Flow-resistive loading and diaphragmatic muscle function in term and preterm infants.

We aimed to assess diaphragmatic function in term and preterm infants with and without history of bronchopulmonary dysplasia (BPD), before and after the application of inspiratory flow resistive loading. Forty infants of a median (range) gestational age of 34 (25-40) weeks were studied. BPD was defined as supplemental oxygen requirement for >28 days of life. Seventeen infants were term, 17 preterm without history of BPD, and six preterm with a history of BPD. The diaphragmatic pressure-time index (PTIdi) was calculated as the mean to maximum trans-diaphragmatic pressure ratio times the inspiratory duty cycle. The PTIdi was calculated before and after the application of an inspiratory-flow resistance for 120 s. Airflow was measured by a pneumotachograph and the transdiaphragmatic pressure by a dual pressure catheter. The median (interquartile range [IQR]) pre-resistance PTIdi was higher in preterm infants without BPD (0.064 [0.050-0.077]) compared with term infants (0.052 [0.044-0.062], p = .029) and was higher in preterm infants with BPD (0.119 [0.086-0.132]) compared with a subgroup of preterm infants without BPD (0.062 [0.056-0.072], p = .004). The median (IQR) postresistance PTIdi was higher in preterm infants without BPD (0.101 [0.084-0.132]) compared with term infants (0.067 [0.055-0.083], p < .001) and was higher in preterm infants with BPD [0.201(0.172-0.272)] compared with the preterm subgroup without BPD (0.091 [0.081-0.108],p = .004). The median (IQR) percentage change of the PTIdi after the application of the resistance was higher in preterm infants without BPD (65 [51-92] %) compared with term infants (34 [20-39] %, p < .001). Preterm infants, especially those recovering from BPD, are at increased risk of diaphragmatic muscle fatigue under conditions of increased inspiratory loading.

Graded onset of parasternal intercostal inspiratory activity detected with surface electromyography in healthy young females and males.

Intramuscular recordings of single motor unit activity from parasternal intercostal muscles show a rostrocaudal gradient in timing and amplitude of inspiratory activity. This study determined the feasibility of surface electromyographic activity (EMG) to measure graded parasternal intercostal activity in young females and males during quiet breathing and breathing with inspiratory resistive loads. Surface EMGs were recorded from the 1st-to-5th parasternal intercostal muscles during 10 min of breathing. EMGs were processed to remove 50 Hz and electrocardiogram artifacts and integrated. Amplitude and onset time of inspiratory activity were measured from waveform averages triggered at the onset of inspiratory flow. Onset times were measured independently by two assessors, blinded to interspace and EMG scale, with excellent agreement (ICC3,k = 0.86). The onset of inspiratory activity in the 1st-to-3rd interspaces was at or within ∼400 ms of the start of inspiratory airflow, but activity in the caudal (4th and 5th) spaces was delayed by up to ∼1,000 ms (P < 0.001). There was no main effect of sex on onset time (P = 0.07), but an interaction with interspace (P < 0.001) revealed that inspiratory activity in the caudal interspaces was delayed by 15% of inspiratory time in female participants compared with 30% of inspiratory time in male participants. Inspiratory loads did not affect EMG onset time (P = 0.31). Thus, surface EMG is feasible to assess the onset time of inspiratory activity as a marker of inspiratory neural drive and pattern of activation across spaces, in both females and males.NEW & NOTEWORTHY We demonstrated that surface EMG is a valid method to measure graded inspiratory EMG in the parasternal intercostal muscles in healthy young male and female participants during quiet breathing and loaded breathing. Across the 1st-to-5th interspaces, there was more homogenous activation in women and more graded activity in men across parasternal intercostal muscles during breathing. By recording surface EMG from both male and female participants, we have revealed sex differences in inspiratory activity across intercostal muscles.

Thoracoabdominal asynchrony in a virtual preterm infant: computational modeling and analysis.

Thoracoabdominal asynchrony (TAA), the asynchronous volume changes between the rib cage and abdomen during breathing, is associated with respiratory distress, progressive lung volume loss, and chronic lung disease in the newborn infant. Preterm infants are prone to TAA risk factors such as weak intercostal muscles, surfactant deficiency, and a flaccid chest wall. The causes of TAA in this fragile population are not fully understood and, to date, the assessment of TAA has not included a mechanistic modeling framework to explore the role these risk factors play in breathing dynamics and how TAA can be resolved. We present a dynamic compartmental model of pulmonary mechanics that simulates TAA in the preterm infant under various adverse clinical conditions, including high chest wall compliance, applied inspiratory resistive loads, bronchopulmonary dysplasia, anesthesia-induced intercostal muscle deactivation, weakened costal diaphragm, impaired lung compliance, and upper airway obstruction. Sensitivity analyses performed to screen and rank model parameter influence on model TAA and respiratory volume outputs show that risk factors are additive so that maximal TAA occurs in a virtual preterm infant with multiple adverse conditions, and addressing risk factors individually causes incremental changes in TAA. An abruptly obstructed upper airway caused immediate nearly paradoxical breathing and tidal volume reduction despite greater effort. In most simulations, increased TAA occurred together with decreased tidal volume. Simulated indices of TAA are consistent with published experimental studies and clinically observed pathophysiology, motivating further investigation into the use of computational modeling for assessing and managing TAA.NEW & NOTEWORTHY A novel model of thoracoabdominal asynchrony incorporates literature-derived mechanics and simulates the impact of risk factors on a virtual preterm infant. Sensitivity analyses were performed to determine the influence of model parameters on TAA and respiratory volume. Predicted phase angles are consistent with prior experimental and clinical results, and influential parameters are associated with clinical scenarios that significantly alter phase angle, motivating further investigation into the use of computational modeling for assessing and managing thoracoabdominal asynchrony.

The detection and sensory perception of inspiratory resistive loads in people with chronic tetraplegia.

This study investigated sensations of breathing following tetraplegia. Fifteen people with chronic tetraplegia and fifteen healthy able-bodied controls matched for age, sex, height, and weight participated. Sensations of breathing were quantified by determining the threshold for detecting an added resistance during inspiration. In a separate task, the perceived magnitudes of six suprathreshold resistive loads were determined with a modified Borg scale. The detection threshold of 0.34 cmH2O/L/s [standard deviation (SD) 0.14] in the tetraplegia group was higher than the 0.23 cmH2O/L/s (SD 0.10) threshold for able-bodied controls (P = 0.004). Both participant groups perceived larger loads to be more effortful, with the Borg effort rating increasing linearly with the peak inspiratory pressure generated at each load. The relationship between Borg effort rating and peak inspiratory pressure was steeper in participants with tetraplegia than in able-bodied controls (P = 0.001), but there was no difference when pressure was divided by maximal inspiratory pressure (P = 0.95). Despite a higher detection threshold, the findings suggest that the perceived magnitude of a suprathreshold inspiratory load is not impaired in chronic tetraplegia and that load magnitude perception is related to the maximal, and not absolute, inspiratory muscle force.NEW & NOTEWORTHY Sensations of breathing are thought to be impaired following chronic tetraplegia. The detection threshold for an added resistive load during inspiration was higher in people with tetraplegia than in healthy able-bodied participants. However, for inspiratory loads above the detection threshold, the perceived magnitude of a resistive load as a function of the peak inspiratory pressure was greater in tetraplegia. Load magnitude perception was comparable between participant groups when peak pressure was divided by maximal inspiratory pressure.

Influence of continuous inspiratory resistive breathing trials on corticospinal excitability of lower limb muscles during isometric contraction

ABSTRACT BACKGROUND Increased work of breathing or fatigue of respiratory muscles has been suggested to reduce exercise performance. PURPOSE The present study investigated the effects of continuous inspiratory resistive breathing (IRB) on the responsiveness of corticospinal pathway innervating the vastus lateralis (VL) to determine whether respiratory muscle fatigue affects the central motor output to exercising limb muscles. METHODS Eight subjects underwent a 6-min IRB trial three times (IRB1–3) in two experiments. During each IRB trial, the subjects performed voluntary hyperventilation through a mask attached with an inspiratory resistive load (approximately 30% of maximal inspiratory pressure (PImax)). In Experiment 1, PImax was measured at baseline and after each IRB trial. In Experiment 2, VL responses to transcranial magnetic stimulation (motor evoked potentials (MEPs) and cortical silent periods were assessed during 5 s isometric knee extension at an intensity of 15% of the maximal voluntary contraction over the same time course as in Experiment 1. RESULTS PImax decreased from baseline after IRB2 and IRB3 (P < 0.05). MEPs increased from baseline after IRB1 and IRB2 (P < 0.05), whereas MEPs after IRB3 were not significantly different from baseline. Cortical silent periods did not change from baseline after IRB1 and IRB2, whereas cortical silent periods after IRB3 increased from baseline (P < 0.05). CONCLUSION These results suggest that as inspiratory muscle fatigue progresses, the corticospinal tract controlling the lower limb muscles can shift from excitatory dominance to inhibitory dominance as a whole, affecting the central motor output to working limb muscles.

Pathophysiological mechanisms of resistive breathing

AIM: This study aimed to explore the pathophysiological mechanisms of resistive breathing by using a model of a conditioned respiratory reflex to external resistance to breathing.&#x0D; MATERIALS AND METHODS: Inspiratory resistive loads were used 11, 28, 54, and 78 cmAq/l/s to model a conditioned respiratory reflex. External respiration was parametrized on the basis of the analysis of motor and ventilatory outputs. Conditioned signals were pure sounds exceeding the threshold of perception by 10 db at 2000 Hz frequency. All the test persons were divided into two groups (large and small groups) according to the initial reinforcement value. (1) In the large group (37 individuals), the conditioned reflex was formed from 11 cmAq/l/s that was subsequently increased stepwise in the load to 76 cmAq/l/s. (2) In the small group (18 individuals), the initial reinforcements were different gradations of resistive loads, with a stepwise transition to the other parameters of an unconditioned stimulus. The period of the isolated application of a conditioned signal (CS) was 20 s, the interval between signals was not fixed, varying from 2 min to 4 min. Six to eight combinations of the conditioned and unconditioned stimuli were used for 1 day of the experiment.&#x0D; RESULTS: The increase in the added respiratory resistance was associated with the pronounced reduction of pulmonary and alveolar ventilation, that is, with the hypoventilation type of resistive load realization. Changes in ventilation during the isolated application of a conditioned signal had an alternative character. In particular, as the reinforcement factor increased, a pronounced shift to hyperventilation was noted.&#x0D; CONCLUSION: The reinforcement value of the conditioned reflex changed stepwise, thereby significantly restructuring the proportion between the effectiveness of the adaptive activity in the realization of external resistance to inspiration (the time of stay under a certain load) and its physiological cost (totalities of the deviations of physiological and energy parameters).

Investigating the specificity of the neurologic pain signature against breathlessness and finger opposition.

Brain biomarkers of pain, including pain-predictive "signatures" based on brain activity, can provide measures of neurophysiological processes and potential targets for interventions. A central issue relates to the specificity of such measures, and understanding their current limits will both advance their development and explore potentially generalizable properties of pain to other states. Here, we used 2 data sets to test the neurologic pain signature (NPS), an established pain neuromarker. In study 1, brain activity was measured using high-field functional magnetic resonance imaging (7T fMRI, N = 40) during 5 to 25 seconds of experimental breathlessness (induced by inspiratory resistive loading), conditioned breathlessness anticipation, and finger opposition. In study 2, we assessed anticipation and breathlessness perception (3T, N = 19) under blinded saline (placebo) and remifentanil administration. The NPS responded to breathlessness, anticipation, and finger opposition, although no direct comparisons with painful events were possible. Local NPS patterns in anterior or midinsula, S2, and dorsal anterior cingulate responded to breathlessness and finger opposition and were reduced by remifentanil. Local NPS responses in the dorsal posterior insula did not respond to any manipulations. Therefore, significant global NPS activity alone is not specific for pain, and we offer insight into the overlap between NPS responses, breathlessness, and somatomotor demand.

ROLE OF RESPIRATORY RATE IN THE TOLERABILITY OF PERSONAL RESPIRATORY PROTECTION EQUIPMENT

The factor limiting the tolerance of personal respiratory protection equipment should be considered the frequency of respiratory movements, which reflexively changes when additional respiratory resistance occurs. Unfortunately, there is almost no information in the available literature about changes in the tolerability of personal respiratory protection equipment at different rates of respiratory movements. The purpose of this work was to study the tolerability of personal respiratory protection equipment when changing the frequency of respiratory movements.The study was conducted on practically healthy persons of both sexes (78 people), aged from 20 to 36 years. To simulate the conditions for the use of personal respiratory protection equipment, inspiratory resistive respiratory loads of 20% of the maximum intraoral pressure during the Mueller test were used. The tolerance of personal respiratory protection equipment was assessed using the Borg visual analogue of dyspnea scale, which reflected the level of subjective discomfort that occurs when additional respiratory resistance is turned on. During the action of additional respiratory resistance, the persons were asked to hold the frequency of respiratory movements, which was set using a special setting.An increase in the rate of respiratory movements against the background of additional respiratory resistance leads to a significant deterioration in the objective and subjective indicators of the functional state of the persons; replacing the inhaled air with an oxygen-rich respiratory mixture with carbon dioxide absorption did not lead to a significant improvement in the functional state. A moderate decrease (up to 70% of the initial frequency of respiratory movements) in the rate of respiration leads to an improvement in the indicators of adaptive activity in conditions of additional respiratory resistance. A significant decrease (up to 35% of the initial frequency of respiratory movements) in the rate of respiration under conditions of additional respiratory resistance leads to a deterioration of objective and subjective indicators of the functional state of the subjects.An increase in peak respiratory flow rates caused by an increase in the rate of respiration, in accordance with the well – known Rohrer equation, significantly increases inelastic resistance and, as a result, respiratory needs. These needs can be met at some time due to a significant increase in the work of the respiratory muscles, but due to fatigue of the latter, psychoemotional tension increases quite quickly and the use of personal respiratory protection equipment is abandoned.

Acute effects of inspiratory loading in older women: Where the breath meets the heart

We tested the hypothesis that inspiratory resistive loading (IRL) increases vagal-mediated complexity of heart rate variability (HRV) in older women. We recorded heart rate continually during 30 breaths with Sham or IRL (30 % of maximal inspiratory pressure) in sitting position. The normalized spectral power in the low (LFn) and high (HFn) frequency bands and the symbolic dynamics measures for 0 V, 2UV and 2 L V were obtained. HFn was higher and LFn was lower during IRL than Sham (p < 0.05). During IRL, 2UV component increased more than Sham (p < 0.05) while 0 V index remained unchanged (p> 0.05). In conclusion, acute IRL improved vagal modulation index of both linear (spectral analysis) and non-linear analysis (symbolic dynamics) in older women.

Acute effect of inspiratory resistive loading on sprint interval exercise performance in team-sport athletes

This study examined acute effects of inspiratory resistive loading (IRL) during rest intervals on sprint interval exercise (SIE) performance. In a randomized crossover design, nine collegiate basketball players performed IRL (15 cmH2O) or passive recovery (CON) at 5-min rest intervals during and immediately after 6 sets of a 30-s SIE test. Performance, muscular oxygenation of vastus lateralis, blood lactate and pH were measured at each condition. Blood lactate at 5-min (−20.5 %) and 20-min (−21.3 %) after SIE were significantly lower in IRL than in CON. The pH at 5-min after SIE was significantly higher in IRL than in CON (+0.8 %, p < 0.05). However, the total work in IRL was significantly lower than in CON (−2.7 %, p < 0.05). Average changes in total hemoglobin at rest intervals in IRL were significantly lower than in CON (−34.5 %, p < 0.05). The IRL could attenuate exercise-induced metabolic acidosis; however, the decreased blood flow at rest intervals might increase the physical challenge in SIE.

Difference in electromyographic activity between the trapezius muscle and other neck accessory muscles under an increase in inspiratory resistive loading in the supine position

The activity of the trapezius muscle is reportedly higher than that of other neck accessory muscles under a condition of increased inspiratory pressure in the standing position. The present study aimed to compare the activity of the trapezius muscle with those of the scalene and sternocleidomastoid muscles under a condition of increased inspiratory pressure in the supine position. This study included 40 subjects, and the muscle activity was measured using surface electromyography. Regarding the results, there was a significant difference in the muscle activity between the trapezius muscle and the scalene and sternocleidomastoid muscles (p = 0.003) in both men and women. Post-hoc analysis showed significant differences between trapezius and the other muscles. Moreover, there was no difference between the scalene and sternocleidomastoid muscles (p = 0.596). The increase in the change in electromyography activity of the muscle is greater in the trapezius muscle than in other muscles when the level of inspiratory pressure increases in the supine position.

The reliability of inspiratory resistive load magnitude and detection testing

ObjectivesTo assess the test-retest reliability of inspiratory load detection and load magnitude perception tests in healthy volunteers. DesignCohort of convenience. SettingRespiratory physiology laboratory. ParticipantsTwenty healthy adults. InterventionsOn two separate occasions participants performed tests of inspiratory loading. Participants breathed through custom made resistive tubing and were asked to indicate when they detected a different resistance during inspiration. In a second test participants rated the magnitude of presented inspiratory loads using the modified Borg score. Main Outcome MeasuresIntra-class Correlation Coefficient (ICC2,1) values for repeated tests of inspiratory load detection threshold and load magnitude rating. ResultsICC2,1 values ranged from 0.657–0.786 for load detection testing and 0.739 to 0.969 for rating of load magnitude. ConclusionsThe tests are simple and reliable measures of inspiratory load detection and magnitude rating. They can be used in future research to determine the effectiveness of interventions to reduce the effort of breathing in health and disease.

Inspiratory muscles contributions to postural control in healthy older women

It was hypothesized that induced inspiratory muscles effort, by an acute inspiratory resistance load (IRL) use, increases the postural sway and the neuromuscular activity of postural muscles in older adults. Fifteen older women breathed spontaneously, controlled (15 cycles/minute), and with acute IRL (15 cycles/minute and target load of 30 % of maximal inspiratory pressure) in random order. The breathing tasks were performed in an orthostatic position (eyes closed) during one minute on a baropodometer plate to acquire center-of-pressure (COP) oscillations and with electromyography (EMG) recording. The root mean square (RMS) of EMG was calculated. COP oscillations were higher with IRL use than spontaneous and controlled breathing tasks, but not between spontaneous and controlled conditions. EMG activity of Tibialis anterior and Soleus muscles were higher with IRL use in comparison to spontaneous and controlled breathing conditions. In conclusion, IRL use influenced the postural control through increased postural sway and neuromuscular compensation of the lower limbs in older women.

The threshold for detecting a rise in airflow resistance during tidal breathing is lower in older patients with COPD than in healthy people of similar age.

To investigate whether or not the threshold for subjectively detecting an increase in the resistance to airflow (LDT) during tidal breathing at rest rises in older age in patients with COPD, as it does in healthy people and asthmatics in remission. We conducted an open cross-sectional study of 31 older patients (age 55-89) with COPD and 60 healthy volunteers (age 55-86). Inspiratory and expiratory resistive load detection thresholds (ILDT and ELDT respectively) and spirometry were measured. The mean (SD) ILDT was 5.93 (7.02) kPa.s/L in COPD patients, compared to 11.11 (8.47) in healthy people (P < 0.001) in the same age range. There was no significant correlation between ILDT and age in the COPD group (r = -0.182, P = 0.326), though significant correlation with age was found in healthy people (r = 0.591, P < 0.001). ILDT and ELDT in COPD patients correlated significantly with the FEV1/FVC ratio (r = 0.367, P = 0.048 and r = 0.481, P = 0.007 respectively) but not with other spirometry indices, height, weight, BMI, oxygen saturation or smoking pack-years. LDT during tidal breathing appears to be sensitized, and thereby lower, in older COPD patients, possibly due to altered central regulation of the threshold or as a consequence of the effect lung compliance, recoil and volume changes have on afferent input from mechano-receptors in COPD. Older COPD patients with good cognition are therefore likely to be as aware of changing airways resistance as younger patients and take appropriate therapeutic action.

Dyspnea perception during the inspiratory resistive loads test in obese subjects waiting bariatric surgery

Identification of low dyspnea perception is relevant, since this condition is significantly associated with worse outcomes. We investigated dyspnea perception during the inspiratory resistive loads test on obese subjects waiting bariatric surgery in comparison with normal subjects. Secondarily, we analysed the proportion of obese subjects with low, moderate and high dyspnea perception. This observational study included subjects with body mass index (BMI) ≥ 35 kg/m2, compared to healthy subjects with BMI ≥ 18 and <25 kg/m2. Subject underwent clinical evaluation, inspiratory test with progressive resistive loads and spirometry. We studied 23 obese subjects (mean BMI = 51.9 ± 9.3 kg/m2) and 25 normal subjects (mean BMI = 24.3 ± 2.3 kg/m2). With the increase magnitude of resistive loads there was a significant increase in dyspnea score (p < 0.001) and progressive increase of the generated inspiratory pressure (p < 0.001), but there was no difference between the groups in terms of dyspnea score (p = 0.191) and no interaction effect (p = 0.372). Among the obese subjects, 4 individuals were classified as low perception, 11 as moderate and 8 as high. In conclusion, the degree of dyspnea perception during the inspiratory progressive resistive loads test did not differ between obese and normal subjects. Among obese subjects, only 17% were classified as low dyspnea perception.

Remote, Automated, and MRI-Compatible Administration of Interoceptive Inspiratory Resistive Loading.

Research on how humans perceive sensory inputs from their bodies (“interoception”) has been rapidly gaining momentum, with interest across a host of disciplines from physiology through to psychiatry. However, studying interoceptive processes is not without significant challenges, and many methods utilized to access internal states have been largely devoted to capturing and relating naturally occurring variations in interoceptive signals (such as heartbeats) to measures of how the brain processes these signals. An alternative procedure involves the controlled perturbation of specific interoceptive axes. This is challenging because it requires non-invasive interventions that can be repeated many times within a subject and that are potent but safe. Here we present an effective methodology for instigating these perturbations within the breathing domain. We describe a custom-built circuitry that is capable of delivering inspiratory resistive loads automatically and precisely. Importantly, our approach is compatible with magnetic resonance imaging (MRI) environments, allowing for the administration of complicated experimental designs in neuroimaging as increasingly required within developing fields such as computational psychiatry/psychosomatics. We describe the experimental setup for both the control and monitoring of the inspiratory resistive loads, and demonstrate its possible utilities within different study designs. This methodology represents an important step forward from the previously utilized, manually controlled resistive loading setups, which present significant experimental burdens with prolonged and/or complicated sequences of breathing stimuli.

Effect of Olfactory Stimulation by L-Menthol on Laboratory-Induced Dyspnea in COPD

Inhalation of L-menthol as a transient receptor potential melastatin 8 agonist induces a cooling sensation in the airway. This cooling sensation induced through olfactory stimulation by L-menthol (OSM) alleviates dyspnea in healthy humans. Therefore, we investigated effects of OSM on the neural respiratory drive (NRD) and multidimensional aspects of dyspnea induced by inspiratory resistive loaded breathing in COPD. In total, 28 patients with COPD and 14 control participants were included in this randomized, single-blinded, placebo-controlled, crossover study. OSM was administered using an L-menthol-scented patch, whereas placebo was administered using olfactory stimulation by strawberry-scented patch: placebo (OSP). Sham condition was measured by breathing through a face mask without a OSP. Dyspnea was evoked by breathing through the inspiratory resistance of 20 or 30cm H2O/L/s and measured using the Multidimensional Dyspnea Profile. NRD was estimated using the electromyogram activities of the parasternal intercostal muscles. Compared with sham and OSP, OSM did not alter the breathing pattern/timing and NRD during inspiratory resistive loading; however, it significantly alleviated the physical and mental breathing effort, air hunger, breathing discomfort, anxiety, and fear during inspiratory resistive loaded breathing in patients with COPD. However, although OSM reduced the air hunger, mental breathing effort, and unpleasantness, no significant improvement on affective dimension of dyspnea by OSM was observed in control participants. Our findings suggest that OSM activated the sense of inspiratory flow that achieved dyspnea relief in patients with COPD. GAS COMPOSITION OF ALVEOLAR AIR AND TOLERABILITY OF PERSONAL RESPIRATORY PROTECTION EQUIPMENT

Introduction. When using personal protective equipment for respiratory organs of an insulating type in case of depletion of a regenerative cartridge, hypoxic-hypercapnic changes in the gas composition of the body occur, which have a negative effect on the tolerance of personal protective equipment for respiratory organs. The combination of additional respiratory resistance with hypoxia and hypercapnia further worsens the tolerance of respiratory protective equipment. The purpose of this study was to study the tolerance of personal respiratory protection when changing the gas composition of the alveolar air. Material and methods. The study was conducted on healthy subjects of both sexes (78 people), aged 20 to 36 years. To simulate the conditions for the use of personal respiratory protective equipment, inspiratory resistive respiratory loads of 20% of the maximum intraoral pressure were used during the Mueller test. The tolerance of respiratory protective equipment was evaluated using the Borg visual analog scale of dyspnea, which reflected the level of subjective discomfort occurring due to additional respiratory resistance is turned on. The methodology for changing the gas composition of the alveolar air in the subjects consisted of using a system that allowed adding oxygen from the line to the closed spirograph circuit and turning the carbon dioxide adsorber on and off. Results. The tolerance of personal respiratory protection is associated with the nature of the gas composition of the alveolar air. A minimum of subjective discomfort was observed in the presence of hyperoxic-hypocapnic composition of the pulmonary air; on the contrary, an increase in subjective discomfort on the Borg scale was observed with a reduced oxygen content and an increased concentration of carbon dioxide. The use of personal respiratory protective equipment against the background of hypoxia-hypercapnia negatively changes the functional state of the body: there was observed an increase in physiological expenditures by leading effectors. Normalization of the gas composition of the body under the use of personal respiratory protection did not lead to complete optimization of the functional state of the subjects. Conclusion. Hypoxia and hypercapnia arising from the depletion of regenerative cartridges of the respiratory protective equipment of an insulating type leads to a significant deterioration in the tolerance to additional respiratory resistance. The alleged mechanism of this phenomenon should be considered as an increase in fatigue of the respiratory muscles.

Respiratory pump contributions in cerebrovascular and postural control responses during orthostatic stress in older women

The acute inspiratory resistive load (IRL) may increase the respiratory pump contributes to hemodynamic responses during orthostatic stress. However, IRL effects on cerebrovascular and postural control interplay is unknown. Fourteen older women (65 ± 4 years) performed two orthostatic tests with spontaneous breathing through acute experimental (−9 cm.H2O) or sham load impedance (0 cmH2O) connected to a pneumotachograph for breath-by-breath recordings on a facemask. Both tests were executed on a force plate, for postural balance analysis, and electromyographic activity records of calf muscles. Blood pressure, middle cerebral artery blood velocity (MCAv), heart rate (HR), stroke volume (SV), and cardiac output (CO) were continuously recorded. IRL reduced postural instability in the first five seconds at orthostatic position compared to sham. Although the mean blood pressure and MCAv were similar in both conditions, IRL reduced the SV and CO drops, and time to recover MCAv. Therefore, IRL improved postural control, CO and MCAv responses during initial orthostasis.

IMMUNOLOGICAL INDICES OF THE HUMAN IN CONDITIONS OF THE APPLICATION OF INDIVIDUAL MEANS OF PROTECTION OF RESPIRATORY ORGANS

The purpose of the work was to study reactions of the immune system with the use of additional respiratory resistance arising from the use of personal respiratory protection. The inspiratory resistance to respiration of 20, 40 and 60% Pmmax was used. The study involved 26 male and female cases at the average age of 21,22,23 years. Based on the results of the study the short-term (3 minutes) effect of inspiratory resistive loads was shown to have a pronounced effect on the population and subpopulation composition of blood lymphocytes, practically without changing the level of secreted immunoglobulins. Different values of additional resistance to respiration were noted to statistically significantly change the level of biogenic amines: the concentration of epinephrine and norepinephrine with elevating values of resistive loads progressively increased; the serotonin concentration shows the opposite dynamics. The work demonstrates the additional respiratory resistance of 20% Pmmax fail to change the immunological status of the subjects. The resistive respiratory load of 40% Pmmax caused immunosuppressive changes in the population composition of lymphocytes and indices of nonspecific immunological resistance. An additional respiratory resistance of 60% Pmmax induced an immunostimulatory effect in the change in the population composition of lymphocytes and in indices of nonspecific immunological resistance. Based on the data obtained, a suggestion has been made that in designing individual respiratory protection devices it is advisable to limit the value of additional inspiratory resistance to respiration of 20% Pmmax.
