Effects of Periodontal Treatment in Patients with Periodontitis and Kidney Failure: A Pilot Study

Periodontitis and chronic kidney disease are both chronic inflammatory diseases and share some common risk factors. This 3-month pilot study aimed to clarify whether non-surgical periodontal therapy is beneficial in clinical, biochemical, and microbiological conditions in patients with periodontitis and kidney failure. Kidney failure patients with moderate to severe periodontitis were recruited from two hospitals. Treatment group received non-surgical periodontal therapy, and control group received oral hygiene instruction only. Outcome assessments were conducted 1 and 3 months after treatment. Non-parametric tests were used to analyze the patient-level data. Periodontal site-level assessments were analyzed by Student t-test and paired t-test. Statistical significance was set at p-value < 0.05. A total of 11 subjects completed the study. There was no significant difference between groups in all-cause mortality, cardiovascular events, infection events, systemic parameters, and serum biomarkers. Comparing to control group, clinical periodontal parameters, gingival crevicular fluid interleukin-1β (IL-1β) level and periodontal pathogens showed significant improvement in the treatment group. Non-surgical periodontal treatment did not change systemic outcomes in kidney failure patients, but changed the local micro-environment.

Low-level laser therapy increases interleukin-1β in gingival crevicular fluid and enhances the rate of orthodontic tooth movement.

The aim of this investigation was to evaluate the effects of low-level laser therapy on interleukin-1β (IL-1β) levels in gingival crevicular fluid and its correlation with orthodontic tooth movement. A split-mouth design was used in 10 subjects (6 female, 4 male) aged 14 to 25years, whose maxillary first premolars were extracted. A gallium-aluminum-arsenide semiconductor diode laser (wavelength, 940nm; energy density, 8J/cm2; power output, 100mW) delivered low-level laser therapy to the experimental canine undergoing distalization at 10 points. The control canine was distalized without low-level laser therapy. The experimental and control canines were distalized using a force of 150g provided by nickel-titanium closed-coil springs. Gingival crevicular fluid was collected at 5 time points from the control and experimental sides, and the levels of IL-1β were analyzed by enzyme-linked immuno-sorbent assay (ELISA). The distal movements of the maxillary canines were measured and compared. Increased levels of IL-1β were observed in the experimental canines compared with the control canines (P<0.001). Cumulative tooth movements over an 8-week experimental period were greater for the experimental canines (occlusogram and software, 4.450 and 4.4903mm, respectively) compared with the control canines (occlusogram and software, 2.025 and 2.0501mm, respectively). A positive correlation existed between the IL-1β levels and the amounts of tooth movement across all time intervals. In combination with light orthodontic force, application of low-level laser therapy increased the levels of IL-1β in gingival crevicular fluid and accelerated orthodontic tooth movement.

Influence of Smoking on Gingival Crevicular Fluid Interleukin 1β and Interleukin-8 in Patients with Severe Chronic Periodontitis among a Rural Population in India.

Background and Objective:Smoking is widely prevalent among rural Indian populations. Smoking is considered a significant risk factor for periodontal diseases. The aim of the present study was to evaluate the influence of smoking on the inflammatory cytokines interleukin (IL) IL-1β and IL-8 in gingival crevicular fluid (GCF) and compare these between smokers and nonsmokers with periodontitis in diseased and healthy sites.Materials and Methods:A total of thirty patients with severe chronic periodontitis (15 smokers and 15 nonsmokers) participated in this study. Clinical parameters assessed were gingival index, bleeding on probing, probing depth (PD), and clinical attachment level. One diseased and one healthy site from each of the periodontitis patients were selected for GCF collection and assigned to the following four groups: healthy sites in smoker (SH), diseased sites in smoker (SD), healthy sites in nonsmoker (NH), and diseased sites in nonsmoker (ND) and were analyzed by enzyme-linked immunosorbent assay test (Quantikine®).Results:Intragroup comparisons revealed statistically significant levels of IL-1β in ND sites compared to NH sites (P < 0.005) and SD sites showed statistically significantly higher levels of IL-1β compared to SH sites (P < 0.0001). However, there was no significant difference in IL-8 levels between NH and ND sites as well as between SH and SD sites. Intergroup comparisons revealed that SH sites exhibited significantly lower amounts of IL-8 (P < 0.01) compared to NH sites and SD sites showed significantly lower levels of IL-8 compared to ND sites (P < 0.001). However, the comparison of NH and SH sites as well as ND and SD sites showed no significant difference in the levels of IL-1β.Conclusions:Cytokine levels were significantly elevated in periodontitis patients. Smokers exhibited a decrease in IL-8 and increase in IL-1β levels. Hence, this reflects the influence of smoking on immunosuppression and its role in the pathogenesis of periodontal disease.

Gingival changes during pregnancy: III. Impact of clinical, microbiological, immunological and socio‐demographic factors on gingival inflammation

To identify predictor variables involved in exacerbated gingival inflammation associated with pregnancy. In this cohort study, 48 pregnant and 28 non-pregnant women without periodontitis were included. The pregnant women were evaluated in the first, second and third trimester and at 3 months postpartum, whilst the non-pregnant women were evaluated twice, with a 6-month interval. At each visit, clinical [plaque index (PlI) and gingival index (GI)], hormonal (salivary progesterone and estradiol), immunological [gingival crevicular fluid interleukin-1β, interleukin-6, tumour necrosis factor-α (TNF-α) and prostaglandin-E(2) ] and microbiological (periodontal pathogens culture) evaluations were performed. Statistical analysis was undertaken using exhaustive chi-square automatic interaction detection (exhaustive CHAID) to analyse the predictive value of the independent outcomes to develop pregnancy GI. PlI was the strongest predictor implicated in the GI throughout pregnancy and after delivery. During the second and third trimesters the presence of Porphyromonas gingivalis significantly contributed to the worsening of gingival inflammation. When compared with the non-pregnant group, significant differences were found in TNF-α amounts and concentrations and in the third trimester site-specific GI. Bacterial challenge to the gingival tissues, both quantitatively (PlI) and qualitatively (harbouring P. gingivalis) appears to affect the level of gingival inflammation observed during pregnancy.

Correlation of Levels of Interleukin-1β in Gingival Crevicular Fluid to the Clinical Parameters of Chronic Periodontitis

IL-1β is a potent stimulator of bone resorption and has been implicated in the pathogenesis of periodontal destruction. Therefore, this study was designed to compare the levels of IL-1β of chronic periodontitis patients with the healthy subjects. Another objective of this study was to correlate IL-1β levels with the clinical parameters of the periodontal disease progression. For this study, total 60 subjects were chosen (30- healthy and 30-chronic periodontitis). Simplified oral hygiene index (OHI-S), gingival index (GI), periodontal disease index (PDI), probing depth (PD), tooth mobility, bleeding on probing (BOP) were recorded for all the subject. Gingival crevicular fluid (GCF) was collected and subjected for ELISA for estimation of IL-1β. At the periodontal diseased sites, the IL-1β levels increased at least 2-fold as compared with healthy subjects. This increase was highly significant (p = 0.0000). Within the test group, IL-1β levels correlated positively and significantly with PDI, PD, BOP and tooth mobility. The correlations of IL-1β with PD (p = 0.000) and IL-1β with BOP (p = 0.0004) were highly significant. These data suggest that amount of GCF IL-1β is closely associated with periodontal status. This relationship may be valuable in monitoring periodontal disease activity. It could be stated from this study on IL- 1β that there seem to be a strong correlation between periodontal tissue destruction and IL-1β. Furthermore IL-1β level could also differentiate between active and inactive periodontal lesions.

Effect of a triclosan-containing dental gel on the levels of prostaglandin I2 and interleukin-1β in gingival crevicular fluid from adolescents with fixed orthodontic appliances

The effect of a triclosan-containing (0.3%) dental gel on inflammatory mediators in gingival crevicular fluid (GCF) was evaluated in 14 healthy adolescents undergoing orthodontic treatment with fixed appliances. A double-blind randomized split-mouth study design was used with color-coded experimental and placebo gels. The gel was self-applied for 5 r min twice daily for 14 days in custom-made soft plastic trays. Clinical data (visible plaque index (VPI) and gingival bleeding index (GBI) and samples of GCF were collected at baseline and after 1, 2, 4, and 6 weeks. The concentrations of prostaglandin I2 (PGI2) and interleukin-1β (IL-1β) were determined by radioimmuno- and enzyme-linked immunosorbent assays, respectively. No clinical effects of the gel applications regarding amount of plaque or gingival bleeding were unveiled. Neither the experimental nor the placebo gel applications caused any statistically significant alterations in the inflammatory mediators, PGI2 and IL-1β, compared to baseline. In conclusion, the present study did not reveal any beneficial effects of the triclosan-containing gel regimen on mild gingivitis in adolescents with fixed orthodontic appliances.

Effect of an antibacterial dental varnish on the levels of prostanoids, leukotriene B4, and interleukin-1β in gingival crevicular fluid

The aim of this study was to investigate the effects of a chlorhexidine/thymol-containing dental varnish on the levels of prostaglandin E2 (PGE2), prostaglandin I2 (PGI2), leukotriene B4 (LTB4), and interleukin-1β (IL-1β) in gingival crevicular fluid (GCF). The material consisted of 15 adolescents undergoing treatment with fixed orthodontic appliances. Four buccal sites adjacent to bands or brackets and exhibiting a mild chronic gingival inflammation were selected in the upper quadrants of each patient. According to a splitmouth technique, the first and second quadrants were randomly treated with either a varnish (Cervitec®) containing 1% chlorhexidine diacetate and thymol (CHX/thymol) or a placebo varnish without active ingredients. The varnishes were applied immediately after the baseline registration, and follow-up examinations were carried out after 3, 8, and 30 days. GCF was sampled with the aid of a paper strip and the volume was determined using a Periotron® 8000. The concentrations of PGE2, PGI2, LTB4, and IL-1β in GCF were assessed using radioimmunoassay and ELISA techniques. The results unveiled statistically significant reductions of PGE2, PGI2, and LTB4 levels in GCF following the active varnish treatment when compared to baseline values. A slight drop in IL-1β levels was registered after both active and placebo varnish applications, but the differences were not significant. The results suggest that treatment with an antibacterial varnish decreases the levels of inflammatory mediators PGE2, PGI2, and LTB4 in gingival crevicular fluid and further support the concept that topical application of a CHX/thymol-containing varnish is beneficial in patients with chronic gingival inflammation.
