<h3>Purpose</h3> Radiation therapy for locally-advanced cervical cancer consists of external beam radiation therapy and brachytherapy. While the recent introduction of image-guided brachytherapy and hybrid intracavitary and interstitial brachytherapy treatments (HBT) may potentially improve clinical outcomes, current literature describes the procedure requiring significant resources (such as general anesthesia, conscious sedation, and MRI-based treatment planning). With increases in treatment time and patient movements, there is greater potential for positional changes of the applicator and neighboring organs at risk (OAR), which can alter dosimetry. Furthermore, these resources might not be universally available to all radiation oncology centers that perform intracavitary cervical brachytherapy, potentially limiting the widespread implementation of HBT. Here we describe our experience with CT-guided interstitial needle placement in a typical freestanding outpatient setting, with an emphasis on minimization of patient movement and treatment duration. <h3>Materials and Methods</h3> All patients that were treated with HBT from June 2018 to May 2020 were included in this series. Initially, patients were taken to the operating room, for an exam under anesthesia and Smit sleeve placement by the radiation oncologist. The patients were determined to need interstitial brachytherapy either by exam under anesthesia, due to anatomy and tumor extent, or after the first brachytherapy treatment, due to dosimetry. All tandem, ovoid and needle insertions and HDR treatments were performed in a freestanding clinic, on the CT simulator table. The patients were treated using minimal sedation (oral lorazepam and oxycodone/acetaminophen). An indwelling urinary catheter was inserted into the bladder. The tandem and ovoids (Elekta) were inserted in the patient, and a vaginal balloon was substituted for packing. Multiple CT scans were performed while advancing the interstitial needles, the bladder was filled with 180 cc of diluted contrast, and a final CT scan was taken for planning purposes. The patients were shifted from the CT table to a surgical bed (single shift), and then transported to the treatment room while treatment planning was performed using Oncentra (Elekta) treatment planning software. After the treatment plan was approved, the bladder was refilled to 180 cc and treatment was delivered using a microSelectron HDR afterloader (Elekta). The applicator was removed and the patient was examined for bleeding. If any vaginal bleeding occurred, vaginal packing was inserted with pressure applied. Time stamps and clinical data were collected during each procedure. <h3>Results</h3> Sixty-three patients underwent placement of a total of 244 interstitial implants. The median time for the entire procedure, from the time of patient entry into the CT simulation room to exit of the HDR treatment room, was 70.0 minutes (mean 70.3, range 54-100). The median time after tandem and ovoid insertion to complete interstitial needle insertion was 9.0 minutes (mean 9.8, range 4-24), using median 2 needles (mean 1.93, range 1-3) and median 3 CT scans (mean 2.63, range 1-4). Bleeding occurred in 22.1% (54/244) of implants, which resolved after median 3 minutes (mean 3.33, range 1-8) of vaginal packing with pressure. All patients (100%) required only one total shift, from the CT table to the mobile surgical bed. <h3>Conclusions</h3> In our series, the interstitial insertion portion of the HBT added 9 minutes, with an overall procedure time of 70 minutes (including insertion, planning, treatment and removal). There was an acceptable bleeding rate which quickly resolved with vaginal packing and pressure. Combining intracavitary and interstitial implants using a hybrid applicator appears to be feasible, efficient and safe in an outpatient freestanding clinic. Further studies aimed at evaluating long-term toxicities and tumor control are warranted.

Pathological complete response with immunotherapy and brachytherapy to 15 metastatic liver lesions in a single patient.

Materials & methods:High dose rate interstitial brachytherapy (HDR-IBT) treatment plan for 15 metastatic liver lesions in a patient with pancreatic cancer was retrieved and analyzed for liver dose parameters and diaphragm dose. Serial 18F-FDG PET-CT scans were reviewed for disease response assessment and left liver lobe volume. Serial laboratory records were analyzed for liver parameters.Results:Left liver lobe volume increased from 241 cm3 pre-HDR-IBT to estimated 600 cm3 after seven sessions of HDR-IBT. Metabolic complete response (CR) and subsequently pathological CR was confirmed in the right hepatotectomy specimen for all the 15 PET-CT avid lesions treated with HDR-IBT. Maximum diaphragm dose in a single fraction was 82 Gy. The liver parameters were stable and patient did not develop radiation induced liver disease.Discussion:This is the largest reported series of HDR-IBT to liver lesions in a single patient. This first ever reported combined treatment of immunotherapy (IT) and HDR-IBT had likely rendered this patient disease free both at local the liver and systemically. Metabolic CR by PET-CT can be seen as early as 46 days after HDR-IBT. Diaphragm can tolerate very high doses of radiation and repeated treatment.Conclusion:In this patient HDR-IBT for multiple liver lesions with IT is well tolerated. PET-CT can be used for response assessment of HDR-IBT liver. Synergistic effect of IT with HDR-IBT and it’s role as bridging for liver resection has clinical potential and should be further studied in prospective trials.

Stereotactic Iodine-125 Brachytherapy for Low-Grade Glioma Treatment: A Monte Carlo study

Background: Stereotactic brachytherapy is an appropriate method that has been used for brain tumors and metastases treatment for more than 40 years for many patients in the world. Also, iodine-125 brachytherapy has been utilized in brain tumors for interstitial brachytherapy treatment since 1979. Even though the physical and biological features make these implants particularly attractive for minimal invasive treatment, the main goal of this paper is to evaluate the I-125 seed time and dose reached to brain glioma tumors of different sizes for treatment using Monte Carlo modeling. Methods: In this paper, Monte Carlo simulation has been applied by the Gate code with 20 (MBq) activity for an iodine seed design for low-grade glioma tumors treatment. Dosimetry features of this source were defined by the updated TG-43U1 recommendations. The absorbed dose distribution around the seed was calculated using the Gate code in liquid water. Result: The ideal condition for brachytherapy is for tumors smaller than 4 cm. With a larger tumor size, the absorption dose at the border of tumor and healthy tissue will be decreased and the implantation time for seeds will increase. Conclusion: Placing an iodine-125 source inside the tumor is not sufficient because of the non-uniform dose distribution in the tumor and the length of treatment time. Using four iodine-125 sources eliminates the tumor, and also, a uniform dose distribution is created in the tumor and the implantation time will be reduced, respectively.

Application of transrectal ultrasound in guiding interstitial brachytherapy for advanced cervical cancer.

PurposeTo investigate the role of transrectal ultrasound guidance in interstitial brachytherapy for cervical cancer.Material and methodsForty-eight patients who underwent interstitial brachytherapy treatment for cervical cancer between January 2017 and January 2018 were enrolled in the study. The distances between each inserted needle and the lesion were measured at seven sites by ultrasound (D1-D7) and compared to the corresponding distances (M1-M7) when visualised with nuclear magnetic resonance imaging (MRI). Measurements were paired on the basis of the observation sites, e.g. D1 and M1, D2 and M2. The statistical differences, intraclass correlation coefficients (ICCs), and linear relationships for the paired measurements were calculated.ResultsNo significant differences were found between the paired M and D measurements, with all ICCs showing high levels of concordance (0.81-0.93).ConclusionsTransrectal ultrasound showed strong agreement with MRI results in determining the position of the inserted needles. Transrectal ultrasound is a useful tool for guided interstitial brachytherapy and is appropriate for widespread use in the treatment of locally advanced cervical cancer.

Long-term effectiveness and safety of image-based, transperineal combined intracavitary and interstitial brachytherapy in treatment of locally advanced cervical cancer

PurposeThe aim of the study was to evaluate the impact of image-based combined intracavitary-interstitial brachytherapy (IC-ISBT) using a transperineal template in locally advanced cervical cancer treatment. Methods and MaterialsA total of 94 patients of cervical cancer stage IIB-IVA underwent image-based transperineal interstitial brachytherapy without tandem (ISBT) or with tandem (IC-ISBT) between June 2008 and June 2018 at our institution. After pelvic chemoradiation, 42 patients underwent ISBT and 52 IC-ISBT. Dosimetric data, clinical response, and toxicity records of these patients were reviewed. ResultsClinical stage distribution was as follows: IIB: 22.4% (21), IIIA: 10.6% (10), IIIB: 56.4% (53), and IVA: 10.6% (10). Mean high-risk clinical target volume was 75.72 cc, and mean cumulative equivalent of 2 Gy per fraction for high-risk clinical target volume was 81 Gy. The median followup was 35.5 months. Overall 3- and 5-year local control, disease-free survival (DFS), and overall survival (OS) were 84% and 84%, 69.1% and 62.9%, and 80.9% and 71.5%, respectively. Local control (90.4% vs. 76.2%; p = 0.048) and DFS (78.8% vs. 57.1%; p = 0.04) were significantly better in the patients of IC-ISBT arm. IC-ISBT (hazard ratio: 0.763; 95% confidence interval 0.217, 1.38; p = 0.046) and D90 dose >85 Gy (hazard ratio: 0.957; 95% confidence interval 0.927, 1.07; p = 0.037) were predictors of better DFS on univariate analysis. Overall survival was not affected significantly by any of the factors. Grade 3 and 4 late complications were recorded in 3.2% (3) of patients and were similar in both arms (p = 0.86). However, the mean rectum 2 cc dose was significantly lower in the IC-ISBT arm (p = 0.038). ConclusionsCombined IC-ISBT is a safe and effective approach to treat ICBT unsuitable cases. It integrates the benefits of ICBT to the adaptability of ISBT around various targets and should be practiced whenever feasible to provide superior outcomes in locally advanced cervical cancer.

Abstract Partial breast irradiation was introduced to reduce treatment times, improve convenience of radiotherapy and reduce radiation exposure to the breast and surrounding normal tissues. A number of techniques have been developed including interstitial brachytherapy, single catheter-based brachytherapy, intraoperative therapy, and 3 dimensional conformal or intensity modulated radiotherapy. The toxicity profile of partial breast irradiation has been dependent on the different techniques used. In general acute toxicity has been less than that observed with whole breast irradiation, particularly for intraoperative approaches where radiotherapy is delivered at the time of breast conserving surgery. Reports of late toxicity have varied. The majority of trials have observed no increase in late toxicity with interstitial brachytherapy and intraoperative treatment when compared to whole breast irradiation. Some studies of 3D conformal radiotherapy have reported an increase in late radiation toxicity such as telangiectasia of the skin and subcutaneous fibrosis of the breast. Limited data are available on cosmetic outcome and quality of life. Again, results to date have varied with some studies reporting similar cosmetic results and others indicating an increase in adverse cosmesis with partial breast irradiation when compared to whole breast irradiation. Efficacy and safety of percutaneous computed tomography-guided high-dose-rate interstitial brachytherapy in treatment of oligometastatic lymph node metastases of retroperitoneal space.

PurposeTo assess efficacy, safety, and outcome of computed tomography (CT)-guided high-dose-rate (HDR) interstitial brachytherapy in patients with oligometastatic lymph node metastases of the retroperitoneal space.Material and methods24 patients with a total of 47 retroperitoneal lymph node metastases from different primary tumors were treated with CT-guided interstitial brachytherapy using an 192Ir source (single fraction irradiation). Every three months after treatment, clinical and imaging follow-up were conducted to evaluate local control and safety.ResultsMedian follow-up was 9.6 months (range, 2.9-39.0 months). Local tumor control rate was 95.7%. The median diameter of the gross tumor volume was 2.2 cm (range, 1-8.6 cm), treated with a median D100 (minimal enclosing tumor dose) of 14.9 Gy (range, 4.5-20.6 Gy). One severe adverse event (grade three) was recorded.Cumulative median progression-free survival was 4.2 months (range, 1.4-23.7 months), and cumulative median overall survival after interstitial brachytherapy was 15.9 months (range, 3.8-39.0 months).ConclusionsCT-guided HDR interstitial brachytherapy is a safe and feasible method for local ablation of oligometastatic lymph node metastases of the retroperitoneal space, and might provide a well-tolerated additional therapeutic option in the multidisciplinary management of selected patients.

The impact of inter-fraction changes for perineal template-based interstitial gynecologic brachytherapy implants.

PurposePerineal template-based interstitial gynecologic brachytherapy (ISBT) treatments are evaluated to determine whether adaptive inter-fraction re-planning is beneficial and necessary to meet the treatment aims of the American Brachytherapy Society (ABS) consensus guidelines for interstitial brachytherapy. Adherence to the EMBRACE II protocol is also assessed.Material and methodsTen patients receiving radical intent treatment for locally advanced or recurrent gynecologic malignancies underwent a three-fraction ISBT treatment with an ABS-recommended prescription regimen of 21 to 24 Gy. Clinical treatment plans were created according to a computed tomography (CT) acquired immediately post-implant. The first fraction was delivered on the same day as the implant (Day 1). The remaining two fractions were delivered on the next day (Day 2), at least six hours apart. Prior to treating on Day 2, a verification CT was acquired, permitting assessment of over-night changes. The Day 2 CT was used to evaluate deviations in 2-Gy-per-fraction equivalent dose (EQD2) from the clinically intended dosimetry for clinical target volume (CTV), bladder, rectum, and sigmoid.ResultsFor all patients, the median (range) difference between the intended and the delivered dosimetry for the CTV D90% was 1.4 Gy10 (0.3-4.4 Gy10). For all normal tissues, the median (range) difference from the intended normal tissue dose was 2.6 Gy3 (0.1-15.5 Gy3). In all cases, the deviation from clinically intended dosimetry did not lead to a violation of recommended normal tissue dose guidelines. For two of 10 patients with large normal tissue differences (> 10 Gy3 from the intended dose), inter-fraction adaptive planning did improve the plan quality, but was not strictly required to meet the normal tissue dose planning aims.ConclusionsThe implementation of perineal template-based ISBT treatment without inter-fraction adaptive planning can be delivered to comply with the ABS normal tissue dose guidelines and EMBRACE II limits for prescribed dose.

Single-Institution Experience with Hdr Brachytherapy Techniques for Skin Cancer

As at the present moments there aren’t definite guidelines for interstitial and superficial brachytherapy treatments for skin carcinomas, based on good clinical results in 274 cases of skin cancer some possible dose fractionation regimes and techniques were presented here. Reaction to radiation, local tumor control and radiation complications were analyzed in each case.

Assessment of the implant geometry in fractionated interstitial HDR breast brachytherapy using an electromagnetic tracking system

PurposeDuring the partial-breast treatment course by interstitial brachytherapy, electromagnetic tracking (EMT) was applied to measure the implant geometry. Implant-geometry variation, choice of reference data, and three registration methods were assessed. Methods and materialsThe implant geometry was measured in 28 patients after catheter implantation (EMTbed), during CT imaging (EMTCT), and in each of up to n = 9 treatment fractions (EMTF(k), k = 1, 2,… n). EMTF(k) were registered to the planned implant reconstruction (CTplan) by using all dwell positions (DPs), the button centers, or three fiducial sensors on the patient's skin. Variation in implant geometry obtained from EMTF(k) was assessed for EMTbed, EMTCT, and CTplan. ResultsEMT was used to measure 3932 catheters. A duration of 6.5 ± 1.7 min was needed for each implant measurement (mean, 17 catheters) plus setup of the EMT system. Data registration based on the DP deviated significantly lower than registration on button centers or fiducial sensors. Within a registration group, there was a <0.5-mm difference in the choice of reference data. Using CTplan as reference for registration, the mean residual distance of DPs on EMT-derived DPs was found at 2.1 ± 1.6 mm (EMTbed), 1.3 ± 0.9 mm (EMTCT), and 2.5 ± 1.5 mm (EMTF(k)). ConclusionsEMT can assess the implant geometry in high-dose-rate interstitial brachytherapy breast treatments. EMTbed, EMTCT, and CTplan data can serve as reference for assessment of implant changes.

Treatment plan dosimetric impact after catheter stretch during multi-catheter interstitial breast brachytherapy.

PurposeNylon 6/6 interstitial brachytherapy catheters may stretch when exposed to moisture, mechanical tension, and body temperature. The purpose of this study is to evaluate the magnitude of catheter stretch during a course of multi-catheter interstitial breast brachytherapy for nylon 6/6 treatment catheters, and to assess the impact this has on treatment plan dosimetry.Material and methodsCatheters were exposed to water at 37°C for six days and the internal catheter length change (ΔL_W) was measured daily. Additionally, the measured internal catheter length change (ΔL_P), performed prior to each treatment fraction, for seven interstitial breast brachytherapy patients (total of 76 catheters) were retrospectively analyzed. The maximum length change seen in ΔL_P and ΔL_W were introduced as a source positional error to retrospective treatment plans, and treatment plan dosimetry was analyzed.Results ΔL_W and ΔL_P were on average +4.0% and +1.6% of the catheter lengths in water or tissue after 48 hours. Weak correlation was seen between the average ΔL_P per catheter and both the catheter length within tissue (ρ = 0.36, p = 0.0007), and the mid-catheter depth in tissue (ρ = 0.42, p < 0.0001). The D90CTV decreased 1.5% (p < 0.05) and 8.2% (p < 0.05) when the ΔL_P and ΔL_W were introduced to the initial plans.ConclusionsNylon 6/6 catheters stretch during a course of multi-catheter interstitial breast brachytherapy treatment. The observed stretch may affect treatment plan dosimetry, if the catheter internal length is only measured immediately after the insertion. Additional catheter length checks are recommended to verify the actual catheter internal length during the treatment.

In Vivo Rectal Wall Dosimetry in Gynecological HDR Brachytherapy Using a Semi-Flexible Rectal Probe Provided with MOSkin Dosimeters

Comparison between the BrachyVision calculated dose and dosimeter measurement was performed using CERR radiotherapy analysis software. Results: The dosimeter was reconstructed with 1 mm voxel spacing and a 3pixel median filter. Aside from the needle track itself, no fractures formed in the dosimeter from the insertion, resulting in a pristine readout free of major image artifacts. Line profiles show strong agreement between the calculated and measured doses across the entire distribution, with exceptionally high matching at the sharp gradients surrounding the site of insertion (Fig. 1). Gamma index calculation shows 99.9% of voxels passing at 3%/2mm criteria. Conclusions: This work introduces a new formulation of low-durometer, deformable Presage dosimeters, and demonstrates strong viability for their use in complete 3D validation of interstitial brachytherapy treatments. These dosimeters are able to address a growing need in brachytherapy dose planning, quality control, and likely treatment commissioning. Further work will test this dosimeter formulation with multiple needle insertions, anthropomorphically-correct anatomy, and varying deformation to closer represent real-life treatment scenarios.

A Brachytherapy Plan Evaluation Tool for Interstitial Applications

Radiobiological metrics such as tumor control probability (TCP) and normal tissue complication probability (NTCP) help in assessing the quality of brachytherapy plans. Application of such metrics in clinics as well as research is still inadequate. This study presents the implementation of two indigenously designed plan evaluation modules: Brachy_TCP and Brachy_NTCP. Evaluation tools were constructed to compute TCP and NTCP from dose volume histograms (DVHs) of any interstitial brachytherapy treatment plan. The computation module was employed to estimate probabilities of tumor control and normal tissue complications in ten cervical cancer patients based on biologically effective equivalent uniform dose (BEEUD). The tumor control and normal tissue morbidity were assessed with clinical followup and were scored. The acute toxicity was graded using common terminology criteria for adverse events (CTCAE) version 4.0. Outcome score was found to be correlated with the TCP/NTCP estimates. Thus, the predictive ability of the estimates was quantified with the clinical outcomes. Biologically effective equivalent uniform dose-based formalism was found to be effective in predicting the complexities and disease control.

Potential role of ultrasound imaging in interstitial image based cervical cancer brachytherapy.

In 2012, more than 500,000 cases of cervical cancer were diagnosed worldwide. Over three quarters of these cases occur in less developed countries [1]. Advancements in image-guided brachytherapy are resulting in improved outcomes and reduced morbidity for women with this disease, but its worldwide adoption is hampered by lack of accessibility to advanced imaging techniques. Ultrasound is emerging as a potential option for tumor visualization, brachytherapy catheter placement, and treatment planning. While additional work is needed, ultrasound can potentially serve as the sole imaging modality for catheter insertion and planning. This paper will review our current knowledge on the use of ultrasound in interstitial brachytherapy treatment for cervical cancer.

Validation of mathematical models for the prediction of organs‐at‐risk dosimetric metrics in high‐dose‐rate gynecologic interstitial brachytherapy

Given the complicated nature of an interstitial gynecologic brachytherapy treatment plan, the use of a quantitative tool to evaluate the quality of the achieved metrics compared to clinical practice would be advantageous. For this purpose, predictive mathematical models to predict the D2cc of rectum and bladder in interstitial gynecologic brachytherapy are discussed and validated. Previous plans were used to establish the relationship between D2cc and the overlapping volume of the organ at risk with the targeted area (C0) or a 1-cm expansion of the target area (C1). Three mathematical models were evaluated: D2cc = α*C1 + β (LIN); D2cc = α - exp(-β*C0) (EXP); and a mixed approach (MIX), where both C0 and C1 were inputs of the model. The parameters of the models were optimized on a training set of patient data, and the predictive error of each model (predicted D2cc - real D2cc) was calculated on a validation set of patient data. The data of 20 patients were used to perform a K-fold cross validation analysis, with K = 2, 4, 6, 8, 10, and 20. MIX was associated with the smallest mean prediction error <6.4% for an 18-patient training set; LIN had an error <8.5%; EXP had an error <8.3%. Best case scenario analysis shows that an error ≤ 5% can be achieved for a ten-patient training set with MIX, an error ≤ 7.4% for LIN, and an error ≤ 6.9% for EXP. The error decreases with the increase in training set size, with the most marked decrease observed for MIX. The MIX model can predict the D2cc of the organs at risk with an error lower than 5% with a training set of ten patients or greater. The model can be used in the development of quality assurance tools to identify treatment plans with suboptimal sparing of the organs at risk. It can also be used to improve preplanning and in the development of real-time intraoperative planning tools.

Monte Carlo characterization of biocompatible beta‐emitting glass seed incorporated with the radionuclide as a SPECT marker for brachytherapy applications

A glass seed consisting of the β−‐emitting radionuclide 90Y incorporated with radionuclide 153Sm as SPECT marker is proposed for potential application in brachytherapy in order to reduce the undesirable dose to healthy adjacent organs. The aim of this work is to determine the dosimetric characteristics, as suggested in the AAPM TG‐60/TG‐149 reports, for this seed using Monte Carlo simulation. Monte Carlo codes MCNP5, EGSnrc, and FLUKA were used to calculate the absorbed dose distribution around the seed. Dosimetric parameters, such as reference absorbed dose rate, radial dose function, and one‐dimensional (1D) and two‐dimensional (2D) anisotropy functions, were obtained. The computational results from these three codes are in agreement within 5.4% difference on average. The absorbed dose rate at the reference point was estimated to be 5.01 cGy h−1 μCi−1 and self absorption of YAS glass seed amounted to 30.51%. The results showed that, with thermal neutron bombardment of 5 hours in a typical flux, sufficient activity for applications in brachytherapy may be achieved. With a 5 mCi initial activity, the total dose of a YAS glass seed was estimated to be 1.38 Gy at 1.0 cm from the seed center. Comparing with gamma emitting seeds, the 90Y seed could reduce undesirable doses to adjacent organs, because of the rapid dose falloff of beta ray. Because of the high R90 value of 5.5 mm, fewer number of 90Y seeds will be required for an interstitial brachytherapy treatment using permanent implant, in comparison with other beta‐emitting seeds. PurposeTo develop a dose verification tool for high-dose-rate interstitial brachytherapy treatment planning in accelerated partial breast irradiation. Methods and MaterialsWe have developed a software tool for interstitial brachytherapy treatment planning assessment. The software contains a database of seven 192Ir source models and is able to estimate the dose distribution using the Task Group 43 and the Sievert integral algorithms. Dose–volume histogram analysis and dose quality assurance (QA) criteria including conformity (COnformal INdex [COIN] and conformation number [CN]), homogeneity (homogeneity index [HI]) parameters were implemented in the software to evaluate and to compare between the doses estimated by the two algorithms and a dose extracted from an external treatment planning system (TPS). ResultsThe tool was evaluated and validated on four clinical cases treated by high-dose-rate interstitial brachytherapy. The doses provided by the Task Group 43 and the Sievert integral algorithms were evaluated by establishing the dose–volume histogram analysis and then by calculating the QA criteria. The algorithms were validated by comparing the dose at different anatomic points with their corresponding dose points provided from TPS. The differences were considered in good agreement (within 5%). ConclusionsPretreatment dose verification is an important step in the QA of brachytherapy accelerated partial breast irradiation. A simple, fast, and accurate method of dose verification is therefore needed. The software proposed in this study could fulfill these requirements. In addition, it is freely available for using by anyone wishing to do a QA on any TPS.

The impact of prostate edema on cell survival and tumor control after permanent interstitial brachytherapy for early stage prostate cancers

Previous studies have shown that procedure-induced prostate edema during permanent interstitial brachytherapy (PIB) can cause significant variations in the dose delivered to the prostate gland. Because the clinical impact of edema-induced dose variations strongly depends on the magnitude of the edema, the temporal pattern of its resolution and its interplay with the decay of radioactivity and the underlying biological processes of tumor cells (such as tumor potential doubling time), we investigated the impact of edema-induced dose variations on the tumor cell survival and tumor control probability after PIB with the 131Cs, 125I and 103Pd sources used in current clinical practice. The exponential edema resolution model reported by Waterman et al (1998 Int. J. Radiat. Oncol. Biol. Phys. 41 1069–77) was used to characterize the edema evolutions previously observed during clinical PIB for prostate cancer. The concept of biologically effective dose, taking into account tumor cell proliferation and sublethal damage repair during dose delivery, was used to characterize the effects of prostate edema on cell survival and tumor control probability. Our calculation indicated that prostate edema, if not appropriately taken into account, can increase the cell survival and decrease the probability of local control of PIB. The magnitude of an edema-induced increase in cell survival increased with increasing edema severity, decreasing half-life of radioactive decay and decreasing photon energy emitted by the source. At the doses currently prescribed for PIB and for prostate cancer cells characterized by nominal radiobiology parameters recommended by AAPM TG-137, PIB using 125I sources was less affected by edema than PIB using 131Cs or 103Pd sources due to the long radioactive decay half-life of 125I. The effect of edema on PIB using 131Cs or 103Pd was similar. The effect of edema on 103Pd PIB was slightly greater, even though the decay half-life of 103Pd (17 days) is longer than that of 131Cs (9.7 days), because the advantage of the longer 103Pd decay half-life was negated by the lower effective energy of the photons it emits (∼21 keV compared to ∼30.4 keV for 131Cs). In addition, the impact of edema could be reduced or enhanced by differences in the tumor characteristics (e.g. potential tumor doubling time or the α/β ratio), and the effect of these factors varied for the different radioactive sources. There is a clear need to consider the effects of prostate edema during the planning and evaluation of permanent interstitial brachytherapy treatments for prostate cancer.

Patient-Specific Monte Carlo-Based Dose-Kernel Approach for Inverse Planning in Afterloading Brachytherapy

Brachytherapy planning software relies on the Task Group report 43 dosimetry formalism. This formalism, based on a water approximation, neglects various heterogeneous materials present during treatment. Various studies have suggested that these heterogeneities should be taken into account to improve the treatment quality. The present study sought to demonstrate the feasibility of incorporating Monte Carlo (MC) dosimetry within an inverse planning algorithm to improve the dose conformity and increase the treatment quality. The method was based on precalculated dose kernels in full patient geometries, representing the dose distribution of a brachytherapy source at a single dwell position using MC simulations and the Geant4 toolkit. These dose kernels are used by the inverse planning by simulated annealing tool to produce a fast MC-based plan. A test was performed for an interstitial brachytherapy breast treatment using two different high-dose-rate brachytherapy sources: the microSelectron iridium-192 source and the electronic brachytherapy source Axxent operating at 50 kVp. A research version of the inverse planning by simulated annealing algorithm was combined with MC to provide a method to fully account for the heterogeneities in dose optimization, using the MC method. The effect of the water approximation was found to depend on photon energy, with greater dose attenuation for the lower energies of the Axxent source compared with iridium-192. For the latter, an underdosage of 5.1% for the dose received by 90% of the clinical target volume was found. A new method to optimize afterloading brachytherapy plans that uses MC dosimetric information was developed. Including computed tomography-based information in MC dosimetry in the inverse planning process was shown to take into account the full range of scatter and heterogeneity conditions. This led to significant dose differences compared with the Task Group report 43 approach for the Axxent source.
