Non-invasive Detection of Diffuse Intracranial Vertebrobasilar Artery Stenosis: A Prospective Comparison with Digital Subtraction Angiography

The aim of this study was to prospectively evaluate diffuse intracranial vertebrobasilar artery stenosis by ultrasonic examination with digital subtraction angiography as the reference. Five hundred forty-one vertebrobasilar arteries with a normal lumen or intracranial stenosis were enrolled. Peak systolic velocity, mean flow velocity and end-diastolic velocity (EDV) at the intracranial vertebrobasilar arteries and extracranial vertebral arteries (VAs) were measured. The resistance index (RI) at extracranial VAs and the difference between the RI of extracranial VAs and the RI of the extracranial internal carotid artery (RIica) were calculated. Compared with normal arteries, all stenotic arteries were divided into the high-velocity group (focal stenosis, multiple-segment stenosis and multiple-artery stenosis) and low-velocity group (critical stenosis and long stenosis). The consistency between ultrasonic examination and digital subtraction angiography for evaluation of vertebrobasilar arteries with multiple-segment stenosis and multiple-artery stenosis was not favorable when applying the widely used Stroke Outcomes and Neuroimaging of Intracranial Atherosclerosis criteria (κ=0.442 and 0.438, respectively). The optimal low-velocity criteria for identifying intracranial vertebrobasilar arteries with critical stenosis and long stenosis were determined by receiver operating characteristic curve analysis and were as follows: EDV ≤15 cm/s and RI ≥0.68 at the extracranial VA and RIica ≥0.10.

Stenting for Symptomatic Intracranial Vertebrobasilar Artery Stenosis in Northeast of China: A Single-Center Study.

Objective: We described the incidence of surgery-related complications to evaluate the safety of endovascular therapy for severe symptomatic intracranial vertebral basilar artery stenosis (IVBS) in our stroke center in Northeast of China.Methods: Consecutive patients with symptomatic IVBS caused by 70–99% stenosis despite standard medical treatment of antiplatelet agents plus statin were enrolled. Either balloon-mounted stent or balloon predilation plus self-expanding stent was performed. Clinical adverse events such as stroke, transient ischemic attack (TIA), and death after the surgery were documented. Radiological events such as in-stent thrombosis, dissection, and guide-wire perforation during the process were recorded as complications as well. The baseline characteristics and outcomes of patients among different Mori types were compared.Results: From January 2017 to December 2018, 97 patients with stroke or TIA due to intracranial IVBS were treated by stenting, including 30 patients with basilar artery (BA) stenosis, 55 patients with intracranial vertebral artery (V4) stenosis, and 12 patients with V4-BA stenosis. The primary events include two intracranial hemorrhage (2.1%, 2/97), seven ischemic events (7.2%, 7/97), and two death (2.1%, 2/97). The successful stent deployment rate was 98.9% (96/97). The Apollo stents were used more for Mori A lesions. Self-expanding stents were more used in Mori C lesions. Mori C lesions were more vulnerable to endovascular procedure and showed higher rate of complications than A (p = 0.008) and B type (p = 0.047).Conclusion: A high technical success rate of IVBS stenting could be achieved, and the safety was acceptable, whereas Mori C lesions were more vulnerable to endovascular procedure and showed a higher rate of complications than A and B types.

Preliminary application of drug-coated balloon in patients with symptomatic intracranial vertebrobasilar artery stenosis

Objective: To explore the safety and short-term efficacy of drug-coated balloon (DCB) angioplasty for patients with symptomatic intracranial vertebrobasilar artery stenosis. Methods: Sixteen patients with symptomatic intracranial vertebrobasilar artery stenosis who received DCB angioplasty from September 2018 to December 2019 at Department of Neurosurgery, Beijing Hospital were retrospectively reviewed. There were 15 males and 1 female, aged (63.1±9.2) years (range: 48 to 77 years). Patients' demographics, lesions characteristics, complications, clinical and imaging follow-up data were collected and analyzed. Results: A total of 19 symptomatic intracranial vertebrobasilar artery stenosis were successfully treated with DCB.The degree of stenosis of lesion was 75% (20%) (M(Q(R))) before operation and 0 (20%) after operation. One posterior circulation stroke due to perforator artery occlusion happened in peri-procedural period.With a mean imaging follow-up time of 5.5 months, there was no restenosis occurred. Within a mean clinical follow-up period of 6.3 months, no new symptoms happened. Conclusion: For patients with symptomatic intracranial vertebrobasilar artery stenosis, DCB angioplasty seems relatively high safety with satisfactory short-term clinical and imaging outcomes.

Balloon-mounted versus self-expanding stents for symptomatic intracranial vertebrobasilar artery stenosis combined with poor collaterals

ABSTRACTObjectives: To compare the technical and 1-year outcomes of balloon-mounted versus self-expanding stents for symptomatic intracranial vertebrobasilar artery stenosis.Methods: 167 patients with severe intracranial vertebrobasilar artery atherosclerotic stenosis with poor collaterals were enrolled. Both balloon-mounted stenting and self-expanding stent placement were selected to treat patients. The baseline characteristics, cerebral angiography and clinical follow-up were assessed between the two groups.Results: The overall mean stenosis degree was reduced from 82.8 ± 11.8% pre-stent placement to 9.41 ± 8.20% post-stent placement. Patients treated with self-expanding stent were more likely to undergo general anesthesia (81.2% versus 67.3%; P = 0.048) and have longer operative times (91.0 ± 25.1 min versus 67.9 ± 17.1 min, P = 0.012) than those treated with self-expanding stents. Patients treated with self-expanding stents were more likely to require longer and larger diameter stents (14.99 ± 3.26 mm versus 9.23 ± 2.46 mm, P = 0.000; 3.28 ± 0.57 mm versus 2.74 ± 0.30 mm, P = 0.000, respectively). The degree of residual stenosis in self-expanding stent group was higher than patients treated with balloon-mounted stents (13.39 ± 8.64% versus 6.70 ± 6.62%, P = 0.000). The rates of stroke, transient ischemic attack or death at 1-year follow-up were not significantly different between two groups (P > 0.05).Conclusions: Stenting for patients with severe symptomatic intracranial vertebrobasilar arterial stenosis with poor collaterals can be performed safely with efficacy. Patients treated with balloon-mounted stents appear to have shorter operative times and lower re-stenosis rates than treated with self-expanding stents.

Stenting for symptomatic intracranial vertebrobasilar artery stenosis: 30-day results in a high-volume stroke center

ObjectivesSymptomatic intracranial vertebrobasilar artery stenosis (IVBS) carries a high annual risk of recurrent stroke. Endovascular therapy was a promising technique but recent trials suggest it may carry a risk of periprocedual complications especially in inexperienced hands. This prospective study was to evaluate the safety of endovascular therapy for severe symptomatic IVBS in a high volume stroke centre. Patients and MethodPatients with symptomatic IVBS caused by 70–99% stenosis despite medical treatment of at least one antiplatelet agent and statin were enrolled. The patients were treated either with balloon-mounted stent or balloon pre-dilation plus self-expanding stent as determined by the operators following a guideline. The primary outcome was 30-day stroke, transient ischemic attack (TIA) and death after stenting. The secondary outcome was successful stent deployment. The baseline characteristics and outcomes of patients with basilar artery (BA) lesions and patients with vertebral artery V4 segment lesions (BA group vs V4 group) were compared. And the outcome of different Mori type lesions was also compared. ResultFrom September 2013 to September 2014, 105 patients with stroke or TIA due to intracranial IVBS were screened and 97 patients were treated by stenting, including 52 patients with BA stenosis and 45 patients with V4 stenosis. The rate of 30-day stroke, TIA and death was 7.1%. All the three strokes happened in the BA group and were perforator strokes. The successful stent deployment rate was 100%. General anesthesia was more preferred in the BA group than in the V4 groups (96.2% vs 75.6%, p=0.005). The Apollo stent was used more for Mori A lesions (30.5% vs 7.9%, p=0.011) and had lower degree of residual stenosis (8.6% vs 12.6%, p=0.014) than Wingspan stent. Mori C lesions were more likely to have higher degree of residual stenosis than Mori A lesion (15.3% vs 7.4%, p=0.005). ConclusionThe short-term safety of endovascular stenting for patients with severe symptomatic IVBS in a high volume stroke centre was acceptable. Mori A lesions may have lower residual stenosis rate than the Mori C type lesions.

Selective use of transradial access for endovascular treatment of severe intracranial vertebrobasilar artery stenosis

ObjectiveTo study the feasibility and safety of transradial artery access for endovascular intervention of severe intracranial vertebrobasilar stenosis for selected patients not suitable for transfemoral access. MethodsThis was a retrospective analysis of 58 patients who had undergone intervention for severe intracranial vertebrobasilar stenosis using transradial access between January 2012 and September 2014. The reasons for transradial access were traced. The outcome measures were the technical success rate, 30-day stroke or death in the territory of the culprit artery, periprocedural and access site complication rates. ResultsOut of the 58 patients, 19 patients (32.8%) used the transradial approach due to poor iliofemoral artery access, 28 (48.3%) due to unfavorable brachiocephalic or subclavian artery anatomy, 11 (19%) due to unfavorable vertebral artery anatomy. The technical success rate was 100%. There were four periprocedural complications, one of which was asymptomatic. The 30-day stroke rate was 5.2% (3/58 patients), with two of them having no residual neurological deficits. There was no mortality. None had access site complications. ConclusionsFor selected patients with severe intracranial vertebrobasilar stenosis with difficult femoral access or anatomical variations precluding provision of a stable support for intracranial intervention, the transradial approach was shown to be a safe and feasible alternative route of access. Future trials of endovascular treatment for intracranial posterior circulation strokes should take into account the route of access.

Endovascular treatment for intracranial vertebrobasilar artery stenosis.

We describe our experience with endovascular treatment for symptomatic intracranial vertebrobasilar artery stenosis. Forty-four patients with intracranial vertebrobasilar artery stenosis (37 vertebral arteries, 7 basilar arteries) were treated with endovascular surgery. Indication criteria for the treatment were (1) medically refractory symptomatic patients; (2) angiographic stenosis of more than 60 %; and (3) short lesion (<15 mm). Under local anesthesia, balloon angioplasty was first performed in all patients. Stenting was performed only in cases with insufficient dilatation, dissection, or restenosis after balloon angioplasty. The degree of stenosis, which was 83 % before treatment, was reduced to 23 % after treatment. The rate of stroke and death within 30 days was 2.3 %. Nine patients (20.5 %) developed restenosis within 6 months. Of these, four patients were symptomatic. All symptomatic patients with restenosis were successfully treated with balloon angioplasty or stenting. Endovascular treatment for vertebrobasilar artery stenoses is feasible and safe in selected patients. Restenosis may be an important cause of recurrent stroke. Therefore, close clinical and neuroradiological follow-ups are essential for patients treated with endovascular surgery to improve long-term results.

Microembolic Signals in Acute Posterior Circulation Cerebral Ischemia

The clinical significance of microembolic signals (MES) in the posterior circulation remains unclear. The aim of this study was to investigate the sources and consequences of MES in acute posterior circulation cerebral ischemia. We evaluated a total of 140 consecutive patients (93 males, mean age 62.9 years) who had acute posterior circulation cerebral ischemia. The MES monitoring was conducted at the basilar artery through the suboccipital window for a 30-minute period. MES were detected in 18 (12.9%) of the 140 patients. Clinical characteristics and laboratory data did not differ between the MES-positive and MES-negative groups. Intracranial vertebrobasilar artery (VBA) stenosis was independently associated with the presence of MES (odds ratio, 9.85; 95% confidence interval, 1.22-79.48; P=0.032), whereas the patients with vertebral artery stenosis that was limited to the extracranial portion did not show an association. Microembolic signals occurred significantly more frequently in patients with severe degree of VBA stenosis compared to those with nonsignificant stenosis (odds ratio, 9.88; 95% confidence interval, 1.23-79.07; P=0.031). In a subgroup analysis of the 79 patients who had lesions on diffusion-weighted images and relevant VBA stenosis, the MES-positive group showed more frequent embolic infarction (P=0.010) and multiple lesion patterns (P=0.007) than single perforating infarctions. In acute posterior circulation cerebral ischemia, intracranial and severe VBA stenosis is associated with MES and may be its root causes. The presence of MES in VBA stenosis suggests that multiple and embolic type infarctions are the mechanisms of stroke.

Abstract 2386: Microembolic Signals in Acute Posterior Circulation Cerebral Ischemia: Sources and Consequences

Wingspan Stent for High-Grade Symptomatic Vertebrobasilar Artery Atherosclerotic Stenosis

This study was designed to present the treatment outcomes with Wingspan stent angioplasty of high-grade intracranial vertebrobasilar artery (VBA) stenosis in symptomatic patients. Between 2007 and 2010, the records of 30 patients with 31 intracranial high-grade VBA stenoses (all ≥ 70%) who underwent elective stenting due to the failure of medical therapy were retrospectively reviewed. Clinical evaluation was performed based on the modified Rankin scale and the National Institutes of Health Stroke Scale. In all cases, the stent deployment was technically successful. The mean stenosis decreased significantly from 82.28 ± 8.02% (range, 72-99%) to 11.18 ± 7.28% (range, 0-25%) after stent-assisted angioplasty (P < 0.05). Periprocedure complications occurred in 3 (10%) of 30 patients; there were 2 cases of perforator strokes and 1 case of transient flow insufficiency with stent overlap. Clinical follow-up (mean, 17.81 ± 11.49 months; range, 5-40 months) was available for 27 patients, and angiographic follow-up (mean, 9.95 ± 5.74 months, range, 5-20 months) was available for 19 patients. Only one case demonstrated recurrent symptoms with restenosis (≥ 50%). There were no recurrent ischemic events and no cases of restenosis in the other patients. According to our data, the Wingspan stent for symptomatic intracranial VBA stenoses is a safe and efficacious treatment alternative in cases with recurrent symptoms despite medical therapy. However, the improvement of outcome requires the reduction in the rate of procedure-related complications and long-term outcomes still have to be demonstrated.

Stenting of Symptomatic Basilar and Vertebral Artery Stenosis in Patients Resistant to Optimal Medical Prevention: The Lyon Stroke Unit Experience

Intracranial angioplasty stenting may be an efficient therapy in patients with intracranial atherosclerotic symptomatic vertebrobasilar artery stenosis unresponsive to optimal medical therapy. We present our experience in this setting. Results: The study included 12 cases (8 men, 4 women), with an age range of 43–78 years (mean 62.6 years). Intracranial stenosis that resulted in qualifying stroke or transient ischemic attack involved the vertebral artery (n = 4), lower basilar artery (n = 1) and mid basilar artery (n = 5). Tandem stenosis included the intracranial vertebral artery and basilar artery (n = 1) and both intracranial vertebral arteries (n = 1). The degree of stenosis ranged between 70 and 90% in the basilar and vertebral arteries. Angioplasty + stenting was performed in all lesions. A successful procedure resulting in 30% of residual stenosis was found in 14 vessels. A periprocedural adverse event occurred in 1 case and was related to a brain hemorrhage. The mean patient follow-up was 15 ± 3 months; 10 patients remained symptom free. All patients underwent a percutaneous endovascular balloon angioplasty and stent placement. Conclusions:This study supports the safety and the potential efficiency of stent-assisted angioplasty in patients resistant to optimal prevention. Randomized larger prospective trials are needed to confirm the benefit of this approach.

Patients who have symptomatic, medically refractory, vertebrobasilar artery stenosis have a high risk of stroke. The benefits of vascular reconstruction surgery and balloon angioplasty for these lesions are limited, and these treatments are associated with considerable complications. Recently stent placement in the intracranial arteries became available and is expected to improve the results of endovascular treatments. We review our experience with endovascular treatment for symptomatic intracranial vertebrobasilar artery stenosis. Forty patients with intracranial vertebrobasilar artery stenosis were treated with endovascular surgery. Indication of the endovascular surgery was medically refractory symptomatic patients with over 60% angiographical stenosis. Balloon angioplasty was firstly performed in all patients. Stenting was performed only in cases with insufficient dilatation, dissection or restenosis after balloon angioplasty. Successful dilatation was obtained in all cases. Twelve patients underwent stenting in initial treatments. The stenosis rate reduced to 25.2% after balloon angioplasty and 16.1% after stenting. No neurological complications occurred after procedure. The restenosis rates after treatments were 25.9% after balloon angioplasty and 23.5% after stenting. During the follow-up period, only 1 patient developed stroke of posterior circulation. Endovascular surgery for symptomatic vertebrobasilar artery stenosis has become more feasible and safer after the introduction of stenting. Prevention of restenosis is the next problem to be solved.

頭蓋内椎骨脳底動脈系動脈硬化性狭窄病変に対するａｎｇｉｏｐｌａｓｔｙおよびｓｔｅｎｔｉｎｇの臨床成績

Recently, angioplasty/stenting has been reported as a treatment option for intracranial atherosclerotic stenosis. We describe clinical results of angioplasty/stenting for 21 patients with intracranial vertebrobasilar stenosis (21 lesions). The procedure was performed under local anesthesia via the percutaneous transfemoral route. In 13 cases, we performed angioplasty only, while we used stents in 8 of 21 cases. The mean stenosis in 13 cases of angioplasty decreased from 76.2% to 26.4%, while 82.5% of the stenotic rate remarkably reduced to 11.3% in 8 cases of stenting. The 30-day morbidity and 30-day mortality rate were 4.8% and 0%, respectively. There was only 1 hemorrhagic complication (cerebellar hemorrhage) in case of stenting, and no ischemic events during or after the procedures. Restenosis (more than 50%) occurred in 3 of 21 cases (14.3%) during the mean follow-up period of 12 months. Our procedure-related morbidity and mortality rates were possibly not higher than those reported by other authors. In this study, angioplasty/stenting for intracranial vertebrobasilar artery stenosis was an effective treatment, but strict indications may be required because the procedure-related 30-day morbidity rate was still high compared with that of extracranial atherosclerotic stenosis, in addition to unknown natural history of intracranial vertebrobasilar stenosis.

Clinical Results of Percutaneous Transluminal Angioplasty and Stenting for Intracranial Vertebrobasilar Atherosclerotic Stenoses and Occlusions

Eighteen patients with intracranial vertebrobasilar stenosis and occlusion were treated by PTA or stenting. In 11 of 18 cases, only PTA was performed and in seven of 18 cases, we used stents. The mean stenosis before and after PTA/stenting was 82.8% and 22.3%, respectively. In 11 cases of PTA only, the stenotic rate decreased from 81.8% to 29.6%, while 85.0% of the stenotic rate remarkably reduced to 6.0% in seven cases of stenting. The 30 days morbidity and 30 days mortality rate were 5.5% and 5.5%, respectively. There was only one haemorrhagic complication (cerebellar haemorrhage) in cases of stenting, and no ischemic events during or after the procedures. Restenosis (more than 50% stenosis) occurred in four of 18 cases(22.2%) during mean followup period of 12 months. Two patients with VA occlusion before treatment, developed restenosis and reocclusion. Complete total occlusion seems to be a high-risk lesion and strict follow-up is required. In this study, PTA/stenting for intracranial vertebrobasilar artery stenosis or occlusion is an effective treatment, but strict indications may be required because procedure-related 30 days morbidity rate was 5.5% in addition to unclear natural history.

Staged stent-assisted angioplasty for symptomatic intracranial vertebrobasilar artery stenosis.

Medically refractory symptomatic vertebrobasilar atherosclerotic disease has a poor prognosis. Studies have shown that longer (> or = 10 mm), eccentric, high-grade (> 70%) stenoses portend increased procedure-related morbidity. The authors reviewed their experience to determine whether a staged procedure consisting of angioplasty followed by delayed (> or = 1 month later) repeated angioplasty and stent placement reduces the morbidity associated with endovascular treatment of symptomatic basilar and/or intracranial vertebral artery (VA) stenoses. The authors retrospectively reviewed the medical records in a consecutive series of eight patients who underwent planned stent-assisted angioplasty for medically refractory, symptomatic atherosclerotic disease of the intracranial posterior circulation between February 1999 and January 2002. Staged stent-assisted angioplasty was planned for these patients because the extent and degree of stenosis of the VA and/or basilar artery (BA) lesion portended an excessive procedure-related risk. The degree of stenosis, recent onset of symptoms (unstable plaque), vessel tortuosity, and lesion length and morphological feaures were contributing factors in determining procedure-related risk. Patient records were analyzed for location and degree of stenosis, preprocedural regimen of antiplatelet and/or anticoagulation agents, devices used, procedure-related complications, and clinical and radiographic outcomes. Among the patients in whom staged stent-assisted angioplasty was planned, vessel dissection, which necessitated immediate stent placement, occurred during passage of the balloon in one of them. In a second patient, the stent could not be maneuvered through the tortuous VA. In a third patient, the VA and BAs remained widely patent after angioplasty alone, and therefore stent placement was not required. Significant complications among the eight patients included transient aphasia and hemiparesis in one and a groin hematoma that necessitated surgical intervention in another; there was no permanent neurological morbidity. The mean stenosis before treatment was 78%, which fell to 54% after angioplasty, and the mean residual stenosis after stent placement was 30%. At the last follow-up examination, none of the treated patients had further symptoms attributable to the treated stenosis. The novel combination of initial angioplasty followed by delayed endoluminal stent placement may reduce the neurological morbidity associated with endovascular treatment of long, high-grade stenotic lesions. Attempting to cross high-grade stenoses with higher-profile devices such as stents may result in an embolic shower. Furthermore, neointimal proliferation and scar formation after angioplasty result in a thickened fibrous layer, which may be protective during delayed stent deployment. Larger-scale studies involving multiple centers are needed to elucidate further the lesion morphological characteristics and patient population most likely to benefit from staged procedures.
