were higher than in control women and the former had a higher WHR than expected for their BMI. 2D:4D in the examined women did not differ from that in control women. However, left 2D:4D was negatively related to the age of gigantomastia onset. There were no correlations between ER and PR expressions and the analysed body and digit ratios. Conclusions The lack of a relationship between 2D:4D and juvenile breast hypertrophy may suggest that foetal exposure to sex hormones may not be crucial in its aetiology. However, the link between high left 2D:4D and early development of gigantomastia suggests that prenatal sex hormones have a role in its development timing. High WHR, and particularly high WHR relative to BMI, may indicate that these women had at some stage of development higher circulating androgens, which may have been converted to oestrogens in breasts due to local aromatase activity. Verification of this hypothesis could allow consideration of the role of aromatase inhibitors in juvenile breast hypertrophy.

Combining Breast Reduction Techniques to Treat Gigantomastia in Ghana

Summary:In this presentation of 2 consecutive cases of symptomatic juvenile breast hypertrophy in Ghana, we review the patient presentation, workup, and discuss outcomes following a combined technique of inferior pedicle stump with free nipple graft reduction mammoplasty. Surgical goals for treatment of gigantomastia were 2-fold: to resect adequate tissue to obtain symptomatic relief with improved quality of life, while avoiding a flat, boxy-appearing breast shape.

Juvenile Breast Hypertrophy: A Successful Breast Reduction of 14.9% Body Weight without Recurrence in a 5-Year Follow-Up

Juvenile hypertrophy of the breast (JHB) is a rare and relentless disease affecting women in the peripubertal period. We present a 13-year-old girl with massive bilateral JHB, successfully treated with a breast reduction and free nipple graft technique. A total of 7300 grams of breast tissue had been removed, accounting for 14.9% of the patient's total body weight. Prophylactic hormonal therapy was not commenced. During the 5-year follow-up period, there was no recurrence and the patient remains satisfied with the aesthetic outcome. A recent meta-analysis study indicates that subcutaneous mastectomy is associated with reduced risk of recurrence, but it is more deforming and the aesthetic result is inferior to a reduction mammaplasty. In patients treated with the latter technique, some evidence exists suggesting that the use of a free nipple graft is associated with a less frequent risk of recurrence than a pedicle technique. This present case is unique as it demonstrates the clinical course of this patient at a considerably longer follow-up period than most reported studies. We adhered to the limited available evidence and highlight the long-term reliability of breast reduction with free nipple grafting as the first line surgical option in JHB, eliminating the need for repeated surgeries.

Unilateral Juvenile Hypertrophy of Breast

Juvenile hypertrophy of the breast (JHB) is an uncommon, benign disorder and typically starts at the adolescence in females. It is characterized by rapid enlargement of breasts, either unilateral or bilateral. The definitive diagnosis is made by histopathologic examination. Treatment recommendations include surgery and hormonal therapy, although hormonal manipulation is still controversial in pediatric patients. Here we report an adult female 35-year-old with right sided JHB who did not undergo surgery or medical treatment.

Hyperplastic breast anomalies in the female adolescent breast.

Macromastia in adolescents is multifactorial and usually idiopathic, associated with obesity or hormonal imbalances. Less commonly, it can result from virginal or juvenile breast hypertrophy, a rare condition of unknown etiology, where an alarmingly rapid breast enlargement occurs during puberty. Breast hypertrophy in the adolescent population can have significant long-term medical and psychological impacts. Although symptoms can be severe, many plastic surgeons, pediatricians, and parents are often reluctant to surgically treat adolescent macromastia. However, reduction mammoplasty is a safe and effective treatment and may be the only way to alleviate the increased social, psychological, and physical strain caused by macromastia in adolescents.

Tamoxifen as First-line Treatment in a Premenarchal Girl with Juvenile Breast Hypertrophy

Background Juvenile breast hypertrophy is uncommon and is characterized by excessive breast enlargement in the peripubertal period. The clinical entity is thought to result from increased sensitivity of mammary tissue to normal levels of circulating hormones. Case Here, we report a female patient, aged 12 years and 6 months, suffering from juvenile breast hypertrophy, who presented at the third month of symptoms and benefited from tamoxifen treatment. Comments In experienced clinics, use of tamoxifen in the treatment of juvenile breast hypertrophy during the brisk growth period may become a medical alternative to reconstructive surgery.

Sculpturing of the Male Breast

During the past decade, more esthetic surgery has been performed on male patients compared with the past. The most common request has been for improvements in the chest, ‘love handles‘ and abdomen. In the youthful male, the musculature, breast tissue and nipple areolar complex are quite tight. As age progresses, laxity of the tissue, skin and musculature becomes a problem. Management of contouring of the male breast depends on the size and shape of the breasts, whether the breasts are glandular or fatty and whether ptosis is present. Surgery aims to reduce fat content, increase skin and subcutaneous tissue contracture, and reposition the tissue in a more anatomical plane. Most of the procedures done by the author have been by ultrasonic liposculpturing. The level of satisfaction has been over 90%. A combination of ultrasound and conservative excision of glandular components can be done. Management of most juvenile breast hypertrophy and enlargement should be discouraged because of incomplete growth of the patient. However, sometimes ultrasound liposuction has a significant role to play in correcting this deformity.

Juvenile Breast Hypertrophy: Report of a Familial Pattern and Review of the Literature

Juvenile hypertrophy of the breast is a relatively rare condition rapidly leading to gigantomastia in peripubertal females. The pathology is limited to the breast, with otherwise normal growth and development. We have reported the first familial pattern seen in this unusual condition. Ablation of the breast is always required. We favor reduction mammaplasty to avoid breast implants, despite the need for additional procedures secondary to recurrent hypertrophy. Our second patient provides support for this approach, demonstrating amelioration in breast growth despite the initial period of rapid regrowth.

Juvenile gigantomastia

Juvenile breast hypertrophy in a 12-year-old girl was treated by bilateral reduction mammoplasty (amputation type) with free transplantation of the areolae and nipples and removal of 8,200 g of breast tissue. Eleven months later, subcutaneous mastectomy with subpectoral tissue expander insertion was performed due to recurrent enlargement of the breasts, with removal of an additional 5,000 g. Three months later, silicone prostheses replaced the tissue expander, with reconstruction of areolae and nipples. One year later, a good result was observed and the breasts have been satisfactorily reconstructed.
