Prominant squamous metaplasia with extensive myxoid degeneration and associated ischaemic infarction in fibroadenoma of breast: A diagnostic dilemma

Fibroadenoma is the most commonly diagnosed benign tumour in adolescents and young women. Squamous metaplasia and prominent myxoid degeneation with associated ischaemia infarction is not a common finding without history of trauma in fibroadenoma. We report a 16 year old unmarried female which posed a diagnostic dilemma due to presence of unusual histopathological findings, which is sparsly reported in English literature. Infarcted Fibroadenoma: An unusual case with its Cyto-histopathological Correlation

Fibroadenoma is a benign breast lesion of adolescents and young women. It constitutes 20 % of all benign breast tumours. Fibroadenoma is associated with many secondary changes like calcification, ossification, hyalinization, myxoid and apocrine squamous metaplasia. Spontaneous infarction in fibroadenoma is very rarely seen. We report one such case of spontaneous infarction of fibroadenoma in a 16 year old unmarried female with its cyto-histopathological correlation, that posed us with diagnostic dilemma in early stage. The possibility of this entity should be considered while diagnosing the breast lumps on cytology. The esion was further confirmed on histology.

Spontaneous infarction in fibroadenoma of breast: a rare pathological finding in a common pathological entity

Fibroadenoma is one of the most prevalent benign tumour in adolescent women. It accounts for 20% of all benign breast tumours. Spontaneous infarction in fibroadenoma is very unwonted and mostly, it is associated with gravidity or lactation. Infarction can additionally occur as a complication of antecedent fine-needle aspiration biopsy. At the institution during 2-years period we incidentally found three such cases of spontaneous infarction of fibroadenoma over varied age groups which surprised us. So, reviewed the literature and presented our findings. Three fibroadenomas with features of central necrosis were diagnosed on excision and biopsy. Spontaneous infarction within fibroadenoma is a rare occurrence. It causes diagnostic difficulties, hence conscientious search and possibility of this entity should be must during the evaluation of breast lump.

Spontaneous infarction in fibroadenoma, a diagnostic challenge on cytology: Report of two cases

Spontaneous infarction is an uncommon complication of fibroadenoma. Lack of knowledge of this rare entity may lead to an erroneous diagnosis of inflammatory lesion or malignancy on fine needle aspiration cytology (FNAC). We describe two cases of fibroadenoma with spontaneous infarction; one in a 20-year-old unmarried female while the other was a 35-year-old married female. Both patients presented with a history of sudden increase in size and pain for which they sought medical help. FNAC followed by histopathology was done. High index of suspicion would help cytopathologists to avoid misdiagnosis.

Cytopathologic diagnosis of spontaneous infarction of fibroadenoma of the breast

Infarction is an uncommon event in a fibroadenoma, which is the commonest benign tumor of the breast. Most often it occurs in pregnancy, lactation or is secondary to fine needle aspiration. Spontaneous infarction of a fibroadenoma in the absence of a predisposing condition is very rare. The cytopathologic features of infarction are necrosis and worrisome nuclear features, which are often misinterpreted as either inflammation or malignancy. We detail a report of accurate cytopathologic diagnosis of spontaneous infarction of fibroadenoma in a 17-year-old adolescent non pregnant girl. Careful attention to the cytopathologic clues like uniform thickness of the necrotic epithelial fragments, branching pattern reminiscent of the staghorn pattern despite atypical nuclear features and clinical details like young age of the patient and recent onset pain in a pre-existing lump helped arrive at the correct diagnosis and spared the patient of a radical excision. To the best of our knowledge, there are no earlier reports of correct cytopathologic diagnosis.
