The influence of the introduction of a national prenatal screening program on late termination of pregnancy, a retrospective cohort study.

To assess the influence of a national prenatal screening program on category 1 (lethal anomalies) late terminations of pregnancy (LTOP). In this population-based retrospective cohort study, we included all category 1 LTOPs from 2004 to 2015 in the Netherlands. The number of LTOPs before and after the introduction of the program was compared as well as the diagnostic process and factors contributing to LTOP. In total, 97 LTOPs were reported. After the introduction of the program, the number of LTOPs decreased from 17 per year to 5 per year on average. The number of cases in which the diagnostic process started with obstetric indications decreased from 55% to 17% (p<0.01) and the number of cases detected by routine screening increased from 11% to 52% (p<0.01). Four factors still contributed to LTOP after the introduction of the screening program: diagnostic or parental delay (40%), absence of screening (24%), false negative results of prior screening (14%) and late onset of disease (12%). The number of LTOPs decreased after the introduction of the screening program. At present, the diagnostic process is mostly screening driven. Parental- and diagnostic delay is still an important factor that contributes to LTOP.

Post treatment imaging in patients with local advanced cervical carcinoma.

Cervical cancer (CC) is the fourth leading cause of death in women worldwide and despite the introduction of screening programs about 30% of patients presents advanced disease at diagnosis and 30-50% of them relapse in the first 5-years after treatment. According to FIGO staging system 2018, stage IB3-IVA are classified as locally advanced cervical cancer (LACC); its correct therapeutic choice remains still controversial and includes neoadjuvant chemo-radiotherapy, external beam radiotherapy, brachytherapy, hysterectomy or a combination of these modalities. In this review we focus on the most appropriated therapeutic options for LACC and imaging protocols used for its correct follow-up. We explore the imaging findings after radiotherapy and surgery and discuss the role of imaging in evaluating the response rate to treatment, selecting patients for salvage surgery and evaluating recurrence of disease. We also introduce and evaluate the advances of the emerging imaging techniques mainly represented by spectroscopy, PET-MRI, and radiomics which have improved diagnostic accuracy and are approaching to future direction.

Risk factors for colorectal cancer in IBD patients

A characteristic feature of inflammatory bowel diseases (IBD) is an increased risk of neoplasia and colorectal cancer (CRC). The introduction of screening programs, as well as modern advances in therapy and endoscopic technologies, awareness of the risk factors of IBD-associated CRC will contribute to reducing the frequency of this dangerous complication of IBD. The presented review describes epidemiological data on the incidence of IBD-associated CRC, established risk factors for CRC, as well as modern methods of prevention, screening and early diagnosis. Literature search was conducted in PubMed and Google Scholar systems by keywords: inflammatory bowel diseases, ulcerative colitis, Crohn’s disease, colorectal cancer, risk factors, screening, endoscopy, dysplasia.

Long-term trends in incidence and prevalence of severe sight impairment due to diabetes in South East Scotland before and after implementation of diabetic retinopathy screening

The study set out to examine the impact of the introduction of a national diabetic retinopathy screening programme in Scotland in 2006 on incidence and trends in severe sight impairment (SSI) certification due to diabetes over three decades. We analysed SSI certification due to diabetic eye disease in Fife, Scotland, between 2010 and 2019 and compared it to previously published estimates from 1990 onwards. Population estimates of the general population and those with diabetes were obtained from the National Records of Scotland and the National Diabetes Register, NHS SCI Diabetes, respectively. Mean annual incidence of SSI in the population with diabetes, 1990–2019, before and after the introduction of diabetic retinopathy screening was compared using a Mann–Whitney U Test. Mean incidence in 2007–2019 was 19.8/100,000 and was significantly less than the period before the systematic screening, 1990–2006 (57.5/100,000, p &lt; .001). Mean SSI incidence due to diabetes in Fife between 2010 and 2019 was significantly lower than in the previous decades ( p &lt; .001) at 16.4 per 100,000 for the population with diabetes, per year ( SD 10.0; 95% confidence interval: [9.2, 23.5]) compared to 42.7 per 100,000 in 2000–2009 and 64.3 per 100,000 in 1990–1999. This study demonstrates a significant reduction in the annual incidence of SSI due to diabetes before and after the introduction of the screening programme provides evidence for the benefit of systematic diabetic retinopathy screening programmes. Other factors including improved management of diabetes and risk factors will have also had an impact emphasizing the importance of a multidisciplinary approach to the prevention of sight loss in diabetes.

Radiofrequency Tag Localisation for Breast Lesions, A single center experience

Background: Impalpable breast lesions are being diagnosed more often since the introduction of screening programs. Wire Localization has been used for decades to aid the surgeons excisions but more recently new localisation techniques have emerged. Radiofrequency (RF) tag localisation is a new localisation technique used for impalpable breast lesions. We describe our experience at NHS Grampian with the first 100 cases in our unit.

Changes in rectal cancer treatment after the introduction of a national screening program; Increasing use of less invasive strategies within a national cohort

AimOrgan preserving treatment strategies and the introduction of a colorectal cancer-screening program have likely influenced the resection rates of rectal cancer. The aim of this study is to assess the influence of these developments on rectal cancer treatment and resection rates in the Netherlands. MethodsPatients diagnosed with non-metastatic rectal cancer between 2013 and 2018, were selected from the Netherlands Cancer Registry. The distribution of surgical and neo-adjuvant treatment and resection rates were analyzed and compared over time. ResultsBetween 2013 and 2018 22640 patients were diagnosed with non-metastatic rectal cancer. The incidence of early stage (cT1) disease increased from 141 (4%) in 2013 to 448 (12%) in 2018. The use of neoadjuvant radiotherapy and chemo-radiotherapy dropped from 39% to 21% and 34%–25%, respectively. A decrease in surgical resection rates (including TEM) was observed from 85% to 73%. The proportion of patients who underwent endoscopic resections increased from 3% to 10%. The decrease in surgical resection rates was larger in patients treated with neo-adjuvant chemo-radiotherapy. ConclusionAn increase in stage I disease is noted after the introduction of the screening program. Surgical resection rates for rectal cancer have fallen over time. Endoscopic resections due to more early-stage disease probably accounts for a large part of this decline. Furthermore, a watch and wait approach after neo-adjuvant chemo-radiotherapy may play an important role as well.

Colorectal cancer incidence, mortality, tumour characteristics, and treatment before and after introduction of the faecal immunochemical testing-based screening programme in the Netherlands: a population-based study

In 2014, a population-based colorectal cancer (CRC) screening programme was stepwise implemented in the Netherlands comprising faecal immunochemical testing once every 2 years, with a cutoff value for positivity of 47 μg haemoglobin per g faeces. We aimed to assess CRC incidence, mortality, tumour characteristics, and treatment before and after introduction of this screening programme. We did a retrospective, observational, population-based study in the Netherlands and gathered CRC incidence data from the Netherlands Cancer Registry from Jan 1, 2010, to Dec 31, 2019, in people aged 55 years or older. Patients with a CRC diagnosis between Jan 1, 2014, and Dec 31, 2018, in the Netherlands Cancer Registry were linked with the nationwide registry of histopathology and cytopathology (PALGA) to identify mode of detection (ie, screening-detected vs clinically detected). We calculated age-standardised CRC incidence rates and used data from Statistics Netherlands to calculate CRC-related mortality in 2010-19. We compared localisation, stage distribution, and treatment of screening-detected CRCs with clinically detected CRCs diagnosed in 2014-18 in patients aged 55-75 years. Between Jan 1, 2010, and Dec 31, 2019, 125 215 CRCs were diagnosed in individuals aged 55 years or older and were included in the analyses for CRC incidence. Before the introduction of the screening programme, the age-standardised CRC incidence rate was 214·3 per 100 000 population in 2013 in people aged 55 years or older. After the introduction of the screening programme, this rate initially increased to 259·2 per 100 000 population in 2015, and subsequently decreased to 181·5 per 100 000 population in 2019. Age-standardised incidence rates for advanced CRCs (stage III and IV) were 117·0 per 100 000 population in 2013 and increased to 122·8 per 100 000 population in 2015; this rate then decreased to 94·7 per 100 000 population in 2018. Age-standardised CRC mortality decreased from 87·5 deaths per 100 000 population in 2010 to 64·8 per 100 000 population in 2019. Compared with clinically detected CRCs, screening-detected CRCs were more likely to be located in the left side of the colon (48·6% vs 35·2%) and to be detected at an early stage (I or II; 66·7% vs 46·2%). Screening-detected CRCs were more likely to be treated by local excision compared with clinically detected CRCs, and this finding persisted when stage I CRCs were analysed separately. After introduction of this national screening programme, a decrease in overall and advanced-stage CRC incidence was observed. In view of this observation, together with the observed shift to detection at earlier stages and more screening-detected CRCs being treated by local excision, we might cautiously conclude that, in the long-term, faecal immunochemical testing-based screening could ultimately lead to a decrease in CRC-related morbidity and mortality. Epidemiology of Multiple Congenital Anomalies Before and After Implementation of a Nationwide Prenatal Screening Program in Denmark.

Surveillance of congenital anomalies is important in order to detect negative influences from environment, medication, or lifestyle as early as possible. Since most teratogens are associated with a spectrum of birth defects rather than a single defect, analysis of the epidemiology of multiple congenital anomalies is important to detect an increase due to environmental or medicine exposure. The aim of the study was to describe changes in prevalence, types of anomalies, and outcome of pregnancies for fetuses and infants with multiple congenital anomalies before and after introduction of the new screening program in the County of Funen, Denmark. The study was based on data from the EUROCAT registry of the County of Funen for the period 1990 to 2014 covering 135,057 births. The registry includes information about livebirths, fetal deaths after 20 weeks of gestation and terminations of pregnancy after prenatal diagnosis of fetal anomalies. All cases with two or more major congenital anomalies in different organ systems, where the pattern of anomalies were not recognized as part of a chromosomal or genetic syndrome or a sequence were included in the study. Overall prevalence of multiple congenital anomalies was 19.7 per 10,000 pregnancies. There was no significant change in prevalence over time. The prenatal detection rate increased from 26 to 57% after introduction of the screening program (p < 0.001). Proportion of terminations of pregnancy increased from 11 to 30% of all cases and 1-week survival for livebirths increased from 64 to 94%. There was no change in combinations of involved organ systems. The implementation of the new screening program in 2004 has led to an increased prenatal detection rate of multiple congenital anomalies followed by an increased rate of termination of pregnancy for the most severe cases and an increased 1-week survival for liveborn infants with multiple congenital anomalies.

Cancer is the second leading cause of death globally. Malignant tumours are responsible for about 9.6 million deaths in 2018 (Ritchie H (2019) How many people in the world die from cancer? https://ourworldindata.org/how-many-people-in-the-world-die-from-cancer ). Worldwide, about 1 in 6 deaths is due to cancer. This confronts researches with the question of their origin and doctors with treatment options. It is common sense that great efforts should be done in order to reduce the number of cancer-specific deaths. In recent years, in lots of countries a variety of cancer screening programs have been developed, investigated and improved. The basic idea of this approach seems to be quite simple: Tumours will be detected at a very early stage when patients do not yet feel clinical symptoms. Thus, using an appropriate therapy, progression of the disease can be prevented and, concerning a whole population, disease-specific mortality should be reduced. Actually, after the introduction of screening programs, an increasing number of new cancer cases can be observed associated with an apparent reduction of the case fatality rate (i.e. the proportion of deaths due to cancer). Partly, the increasing number of cancers may be explained by the fact that people have a higher life expectancy. Under this aspect, the decreased case fatality rate could be considered as a success which may be attributed to screening efforts. However, there is still insufficient evidence affirming benefits of screening programs for crucial outcomes, i.e. all-cause mortality. In this narrative review, the phenomenon that probabilities and risks are rather often interpreted in an inadmissible way will be described. Furthermore, conceptual issues and inconsistencies between evidence and opinion about screening will be explored.

Expression of Extracellular Matrix-Related Genes and Their Regulatory microRNAs in Problematic Colorectal Polyps.

Simple SummaryDuring bowel cancer screening programs, many diagnostically problematic polyps are removed. The greatest challenge is to distinguish between adenomas with epithelial misplacement and adenomas with early carcinoma, considering the diagnosis affects prognosis and treatment. Our aim was to analyze the expression of extracellular matrix related genes and proteins DCN, EPHA4, FN1, SPARC, SPON2, and SPP1, in 44 biopsies. Differences were observed in most of the analyzed genes and proteins in adenoma with epithelial misplacement in comparison to adenoma with early carcinoma, reflecting inflammatory stromal reaction to traumatisation and misplacement of dysplastic glands in the submucosa in the former, and desmoplastic stromal reaction to true invasion of dysplastic glands in the submucosa in the latter. The observed expression patterns are too complex to be used in diagnostic work, but might contribute to better understanding extracellular matrix changes in colorectal cancerogenesis and help to find new diagnostic markers in the future.Colorectal carcinoma usually evolves gradually, forming a spectrum of lesions, due to accumulation of genetic mutations and epigenetic alterations. Many early lesions are detected since the introduction of screening programs. Differences were observed in AEM in comparison to AEC for DCN, EPHA4, FN1, SPON2, and SPP1, reflecting inflammatory stromal reaction to traumatisation and misplacement of dysplastic glands in the submucosa in the former, and desmoplastic stromal reaction to true invasion of dysplastic glands in the submucosa in the latter. Expression of regulatory microRNAs hsa-miR-200c and hsa-miR-146a significantly negatively correlated with the expression of their regulated genes, while significant difference between AEM and AEC was observed only for hsa-miR-29c. The described expression patterns are too complex to be used in diagnostic work, but might contribute to better understanding ECM changes in colorectal carcinoma development, helping to find new markers in the future.

Recent advances in X-ray imaging of breast tissue: From two- to three-dimensional imaging.

Breast cancer is a globally widespread disease whose detection has already been significantly improved by the introduction of screening programs. Nevertheless, mammography suffers from low soft tissue contrast and the superposition of diagnostically relevant anatomical structures as well as from low values for sensitivity and specificity especially for dense breast tissue. In recent years, two techniques for X-ray breast imaging have been developed that bring advances for the early detection of breast cancer. Grating-based phase-contrast mammography is a new imaging technique that is able to provide three image modalities simultaneously (absorption-contrast, phase-contrast and dark-field signal). Thus, an enhanced detection and delineation of cancerous structures in the phase-contrast image and an improved visualization and characterization of microcalcifications in the dark-field image is possible. Furthermore, latest studies about this approach show that dose-compatible imaging with polychromatic X-ray sources is feasible. In order to additionally overcome the limitations of projection-based imaging, efforts were also made towards the development of breast computed tomography (BCT), which recently led to the first clinical installation of an absorption-based BCT system. Further research combining the benefits of both imaging technologies is currently in progress. This review article summarizes the latest advances in phase-contrast imaging for the female breast (projection-based and three-dimensional view) with special focus on possible clinical implementations in the future.

Regional clinical and economic model of colorectal cancer screening

Purpose of the study. To evaluate the clinical and economic efficiency of colorectal cancer screening model in the Yaroslavl region. Materials and methods. To achieve this goal, we have given a clinical and economic assessment of the existing approach to the diagnosis and treatment of colon cancer. We have also developed a clinical and economic model of colorectal cancer screening strategy in the Yaroslavl region. After that, we made a comparative assessment between the strategies on such parameters as total costs, efficiency, and CER coefficient. Results. The strategy for the absence of colorectal cancer screening has a cost-effectiveness rate equal to 246 712 rubles (3820$)/LYG, with the value of added years of life in 2.9 years. According to our calculations, the implementation of the program of screening of CRC based on fecal occult blood immunochemical method, followed by the implementation of colonoscopy in the case of a positive result, will require an increase in funding for patients with colon cancer by 6.9% per year. At the same time, by changing the structure of morbidity and increasing the part of early forms in the implementation of the screening program, an increase in the estimated life expectancy of patients will be achieved, which is consistent with the long-term results of programs conducted in Europe and the United States. The CER for the screening strategy is 103.95 thousand rubles (2030$) / LYG. Long-term modeling shows that the introduction of screening program will reduce the incidence of CRC by 12% from the baseline by the 15th year of implementation and reduce the cost of diagnosis and treatment of colon cancer by 16.1% per year without taking into account inflation expectations. Conclusion. The introduction of a colorectal cancer-screening program at the regional level is clinically effective and cost-effective.

Geographical inequalities in breast cancer incidence, mortality and late stage at diagnosis in Cyprus

Abstract Background A national breast cancer (BC) screening programme, targeting women 50-69 years old, was introduced in Cyprus in 2007. This study aims to assess rural/urban differences in BC incidence, tumour stage at diagnosis (TSD) and mortality, since 2004 and since the introduction of the screening programme. Methods Data were obtained for 6589 new cases of BC (diagnosis 2004-2016) from a national population-based cancer registry (1998-today) in Cyprus. TSD was recorded according to the Surveillance, Epidemiology, and End Results categories. We defined late TSD as regional or distant stage. Unknown stages were excluded. We linked cases to the population census data and place of residence (rural/urban) and calculated incidence and mortality rate ratios (RR) by place of residence. We conducted logistic regression to measure the association between place of residence and TSD or mortality, adjusting for sex, age, ethnicity, smoking and marital status. Adjusted odds ratios (aOR) and 95% confidence intervals (CI) are reported. Results Since 2004, 54.7% of BC cases were diagnosed at localised stage, 40.6% at regional stage and 4.7% at distant stage. Incidence, but not mortality, rate for all BC was higher in urban than in rural areas (RRincidece=1.22;95%CI=1.51-1.28 and RRmortality=1.10;95%CI=0.97-1.24). Rural place of residence was associated with higher odds of late TSD (aOR=1.34; 95%CI=1.15-1.58) and death (aOR=1.36; 95%CI=1.11-1.68). After the introduction of the screening programme, for the targeted age group, rural place of residence was still associated with higher odds of late TSD (aOR=1.31; 95%C= 1.03-1.67), but not with death (p &amp;gt; 0.05). Conclusions Even though BC incidence is higher in urban than in rural areas, women residing in rural areas had higher odds of late TSD and death. The introduction of the national BC screening programme only slightly reduced geographical inequalities in TSD. Further studies are needed to understand and reduce the role of inequalities in TSD. Key messages Rural/Urban inequalities in TSD are evident among BC cases in Cyprus, diagnosed between 2004 and 2016. The introduction of the national BC screening programme reduced but did not overcome geographical inequalities in TSD.

The effect of the population-based cervical cancer screening program on 5-year survival in cervical cancer patients in Lower Silesia.

Poland is considered among the European countries with an average incidence of cervical cancer (CC; about 3,000-3,500/year) and at the same time with high mortality (5-year survival rate - 55.2%). For this reason, in 2006 Poland introduced a Population-Based Cervical Cancer Prevention and Early Detection Program addressed to women aged 25-59 years, in which a cytological test is carried out every 3 years. The aim of the study was to assess the changes in the curability of CC patients brought by the introduction of the Screening Program in the Lower Silesian voivodeship and to identify the subpopulation of women for whom activities aimed at increasing adherence rates must be intensified. The 5-year relative survival in 3,586 CC patients from 2000-2010 registered in the Lower Silesian Cancer Registry was analyzed. In the Lower Silesian voivodeship, a 55.1% 5-year survival rate was recorded in 2000-2004 and 70.5% in 2010. The highest increase in 5-year relative survival rates was found in rural communities (from 53.1% in 2000-2004 to 77.7% in 2010) and in Wrocław (56.8% and 74.2%, respectively). In the study group, the number of patients with invasive CC (C53) detected in the local stage of the disease increased systematically from 61.5% in 2000-2004 to 74.3% in 2010. The introduction of the population-based screening program improved the curability rate in CC patients in the Lower Silesian voivodeship. In order to maintain the recent positive trends, further education should be continued, and activities aimed at increasing adherence to screening tests should be intensified, especially in urban-rural communities.

An educational interventional study on knowledge, attitude of cervical cancer among nursing students of HKES’ college of nursing, Kalaburagi

Introduction: Cervical cancer is one of the most common cancers in women, with an estimate of 468,000 new cases annually in the world, 80% of these countries. India accounts for one-fifth of the world burden of cervical cancer the incidence of cervical cancer has declined in western countries following the introduction of screening programs which are often rudimentary or nonexistent in countries like India due to infrastructural, financial and personnel constraints. In developing countries like ours, more than three fourths of cervical cancer patients are diagnosed at advanced stages leading to poor prospects of long-term survival and cure.Objectives1. To assess the awareness of nursing students on aetiology, symptoms, risk factors, and prevention of cervical cancer.2. To assess the effect of educational intervention regarding risk factors, and prevention of cervical cancerMethodology: An educational interventional study was done after calculate the sample size 110. Pretest awareness level was asses using a questionnaire and educational intervention was given and posttest date was collected after applying the same questionnaire. The statistical significance was set at 5% (p < 0.05). Appropriate statistical test was applied to determine the effectiveness of educational intervention.Results: A total 110 nursing girl students belonging to 3rd and 4th year BSC were registered for our study. About 63.6% nursing students were aware about the disease. More improvement in their knowledge was seen in areas related to disease, screening methods, Prevention aspects, vaccine and symptoms with mean paired differences of 1.36, 2.064, 0.209, 1.055 and 1.067 respectively.Conclusion: This study highlights inadequate knowledge about cervical cancer, its screening and prevention amongst the nursing students. Implementation of the educational sessions was successful in improving their knowledge. Study highlights the need to undertake similar community based studies about awareness and education interventions.

Early diagnosis and detection of breast cancer.

Breast cancer is the most common malignancy in women. It is often characterized by a lack of early symptoms, which results in late detection of the disease. Detection at advanced stages of the decease implies the treatment is more difficult and uncertain. The appropriate screening programs have been conducted within the organized preventive examinations and have made significant contributions to the early breast cancer detection. It is necessary to improve the screening process in order to reduce the percentage of female population that is not covered by screening programs and increase the number of early-detected breast cancers. The improvement of the screening program may be reflected in the following: more efficient determination of the list of the women who have to undergo preventive examination, introduction of screening program in thermography as a diagnostic method applied in pre-screening stage, more efficient analysis of mammograms and continuous follow up of patients. The identification of target population for breast cancer screening program has been based on the age of women. The improvement of the early breast cancer diagnosis process proposed in this paper is reflected in more efficient determination of the group of women who have to undergo preventive examination based on the factors affecting the occurrence of breast cancer. Inclusion of the pre-screening phase in which thermal imaging could be applied and software support to mammographic detection of tumor are suggested. This paper describes the breast cancer, current screening program and techniques for early-stage breast cancer detection, module of medical information system MEDIS.NET for creating screening list based on the analysis of risk factors affecting the occurrence of breast cancer, mammography and role of thermal imaging in the process of early breast cancer detection. It also presents an overview on important achievements in computer-aided detection and diagnosis of breast cancer in mammography and thermography. Based on the obtained results, dynamics of preventive examinations for particular groups of women that is different from the standard two-year examinations, can be successfully defined. It can be concluded that the use of a computer system for tumor diagnosis in mammogram based on various methods of image processing can help doctors in decision-making, while the use of thermal imaging in the pre-screening phase would significantly reduce the list of women for screening mammograms.

STEREOTACTIC BODY RADIATION THERAPY FOR EARLY-STAGE NON-SMALL CELL LUNG CANCER

An active introduction of screening programs potentially leads to a significant increase in the proportion of patients with early forms of non-small cell lung cancer. Surgical treatment, which is the standard of care for localized forms, due to functional limitations can be performed only in 65-70% of patients. The solution to this problem can be found in the improvement of the results of radiotherapy by using modern equipment, the planning systems, improved fractionation schemes and introduction of methods for summing radiation doses. Stereotactic radiotherapy allows high-precision delivery of high radiation dose to tumor with a minimal damage to surrounding healthy tissues. In this literature review based on the analysis of a large number of publications we show that it is not yet possible to make valid conclusions about the effectiveness and safety of stereotactic radiation therapy as an alternative to the surgical methods. It is necessary to plan and conduct randomized trials without further delay taking into account the expected high relevance of the method.

Optimal configuration of a low-dose breast-specific gamma camera based on semiconductor CdZnTe pixelated detectors

Breast cancer is one of the most frequent tumours in women. During the ‘90s, the introduction of screening programmes allowed the detection of cancer before the palpable stage, reducing its mortality up to 50%. About 50% of the women aged between 30 and 50 years present dense breast parenchyma. This percentage decreases to 30% for women between 50 to 80 years. In these women, mammography has a sensitivity of around 30%, and small tumours are covered by the dense parenchyma and missed in the mammogram. Interestingly, breast-specific gamma-cameras based on semiconductor CdZnTe detectors have shown to be of great interest to early diagnosis. Infact, due to the high energy, spatial resolution, and high sensitivity of CdZnTe, molecular breast imaging has been shown to have a sensitivity of about 90% independently of the breast parenchyma. The aim of this work is to determine the optimal combination of the detector pixel size, hole shape, and collimator material in a low dose dual head breast specific gamma camera based on a CdZnTe pixelated detector at 140 keV, in order to achieve high count rate, and the best possible image spatial resolution. The optimal combination has been studied by modeling the system using the Monte Carlo code GATE. Six different pixel sizes from 0.85 mm to 1.6 mm, two hole shapes, hexagonal and square, and two different collimator materials, lead and tungsten were considered. It was demonstrated that the camera achieved higher count rates, and better signal-to-noise ratio when equipped with square hole, and large pixels (> 1.3 mm). In these configurations, the spatial resolution was worse than using small pixel sizes (< 1.3 mm), but remained under 3.6 mm in all cases.

Patients with Small Abdominal Aortic Aneurysm are at Significant Risk of Cardiovascular Events and this Risk is not Addressed Sufficiently

Patients with abdominal aortic aneurysm (AAA) are at significant risk of cardiovascular (CV) events. Recent implementation of AAA-screening means thousands of patients are now diagnosed with small-AAA; however, CV risk factors are not always addressed. This study aimed at assessing and quantifying the CV characteristics of patients with small AAA following the introduction of screening programmes. CV profiles of 384 men with a small AAA (<55 mm diameter) were assessed through the United Kingdom Aneurysm Growth Study (UKAGS), a nationwide prospective cohort study of men with small AAA. A prospective local cohort of an additional 142 patients with small AAA with available blood pressure (BP) and lipid profiles was also included and followed-up for 1 year. In the UKAGS population, 54% were current and 30% ex-smokers; 58% were hypertensive and 54% hypercholesterolaemic. In the local group, 54% were current and 40% were ex-smokers, and 94% were hypertensive. Patients were not more likely to receive CV medication after entering AAA surveillance in either group. All local patients were clustered “high-risk” for future CV events based on the Framingham score (mean 21.8%, 95% CI 20.0-23.6), JBS-2 (16.3%, 14.7-17.9) and ASSIGN (25.2%, 22.7-27.7). No change was seen in systolic BP levels between baseline and 1 year (140.9 mmHg vs. 142.5 mmHg, P =.435). A rise was seen in cholesterol (4.0 mmol-4.2 mmol, P <.0001) values at 1 year. This study suggests that patients with small AAA are at significant risk for developing CV events and this is not currently addressed, which is evident by the “high-risk” CV risk profiles of these patients despite being in AAA surveillance. Design and implementation of a CV risk reduction programme tailored for this population is necessary.
