Hypo‐ or hyperarousal? The mechanisms underlying social information processing in autism

AbstractAutistic children tend to show divergent social information processing, and controversies exist regarding the mechanisms underlying this processing. In this article, we summarize and categorize the current theories and evidence related to hypo‐ and hyperarousal accounts of social information processing in autism. The hypo‐arousal account proposes reduced motivation or sensitivity to process social information and is supported by behavioral, eye‐tracking, and neuroimaging studies. Recent evidence has challenged this account, highlighting the role of eye gaze in interpreting the hypo‐brain activation. The hyperarousal account posits that social context could evoke anxiety and discomfort in autistic children, which could interfere with their social information processing and result in social avoidance. Most research in this field has studied children and adolescents, but some studies have included adults since autism is a lifelong condition. Here, we highlight the importance of considering the heterogeneity of arousal patterns in autistic children when developing individualized supporting protocols.

Sensor-Based Technology for Social Information Processing in Autism: A Review.

The prevalence of autism spectrum disorders (ASD) has increased strongly over the past decades, and so has the demand for adequate behavioral assessment and support for persons affected by ASD. Here we provide a review on original research that used sensor technology for an objective assessment of social behavior, either with the aim to assist the assessment of autism or with the aim to use this technology for intervention and support of people with autism. Considering rapid technological progress, we focus (1) on studies published within the last 10 years (2009–2019), (2) on contact- and irritation-free sensor technology that does not constrain natural movement and interaction, and (3) on sensory input from the face, the voice, or body movements. We conclude that sensor technology has already demonstrated its great potential for improving both behavioral assessment and interventions in autism spectrum disorders. We also discuss selected examples for recent theoretical questions related to the understanding of psychological changes and potentials in autism. In addition to its applied potential, we argue that sensor technology—when implemented by appropriate interdisciplinary teams—may even contribute to such theoretical issues in understanding autism.

Uncovering the Social Deficits in the Autistic Brain. A Source-Based Morphometric Study.

Autism is a neurodevelopmental disorder that mainly affects social interaction and communication. Evidence from behavioral and functional MRI studies supports the hypothesis that dysfunctional mechanisms involving social brain structures play a major role in autistic symptomatology. However, the investigation of anatomical abnormalities in the brain of people with autism has led to inconsistent results. We investigated whether specific brain regions, known to display functional abnormalities in autism, may exhibit mutual and peculiar patterns of covariance in their gray-matter concentrations. We analyzed structural MRI images of 32 young men affected by autistic disorder (AD) and 50 healthy controls. Controls were matched for sex, age, handedness. IQ scores were also monitored to avoid confounding. A multivariate Source-Based Morphometry (SBM) was applied for the first time on AD and controls to detect maximally independent networks of gray matter. Group comparison revealed a gray-matter source that showed differences in AD compared to controls. This network includes broad temporal regions involved in social cognition and high-level visual processing, but also motor and executive areas of the frontal lobe. Notably, we found that gray matter differences, as reflected by SBM, significantly correlated with social and behavioral deficits displayed by AD individuals and encoded via the Autism Diagnostic Observation Schedule scores. These findings provide support for current hypotheses about the neural basis of atypical social and mental states information processing in autism.

Differences in global and local level information processing in autism: An fMRI investigation

People with autism spectrum disorders (ASD) have atypical visual perception of global and local information. Previous neuroimaging studies have examined the functional anatomy of locally directed attention during visual processing in ASD, but few have examined differences in both globally and locally directed attention. We performed functional magnetic resonance imaging (fMRI) in 17 adults with ASD and 16 typically developing (TD) subjects to examine the neurobiology of both global- and local-level information processing in ASD using an abstract hierarchical design task. TD subjects showed no regions of increased brain activation relative to subjects with ASD as assessed using whole brain analysis. Subjects with ASD exhibited greater activation in right superior frontal gyrus during locally directed attention. During globally directed attention, the ASD group showed greater right lateral occipital activation. Additionally, subjects with ASD showed less deactivation in medial prefrontal cortex (part of the default mode network) in the globally directed attention condition. Our findings help elucidate networks of brain activation related to atyipcal global and local feature processing in ASD.

Altered posterior cingulate cortical cyctoarchitecture, but normal density of neurons and interneurons in the posterior cingulate cortex and fusiform gyrus in autism

Autism is a developmental disorder with prenatal origins, currently estimated to affect 1 in 91 children in the United States. Social-emotional deficits are a hallmark of autism and early neuropathology studies have indicated involvement of the limbic system. Imaging studies demonstrate abnormal activation of the posterior cingulate cortex (PCC), a component of the limbic system. Abnormal activation has also been noted in the fusiform gyrus (FFG), a region important for facial recognition and a key element in social interaction. A potential imbalance between excitatory and inhibitory interneurons in the cortex may contribute to altered information processing in autism. Furthermore, reduced numbers of GABA receptors have previously been reported in the autistic brain. Thionin-stained sections were used to qualitatively assess cytoarchitectonic patterning and quantitatively determine the density of neurons and immunohistochemistry was used to determine the densities of a subset of GABAergic interneurons utilizing parvalbumin-and calbindin-immunoreactivity. In autism, the PCC displayed altered cytoarchitecture with irregularly distributed neurons, poorly demarcated layers IV and V, and increased presence of white matter neurons. In contrast, no neuropathology was observed in the FFG. There was no significant difference in the density of thionin, parvalbumin, or calbindin interneurons in either region and there was a trend towards a reduced density of calbindin neurons in the PCC. This study highlights the presence of abnormal findings in the PCC, which appear to be developmental in nature and could affect the local processing of social-emotional behaviors as well as functioning of interrelated areas.

Far visual acuity is unremarkable in autism: Do we need to focus on crowding?

Although autism presents a unique perceptual phenotype defined in part by atypical (often enhanced) analysis of spatial information, few biologically plausible hypotheses have been advanced to explain its neural underpinnings. One plausible explanation is functional but altered lateral connectivity mediating early or local mechanisms selectively responsive to different stimulus attributes, including spatial frequency and contrast. The goal of the present study was first to assess far visual acuity in autism using Landolt-C optotypes defined by different local stimulus attributes. Second, we investigated whether acuity is differentially affected in autism when target optotypes are simultaneously presented with flanking stimuli at different distances. This typical detrimental "crowding effect" of flanking stimuli on target optotype discrimination is attributed to lateral inhibitory interaction of neurons encoding for visual properties of distracters close to the target. Results failed to demonstrate a between-group difference in acuity to Landolt-C optotypes, whether defined by luminance- or texture-contrast. However, the expected crowding effect at one gap-size opening distance was evidenced for the control group only; a small effect was observed for the autism group at two gap-size opening. These results suggest that although far visual acuity is unremarkable in autism, altered local lateral connectivity within early perceptual areas underlying spatial information processing in autism is atypical. Altered local lateral connectivity in autism might originate from an imbalance in excitatory/inhibitory neural signaling, resulting in changes regarding elementary feature extraction and subsequent downstream visual integration and visuo-spatial analysis. This notion is discussed within the context of characteristic lower- and higher-level perceptual processing in autism.

Demonstrations of Decreased Sensitivity to Complex Motion Information Not Enough to Propose an Autism-Specific Neural Etiology

Interest regarding neural information processing in autism is growing because atypical perceptual abilities are a characteristic feature of persons with autism. Central to our review is how characteristic perceptual abilities, referred to as perceptual signatures, can be used to suggest a neural etiology that is specific to autism. We review evidence from studies assessing both motion and form perception and how the resulting perceptual signatures are interpreted within the context of two main hypotheses regarding information processing in autism: the pathway- and complexity-specific hypotheses. We present evidence suggesting that an autism-specific neural etiology based on perceptual abilities can only be made when particular experimental paradigms are used, and that such an etiology is most congruent with the complexity-specific hypothesis.

Enhanced and diminished visuo-spatial information processing in autism depends on stimulus complexity

Visuo-perceptual processing in autism is characterized by intact or enhanced performance on static spatial tasks and inferior performance on dynamic tasks, suggesting a deficit of dorsal visual stream processing in autism. However, previous findings by Bertone et al. indicate that neuro-integrative mechanisms used to detect complex motion, rather than motion perception per se, may be impaired in autism. We present here the first demonstration of concurrent enhanced and decreased performance in autism on the same visuo-spatial static task, wherein the only factor dichotomizing performance was the neural complexity required to discriminate grating orientation. The ability of persons with autism was found to be superior for identifying the orientation of simple, luminance-defined (or first-order) gratings but inferior for complex, texture-defined (or second-order) gratings. Using a flicker contrast sensitivity task, we demonstrated that this finding is probably not due to abnormal information processing at a sub-cortical level (magnocellular and parvocellular functioning). Together, these findings are interpreted as a clear indication of altered low-level perceptual information processing in autism, and confirm that the deficits and assets observed in autistic visual perception are contingent on the complexity of the neural network required to process a given type of visual stimulus. We suggest that atypical neural connectivity, resulting in enhanced lateral inhibition, may account for both enhanced and decreased low-level information processing in autism.

Quantification Judgement in High Functioning Autism: Superior or Different?

This study examined the hypothesis of superior quantification abilities of persons with high functioning autism (HFA). Fourteen HFA individuals (mean age: 15 years) individually matched with 14 typically developing (TD) participants (gender, chronological age, full-scale IQ) were asked to quantify as accurately and quickly as possible numerosities, represented by the number of squares (2-9) presented in random configurations. In addition, the visual angles of stimuli presentation were manipulated in order to induce a local (large visual angle) and a global (small visual angle) bias on participants' quantification performance (accuracy and naming time). Findings revealed no effect of local and global bias of stimuli presentation in the two groups' performance, and no superior quantification abilities in HFA participants. However, analyses of the naming time slopes for identification by HFA participants of small consecutive numerosities (2-5), suggested their use of counting processes instead of subitizing (or immediate apprehension of numerosities) as in TD participants. Possible explanations for these results are discussed with reference to models of locally-oriented information processing in autism.

Functional imbalance of visual pathways indicates alternative face processing strategies in autism

To investigate whether autistic subjects show a different pattern of neural activity than healthy individuals during processing of faces and complex patterns. Blood oxygen level-dependent (BOLD) signal changes accompanying visual processing of faces and complex patterns were analyzed in an autistic group (n = 7; 25.3 [6.9] years) and a control group (n = 7; 27.7 [7.8] years). Compared with unaffected subjects, autistic subjects demonstrated lower BOLD signals in the fusiform gyrus, most prominently during face processing, and higher signals in the more object-related medial occipital gyrus. Further signal increases in autistic subjects vs controls were found in regions highly important for visual search: the superior parietal lobule and the medial frontal gyrus, where the frontal eye fields are located. The cortical activation pattern during face processing indicates deficits in the face-specific regions, with higher activations in regions involved in visual search. These findings reflect different strategies for visual processing, supporting models that propose a predisposition to local rather than global modes of information processing in autism.

Central coherence and theory of mind in autism: Reading homographs in context

The hypothesis that people with autism have a specific deficit in ‘theory of mind’ has been successful in explaining the characteristic triad of social, communication and imagination impairments. It cannot, however, explain the non‐social impairments and skills shown by people with autism. Frith (1989) and Frith & Happé (1994a) have suggested that these aspects of autism can, instead, be understood as manifestations of a characteristic of general information processing in autism: ‘weak central coherence’. The relationship between theory of mind deficits and weak coherence is examined in the present paper. Sixteen relatively able participants with autism, who differed in their theory of mind task performance, were tested with a homograph reading task, in which pronunciation of target words is determined by integration of whole sentence context (e.g. pronouncing tear in ‘In her eye/dress there was a big tear’). The results suggest that people with autism at all levels of theory of mind performance show a relative failure to process information for context‐dependent meaning in this task. The implications of these findings for current theories of autism are discussed.

A developmental cognitive neuropsychological approach to the assessment of information processing in autism

This paper explores the application of the cognitive neuropsychological model to two children within the autistic spectrum. Both children were assessed individually over five sessions in an attempt to establish their strategies for information processing. Different input modes were com pared in combination and in isolation - the spoken word, sign, and print. The results offer insights into their processing, highlighting indi vidual differences, which led to subsequent hypotheses for optimum teaching approaches.

Why do autistic individuals show superior performance on the block design task?

Systematic variations of the block design task were given to 20 autistic, 33 normal and 12 mildly retarded subjects. Designs were contrasted which were either "whole" or segmented, rotated or unrotated, and which did or did not contain obliques. Only segmentation, but neither of the spatial orientation factors, revealed a significant group difference. Autistic subjects, regardless of age and ability, performed better than controls when presented with unsegmented designs. This result suggests that they need less of the normally required effort to segment a gestalt, and thus supports the hypothesis of weak central coherence as a characteristic of information processing in autism.
