Comparison of 3T MR arthrography and 3T MRI in intra-articular hip pathology: a cost-analysis.

MR arthrography (MRA) has previously been the radiological gold standard for investigating labral and chondral lesions of the hip joint. In recent years, 3T MRI has demonstrated comparable accuracy, being adopted as the first-line imaging investigation in many institutions. We compare the associated increased cost and radiation dose of the fluoroscopic component of the MRA compared to MRI. In this retrospective review over 2years, 120 patients (mean age 27.3years ± 13.2, range 8-67) underwent 3T MRA or non-contrast 3T MRI. Three musculoskeletal radiologists reported the data independently. Primary objectives included cost-comparison between each and radiation dose of the fluoroscopic component of the MRA. Secondary objectives included comparing detection of pathology involving the acetabular labrum, femoral cartilage, and acetabular cartilage. Then, 58 (48%) underwent 3T MRA and 62 (52%) patients underwent 3T MRI. The added cost of the fluoroscopic injection prior to MRA was €116.31/patient, equating to €7211.22 savings/year. MRA was associated with a small radiation dose of 0.003mSv. Transitioning from 3T MRA to 3T MRI in the investigation of intra-articular hip pathology increases cost savings and reduces radiation dose.

Slipped capital femoral epiphysis (SCFE) is a hip condition that occurs in teens and preteens who are still growing. Treatment for SCFE involves surgery to stop the head of the femur from slipping any further. Without early detection and proper treatment, SCFE can lead to potentially serious complications, including rapid degeneration of the femoral head and/or painful arthritis in the hip joint. Objective. Our case is presented to familiarize with the technique of safe surgical hip dislocation for the treatment of intraarticular hip pathologies. Case report and Methods. After collision while playing the 11 year old girl developed left hippain still allowing to walk. Imaging documented an acute slipped femoral capital epiphysis with the metaphysis articulating against the acetabulum. The patient was underwent open reduction could be performed using the DUNN procedure modified by GANZ. 6 weeks later prophylactic screw stabilization of the healthy right hip followed. Results. At 9 months follow-up the patient walks painfree with symmetric range of motion. Conclusion. The goal of treatment is to prevent the mildly displaced femoral head from slipping any further. This is always accomplished through surgery. Early diagnosis of SCFE provides the best chance of stabilizing the hip and avoiding complications. When treated early and appropriately, long-term hip function can be expected to be very good. Once SCFE is confirmed, the child will not be allowed to put weight on their hip and will be admitted to the hospital. In most cases, surgery is performed within 24 to 48 hours. In patients with unstable SCFE, the surgeon may first make an open incision in the hip, then gently manipulate (reduce) the head of the femur back into its normal anatomic position. The surgeon will then insert one or two metal screws to hold the bone in place until the growth plate closes. Sometimes surgeon may recommend inserting a screw into the unaffected hip at the same time to reduce the risk of SCFE. Our case demonstrates the value of open reduction of a severely dislocated femoral capital epiphysis in a case otherwise probably needing endoprosthetic hip replacement for restitution of hip function or hip arthrodesis at short-term follow-up

Decision Making in Combined Hip Arthroscopy and Femoral Derotational Osteotomy Surgeries

Background: Femoral version abnormalities can contribute to intra-articular hip pathology. Combined hip arthroscopy with femoral derotational osteotomy (FDRO) has been shown to successfully treat those with intra-articular hip pathology with excessive anteversion or retroversion of the femur. Indications: We describe the technique for combined hip arthroscopy and FDRO in patients with symptomatic intra-articular hip pathology in the setting of excessive anteversion or retroversion of the femur. Technique Description: Hip arthroscopy is performed using standard anterolateral, modified mid-anterior, and distal anterolateral accessory portals. The labrum is repaired using a narrow diameter suture. Femoroplasty is performed with utilization of fluoroscopic imaging to assess resection. Dynamic flexion is performed as a final check of adequacy of resection. Capsular closure is performed in all cases. After hip arthroscopy, the patient is repositioned on a radiolucent table. A piriformis start point is obtained with a guide pin followed by standard opening reaming and ball-tipped guidewire placement. A femoral osteotomy is made just proximal to the isthmus and made through a lateral approach to the femur. Two K-wires are placed distal and proximal to the osteotomy site. A goniometer is then used to measure rotation. A drill is used to perforate the cortex circumferentially at the osteotomy site. After standard reaming, a sagittal saw is then used to start the osteotomy cut followed by an osteotome. An intramedullary nail is inserted over a ball-tipped guidewire while rotational reduction is assessed from the foot of the operating table. Distal interlocking screws are placed, and the nail is backslapped to create compression at the osteotomy site prior to placing proximal interlocking screws. Results: Recent studies show improved hip outcome scores in patients who undergo concomitant hip arthroscopy and FDRO for symptomatic abnormal femoral version. Discussion/Conclusion: Identifying patients who would benefit from concomitant surgeries requires thorough preoperative evaluation. Correct identification and treatment of these patients leads to improved outcomes. Pediatric Hip Arthroscopy: a Review of Indications and Treatment Outcomes.

The use of hip arthroscopy has expanded substantially over the last decade, including in pediatric and adolescent populations. Indications for hip arthroscopy in the pediatric population continue to be refined and research of outcomes following hip arthroscopy has increased. The purpose of this review is to provide an overview of current indications for hip arthroscopy in the pediatric population and the outcomes for each indication. Hip arthroscopy is used in the treatment of a range of pediatric hip conditions, spanning from the infant to young adult. In femoroacetabular impingement, hip arthroscopy in young adolescents has shown improvement in patient-reported outcome measures, high return to sport rates, and low complications. Intra-articular hip pathology secondary to Legg-Calve-Perthes and the persistent deformities following slipped capital femoral epiphysis can be managed with primary hip arthroscopy, and outcomes show significant improvements in patient-reported outcomes. Arthroscopy can be used safely as a reduction aid in developmental hip dysplasia, and as a primary treatment for borderline hip dysplasia in adolescents. In septic hip arthritis, arthroscopic drainage is a safe and effective treatment. Hip arthroscopy is used in the pediatric and adolescent population in the management of femoroacetabular impingement, Legg-Calve-Perthes disease, the sequelae of slipped capital femoral epiphysies, developmental hip dysplasia, and septic arthritis. Research for each of these conditions shows that arthroscopy is a safe and effective treatment when performed for the correct indications, and results are comparable to open surgical options.

Rectus Femoris Heterotopic Ossification: Combined Arthroscopic and Open Resection With Allograft Reconstruction and Concomitant Treatment of Femoroacetabular Impingement

Background: Heterotopic ossification (HO), or the abnormal formation of bone in extra-skeletal tissue, is a well-known complication of orthopedic trauma, tendon avulsions, chronic injuries, spinal cord injuries, and soft tissue damage from surgery. Heterotopic ossification commonly develops at the direct or indirect head of the rectus femoris. Athletes are especially susceptible to chronic microtearing and acute tendon avulsion, which may result in HO. When HO develops in the setting of concurrent intra-articular hip pathology, it may be amendable to arthroscopic excision, depending on its size and location. Indications: Heterotopic ossification that develops adjacent to the hip joint can affect range of motion of the joint and is often a source of pain. Arthroscopic excision is indicated when the HO that develops within or about the hip joint is symptomatic and is of a location and size that it can be reached and excised arthroscopically. Technique Description: In this technical note, we describe our method to excise HO using initial arthroscopic surgery to address femoroacetabular impingement syndrome (FAIS) and perform initial dissection of the HO fragment. Heterotopic ossification resection was then completed via an anterior open approach followed by reconstruction of the rectus femoris origin with Achilles allograft. This is followed by our HO prophylaxis protocol of indomethacin 75 mg daily for 4 days, followed by naproxen 500 mg 2 times daily through postoperative day 30, although many other regimens exist for HO prophylaxis. Results: Using arthroscopy for the removal of symptomatic HO at the time of surgery allows for the management of concomitant intra-articular pathology and efficient and precise dissection of the undersurface of the HO fragment. Discussion/Conclusion: Heterotopic ossification is a well-characterized complication of soft tissue damage, including tendon avulsion, trauma, previous surgery, and chronic microtear of the hip musculature. Oftentimes, HO develops in a location that can be reached arthroscopically. Surgeons should consider combining intra-articular arthroscopic surgery with arthroscopic excision of HO when appropriate, noting that there are many advantages to arthroscopic removal when compared with open procedures. Use of Warmed Irrigation Solution in Arthroscopy: a Systematic Literature Review and a Perspective of Ten Years of Experience in Hip Arthroscopy

The number of hip arthroscopies has rapidly increased worldwide due to the new knowledge of intraarticular hip pathology. This procedure is successful with a low rate of complications but severe. One of the most important, from the anesthetic point of view, is hypothermia, carrying out a physiological response that could be dangerous for the patients. In hip arthroscopies, a warmed irrigation solution has been implemented as a simple, low-cost, and effective method for preventing hypothermia. There is limited literature on hip arthroscopy; therefore, much of the literature is focused on shoulder and knee arthroscopy. This systematic review of the literature aims to analyze the current evidence of warmed irrigation solutions in hip, shoulder, and knee arthroscopy. Eight studies of patients undergoing arthroscopy were included in the qualitative synthesis (6 shoulder studies, 1 hip study, and 1 knee study; 526 patients). Warm irrigation fluid reduced the degree of core body temperature drop and the incidence of hypothermia in 6 studies (5 shoulder studies and 1 hip study). In conclusion, warming irrigation fluids for shoulder and hip arthroscopy significantly decreased the risk of hypothermia, increased the lowest mean temperature, decreased the maximum temperature drop, and decreased the risk of shivering and hypothermia.

Treatment of Concomitant Intra-articular Pathology and Hip Dysplasia With Combined Hip Arthroscopy and Periacetabular Osteotomy Using a Single Radiolucent Traction Table Attachment Without a Perineal Post

Combined hip arthroscopy and periacetabular osteotomy are used for the treatment of concomitant intra-articular hip pathology and acetabular dysplasia or instability. Traditionally, the procedure has been achieved with the use of a traction table or table attachment for the arthroscopic portion and then subsequent transfer of the patient to a fully radiolucent flat bed. In this article, we highlight the technique of a combined hip arthroscopy and periacetabular osteotomy procedure using a single bed attachment system.

Amorphous calcium deposits of the hip joint: current observations and state of the scientific discussion.

As a result of increasing hip arthroscopies, rare pathologies as intra-articular amorphous calcium deposits in the capsule-labral (perilabral) recess can be recognized. There is a lack of publications on this pathology. The largest case series included 18 patients. An association between femoroacetabular impingement syndrome (FAIS) and female sex was observed. Furthermore, a correlation between the size of the calcific deposit and the preoperative hip function score was reported. Our hypothesis was that the data of our patient collective with intraoperative amorphous calcium deposits of the hip joint are comparable to the existing data to confirm previous observations. From 01/2018 to 08/2020, a total of 714 hip arthroscopies were performed. 12 (1.7%) patients who presented intra-articular amorphous calcium deposits during arthroscopy were included. On radiographs, signs of impingement and osteoarthritis were determined. Characteristics and size of the calcific deposits were examined. Preoperative and at the time of follow-up (23months), patient-reported outcome scores (PROS) were evaluated. Duration of symptoms, pain medication, comorbidities, and return-to-work were evaluated too. The PROS of the four female and eight male patients improved significantly. The average size of the calcific deposit was 6.9mm in the anteroposterior radiographs. Separation of the calcific deposit from the acetabular rim was seen in nine cases. No correlation between deposit sizes and PROS was found. Cam morphology was treated in ten cases. All patients returned to work after a median of 7weeks (2.5-13weeks). Amorphous calcium deposits were found in approximately 1% of all hip joints with indication for hip arthroscopy. They are not consistently associated with gender, intra-articular hip pathologies or comorbidities. The clustered occurrence in cam FAI can be justified solely by the fact that impingement is by far the most common indication for hip arthroscopy.

Core Muscle Injury: Evaluation and Treatment in the Athlete

Background: Pain in the groin region, where the abdominal musculature attaches to the pubis, is referred to as a “sports hernia,”“athletic pubalgia,” or “core muscle injury” and has become a topic of increased interest due to its challenging diagnosis. Identifying the cause of chronic groin pain is complicated because significant symptom overlap exists between disorders of the proximal thigh musculature, intra-articular hip pathology, and disorders of the abdominal musculature. Purpose: To present a comprehensive review of the pathoanatomic features, history and physical examination, and imaging modalities used to make the diagnosis of core muscle injury. Study Design: Narrative and literature review; Level of evidence, 4. Methods: A comprehensive literature search was performed. Studies involving the diagnosis, treatment, and rehabilitation of athletes with core muscle injury were identified. In addition, the senior author’s extensive experience with the care of professional, collegiate, and elite athletes was analyzed and compared with established treatment algorithms. Results: The differential diagnosis of groin pain in the athlete should include core muscle injury with or without adductor longus tendinopathy. Current scientific evidence is lacking in this field; however, consensus regarding terms and treatment algorithms was facilitated with the publication of the Doha agreement in 2015. Pain localized proximal to the inguinal ligament, especially in conjunction with tenderness at the rectus abdominis insertion, is highly suggestive of core muscle injury. Concomitant adductor longus tendinopathy is not uncommon in these athletes and should be investigated. The diagnosis of core muscle injury is a clinical one, although dynamic ultrasonography is becoming increasingly used as a diagnostic modality. Magnetic resonance imaging is not always diagnostic and may underestimate the true extent of a core muscle injury. Functional rehabilitation programs can often return athletes to the same level of play. If an athlete has been diagnosed with athletic pubalgia and has persistent symptoms despite 12 weeks of nonoperative treatment, a surgical repair using mesh and a relaxing myotomy of the conjoined tendon should be considered. The most common intraoperative finding is a deficient posterior wall of the inguinal canal with injury to the distal rectus abdominis. Return to play after surgery for an isolated sports hernia is typically allowed at 4 weeks; however, if an adductor release is performed as well, return to play occurs at 12 weeks. Conclusion: Core muscle injury is a diagnosis that requires a high level of clinical suspicion and should be considered in any athlete with pain in the inguinal region. Concurrent adductor pathology is not uncommon.

Therapeutic and diagnostic arthroscopy of the hip joint (first experience)

The experience in therapeutic diagnostic hip arthroscopy is presented. Hip arthroscopy was performed in 11 patients, aged 3-11 years. There were 5 patients with congenital hip joint dislocation, 3 patients with Legg-Kalve-Perthes disease, 2 patients with deforming coxarthrosis and 1 patient with pathologic hip dislocation. The original technique for hip arthroscopy is suggested. High efficacy of arthroscopy for the diagnosis and treatment of intraarticular hip pathology is shown.

Isolated Endoscopic Gluteus Medius Repair Can Achieve Successful Clinical Outcomes at Minimum 2-Year Follow-up

PurposeTo report on clinical presentations and outcomes in patients who underwent an isolated endoscopic gluteus medius (GM) repair.MethodsWe retrospectively reviewed and prospectively collected data on patients who underwent a primary isolated endoscopic GM repair. Patients were included if the following patient-reported outcome scores were obtained preoperatively and at minimum 2-year follow-up: modified Harris Hip Score (mHHS), Non-arthritic Hip Score (NAHS), and visual analog scale (VAS) score. The minimal clinically important difference (MCID) for the mHHS, NAHS, and Hip Outcome Score–Sports Specific Scale was uniquely calculated for this study. Patients who underwent concomitant procedures including hip arthroplasty and hip arthroscopy were excluded.ResultsA total of 26 hips met all inclusion and exclusion criteria, of which 23 hips (88.5%) (22 patients) had a minimum 2-year follow-up. Nineteen hips had a partial-thickness GM tear; 17 (89.5%) were high-grade partial-thickness tears treated with a side-to-side repair through a transtendinous window. Four full-thickness tears with no retraction or fatty infiltration were able to be repaired endoscopically. The mHHS, NAHS, and VAS score all significantly improved (P < .05) from baseline: The mHHS improved from 55.9 to 75.4 (P = .005); the NAHS, from 58.8 to 79.2 (P = .003); and the VAS score, from 5.9 to 3.6 (P = .009). The MCID was calculated for the mHHS, NAHS, and Hip Outcome Score–Sports Specific Scale as 5.8, 7.1, and 11.0, respectively. Most patients achieved the MCID for all 3 patient-reported outcome scores.ConclusionsAppropriately selected patients without concomitant intra-articular hip pathology may achieve successful outcomes at a minimum 2-year follow-up after an isolated endoscopic GM repair. Most isolated endoscopic GM repairs were performed for partial-thickness GM tears.Level of EvidenceLevel IV, case-series study.

Hip arthroscopy and T-shaped capsular plication for the treatment of borderline dysplasia: a minimum 2-year follow-up.

The aim of the study was to evaluate the clinical outcomes of patients treated arthroscopically for symptomatic borderline dysplasia of the hip (BHD) with a T-shaped capsular plication, with a minimum follow-up of 24 months. Twenty-seven patients who had undergone hip arthroscopy and T-shaped capsular plication for symptomatic BHD were included in the study. All patients were clinically evaluated prior to surgery (T0) and at two consecutive follow-ups (T1: 12 ± 1.2 months; T2: 52.0 ± 23.2 months) using the visual analogue scale (VAS) pain score, the Hip Outcome Score in activities of daily living (HOS-ADL) and sport-specific subscale (HOS-SSS), the Western Ontario and McMaster Universities Osteoarthritis Index (WOMAC) and Short Form-12 in its physical (PCS) and mental component scores (MCS). We also evaluated the presence of correlations and analysed the possible differences between subgroups. We found a significant difference for HOS-ADL, HOS-SSS, VAS and WOMAC with the Friedman test for repeated measures through time points (p < 0.0001), but not for MCS. All clinical scores reported a significant improvement compared to the previous time point (p < 0.05), with the exception of HOS-SSS, for which p = 0.2570 between T1 and T2. The body mass index was positively correlated with preoperative VAS (R = 0.42857; p = 0.0257) and negatively correlated with PCS at T0 (R = - 0.40999; p = 0.0337) and with PCS (R = - 0.40491; p = 0.0362), MCS (R = - 0.45939; p = 0.0159) and WOMAC (R = - 0.40573; p = 0.0216) at T2. No complications were reported in our cohort. The results of the current study confirm that patients with intra-articular hip pathology and concomitant BHD (LCEA 20°-25°) can benefit from arthroscopic treatment and that T-shaped capsular plication is one of the available options. Orthopaedic surgeons specialising in hip arthroscopy should consider arthroscopy for the treatment of symptomatic BHD for stabilising and strengthening the joint. Future studies should focus on finding out which type of patients can benefit most from this procedure. Prospective Case Series.

Excision of Heterotopic Ossification around the Hip: Arthroscopic and Open Techniques

Heterotopic ossification (HO) can occur as a complication of various pathologies affecting the hip including trauma, tendon avulsions, chronic injury, spinal cord injury, and soft-tissue disruption caused by surgery. When HO is present alongside intra-articular hip pathology such as femoroacetabular impingement syndrome (FAIS) or labral pathology, consideration should be made to combine the surgical excision of the HO with the FAIS decompression or labral repair if the location and size of the HO is appropriate for arthroscopic excision. Often times, the HO is located in such a position that any central compartment work can be completed before turning to the HO excision. If an open approach is required, the modified Gibson approach can be used for lateral hip access, whereas the Smith-Petersen approach provides anterior hip access. In this Technical Note we discuss arthroscopic techniques for excision of HO in the setting of concomitant FAIS, with discussion of when HO excision occurs in relation to cam decompression and labral repair, and mention tips on how to approach HO excision through an open approach.

Determining Trendelenburg test validity and reliability using 3-dimensional motion analysis and muscle dynamometry

BackgroundThe hip abductor muscle group stabilises the pelvis during gait to prevent excessive pelvic drop. Hip abductor weakness has been linked to musculoskeletal conditions such as chronic low-back pain. As such, it is important that practitioners can correctly diagnose hip abductor weakness in a clinical setting. Although the Trendelenburg test is commonly used by practitioners, the validity of this test to assess hip abductor weakness in the absence of musculoskeletal injury remains questionable. The aim of this study was to determine the validity of the Trendelenburg test, as observed by a practitioner, to assess frontal plane pelvic motion and hip abductor strength in a population without intra-articular hip disorders.MethodsThis study was performed between June 14th and October 16th 2019. Eighteen participants were recruited for this study. Peak normalised isometric and isokinetic hip abductor torque were measured bilaterally (n = 36) using the Biodex System 4 isokinetic dynamometer. Each participant performed the Trendelenburg test bilaterally (n = 36) while a graduate year chiropractic practitioner assessed for a “positive” or “negative” sign. The test was simultaneously recorded using Vicon 3-Dimensional motion capture to measure frontal plane pelvic motion and elevation. Correlation analyses were performed between the measures of peak hip abductor torque and pelvic motion to determine if any relationship existed. Agreement between the practitioner and 3-Dimensional analysis was calculated using the kappa (κ) statistic.ResultsWeak, non-significant correlations were found between hip abductor strength and pelvic motion before outlier removal. Significant (p < 0.05) yet weak correlations were found after outlier removal, except for isometric hip abductor strength. Weak agreement was found between the chiropractic practitioner and 3-Dimesnional analysis for the Trendelenburg test assessment (κ = 0.22–0.25).ConclusionsThis study found no significant relationship between normalised peak isometric and isokinetic hip abductor torque and frontal plane pelvic motion during the Trendelenburg test in a healthy young adult population. There was also poor agreement between the practitioner and pelvic motion assessments. Caution should be used when using this test, in the absence of intra-articular hip pathology, to assesses hip abductor weakness. Before any definitive conclusion can be made, studies with a larger sample size should be performed.

Arthroscopic classification of intra-articular hip pathology demonstrates at best moderate interrater reliability.

The purpose of this study was to report several novel classification systems for intra-articular lesions observed during hip arthroscopy, and to quantify the interrater reliability of both these novel systems and existing classifications of intra-articular lesions when tested by a group of high-volume hip arthroscopists. Five hip arthroscopists deliberated over shortcomings in current classification systems and developed several novel grading systems with particular effort made to capture factors important to the treatment and outcomes of hip arthroscopy for labral injury. A video learning module describing the classifications was then developed from the video archive of surgeries performed by the senior author and reviewed by study participants. Following review of the module, a pilot study was completed using five randomly selected videos, after which participating surgeons met once more to discuss points of disagreement and to seek clarification. The final video collection for testing reliability was composed of 29 videos selected with the intent of capturing all sublevels of each classification scheme. Study participants recorded their assessments using each classification scheme, and interrater reliability was calculated by a study participant not involved in grading. The average kappa coefficients for the classification schemes ranged from 0.38 to 0.54, with the interrater reliability of all classification schemes except labral degeneration qualifying as moderate. The percent of cases with absolute agreement ranged from 17.2% to 51.7% across the classification systems. Even among a group of high-volume hip arthroscopists who engaged in several discussions about the proposed classification schemes, grades were found to have at best moderate interrater reliability. Moderate interrater reliability is demonstrated for novel grading systems for describing labral tear complexity, labral bruising, labral size, and extent of synovitis, and fair reliability is demonstrated for labral degeneration. Further development and refinement of multifactorial grading systems for describing labral injury are indicated. Evaluating the multifactorial nature of intra-articular lesions in the hip is an important part of intraoperative decision-making and defining reliable classifications for intra-articular lesions is a critical first step towards developing generalizable criteria for guiding treatment type. Responsiveness and ceiling effects are key properties of an outcome score. No such data have been reported for the original English version of the International Hip Outcome Tool 12 (iHOT-12) at a follow-up of more than four months. The aim of this study was to identify the responsiveness and ceiling effects of the English version iHOT-12 in a series of patients undergoing hip arthroscopy for intra-articular hip pathology at a minimum of one year postoperatively. A total of 171 consecutive patients undergoing hip arthroscopy with a diagnosis of femoroacetabular impingement (FAI) under the care of a single surgeon between January 2013 and March 2017 were included. iHOT-12 and EuroQol 5D-5L (EQ-5D-5L) scores were available pre- and postoperatively. Effect size and ceiling effects for the iHOT-12 were calculated with subgroup analysis. A total of 122 patients (71.3%) completed postoperative PROMs scores with median follow-up of 24.3 months (interquartile range (IQR) 17.2 to 33.5). The median total cohort iHOT-12 score improved significantly from 31.0 (IQR 20 to 58) preoperatively to 72.5 (IQR 47 to 90) postoperatively (p < 0.001). The effect size (Cohen's d) was 1.59. In all, 33 patients (27%) scored within ten points (10%) of the maximum score and 38 patients (31.1%) scored within the previously reported minimal clinically important difference (MCID) of the maximum score. Furthermore, nine (47%) male patients aged < 30 years scored within 10% of the maximum score and ten (53%) scored within the previously reported MCID of the maximum score. There is a previously unreported ceiling effect of the iHOT-12 at a minimum one-year follow-up which is particularly marked in young, male patients following hip arthroscopy for FAI. Cystoscopy as a tool for hip arthroscopy for treating morbidly obese patients: a case report of treating a Sumo wrestler.

Sports medicine surgeons sometimes encounter morbidly obese athletes with femoroacetabular impingement, such as Sumo wrestlers. In such cases, traditional arthroscopic equipment will not reach the joint. This case report describes the use of a cystoscope to perform arthroscopy to treat borderline developmental dysplasia of the hip combined with cam impingement in a morbidly obese athlete. The cystoscope enables hip arthroscopy to be performed when traditional instruments are not of sufficient length to access the hip and/or an extra-long arthroscope is not available. The use of the cystoscope provides a practical, feasible and minimally invasive option to treat non-arthritic intraarticular hip pathology in the morbidly obese or extremely muscular athletes.

Practical office ultrasound for the hip surgeon: current concepts

Ultrasound imaging allows for real-time dynamic visualisation of muscles, tendons, fascial planes and joints. This modality enhances the sensitivity of physical examination for the localisation of pathology and improves clinical diagnostic accuracy. Ultrasound improves the accuracy of targeted injections without the need for fluoroscopy. This modality is economically affordable, portable, and does not expose the patient or provider to radiation. A thorough understanding of the anatomy and biomechanics of the hip and a working knowledge of both intra-articular and extra-articular hip pathologies is essential to optimising the utility of this modality. Ultrasound-guided injections allow for real-time visualisation of both the needle and soft tissue or intra-articular target and are superior to unguided hip injections. Improved patient care can be achieved through the skilled use of ultrasound by a hip surgeon, radiologist or ultrasound technician with appropriate training.
