An Overview of the Research Reports on Addiction in Iranian Journals from 2010 to 2015.

BackgroundThe articles and research reports presented in the scientific journals are selected without a proper critical appraisal in many cases. This is a major problem, as it leads to the opinions of the experts being neglected in many of these scientific journals. A descriptive study was done in fall and winter of 2016.MethodsJournals were identified searching the electronic databases, including PubMed, Medline, Scopus, Google Scholar, and national Persian databases of SID and Magiran. All the articles on addiction published in the time span of 2010-2015 in 8 psychiatry, psychology, and addiction journals were included in this study. The journals included Journal of Behavioral Sciences, Iranian Journal of Psychiatry and Clinical Psychology, Journal of Fundamentals of Mental Health, and Journal of Research on Addiction in Persian, and Health and Addiction Journal, International Journal of Psychiatry and Behavioral Sciences, Journal of Research in Behavioral Sciences, and International Journal of Psychological Studies in English. These journals were evaluated according to their research design and methodology, scientific writing style, and the validity of the references. For all of the 264 articles, the research design and references were thoroughly evaluated.FindingsThe most frequent problems were seen in the introduction section, the sampling method of study, the exclusive criteria, analysis, and the limitations of the study.ConclusionMajor deficiencies in the articles on addiction in Iranian journals were seen. It can be prevented by including important items in the checklists published by some of the journals for reviewers.

The relationship between the services available to patients in primary care and at local psychiatric clinics and the use of coercion: recent findings from Northern Norway

Introduction: Psychiatric patients may be subjected to coercion in many different forms, including involuntary admission to psychiatric hospital, involuntary outpatient treatment, and involuntary treatment with medications [1]. The use of coercion in the psychiatric services involves a range of ethical, clinical, and legal issues [2,3]. The Norwegian authorities have stated that it is a goal to reduce the use of coercion in the psychiatric services, as it is believed that this will improve the services and increase the quality of care [4]. Purpose and methods: We review and discuss findings from studies on coercion in North Norway, focusing on the relationship between the services available to patients in primary care and at local psychiatric clinics and the use of coercion. Results and discussion: A lack of services at the municipal level might increase the use of coercion. For instance, approximately half of the involuntary admissions had been referred from doctors working at municipal out-of-hours clinics [5]. These doctors often felt pressured to commit patients to psychiatric hospital, as few other options were available at nights and week-ends [5-7]. The increased availability of other services at nights and week-ends could therefore possibly result in reduced levels of coercion. Having sufficient resources available at the secondary level might also reduce the amount of coercion patients are subjected to. For instance, an area that had beds available for emergencies at local psychiatric clinics had significantly fewer (95% CI for EXP(B)=1.133-2.206, p=0.005) involuntary admissions than a comparable area without such beds [8]. While much of the coercion of psychiatric patients takes place at the tertiary/hospital level, this study suggests that the availability of services at the primary and secondary levels might influence the level of coercion at the tertiary/hospital level. Conclusion: The present study suggests that increasing the availabilty of voluntary psychiatric services at the primary and secondary levels might represent one way of achieving the goal of reducing coercion in the psychiatric services. This relationship should be examined further in future research involving the North Norwegian psychiatric health services.

It has been associated with long-term devastating consequences such as impotence, gangrene, and urinary retention. Psychotropic medication, notably, trazodone, is well known for its side effect of priapism; however, antipsychotic medications have also been reported to cause priapism.1A 24-year-old African American man with a history of bipolar disorder presented with a severe manic episode associated with psychotic symptoms. He was on valproic acid with good results in the past, however had been noncompliant with his medication for 2 weeks prior to his presentation. He denied any substance use, and his urine toxicology screen was negative. There were no concomitant medical problems, and home medication list was restricted to valproic acid. The patient was hospitalized, and valproic acid was resumed. Olanzapine 5 mg at bedtime was started as an augmentation agent for his severe manic episode. On the mornings of Day 3 and 4 of hospital stay, the patient complained of prolonged, painless erection not associated with any sexual behavior or thoughts. These episodes lasted for around 30 minutes and resolved without any intervention. However, on Day 5, the erection was painful and did not resolve up to 2 hours after the patient woke up in the morning. The urology service was consulted and immediate evacuation of blood from the patient’s penis was performed and phenylephrine was injected, achieving complete flaccidity. The Naranjo algorithm determined a probable association between olanzapine and priapism.2 Olanzapine was discontinued, and the patient was continued on valproic acid. He stabilized with regard to his mood disorder in the next 3 days and did not have a recurrence in the symptoms of prolonged erections. Olanzapine was documented in the medical records to cause this adverse reaction and was never administered in the future. The patient has not had recurrence of priapism in the next 2 years after his initial episode.Priapism results from alpha1-adrenergic blockade in the corpora cavernosa, leading to a parasympathetic-mediated arteriodilation, as well as inhibition of the sympathetic system that leads to detumescence. This leads to an intracavernosal stasis, owing to inadequate venous outflow caused by the obstruction of subtunical venules, resulting in hypoxia, acidosis, and pain.3 Alpha2-adrenergic blockade exacerbates the priapism by stimulating the release of a nitric oxide-like substance, which further stimulates erections.4 Antipsychotic-induced priapism has been linked to the alpha1-adrenergic antagonism of these medications. She was diagnosed with a right-parasagittal parietal meningioma, and subsequently underwent surgery, which resulted in seizure frequency-reduction >90%. Six years later, at the age of 41, the neurological condition of the patient deteriorated, with significant increase in seizure frequency, and development of left hemiparesis and schizophrenia-like psychosis. Brain MRI revealed recurrence of a large meningioma, and another surgery was performed. After surgery, the patient became seizure-free. The psychotic symptoms were characterized by severe paranoid delusions of persecution and reference related to her neighbors and family members, and auditory hallucinations, to a lesser extent. Schizoaffective disorder and mood disorder (e.g., major depression) with psychotic features were ruled out. After initiation of treatment with antipsychotics (i.e., risperidone), the psychotic symptoms showed gradual improvement. Postoperative magnetoencephalography showed no epileptic discharge but prominent delta/theta activity surrounding the tumor, likely representing perilesional edema and dysfunctional cortex.DiscussionTumoral lesions, especially gangliogliomas and dysembryoblastic neuroepitheliomas, are commonly seen in the excised tissues of patients undergoing temporal lobe surgery who present with schizophrenia-like symptoms.1 However, extratemporal tumors have rarely been reported to be associated with schizophrenia-like disorders. In this study, we describe a patient who developed schizophrenia-like psychosis along with neurological deterioration associated with recurrence of a right parietal meningioma. A few studies have reported that right-sided tumors, including meningiomas affecting the parietal cortex2 were found in patients with delusional schizophrenia-like disorders. This is consistent with recent evidence indicating that the right parietal cortex is implicated in the pathogenesis of delusions and schizophrenia-like symptoms.3 In this context, it is interesting that our patient presented with a psychotic disorder with predominant paranoid delusions. Bipolar Disorder and Pregnancy: Maintaining Psychiatric Stability in the Real World of Obstetric and Psychiatric Complications

This article describes complex, real-life issues faced by a woman with bipolar I disorder who wished to bear a healthy child while remaining psychiatrically well. The therapeutic issues include balancing treatment decisions that affect fetal and maternal risks. The authors address the importance of carefully considering the patient’s history of response to medications when evaluating risks to maternal and fetal health. They discuss the role of the psychiatrist as a part of the treatment team faced with unpredictable but not unexpected complexities, such as miscarriage, abnormal or questionable prenatal screening tests, gestational diabetes, and the emergence of fetal decelerations, preterm labor, and psychiatric decompensation. The article presents and evaluates treatment decisions made in the setting of multiple obstetric and psychiatric complications that do not clearly fit published algorithms. The importance of incorporating family and social supports as an integral part of the treatment plan is emphasized.

1167 T important role of general practitioners in prescribing antidepressant medications and treating depression has been documented. However, the extent to which general practitioners are prescribing other types of psychotropic medications has received less emphasis. This study used data from August 2006 to July 2007 from the National Prescription Audit (NPA) Plus database of IMS to examine this question. IMS collects transaction information each month from approximately 36,000 retail pharmacies, representing about 70% of all retail pharmacies, which when weighted represent all prescriptions filled in retail outlets in the United States. Using a separate sample of retail pharmacy transactions that includes the physician’s Drug Enforcement Administration number, IMS assigns physician specialty information to obtain an estimate of the total number of prescriptions filled in retail pharmacies by medical specialty. As shown Figure 1, of the 472 million prescriptions for psychotropic medications, 59% were written by general practitioners, 23% by psychiatrists, and 19% by other physicians and nonphysician providers. General practitioners wrote prescriptions for 65% of the anxiolytics in the sample, 62% of the antidepressants, 52% of the stimulants, 37% of the antipsychotics, and 22% of the antimania medications. Conversely, psychiatrists and addiction specialists wrote prescriptions for 66% of the antimania medications, 49% of the antipsychotics, 34% of the stimulants, 21% of the antidepressants, and 13% of the anxiolytics. Pediatricians were included as general practitioners and wrote 25% of all stimulant prescriptions but only 3% of all other types of psychotropic medications (data not shown). Prescribing of psychotropic medications by nonpsychiatrists improves access to treatment. However, concerns remain about whether patients treated in the general medical setting are receiving treatment concordant with evidence-based guidelines, psychotherapy, adequate medication monitoring, and appropriate intensity of treatment.
