557 Results of Microsurgical Clipping of Unruptured Anterior Communicating Artery Aneurysms (ACOMa) Using the Pretemporal Minimal Durotomy Approach

INTRODUCTION: Recent reports suggest the superiority of microsurgical clipping of ACOMa compared to endovascular treatments. We present our experience with the pretemporal minimal durotomy (minimal brain exposure) approach in 168 patients. METHODS: Data of 168 patients were prospectively collected and retrospectively reviewed. The pretemporal approach with the removal of the posterior roof of the orbit and the anterior clinoid process with a minimized subfrontal durotomy allowed broad exposure to the ACOMa complex and minimal exposure to the surrounding brain. The technique allowed early proximal control and 360-degree visualization of the aneurysm and its surroundings. RESULTS: The average aneurysm size was 6.4 mm. Total clip occlusion was achieved in 100%, confirmed by intraoperative indocyanine green videography and post-operative computer tomography angiography/cerebral angiography. At discharge, 93.5% were modified Rankin Scale (mRS) 0-2, and 99.0% were mRS 0-2 at 1 year follow up. There was zero mortality, and one patient with a giant aneurysm had a stable residual on follow-up. CONCLUSIONS: Microsurgical clipping of ACOM aneurysms using the pretemporal minimal durotomy approach results in more durable outcomes than endovascular technique with excellent functional outcomes and should be strongly considered as a first treatment option, especially in aneurysms that have anatomic endovascular disadvantages that have a high likelihood of recurrence.

Multispectrum Indocyanine Green Videography for Visualizing Brain Vascular Pathology

Currently, neurosurgical vascular surgery frequently uses indocyanine green (ICG)-videography (VG) to evaluate the blood flow in brain vessels. Although ICG-VG delineates intravascular ICG fluorescence as a high-intensity signal in gray-scale with dark background, it is hard to identify anatomical structures, including vasculature or surgical devices simultaneously. This report developed combination of a near-infrared (NIR) camera with particular sensitivity and an optical filter to observe the blood-flow conditions and anatomical structures. To overcome the specific issues of ICG-VG, we applied a high-sensitivity camera with a 980-nm NIR component to delineate anatomical and fluorescence images, detecting signals between 830 and 1000 nm simultaneously during operation. We used a diluted ICG phantom to evaluate fluorescence signal changes by changing wavelength of the emission light. For clinical use, we used a high-sensitivity NIR camera with a high-pass filter on a surgical microscope. The new NIR system detected signals between 770 and 1000 nm, and the lighting system illuminated objects mainly at 980-nm wavelength. Both images with the blood flow and anatomical structures were projected to the smart glasses in real time. In the phantom experiment, we found that the emission light with wide band widths (575-800 nm) evoked various intensities of ICG fluorescence. This new NIR system allowed us to observe ICG fluorescence and anatomical structures without image fusion or time-delay. The both information of anatomy and fluorescence was projected on wearable smart glasses. Furthermore, the new NIR system detected ICG-fluorescence signals for a longer duration than the original camera, which allowed us to achieve careful and detailed observation of more vasculature and fine vessels. This study proposes a new NIR system and emphasizes simultaneous observation of anatomy and fluorescence signals during operation. It paves the way for further possibilities in the development of optical systems. To understand the natural phenomena and combination of different scientific and clinical fields, it might be important to understand and combine not only fluorescence, but also natural science, optics, and background pathology. This simple system would be available for neuroendoscope and robotic surgery.

Microsurgical Resection of Thoracic Meningioma: 2-Dimensional Operative Video.

The primary goal of surgery with spinal meningioma is complete safe tumor removal and decompression of the spinal cord. For the surgical removal of spinal meningioma, internal debulking before dissection of the tumor capsule is essential. Intraoperative ultrasonography to localize the tumor is recommended by some authors, but we use indocyanine green (ICG) videography to visualize the localization of tumor before dural incision. ICG videography allows safe and complete delineation of intradural tumors before dural opening. This technique is quick, cost-effective, and simple to use, especially with its integration into the surgical microscope. Herein, we present a case of a 54-yr-old female patient presenting with a 1-yr history of numbness of the right lower limb. Neurological examinations demonstrated temperature pain disorder of right lower limb and slight dysuria. Magnetic resonance images demonstrated intradural extramedullary tumor at the level of Th6. Preoperative diagnosis was spinal meningioma. In this surgical video, we show ICG angiography before dural opening, in addition to the basic surgical procedure of the thoracic meningioma. We believe this operative video will be useful for those in training as well as practicing surgeons. We received written informed consent from the patient for this publication.

Feasibility of real-time intraoperative fluorescence imaging of dural sinus thrombosis

Dural sinus thrombosis is a well-recognized and potentially serious complication following lateral skull base surgery. Methods of diagnosis are limited to post-operative computerized tomography scans or magnetic resonance venography. There are currently no reports of an intraoperative technique used to detect dural sinus thrombosis. Here, we describe the case of a 62-year-old woman who underwent translabyrinthine resection of a right vestibular schwannoma with no evidence of sinus thrombosis on pre-operative scans. Following tumor resection, patency of the sigmoid sinus was assessed intraoperatively using indocyanine green (ICG) videography which revealed a lack of flow in the right sigmoid sinus. Postoperative CT scan confirmed thrombosis of the right sigmoid sinus. We present the first report of real-time intraoperative diagnosis of sigmoid sinus thrombosis during removal of a vestibular schwannoma. ICG videography may be used for intraoperative visualization of dural sinus integrity and patency during prolonged or technically challenging microsurgical procedures. This technique may enable periodic monitoring and early identification of filling defects which can guide further intraoperative strategies and postoperative monitoring.

Novel techniques of real-time blood flow and functional mapping: technical note.

There are two main approaches to intraoperative monitoring in neurosurgery. One approach is related to fluorescent phenomena and the other is related to oscillatory neuronal activity. We developed novel techniques to visualize blood flow (BF) conditions in real time, based on indocyanine green videography (ICG-VG) and the electrophysiological phenomenon of high gamma activity (HGA). We investigated the use of ICG-VG in four patients with moyamoya disease and two with arteriovenous malformation (AVM), and we investigated the use of real-time HGA mapping in four patients with brain tumors who underwent lesion resection with awake craniotomy. Real-time data processing of ICG-VG was based on perfusion imaging, which generated parameters including arrival time (AT), mean transit time (MTT), and BF of brain surface vessels. During awake craniotomy, we analyzed the frequency components of brain oscillation and performed real-time HGA mapping to identify functional areas. Processed results were projected on a wireless monitor linked to the operating microscope. After revascularization for moyamoya disease, AT and BF were significantly shortened and increased, respectively, suggesting hyperperfusion. Real-time fusion images on the wireless monitor provided anatomical, BF, and functional information simultaneously, and allowed the resection of AVMs under the microscope. Real-time HGA mapping during awake craniotomy rapidly indicated the eloquent areas of motor and language function and significantly shortened the operation time. These novel techniques, which we introduced might improve the reliability of intraoperative monitoring and enable the development of rational and objective surgical strategies.

ntraoperative imaging has become a beneficial and standard adjunct in cerebrovascular microsurgery. Intraoperative vasI cular evaluation may avoid parent vessel compromise or occlusion and devastating postoperative results. Detection of these complications with postoperative imaging is often too late to preclude infarction. Additionally, intraoperative evaluation may obviate the need for future surgery for clip repositioning or failure of complete aneurysm, arteriovenous malformation (AVM), or arteriovenous fistula (AVF) obliteration. Common intraoperative modalities used alone or in combination include cerebral angiography, intraoperative magnetic resonance imaging (MRI) or computed tomography (CT), Doppler ultrasound, cerebral blood flow (CBF) probe, endoscopic assistance, indocyanine green (ICG) videography, and laser speckle flow imaging (LSFI). Hybrid operative suites with biplane angiography and intraoperative MRI or CT have improved access to these imagingmodalities and intraoperative assessment. Improvements in imaging and technology have led to many means to improve visualization through both extravascular and intravascular techniques aswell as assess blood velocity and calculate CBF. Intraoperative quantification of vascular dynamics also can help in surgical assessment and planning and may help in the prediction of postoperative complications. Identification of venous sinus, tumor location, and pial supply during meningioma surgery by transdural indocyanine green videography

Indocyanine green (ICG) videography is commonly used in the neurosurgical field for minimally invasive neurosurgery. The aim of this study was to evaluate a new intraoperative imaging modality by performing transdural ICG videography during surgery for meningiomas. Between March 2011 and April 2012, 10 patients with meningiomas received intravenous injection of 12.5 mg ICG just prior to dural opening. The cases comprised 8 convexity meningiomas and 2 foramen magnum meningiomas. Efficacy of the transdural ICG videography was assessed in terms of the tumor volume, the circulation time from the first appearance of the vessel to the appearance of the venous sinus, the tendency to bleed, and the discrimination of the venous sinus. The mean tumor volume was 71.6 ± 87.9 ml (the mean is expressed ± SD throughout). The cortical arteries, veins, and the venous sinus were identified by the ICG videography transdurally. The projection of the meningiomas was identified by a shadow (which the authors call the eclipse sign). Total eclipse signs were obtained in 8 cases and partial eclipse signs were obtained in 2 cases; tumor volume in the latter was more than 200 ml. In 5 of 10 cases the adjacent venous sinuses were exposed and were successfully visualized by ICG videography in 5.92 ± 1.05 seconds from the first appearance of the vessel. In 5 of 10 cases the total and the partial eclipse signs were diminished in 3.46 ± 1.31 seconds. The diminishment of the total and the partial eclipse sign was earlier than the visualization of the venous sinus (p = 0.011, t-test), revealing bleeding from the tumor that was observed until coagulation of the feeding arteries from the intracranial arteries. Prior to opening of the dura mater, transdural ICG videography was used successfully to visualize the dural attachment of meningiomas and the venous sinus, resulting in safe and appropriate dural opening. The diminishment of the total and partial eclipse signs may represent significant feeding from the intracranial arteries and a tendency to bleed during resection.

Cortical Indocyanine Green Videography for Quantification of Acute Hypoperfusion After Subarachnoid Hemorrhage

Acute neurological deficits after subarachnoid hemorrhage (SAH) correlate with outcome, and a phase of acute hypoperfusion was characterized recently. Indocyanine green (ICG) videography is an established intraoperative imaging technique with important descriptive potential. To analyze whether ICG can be used to analyze and confirm perfusion changes early after SAH. We prospectively enrolled 11 patients with acute SAH within the past 24 hours and 14 patients undergoing surgery for unruptured aneurysms. Cortical ICG videography was performed, and offline analysis included the arterial, parenchymal, and venous cortical compartment. Transit times, signal gradient, maximum of fluorescence intensity, and the area under the curve were calculated as surrogate markers for perfusion characteristics. Arterial, parenchymal, and venous transit times were comparable in both groups. The velocity of signal change in SAH patients was significantly lower in all 3 compartments (P < .001, P < .01, P < .001, respectively), as was the peak fluorescence intensity (P < .001). In SAH patients, fluorescence intensity did not vary between areas with and without diffuse cortical blood. Area under the curve analysis showed significantly lower values in SAH patients compared with the control group (P < .001). Cortical ICG videography and analysis are feasible during surgery. Patients early after SAH have a significantly lower velocity of signal change, lower peak of fluorescence intensity, and lower overall area under the curve, but similar transit times. This technique can be used to quantify perfusion alteration, in this case, acute SAH, and may be used as an adapted measurement tool for intraoperative therapy.

Transdural Imaging of Meningiomas by Indocyanine Green Videography: The Eclipse Sign

Indocyanine green videography has been introduced into neurosurgical fields for minimally invasive neurosurgery. To establish a new intraoperative imaging modality, we performed transdural indocyanine green videography during the surgery of meningiomas. A dose of 12.5 mg of indocyanine green was injected transvenously in two cases of meningiomas just before the dural opening. Transdural indocyanine green videography was monitored. The cortical arteries and veins and the venous sinus were identified by the indocyanine green videography transdurally in both cases. The projection of meningiomas was identified as the shadow and signal negative regions, and was visualized as "the Eclipse." Transdural observation of the cortical arteries and veins and the venous sinus was successfully performed followed by the visualization of the projection of meningiomas as the shadow. We propose this sign as "the Eclipse sign." This transdural imaging method was of value in terms of precise and minimal dural incision.

Efficacy of indocyanine green videography and real-time evaluation by FLOW 800 in the resection of a spinal cord hemangioblastoma in a child

A 19-month-old child was gradually suffering from gait disturbance and was referred by his pediatrician to the authors' institution. Spinal MRI showed Gd-enhanced spinal cord tumor and congestive myelopathy. Intraoperatively the lesion was seen to be a hemangioblastoma. Because discrimination of the arterialized draining veins from the feeding arteries was difficult, indocyanine green videography was conducted to differentiate them. Real-time evaluation by FLOW 800 revealed that the slope of the average signal intensity in the feeding artery was steeper than that of the arterialized veins. The tumor was successfully resected, and postoperative indocyanine green videography showed total removal of the tumor as a signal-negative region; the circulation time between the feeding artery and the main draining vein was prolonged from 2.5 to 5.5 seconds. Indocyanine green videography and real-time evaluation by FLOW 800 were objective and effective for the excision of a tumor retaining the arteriovenous shunt. The patient recovered from congestive myelopathy and gait disturbance.

Intraoperative Use of Indocyanine Green Fluorescence Videography for Resection of a Spinal Cord Hemangioblastoma

Indocyanine green (ICG) fluorescence videography has been recently applied to the neurosurgical field, mostly in the management of cerebral aneurysms, but has had limited description in the subspecialty of spine or oncological neurosurgery. We describe a novel application of this previously defined surgical tool to assist in the resection of a residual spinal cord hemangioblastoma. Our patient is a 49-year-old woman with a residual symptomatic cervical hemangioblastoma that was previously embolized and resected at another institution. After initial symptomatic improvement, she returned with progressive symptoms, increasing radiographic spinal cord edema, and a residual lesion at the level of C1. We resected the remaining tumor with the adjuvant use of ICG fluorescence videography. Intraoperative injection of ICG clearly identified a component of the tumor underlying adhesive, opaque arachnoid that was not visualized by direct microscopy. Immediate postresection ICG videography suggested a complete resection was achieved which was later corroborated by postoperative magnetic resonance imaging. The adjuvant use of ICG videography is a useful surgical tool that permits greater visualization of the complete extent of the lesion, particularly in managing recurrent or residual lesions obscured by adhesions.

The Use of Indocyanine Green Videography for Intraoperative Localization of Intradural Spinal Tumors

Prospective. To characterize the role of indocyanine-green videography in the intraoperative localization of intradural spinal tumors. Correct intraoperative localization of intradural spinal tumors as well as tailored dural opening is essential in surgery and can be demanding in some instances. We investigated the role of indocyanine green (ICG) videography to correctly localize intradural, spinal tumors. We report a series of 30 patients with intradural spinal tumors, in which (hemi-)laminectomy or extended laminotomy was followed by ICG videography to determine intradural tumor margins (injection of intravenous ICG, visualization with operating microscope in combination with an additional, fluorescent light source). Tumor projection was verified after dural opening, and surgical resection was continued thereafter. Imaging was conducted without complications in all cases. Identification of the tumor margins before dural opening was achieved in 28 cases (93%), either by tumor enhancement or absence of ICG uptake in relation to the surrounding spinal cord or nerve roots. In 1 case, ICG videography helped to correct the previously determined level of exposure and led to cranial extension of the bony exposure. No significant contrast enhancement was seen in 2 patients who demonstrated a small lesion ventrolaterally to the cervical spinal cord. ICG videography represents a helpful tool to localize the position of intraspinal, intradural tumors, which are lateral or dorsal to the spinal cord or nerve roots. Bony exposure can be optimized before dural incision. Positioning and length of dural incision may be tailored to reduce the incidence of complications such as leakage of cerebrospinal fluid and/or accidental fiber damage.
