Consequences of Juvenile Idiopathic Arthritis on Single Leg Squat Performance in Youth.

Juvenile idiopathic arthritis (JIA) affects body structure and function outcomes that may increase the risk of acute joint injury. The purpose of this study was to examine single leg squat (SLS) biomechanics for youth with JIA and their healthy peers. The study design was a matched pair cohort study. Sixty-five youth (JIA n = 30; control n = 35) participated in this ethics-approved study. Participants performed 3 sets of 5 consecutive SLS tasks. Disease activity and functional status were assessed using the Juvenile Arthritis Disease Activity Score and Child Health Assessment Questionnaire. Indexed (most-affected leg [JIA]; dominant leg [control]) and contralateral extremity biomechanics were obtained using a 12-camera system. Outcomes included hip flexion/extension (FE), adduction/abduction (AA), and internal/external (IE) rotation range of motion (ROM). Data were analyzed using a multivariate random coefficient model in R (α⍺ = 0.05). A total of 29 matched pairs were analyzed. Youth with JIA had low disease activity and performed the SLS with a more internally rotated hip (indexed leg P = 0.023, β = -1.9°). Female participants displayed greater hip FE (indexed leg P = 0.015, β = -4.3°; contralateral leg P = 0.005, β = -4.8°) and IE ROM (indexed leg P = 0.021, β = -2.1°) than male participants. Associations were observed for body mass index and hip IE ROM (contralateral leg P = 0.001, β = -0.4°), knee flexion angle, and hip FE ROM (indexed leg P = 0.001, β = 0.4°; contralateral leg P = 0.001, β = 0.5°) and AA (indexed leg P = 0.010, β = 0.1°; contralateral leg P = 0.002, β = 0.2°). This study identified functional alterations for an SLS in youth with JIA. These findings support the use of physical therapy as part of a multidisciplinary management approach, to restore normal hip posture and movement.

No Differences Identified in Transverse Plane Biomechanics Between Medial Pivot and Rotating Platform Total Knee Implant Designs

BackgroundTotal knee arthroplasties (TKAs) using well-designed, fixed bearing prostheses, such as medial pivot (MP), have produced good long-term results. Rotating-platform, posterior-stabilized (RP-PS) mobile bearing implants were designed to decrease polyethylene wear. Sagittal and coronal plane TKA biomechanics are well examined and correlated to polyethylene wear. However, limited research findings describe this relationship in transverse plane. We assumed that although axial plane biomechanics might not be the most destructive parameters on polyethylene wear, it is important to clarify their role because both joint kinematics and kinetics in all 3 planes are important input parameters for TKA wear testing (International Organization for Standardization 14243-1 and 14343-3). Our hypothesis was that transverse plane overall range of motion (ROM) and/or peak moment show differences that reflect on wear advantages when compared RP-PS implants to MP designs. MethodsTwo groups (MPs = 24 and RP-PSs = 22 subjects) were examined by using 3D gait analysis. The variables were total internal-external rotation (IER) ROM and peak IER moments. ResultsNo statistically significant difference was demonstrated between the 2 groups in kinetics (P = .389) or kinematics (P = .275). ConclusionIn the present study, no wear advantages were found between 2 TKAs. Both designs showed identical kinetics at the transverse plane in level-ground walking. Kinematic analysis could not illustrate any statistically significant difference in terms of overall IER ROM. Nevertheless, kinematic gait pattern differences observed possibly reflect different patterns of joint surface motion or abnormal gait patterns. Thus, wear testing with various input waveforms combined with functional data analysis will be necessary to identify the actual effects of gait variability on polyethylene wear.

The shoulder remplissage procedure for Hill-Sachs defects: does technique matter?

This biomechanical study evaluated the effects of 3 remplissage techniques on shoulder stability and motion in a Hill-Sachs (HS) instability model. Cadaveric forequarters were tested on an active shoulder simulator. Three remplissage techniques were performed for 15% and 30% HS defects. Testing conditions included intact and 15% and 30% HS defects, and the 3 remplissage techniques: T1, anchors in the defect valley; T2, anchors in humeral head rim; and T3, anchors in valley with medial suture placement. Outcomes included stability, internal-external rotation range of motion (IE-ROM), and joint stiffness. All remplissage techniques improved shoulder stability. In 15% HS defects tested in adduction, T3 significantly reduced IE-ROM (P = .037), whereas T1 and T2 did also (mean IE-ROM reductions: T1, 14°; T2, 11°; T3, 21°), but not to significance (P ≥ .088). In abduction, no significant reductions in IE-ROM occurred (P ≥ .060). In 30% HS defects tested in adduction (mean reduction IE-ROM: T1, 11°; T2, 19°; T3, 28°) and abduction (mean reduction: T1, 9°; T2, 15°; T3, 21°), all techniques significantly reduced IE-ROM (P ≤ .046). All techniques increased joint stiffness from 100% to 320% beyond the Bankart repair alone. A significant increase in joint stiffness was observed for T3 compared with the 30% HS group (P = .004), whereas T2 trended toward an increase (P = .078). There was no significant increase in joint stiffness with T1 (P = .249). All remplissage techniques enhanced shoulder stability, restricted ROM, and increased joint stiffness. No significant differences were found between anchors placed in the valley (T1) vs those placed in the humeral head rim (T2). Medial suture placement (T3) resulted in the greatest joint stiffness values and mean restriction in motion.
