Effect of brimonidine on vascular density and imagej-derived flow index of optic nerve head and macula in primary open angle glaucoma.

To study the effect of brimonidine on vascular density and flow index of optic nerve head (ONH) and macula in primary open angle glaucoma (POAG) using optical coherence tomography angiography (OCTA). Twenty-three brimonidine-naïve POAG patients were started on brimonidine. They underwent OCTA ONH and macula before commencing brimonidine and one month thereafter. Systemic arterial blood pressure (SABP) and intraocular pressure (IOP) were measured at each visit to calculate mean ocular perfusion pressure (MOPP). The OCT angiograms were analyzed using ImageJ software to calculate ONH and macular flow indices. Thirty-seven eyes (23 patients) with a mean age of 56.7 ± 12.49years were included of whom 60.8% were males. Brimonidine was associated with an increase in the superficial flow index (SFI) (P-value = 0.02) and optic nerve head flow index (ONHFI) (P-value = 0.01). Also, superficial vascular density (SVD) for whole image, superior-hemi and fovea increased (P-value = 0.03, 0.02, 0.03 respectively). ONH inferior-hemi vascular density decreased (P-value = 0.01) despite an increase in inferior quadrant retinal nerve fiber layer thickness (RNFLT) (P-value = 0.03). There was no statistically significant correlation between flow indices and MOPP at baseline and follow-up. A moderate negative correlation was found between SVD and DVD at the fovea and MOPP at baseline and follow-up (P-value = 0.03, 0.05) (P-value = 0.02, 0.01) respectively. Brimonidine was associated with an increase in SFI, ONHFI and SVD indicating improved GCC and RNFL perfusion in POAG. Despite the increase in inferior quadrant RNFLT, the concomitant decrease in inferior-hemi ONHVD precluded a conclusion of hemodynamically-mediated improvement of RNFLT.

Retinal Nerve Fiber Layer Thickness in Patients with Schizophrenia and Its Relation with Cognitive Impairment.

Schizophrenia is a chronic severe mental illness with heterogeneous clinical presentation, course, and outcome. Cognitive impairment is one of its core features. Retinal nerve fiber layer (RNFL) imaging using OCT (optical coherence tomography) could provide easy access for in vivo imaging of the retina, rendering it as a "window to the brain." Studies done on schizophrenia have shown RNFL thinning. This study attempts to look into the association between cognitive impairment, disease duration, and RNFL abnormality in patients with schizophrenia using OCT. Patients diagnosed with schizophrenia meeting DSM 5 (Diagnostic and Statistical Manual of Mental Disorders) criteria and who were confirmed to be in remission for at least six months clinically and scoring less than three on PANSS-8 (positive and negative symptom scale-8) remission scale were included. They were administered the Montreal Cognitive Assessment Scale (MoCA) for cognitive assessment. RNFL measures were taken using spectral domain-OCT. Variables were compared using Pearson's correlation test, one-way ANOVA test, and independent t-test as appropriate. A total of 36 patients were studied. MoCA scores and RNFL thickness showed a positive correlation. Patients with schizophrenia had reduced average RNFL thickness and reduced RNFL thickness in superior, inferior, and temporal quadrants. Average RNFL thickness, Superior and inferior quadrant RNFL thickness showed a positive correlation with MoCA scores. No correlation was obtained between macular volume, macular thickness, duration of illness, and MoCA scores. Patients with schizophrenia have reduced average RNFL thickness. Patients with low MoCA scores have RNFL thinning.

Retinal nerve fiber layer and ganglion cell complex thickness in diabetic smokers without diabetic retinopathy

Purpose To compare the thickness of the retinal nerve fiber layer (RNFL) and macular ganglion cell-inner plexiform layer (GC-IPL) in smoker and nonsmoker diabetics without diabetic retinopathy. Materials and Methods Patients with diabetes were divided into two groups according to their smoking status: Group 1 consisted of 38 smoker diabetics who had chronically smoked more than 20 cigarettes per day for more than five years; Group 2 consisted of 38 nonsmoker diabetics. After a detailed ophthalmologic examination, the mean and regional (superior, supratemporal, inferior, inferotemporal, temporal, nasal, superonasal, and inferonasal) RNFL and GC-IPL thicknesses were measured with spectral-domain optic coherence tomography (SD-OCT) and compared between groups. Results The mean age was 54.7 ± 10.5 and 51.2 ± 9.7 years in the smoker and nonsmoker groups, respectively (p = 0.14). Gender, duration of diabetes, and the mean axial length were similar between groups (p:0.43, p:0.54, p: 0.52, respectively). Mean RNFL thickness was 89.1 ± 8.0 µm in the smoker group and 93.4 ± 7.0 µm in the nonsmoker group, and it was significantly thinner in the smoker group (p = 0.01). The temporal RNFL thickness in the smoker group was thinner than in the nonsmoker group (p = 0.02). There was no difference in superior, inferior, and nasal RNFL thicknesses between the groups (p = 0.31, p = 0.12, p = 0.39, respectively). The mean macular GC-IPL thickness of the smoker and nonsmoker groups was 78.53 ± 15.74 µm and 83.08 ± 5.85 µm, respectively (p = 0.09). Superior, superonasal, inferonasal, inferior, inferotemporal, and superotemporal quadrant GC-IPL thicknesses were similar between the groups (p = 0.07, p = 0.60, p = 0.55, p = 0.77, p = 0.71, p = 0.08, respectively). The groups showed no difference in minimum GC-IPL thickness (p = 0.43). There was a significant negative correlation between smoking exposure and mean, inferior quadrant RNFL thicknesses in the smoker group (p = 0.04, r= −0.32, and p = 0.01, r= −0.39, respectively). Conclusion Mean RNFL thickness was significantly thinner in smoker diabetics. Although not statistically significant, especially mean, superior, and superotemporal GC-IPL was thinner in smoker diabetics. The results suggest a potential association between the coexistence of diabetes and smoking with alterations in RNFL and GC-IPL thickness.

Decreased perifoveal ganglion cell complex thickness - a first sign for macular damage in patients using hydroxychloroquine.

Aim: To examine ganglion cell complex (GCC) thickness detected by optical coherence tomography (OCT) in patients using hydroxychloroquine (HCQ), without any structural and functional macular changes to evaluate the initial symptoms of macular toxicity for early diagnosis before clinical evaluation. Methods: Eighty eyes of forty patients (Group 1) and forty eyes of twenty healthy volunteer persons (Group 2) were included in the study. Detailed ophthalmologic and mydriatic fundus examination were applied to all patients and volunteers (controls). Spectral domain OCT, visual field (VF) and color vision test were performed. Measurements of macula thickness, GCC thickness (involving nerve fiber layer, ganglion cell layer and inner plexiform layer) and peripapillary retinal nerve fiber layer (RNFL) were performed with OCT. Patients with retinal pigment epithelial changes, VF paracentral scotoma and defected color vision were excluded from the planned study. Results: Perifoveal GCC layer thickness in all quadrants was significantly thinner in group 1 compared to group 2 (p=0.017, p=0.001, p=0.019, p=0.001). The mean global inferior hemifield and nasal quadrant RNFL thickness were lower than in the control groups (p=0,012, p=0,009, p=0,005, respectively). Five-year outcomes of selective laser trabeculoplasty: A retrospective study.

Studies have shown the efficacy of selective laser trabeculoplasty (SLT) to lower intraocular pressure (IOP) as adjuvant therapy during short-term follow-up. However, few studies have assessed the long-term efficacy of SLT on preventing worsening Humphrey visual field (HVF) parameters and thinning of the retinal nerve fiber layer (RNFL) with continued medical therapy. A retrospective chart review was conducted of 51 eyes of 39 patients with glaucoma treated with SLT at Boston Medical Center between 2012 and 2016 with 3- and 5-year follow-up. Outcome measures included IOP, visual acuity, number of glaucoma medications, number of months to subsequent surgical intervention. HVF outcome measures included mean deviation (MD) and pattern standard deviation (PSD). Optical coherence tomography (OCT) outcome measures included RNFL mean thickness, and superior and inferior thicknesses. Twenty-five eyes received subsequent surgical intervention (mean time to intervention = 33.6 ± 20.0 months). In the eyes that did not receive another intervention, mean IOP was significantly decreased by 3.2 and 3.5 mmHg at 3- and 5-year after SLT, respectively. Mean number of glaucoma medications was significantly increased at 5-year (2.7 ± 1.6; P = 0.04), compared to pre-SLT (2.0 ± 1.1). Mean HVF MD was significantly higher at 5-year (-7.64 ± 6.57 dB) compared to pre-SLT (-5.61 ± 3.90 dB). Mean PSD significantly increased at 3-year (5.30 ± 2.91 dB) and 5-year (6.84 ± 2.62 dB), compared to pre-SLT (4.63 ± 2.70 dB; P = 0.04 and ≤0.01, respectively). On OCT, inferior quadrant RNFL thickness decreased significantly at 5-year (88.5 ± 19.3 μm), compared to pre-SLT (94.0 ± 23.2 μm). Although 51% of eyes had IOP controlled at 5-year post-SLT, mean number of glaucoma medications was significantly higher. Also, there was progression of MD and PSD on HVF and inferior quadrant thinning on OCT at 5-year. We found a significant association between age at SLT and risk of subsequent surgical intervention over 5-year follow-up. Our study adds to our understanding of long-term outcomes of adjuvant SLT for glaucoma patients receiving medical therapy.

Effect of oral citicoline therapy on retinal nerve fiber layer and ganglion cell-inner plexiform layer in patients with primary open angle glaucoma

To evaluate the short-term effects of oral citicoline therapy on the retinal nerve fiber layer (RNFL) and the macular ganglion cell-inner plexiform layer (mGCIPL) in patients with primary open angle glaucoma (POAG). Fifty-four eyes of 54 patients with POAG glaucoma included in the study. In addition to a topical hypotensive, 250-mg oral citicoline was administered to 27 patients, while 27 patients were assigned as the control group. RNFL and mGCIPL values were measured using optical coherence tomography (OCT) at 1d before treatment and 3mo after the initiation of treatment. At the third month visit, citicoline treatment was discontinued and drug-free control (wash-out) measurements were obtained at the fourth month in citicoline group. The average RNFL thickness was significantly higher at month 3 than the baseline (P=0.038) in citicoline group. However, this improvement partially regressed after a 1-month wash-out period. No statistically significant changes in RNFL were observed in the superior, nasal, temporal and inferior quadrants at months 3 and 4 (P>0.05). The change in the average and inferior quadrant RNFL thickness in the citicoline group at 3mo was significantly greater than the control group (P=0.006 and P=0.014, respectively). There were no significant differences between the groups according to the change in mGCIPL thickness and the superior, nasal and temporal quadrant RNFL thickness (P>0.05). With oral citicoline treatment, the loss in the average RNFL is prevented in POAG patients in the short-term. Study data show that citicoline may have a significant impact on slowing glaucoma progression, which could have a potential neuroprotective effect.

Optical Coherence Tomography Findings of Nonarteritic Ischemic Optic Neuropathy

Objective: To evaluate the optical coherence tomography (OCT) parameters in patients with nonartheritic ischemic optic neuropathy (NAION) and glaucomatous optic atrophy. Material-Method: Datas of patients diagnosed with glaucomatous optic atrophy (Group 1) and NAION (Group 2) and followed-up at the the glaucoma department between 2017-2020 were retrospectively analyzed. Detailed ophthalmologic examination including demographic data, best corrected visual acuity (BCVA), intraocular pressure (IOP), central corneal thickness (CCT) obtained by ultrasonic pachymetry, retinal nerve fiber layer (RNFL) and ganglion cell complex thickness obtained by OCT (Carl Zeiss Meditec, Dublin, CA, USA). The mean and regional (superior, superotemporal, inferior, inferotemporal, temporal, nasal, superonasal, and inferonasal) RNFL and ganglion cell complex thicknesses were compared between the groups. Results: The mean age of 30 patients (14 female and 16 male) in Group 1 was 66.2 ± 12 and the mean age of 36 patients (20 male, 16 female) in Group 2 was 61.6 ± 13.05 years. Age and gender were similar between the groups (p=0.25). The mean BCVA was 0.43 ± 0.3 and 0.16 ± 0.1 in groups, respectively (p <0.01). The mean RNFL was 50.8 ± 10 µ in Group 1 and 59.05 ± 11 µ in Group 2 (p:021). The mean ganglion cell thickness was 50.2 ± 10.4 and 48 ± 15 µ in Group 1 and Group 2, respectively. It was found to be significantly thinner in Group 2 (p:0.04). The mean inferior quadrant RNFL thickness was 53.96 ± 13 and 74.18 ± 23 µ, in Group 1 and Group 2, respectively and it was significantly thinner in Group 1 (p: 0.02). There was no significant difference between the two groups in the other quadrants’ RNFL thickness. The mean inferotemporal quadrant ganglion cell thickness was significantly thinner in Group 1 and the mean superoquadrant ganglion cell thickness was significantly thinner in Group 2 (p1 <0.01, p2:0.09). Discussion: The mean ganglion cell complex thickness was found to be thinner in the NAION group. This may indicate that the ganglion cell complex is more susceptible to ischemia-related damage.

Effect of Inconsistent Optical Coherence Tomography Scan Location on Glaucoma Progression Analysis.

Circumpapillary retinal nerve fiber layer (RNFL) thickness change induced by optical coherence tomography (OCT) scan circle location displacement was not completely compensated despite the use of Guided Progression Analysis (GPA), especially when the "R1" registration method was applied. The purpose of this study was to evaluate the effect of inconsistent OCT scan location on glaucoma progression detection using GPA. Eighty-four glaucomatous eyes with at least 4 serial OCT tests were included. To evaluate the ability of OCT GPA to adjust the OCT scan location, an analysis was performed without manipulation (control set), and after the OCT scan of the latest test was intentionally moved inferotemporally (test set). The eyes were classified into the adjustment and nonadjustment groups based on the superior or inferior quadrant RNFL thickness agreement between the control and test sets. When the OCT GPA parameters between the control and test sets were compared, the test set showed a greater superior RNFL thickness and lesser inferior RNFL thickness compared with the control set (P<0.05). The eyes in the nonadjustment group (n=21, 25%) had a lower chance of applying the eye-tracking function (P=0.003) and a higher frequency of the "R1" registration method of OCT GPA (P<0.001) than the adjustment group (n=63, 75%); all eyes with the "R1" method were in the nonadjustment group and all eyes with the "R2" method were in the adjustment group. Inconsistent OCT scan location induced changes in RNFL thickness that were not completely compensated despite the use of OCT GPA, especially when the "R1" registration method was applied. These findings indicates a potential for misidentified glaucoma progression.

THE EFFECT OF CORNEAL ASTIGMATISM ON RETINAL NERVE FIBER LAYER THICKNESS AND OPTIC NERVE HEAD MEASUREMENTS BY OPTICAL COHERENCE TOMOGRAPHY

ABSTRACT Background: Optical Coherence Tomography (OCT) is a non-invasive investigative technique that functions as a type of optical biopsy. It has been an important diagnostic tool in evaluation of optic nerve head (ONH) and macula. Evaluation of OCT ONH is essential, since the thickness of the RNFL and ONH parameters can be affected in various optic neuropathies like glaucoma, optic atrophy and optic disc edema. Objective: To evaluate the effect of corneal astigmatism on retinal nerve fiber layer (RNFL) thickness and optic nerve head (ONH) parameters measured with spectral-domain optical coherence tomography (OCT). Methods: Forty eyes of 32 patients were included in this study.Group I; 20 non/low astigmatic eyes to be compared with Group II; 20 astigmatic eyes (10 eyes have with the rule corneal astigmatism WTR and 10 eyes have against the rule corneal astigmatism ATR). The average corneal astigmatism in group I was -0.33±0.17 D (range -0.5-0 D), in group II it was -3.18±0.87 D (range -4.5- -2 D) in eyes of with the rule corneal astigmatism, and -3.13±0.84 D (range -4.5- -2.25 D) in eyes of against the rule corneal astigmatism. All forty eyes underwent full ophthalmic examination and performed OCT ONH imaging by SD-OCT Angio Vuein Ophthalmology Department, Faculty of Medicine, Al-Azhar University, Cairo, from January to November 2019. Results: No significant difference was noted in the disc area, rim area, vertical C/D ratio and cup volume (all P values > 0.05). There were no significant differences between the two groups in global average RNFL thickness, as well as superior, inferior, nasal and temporal quadrant RNFL thickness (P values > 0.05). Conclusions: corneal astigmatism had statistically insignificant effect on optic nerve head parameters and retinal nerve fiber layer thickness measured with spectral domain optical coherence tomography.

To evaluate the retinal nerve fiber layer thickness in different types of glaucoma

Objective: Glaucoma is a multifaceted eye disease which is classified as physical damage of retinal ganglion cells which may effect in loss of vision and permanent blindness. While physical damage of glaucoma can be clinically evaluated the optic nerve head and peripapillary retinal nerve fiber layer (RNFL). Our objective is to evaluate the mean RNFL thickness in all types of glaucoma. Methodology: This prospective and cross sectional study was conducted in Glaucoma Clinic of Al Ibrahim Eye Hospital (AIEH), Karachi, for the period from May 2019 to October 2019, after ethical approval from Institutional Research Committee. A total of 64 glaucoma patients were chosen by using non-probability purposive sampling technique. The participant comprised, no known eye disease, no visual impairment, IOP below 22 mmHg, and no obvious retinal disease or defect were included while individuals having a history of ocular diseases or pathology with residual visual impairment, retinal diseases, amblyopia, and history of intraocular surgery or laser therapy were excluded from the study. The collected data were analyzed using Statistical Package for Social Sciences (SPSS) version 20. Results: 64 patients of both genders with mean age of 55.54±15.58 years. The mean intraocular pressure of right eye and left eye was 16.46±8.06 and 16.75±7.82 mmHg, respectively. The mean RNFL thickness in superior, inferior, temporal and nasal quadrant of the right eye was measured 68.23±25.44, 66.79±27.50, 51.75±12.58 and 47.73±18.82 microns, respectively while the mean RNFL thickness in superior, inferior, temporal and nasal quadrant of the left eye was measured 76.01±22.72, 67.42±21.25, 54.37±13.0 and 49.62±11.57 microns, respectively. Conclusion: Our study showed that analysis of retinal nerve fiber layer thickness with optical coherence tomography is the best tool for the diagnosis of glaucoma. It has also been observed in our study that frequency of changes in retinal nerve fiber layer thickness was mostly found in primary open angle glaucoma patients

Diagnostic value of optic disc retinal nerve fiber layer thickness for diabetic peripheral neuropathy.

To investigate the value of optic disc retinal nerve fiber layer (RNFL) thickness in the diagnosis of diabetic peripheral neuropathy (DPN). Ninety patients with type 2 diabetes, including 60 patients without DPN (NDPN group) and 30 patients with DPN (DPN group), and 30 healthy participants (normal group) were enrolled. Optical coherence tomography (OCT) was used to measure the four quadrants and the overall average RNFL thickness of the optic disc. The receiver operator characteristic curve was drawn and the area under the curve (AUC) was calculated to evaluate the diagnostic value of RNFL thickness in the optic disc area for DPN. The RNFL thickness of the DPN group was thinner than those of the normal and NDPN groups in the overall average ((101.07± 12.40) µm vs. (111.07±6.99) µm and (109.25±6.90) µm), superior quadrant ((123.00±19.04) µm vs. (138.93±14.16) µm and (134.47±14.34) µm), and inferior quadrant ((129.37±17.50) µm vs. (143.60±12.22) µm and (144.48±14.10) µm), and the differences were statistically significant. The diagnostic efficiencies of the overall average, superior quadrant, and inferior quadrant RNFL thicknesses, and a combined index of superior and inferior quadrant RNFL thicknesses were similar, and the AUCs were 0.739 (95% confidence interval (CI) 0.635-0.826), 0.683 (95% CI 0.576-0.778), 0.755 (95% CI 0.652-0.840), and 0.773 (95% CI 0.672-0.854), respectively. The diagnostic sensitivity of RNFL thickness in the superior quadrant reached 93.33%. The thickness of the RNFL in the optic disc can be used as a diagnostic method for DPN.

Evaluation of retinal fiber thickness and visual pathways with optic coherence tomography and pattern visual evoked potential in different clinical stages of obstructive sleep apnea syndrome.

To investigate the possible changes in retinal nerve fiber layer (RNFL) by optic coherence tomography and in the amplitudes and peak times (PTs) in pattern visual evoked potential (pVEP) and to compare them in obstructive sleep apnea syndrome (OSAS). This prospective study included patients with mild OSAS (n = 30), severe OSAS (n = 30), and 30 control subjects. All patients were assessed after obtaining the approval from our hospital's ethics committee. There was no difference in age and gender between the groups (p = 0.184, p = 0.954). By analysis of variance, there was a significant difference in RNFL values among patients with mild OSAS, severe OSAS, and control for three measures of RNFL (average p = 0.044, nasal p = 0.003, inferior p = 0.027). In severe OSAS group, nasal and inferior quadrants of the RNFL were found to be thinner than the control group (p = 0.008, p = 0.031). We showed that the PT of P100 and N145 was prolonged in severe OSAS compared to the control group (p < 0.001) and that PT of P100 was prolonged in mild OSAS compared to the control group (p < 0.05). The amplitude of N75-P100 was significantly decreased in patients with both severe OSAS and mild OSAS compared to the control group (p < 0.001). Correlation of RNFL and pVEP values showed that the inferior quadrant RNFL thickness is correlated with both P100 and N145 PTs (r = 0.271*, p = 0.036 and r = 0.290*, p = 0.043, respectively) and N75-P100 amplitude (r = 0.378**, p = 0.003) in severe OSAS group. In mild and severe stages of the disease, edema and inflammation were evident and VEP PT and amplitudes were affected in both groups. Furthermore, thinning in RNFL in the severe stage of the disease might be associated with higher atrophy levels and prolonged exposure to hypoxia.

Retinal nerve fiber layer thickness is associated with hippocampus and lingual gyrus volumes in nondemented older adults

ObjectivesAbnormal retina structures, such as thinner retinal nerve fiber layer (RNFL), have been frequently reported in patients with Alzheimer's disease (AD). However, the association between RNFL and brain structures in cognitively normal adults remains unknown. We therefore set out to conduct a cross-sectional investigation to determine whether RNFL thickness is associated with brain structure volumes in nondemented older adults. MethodsWe measured RNFL thickness by optical coherence tomography and brain structure volumes by 3 T magnetic resonance imaging. Cognitive function was assessed using the Chinese version of Repeatable Battery for the Assessment of Neurological Status. Pearson correlation was initially employed to screen for the potential associations among RNFL thickness, brain structure volumes and cognitive function. And then, multivariable linear regression models were conducted to further examine such associations adjusting for possible confounding factors, including age, sex, years of education and the estimated total intracranial volume (eTIV). Results113 participants (≥ 65 years old) were screened and 80 of them (mean age: 68 ± 5.3 years; 48% male) were included in the final analysis. RNFL thickness in temporal quadrant was associated with medial temporal lobes volumes [unadjusted: r = 0.155, P = 0.175; adjusted: β = 0.205 (0.014, 0.383), P = 0.035], and especially associated with the hippocampus volume [unadjusted: r = 0.213, P = 0.062; adjusted: β = 0.251 (0.060, 0.435), P = 0.011] after adjusted for age, sex, years of education and eTIV. Moreover, it showed that RNFL thickness in inferior quadrant [unadjusted: r = 0.221, P = 0.052; adjusted: β = 0.226 (0.010. 0.446), P = 0.041] was significantly associated with occipital lobes volumes after the adjustment of age, sex, years of education and eTIV, and selectively associated with the substructure of lingual gyrus volume [unadjusted: r = 0.223, P = 0.050; adjusted: β = 0.278 (0.058, 0.487), P = 0.014]. In addition, average RNFL thickness was associated with the cognitive domain of visuospatial/constructional [unadjusted: r = 0.114, P = 0.322; adjusted: β = 0.216 (0.006, 0.426), P = 0.044] after the adjustment in these nondemented older adults. ConclusionsQuadrant-specific associations exist between RNFL thickness and brain regions vulnerable to aging or neurodegeneration in older adults with normal cognition. These findings would promote further investigations into using RNFL as a noninvasive and less expensive biomarker of neurocognitive aging and AD-related neurodegeneration.

Structural evaluation of perimetrically normal and affected hemifields in open angle glaucoma

Purpose:To study macular ganglion cell layer-- inner plexiform layer complex (GCL + IPL) in relation to peripapillary retinal nerve fiber layer (RNFL) in glaucomatous eyes with superior or inferior hemifield defects (HD) and to study structural configuration in normal hemifield.Methods:This was an observational cross-sectional study. Data from consecutive 45 superior HD (SHD) and 50 inferior HD (IHD) eyes were analyzed. Each patient underwent detailed ocular examination, standard automated perimetry, and spectral domain optical coherence tomography (SD-OCT). After adjusting for age, gender, and signal strength, area under receiver operating characteristic curve (AUC) was calculated to determine diagnostic ability of GCL + IPL and peripapillary RNFL. Apparently normal hemifield was compared with true normal hemifield. Data were analyzed with SPSS, analysis of variance, t-test, Chi-square test, and receiver operating curve.Results:In the SHD glaucoma group, best parameters for discriminating normal eyes from glaucomatous eyes were inferotemporal GCL + IPL thickness (0.935) and inferior quadrant RNFL thickness (0.971). For IHD glaucoma, average GCL + IPL thickness (0.877) and average RNFL thickness (0.950) had best AUC values. When evaluating apparently normal hemifield in both groups, statistically significant difference was found in inferior GCL + IPL sector (0.865) and inferior quadrant RNFL (0.883) in IHD and superonasal GCL + IPL (0.725) and superior quadrant RNFL (0.842) in SHD groups.Conclusion:SD-OCT may be a useful ancillary diagnostic tool for evaluation of early macular and circumpapillary structural changes in glaucomatous eyes with localized visual field defects. Apparently normal hemifields show structural damage and should be considered in management of glaucoma.

Retinal nerve fiber layer and ganglion cell layer changes on optical coherence tomography in early multiple sclerosis and optic neuritis cases.

Purpose:To study the retinal nerve fiber layer (RNFL) and ganglion cell layer (GCL) changes on optical coherence tomography in early multiple sclerosis (MS) patients.Methods:A prospective cohort study was conducted at a tertiary care center. Patients of early MS (expanded disability status scale <3) with or without optic neuritis (ON) and idiopathic ON were included. Twenty age-matched individuals were taken as controls. Changes in RNFL and GCL thickness were evaluated along with the correlation with visual function parameters such as visual acuity, contrast sensitivity, and visual evoked response at first visit and again at six months.Results:Forty-four patients of MS with or without ON (24 and 20 patients respectively), 29 patients with idiopathic ON, and 20 healthy controls constituted the cohorts. Mean LogMAR best-corrected visual acuity was found to be significantly reduced in all groups except fellow eyes (FE) of ON group. Mean values of average RNFL thickness and values in superior, temporal, and inferior quadrant were significantly reduced. Similarly, overall mean values of average GCL-inner plexiform layer (IPL) thickness and values in superior, superonasal, superotemporal, inferonasal, and inferotemporal quadrant were significantly reduced in all groups except FE of ON group (P < 0.05). All the visual parameters significantly correlated with GCL + IPL thickness.Conclusion:GCL + IPL thickness is a more sensitive clinical structural marker than RNFL in early MS with/without ON and ON patients and correlates with all the visual parameters better than RNFL thickness.

OCT Glaucoma Staging System: a new method for retinal nerve fiber layer damage classification using spectral-domain OCT.

PurposeTo describe a new method, the Optical Coherence Tomography (OCT) Glaucoma Staging System, for classifying retinal nerve fiber layer (RNFL) damage assessed with OCT.Patients and methodsThe OCT Glaucoma Staging System was created based on data obtained from Nidek RS 3000 spectral-domain (sd)-OCT. This system uses the superior and inferior quadrant RNFL thickness values, plotted on an x-y diagram for staging structural damage severity in glaucoma. A non-linear equation and two regression lines describe the boundary lines which separate the different sectors of the diagram. These mathematical formulas have been used to create a software, which provides a quick classification of the RNFL damage. Sensitivity and specificity of the system were assessed in a different cohort including 64 patients with early OAG, and 62 normal subjects.ResultsThree hundred and two OCT tests from 98 healthy controls and 284 patients affected by either ocular hypertension or chronic open-angle glaucoma were considered in order to design the new classification system. The OCT Glaucoma Staging System classifies RNFL defects into 6 stages of increasing severity ranging from borderline to stage 5, and 3 groups according to defect localization (superior, inferior, or diffuse). Sensitivity and specificity in discriminating between healthy and glaucomatous eyes were 95.2 and 91.9%, respectively, considering borderline results as abnormal.ConclusionsThe OCT Glaucoma Staging System appears to provide a standardized and objective classification of glaucomatous RNFL damage. It can be used in day-to-day clinical practice for an easy and fast interpretation of RNFL measurements obtained with OCT.

Anxiety During Pregnancy and Postpartum: Course, Predictors and Comorbidity with Postpartum Depression.

Previous findings on peripartum anxiety are inconsistent in respect to the prevalence and course of peripartum anxiety with comorbidity of depression. Our aim was threefold: (1) to examine the course of elevated anxiety during pregnancy, immediately after childbirth, and six weeks postpartum; (2) to establish comorbidity of postpartum anxiety and postpartum depression (PPD); and (3) to examine predictors of anxiety 6 weeks postpartum. A sample of women (N=272) who were below the cut-off score for clinical depression during pregnancy were assessed in the third trimester of pregnancy, then 2 days and 6 weeks postpartum. Questionnaires on anxiety, pregnancy specific distress, stress, coping styles, social support, and depression were administered at each assessment. Obstetric data were collected from the participants' medical records. The estimated rate of high anxiety was 35% during pregnancy, 17% immediately after childbirth, and 20% six weeks postpartum, showing a decrease in anxiety levels after childbirth. Comorbidity of anxiety and PPD was 75%. Trait anxiety and early postpartum state anxiety are significant predictors of postpartum anxiety. Anxiety is a common peripartum psychological disturbance. Anxiety symptoms overlap with PPD, but not com-pletely, indicating that screening for postpartum mental difficulties should include both depression and anxiety.

Effect of Refractive Correction Error on Retinal Nerve Fiber Layer Thickness: A Spectralis Optical Coherence Tomography Study.

BackgroundIdentifying and assessing retinal nerve fiber layer defects are important for diagnosing and managing glaucoma. We aimed to investigate the effect of refractive correction error on retinal nerve fiber layer (RNFL) thickness measured with Spectralis spectral-domain optical coherence tomography (SD-OCT).Material/MethodsWe included 68 participants: 32 healthy (normal) and 36 glaucoma patients. RNFL thickness was measured using Spectralis SD-OCT circular scan. Measurements were made with a refractive correction of the spherical equivalent (SE), the SE+2.00D and the SE–2.00D.ResultsAverage RNFL thickness was significantly higher in the normal group (105.88±10.47 μm) than in the glaucoma group (67.67±17.27 μm, P<0.001). In the normal group, +2.00D of refractive correction error significantly affected measurements of average (P<0.001) and inferior quadrant (P=0.037) RNFL thickness. In the glaucoma group, +2.00D of refractive correction error significantly increased average (P<0.001) and individual quadrant (superior: P=0.016; temporal: P=0.004; inferior: P=0.008; nasal: P=0.003) RNFL measurements compared with those made with the proper refractive correction. However, −2.00D of refractive correction error did not significantly affect RNFL thickness measurements in either group.ConclusionsPositive defocus error significantly affects RNFL thickness measurements made by the Spectralis SD-OCT. Negative defocus error did not affect RNFL measurement examined. Careful correction of refractive error is necessary to obtain accurate baseline and follow-up RNFL thickness measurements in healthy and glaucomatous eyes.

Evaluation of retinal nerve fiber layer, ganglion cell layer and macular changes in patients with migraine.

The aim of this study was to investigate retinal nerve fiber layer (RNFL), ganglion cell layer (GCL) thickness, macular changes (central subfield thickness (CST), cube average thickness (CAT), cube volume (CV) in patients with migraine using spectral-domain optical coherence tomography (OCT) and to assess if there was any correlation with white matter lesions (WML). In this prospective case-control study, RNFL, GCL thickness and macular changes of 19 migraine patients with aura (MA), 41 migraine without aura (MO) and 60 age- and gender-matched healthy subjects were measured using OCT device. OCT measurements were taken at the same time of the day to minimize the effects of diurnal variation. The average, inferior and superior quadrant RNFL thickness were significantly thinner in patients with migraine (p=0.017, p=0.010, p=0.048). There was also a significant difference between patients with and without aura in the mean and superior quadrant RNFL thickness (p=0.02, p=0.043).While there was a significant thinning in CST and CAT in patients with migraine (p=0.020), there were no significant difference in GCL measurements (p=0.184). When the groups were compared to the control group, there were significant differences between MA and the control group regarding average, superior and inferior quadrant RNLF thickness (p<0.001, p=0.025, p<0.001). On the other hand, there were significant differences between MO and the control group regarding average and inferior faces (p=0.037, p=0.04). When OCT measurements were evaluated according to the frequency of attacks, CST and GCL thickness were significantly thinner in patients who had more than four attacks a month (p=0.024, p=0.014). In patients with WML, only CV measurements were significantly thinner than migraine patients without WML (p=0.014). The decreased RNFL, CST, CAT and CV of the migraine patients might be related to the vascular pathology of the disease. Because WML was not correlated with the same measurements except CV, we think that further studies are needed to evaluate the etiopathologic relationship between OCT measurements and WML in migraine patients.
