Primary Care 2.0: Design of a Transformational Team-Based Practice Model to Meet the Quadruple Aim.

A new transformational model of primary care is needed to address patient care complexity and provider burnout. An 18-month design effort (2015-2016) included the following: (1) Needs Finding, (2) Integrated Facility Design, (3) Design Process Assessment, and (4) Development of Evaluation. Initial outcome metrics were assessed. The design team successfully applied Integrated Facility Design to primary care transformation design; qualitative survey results suggest that design consensus was facilitated by team-building activities. Initial implementation of Quadruple Aim-related outcome metrics showed positive trends. Redesign processes may benefit from emphasis on team building to facilitate consensus and increased patient involvement to incorporate patient voices successfully.

Utilizing integrated facility design to improve the quality of a pediatric ambulatory surgery center

The aim was to use Integrated Facility Design (IFD) to design a surgery center that enhances the delivery of health care by developing processes that provide highly efficient patient, family, and provider flows while adding value and utilizing costly resources effectively. Integrated Facility Design is an adaptation of the Toyota 3P (Production, Preparation, Process) Program. The goal of IFD is to accelerate development time and lower start-up costs. The use of IFD produced a savings of $30 million in project costs and enabled a completion date 3.5 months ahead of schedule. The designed patient flow processes resulted in dramatic improvements in patient, family, and provider throughput. The use of IFD in the design of a pediatric ambulatory clinic and surgery resulted in significant cost savings and improved clinical efficiency.

Facility description language for integrating distributed designs

A principal goal of concurrent engineering has been to reduce the development cycle and time-to-market for new products. The success of such efforts brings about a parallel need for rapid design, evaluation, and reconfiguration of the facilities required to manufacture these products. Traditional facility layout methods rely on a sequence of actions progressing through a series of increasingly detailed analyses. More recent techniques automate and improve this process. However, there has been little progress toward an integrated facility design environment that would support the interactions of multidisciplinary designer teams. Such teams have been created with significant benefits for realizing the concurrent engineering process, and can be more effective if given a common design language. This article describes the generic Facility Description Language (FDL), which provides a common formal base for collaborative work and detailed analysis of process layout, flow geometry, visual aspects of control and safety. FDL augments the capabilities of simulator-emulator design workstations to integrate the physical and logical aspects of production facility design. Specifically, our research has defined the basic features and functions of FDL and resulted in a prototype implementation architecture. FDL can serve as a useful structure for the definition and description of a production facility at different levels of abstraction, allowing the same basic design model to be iteratively refined throughout the development process. It is demonstrated how an interactive graphical environment provides a powerful modelling tool that permits distributed users to achieve a higher level of design synergy.
