Urethral cutaneous fistula correction using an autologous tunica vaginalis pedicle flap in a four-year-old intact male German shepherd dog.

Urethral cutaneous fistulas are rarely reported in dogs. Several techniques have been described to treat urethrocutaneous fistulas with different degrees of success. This case report is on a four-year-old male intact German shepherd diagnosed with urethrocutaneous fistulas after two weeks of dysuria. The urethra was repaired using an autologous tunica vaginalis pedicle flap after open castration and scrotal ablation. The dog was catheterised for two weeks after which a positive contrast urethrogram revealed a healed urethra with no stricture or leakage into surrounding tissue. The dog was discharged the following day after normal urination. Follow-up examination with the referring veterinarian at five months confirmed that the dog was clinically healthy and urinating normally. Literature about urethrocutaneous fistulas is lacking in the small animal field. To our knowledge there are no reports in the literature describing the use of an autologous tunica vaginalis pedicle flap for the repair of a urethral defect in dogs. This case report describes a novel technique to treat urethrocutaneous fistulas in an intact male dog.

Pulmonary veno-occlusive disease as a cause of severe pulmonary hypertension in a dog

BackgroundPulmonary veno-occlusive disease (PVOD) is a rare cause of pulmonary arterial hypertension (PAH) in humans and can be classified in idiopathic, heritable, drug and radiation-induced, and associated with connective tissue disease or human immunodeficiency virus infection. Recently, biallelic mutations of the EIF2AK4 gene have been discovered as a cause for an autosomal recessive form of PVOD in humans. In dogs, PAH is poorly characterized and is generally considered to be idiopathic or secondary to (for example) congenital left-to right cardiovascular shunts or heartworm disease. However, recently, the pathologic features resembling human PVOD were retrospectively described in post-mortem lung samples of dogs presenting with respiratory distress and idiopathic pulmonary hypertension (PH), which suggests that PVOD contributes to an unknown percentage of cases with unexplained PH. In dogs, information on the clinical presentation of PVOD is scarce and the cause and pathogenesis of this disease is still unknown.Case presentationAn 11-year-old, intact male German Shepherd dog (GSD) was presented with a 2-day history of acute-onset dyspnea and generalized weakness. Physical examination, laboratory analysis, thoracic radiography, echocardiography, a computed tomography scan and an ante mortem lung biopsy demonstrated severe arterial hypoxemia and severe PH but were not diagnostic for a known disease syndrome. Based on the poor reaction to therapy with oxygen, sildenafil, pimobendan and dexamethasone the dog was euthanized. Histopathology of the lungs showed venous and arterial remodelling, segmental congestion of alveolar capillaries and foci of vascular changes similar to human pulmonary capillary hemangiomatosis, indicating that the dog suffered from PVOD. Whole genome sequencing analysis was performed on the case and a healthy GSD. Validation was performed by Sanger sequencing of five additional GSD's unknown for any form of respiratory stress and aged ≥ 10 years. No causal variants were found in the genes that are known to be involved in human PVOD and PAH.ConclusionsThis case report confirms that PVOD should be a diagnostic consideration in dogs presenting with dyspnea and unexplained PH. In the present case, no casual genetic mutations known to be involved in humans with PVOD and PAH were found.

A Case of Poorly Differentiated Cutaneous Canine Mast Cell Tumor with an Atypical Dermatologic Presentation

Canine cutaneous mast cell tumors occur as nodular dermal lesions. A condition of mastocytosis is also described in dogs. The purpose of this study is to report the clinical and histopathological findings in a dog with high grade non-nodular cutaneous mast cell tumor. An 11-year-old intact male German Shepherd dog presented a severe erythematous dermatitis and erythematous plaques on the left thoracic wall, rapidly increasing in thickness, ulcerating, and spreading to the head. The fine needle cytology was indicative of a round cell neoplasm. The histological appearance of the lesions was suggestive of a high grade cutaneous mast cell tumor. Neoplastic cells were immunoistochemically Tryptase positive and c-kit positive (focal cytoplasmic staining), with a high Ki67 index. On the basis of this report high grade canine cutaneous mast cell tumors may show an atypical non-nodular presentation and should be included in the differential diagnosis for different lesions grossly appearing as erythematous dermatitis/plaques.

Bifid phallus with complete duplication and a separate scrotum in a German shepherd dog: a case report

Diphallia or penile duplication is a rare congenital abnormality thought to result from duplication of the cloacal membrane in the early developmental stages in the uterus. The present case concerned a 4-year old intact male German shepherd dog. The patient presented with a complaint of paraphimosis. Clinical, laboratory, radiological and ultrasonographic examinations were carried out. Semen samples collected prior to surgery were examined. Following amputation, the penis with paraphimosis was sent for histopathological investigation. Karyotype analysis of the patient was also performed. As well as the first report of this abnormality in Turkey, this case report represents the first report of this condition in a German shepherd dog.

Diagnosis and treatment of blastomycosis affecting the nose and nasopharynx of a dog

A 2-year-old 38.9-kg (85.58-lb) sexually intact male German Shepherd Dog was examined because of a 4-month history of severe nasal swelling and nasal mucosa congestion. The signs were slowly progressive. Physical examination revealed that the dorsal aspect of the dog's nose was swollen and hard. Mucous membranes in both nostrils were hyperemic and edematous. Diagnostic investigation revealed severe nasal osteolysis and pyogranulomatous rhinitis and nasopharyngitis attributable to blastomycosis. Oral administration of itraconazole was initiated (5 mg/kg [2.27 mg/lb], q 12 h for 5 days and then q 24 h). After a treatment period of 3 months, the nose had regained its normal appearance. After 5 months of treatment, the Blastomyces infection was eliminated as confirmed by results of rhinoscopy and biopsy specimen examination. No relapse was evident within 1 year after discontinuation of treatment. In dogs, nasal and nasopharyngeal blastomycosis can result in severe osteolysis of the nasal bone. Resolution of disease can be achieved with oral administration of itraconazole for a period of at least 5 months.

Use of a percutaneous atrial septal occluder device for complete acute occlusion of an intrahepatic portosystemic shunt in a dog

A 3-month-old sexually intact male German Shepherd Dog was evaluated because of signs of depression, ataxia, and collapse. Clinicopathologic abnormalities included low serum BUN and albumin concentrations and high serum liver enzyme activities and plasma ammonia and serum bile acids concentrations. Abdominal ultrasonography revealed an intrahepatic portosystemic shunt (PSS). The dog was anesthetized; via a transjugular approach, guidewires and catheters were directed with fluoroscopic guidance to locate the shunt and determine its anatomic features. Minimal changes in portal vein pressure during temporary shunt balloon occlusion enabled complete shunt attenuation, which was performed by use of a self-expanding septal occlusion device that is typically used for treatment of atrial septal defects in humans. Following initial misplacement of the device, the procedure was repeated successfully 2 months later and resulted in complete shunt occlusion. One year after this second procedure, the dog was clinically normal and serum bile acids concentration was within reference limits. In certain dogs with intrahepatic PSSs, treatment with minimally invasive interventional techniques involving fluoroscopy may reduce the morbidity and mortality rates associated with more invasive surgical procedures.

Total hip replacement in a dog

An intact male German shepherd dog (4 y, body wt. 35.5 kg) was referred to the Animal Medical Center, Chonbuk National University with severe lameness, pain and gait abnormality on the right hind limb. Survey radiographs of the pelvis revealed dysplasia of the right coxofemoral joint with subluxation. The dog was surgically treated performing total hip replacement (THR) using Modular Biolox Canine Modular THR System. The dog began to bear weight and slowly returned to a normal exercise pattern 2 months after surgery. THR resulted in satisfactory clinical functions with 6 months follow up and no complications were observed. Canine modular THR could be a successful modality for the management of disabling conditions of the coxofemoral joint.
