A Rapid Release of Corticosteroid-Binding Globulin from the Liver Restrains the Glucocorticoid Hormone Response to Acute Stress

A strict control of glucocorticoid hormone responses to stress is essential for health. In blood, glucocorticoid hormones are for the largest part bound to corticosteroid-binding globulin (CBG), and just a minor fraction of hormone is free. Only free glucocorticoid hormone is able to exert biological effects, but little is known about its regulation during stress. We found, using a dual-probe in vivo microdialysis method, that in rats, the forced-swim stress-induced rise in free corticosterone (its major glucocorticoid hormone) is strikingly similar in the blood and in target compartments such as the subcutaneous tissue and the brain. However, in all compartments, the free corticosterone response was delayed by 20-30 min as compared with the total corticosterone response in the blood. We discovered that CBG is the key player in this delay. Swim stress evoked a fast (within 5 min) and profound rise in CBG protein and binding capacity in the blood through a release of the protein from the liver. Thus, the increase in circulating CBG levels after stress restrains the rise in free corticosterone concentrations for approximately 20 min in the face of mounting total hormone levels in the circulation. The stress-induced increase in CBG seems to be specific for moderate and strong stressors. Both restraint stress and forced swimming caused an increase in circulating CBG, whereas its levels were not affected by mild novelty stress. Our data uncover a new, highly dynamic role for CBG in the regulation of glucocorticoid hormone physiology after acute stress.

Characterization and quantification of corticosteroid-binding globulin in a southern toad, Bufo terrestris, exposed to coal-combustion-waste

Corticosteroid-binding globulin (CBG) is a plasma protein that binds corticosterone and may regulate access of hormone to tissues. The role of CBG during a stress response is not clear. At least two hypotheses have been proposed: 1) CBG levels may increase in response to a stressor, thereby decreasing the amount of circulating free corticosterone, or 2) CBG levels may decline, making corticosterone available for its role in increased metabolic needs during stress. In this study, southern toads, Bufo terrestris, were exposed to a chronic pollutant (coal-combustion-waste), to determine changes in CBG and free corticosterone levels. Since toads exposed to chronic pollutants in previous studies did not exhibit the predicted changes in metabolic rate and mass, but did experience a significant elevation in total corticosterone, we hypothesized that CBG would likewise increase and thus, mitigate the effects of a chronic (i.e. 2 months) pollutant stressor. To conduct this study, we first characterized the properties of CBG in southern toads. Toad CBG has a K d = 20.6 ± 1.0 nM and a B max = 332.2 ± 5.1 nmol/L plasma. The rank order potencies for steroid inhibition of tritiated corticosterone are: dihydrotestosterone > corticosterone ≫ progesterone = testosterone ⋙ estrogen = dexamethasone. After characterization, we monitored the changes in CBG, total corticosterone, and free corticosterone in male toads that were exposed to either coal-combustion-waste or control conditions. CBG increased in all groups throughout the experiment. Total corticosterone, on the other hand, was only significantly elevated at four weeks of exposure to coal-combustion-waste. The increase in CBG did not parallel the increase in total corticosterone; as a result, free corticosterone levels were not buffered by CBG, but showed a peak at four weeks similar to total corticosterone. This finding indicates that, in this species, CBG may not provide a protective mechanism during long-term pollution exposure.

Free cortisol levels in older hysterectomized women treated with estradiol or estradiol/progesterone: Results from a randomised placebo controlled double-blind study

An age associated increase in basal activity of the hypothalamus pituitary adrenal (HPA) axis has been reported repeatedly in humans even though the interindividual variance appears to substantial. Some studies have suggested that these alterations are more pronounced in older women. Similarly pharmacological challenge studies often observed a stronger age associated HPA hyperactivity in older women then in older men. It has been hypothesised that the decrease in gonadal hormones after menopause might be responsible for these sex differences, but experimental studies in humans on this topic are rare. The present study wanted to characterize the effects of sex steroid treatment on basal free cortisol levels in older women. In a double blind experiment 51 healthy hysterectomized women (age 58–75 years) were randomly assigned to one of three treatment groups: 1) placebo, 2) estradiol (2mg estradiol valerate), or 3) estradiol/progesterone (2mg estradiol valerate/100mg micronized progesterone). Salivary samples (5 samples for a circadian day profile) for the determination of free cortisol levels were collected at baseline and after 4 and 24 weeks of treatment. In addition estradiol, progesterone and corticosteroid-binding globulin (CBG) were measured in serum. Results showed the expected strong increases in sex steroids in the two active treatment groups, which was paralleled by an increase in CBG. In contrast, free cortisol levels remained unchanged in both groups after acute (4 weeks) or chronic (24 weeks) sex steroid treatment. The current results show that the HPA axis of older women maintains stable free cortisol levels under basal conditions despite a sex steroid-induced increase in CBG. Similar observations have been reported in young women taking oral contraceptives. The present results appear to argue against a major role for sex steroid treatment in preventing or treating an age-associated increase in basal activity of the HPA axis.

Identification of differentially expressed genes in aflatoxin B1-treated cultured primary rat hepatocytes and Fischer 344 rats.

Aflatoxin B1 (AFB1), a mutagen and hepatocarcinogen in rats and humans, is a contaminant of the human food supply, particularly in parts of Africa and Asia. AFB1-induced changes in gene expression may play a part in the development of the toxic, immunosuppressive and carcinogenic properties of this fungal metabolite. An understanding of the-role of AFB1 in modulating gene regulation should provide insight regarding mechanisms of AFB1-induced carcinogenesis. We used three PCR-based subtractive techniques to identify AFB1-responsive genes in cultured primary rat hepatocyte RNA: differential display PCR (DD-PCR), representational difference analysis (RDA) and suppression subtractive hybridization (SSH). Each of the three techniques identified AFB1-responsive genes, although no individual cDNA was isolated by more than one technique. Nine cDNAs isolated using DD-PCR, RDA or SSH were found to represent eight genes that are differentially expressed as a result of AFB1 exposure. Genes whose mRNA levels were increased in cultured primary rat hepatocytes after AFB1 treatment were corticosteroid binding globulin (CBG), cytochrome P450 4F1 (CYP4F1), alpha-2 microglobulin, C4b-binding protein (C4BP), serum amyloid A-2 and glutathione S-transferase Yb2 (GST). Transferrin and a small CYP3A-like cDNA had reduced mRNA levels after AFB1 exposure. Full-length CYP3A mRNA levels were increased. When liver RNA from AFB1-treated male F344 rats was evaluated for transferrin, CBG, GST, CYP3A and CYP4F1 expression, a decrease in transferrin mRNA and an increase in CBG, GST, CYP3A and CYP4F1 mRNA levels was also seen. Analysis of the potential function of these genes in maintaining cellular homeostasis suggests that their differential expression could contribute to the toxicity associated with AFB1 exposure.

Effects of Experimentally Elevated Testosterone on Plasma Corticosterone and Corticosteroid-Binding Globulin in Dark-Eyed Juncos ( Junco hyemalis)

An earlier study of free-living male dark-eyed juncos found an increase in plasma corticosterone (B) in response to experimental elevation of plasma testosterone (T) (E. D. Kettersonet al.,1991,Horm. Behav.25,489–503). To investigate whether the increase was caused by enhanced secretion of corticosterone or by a slower clearance rate, or both, we exposed 52 captive yearling male dark-eyed juncos (Junco hyemalis) to day lengths corresponding to those of spring and implanted them with one or two testosterone-filled or sham implants (10 T-I, 22 T-II, and 20 C-males). We then examined the effect of experimentally elevated testosterone on plasma corticosterone and on corticosteroid-binding globulin (CBG), as measured by the ability of steroid-stripped plasma to bind labeled corticosterone. Plasma samples were taken five times, 2 weeks before experimental prolongation of day length and approximately every 3 weeks thereafter. Treatment with testosterone increased both plasma testosterone and plasma corticosterone two to three times above control levels, and the degree of elevation was dose-dependent. Only when all treatment groups were pooled, however, were plasma testosterone and corticosterone significantly correlated. The relationship between plasma corticosterone and time required to bleed the birds was similar for all three treatment groups, suggesting that there was no effect of treatment on the stress response. Testosterone significantly increased the capacity of the plasma to bind corticosterone, presumably because it contained more CBG, when compared to the plasma of controls. However, treatment with testosterone did not affect the affinity of the plasma for corticosterone. It seems likely that exogenous testosterone elevated corticosterone by slowing the corticosterone clearance rate via an increase in CBG. It is not clear what the net effect of chronic elevation of testosterone would be on the availability of corticosterone to target tissues.

Effects of incremental cortisol and adrenalectomy on plasma corticosteroid binding capacity in fetal sheep.

The effects of incremental cortisol infusion or fetal adrenalectomy on plasma corticosteroid-binding capacity (CBC) were examined in sheep fetuses during late gestation (term approximately 150 days). Cortisol, infused from day 120 at 1.5 mg/day for the first 3 days, 2.5 mg/day for the next 5 days, and 3.5 mg/day for the final 2 days, stimulated a significant rise in plasma CBC and immunoreactive corticosteroid binding globulin (CBG). There was a significant positive correlation between individual values for total plasma cortisol concentrations and CBC values. In contrast, fetal adrenalectomy at day 115 prevented the rise in plasma CBC found in intact fetuses at term. These experiments show that exogenous cortisol, given in a manner that mimics the prepartum rise in fetal plasma cortisol, stimulates CBG biosynthesis, whereas abolition of the cortisol rise prevents the increase in CBG. The study provides strong support for the proposal that the prepartum increase in CBG biosynthesis in fetal sheep occurs in response to the progressive rise in adrenal cortisol output by the fetus towards term.

Corticosteroid-binding globulin (CBG) in fetal development.

In fetal sheep the prepartum increase in plasma cortisol concentration is associated with an increase in high affinity corticosteroid binding activity in plasma. This appears to reflect an increase in corticosteroid-binding globulin (CBG) biosynthesis from the fetal liver, and evidence is presented that hepatic CBG gene expression is increased by exposure to glucocorticoids in the fetus. Immunoreactive CBG is found in other fetal tissues, and CBG mRNA is present in fetal pituitary. CBG reduces the ability of cortisol to exert negative feedback on basal or CRH-stimulated ACTH output by fetal sheep pituitary cells in culture. We suggest that CBG interacts with cortisol in a manner that maintains a low negative feedback on the pituitary, and perhaps hypothalamus. This constitutes a component of the cascade of events that is associated with hypothalamic-pituitary-adrenal activation in the late gestation fetus, and with the onset of parturition.

Glucocorticoid-induced increase in plasma corticosteroid-binding globulin levels in fetal sheep is associated with increased biosynthesis and alterations in glycosylation.

In fetal sheep, there is a concomitant prepartum rise in cortisol and corticosteroid-binding globulin (CBG) that maintains a low free plasma cortisol level and allows for a low negative feedback effect of cortisol on the secretion of ACTH from the fetal pituitary. However, the stimulus for the prepartum increase in CBG and the mechanism(s) of this effect are not known. It has been proposed that glucocorticoids increase CBG concentrations, and therefore, we infused fetal sheep with the synthetic glucocorticoid dexamethasone (DEX; 2 micrograms/min over 15 min every 2 h for 96 h, n = 5) or saline (n = 5). The plasma corticosteroid-binding capacity increased from 30.0 +/- 2.4 to 55.6 +/- 7.7 and 92.6 +/- 11.1 ng/ml at 48 and 96 h, respectively, of DEX infusion. To examine possible mechanisms of increasing fetal plasma CBG, we first cloned and sequenced a sheep CBG cDNA and purified the protein. This allowed us to deduce the primary structure of ovine CBG and to demonstrate that hepatic CBG mRNA abundance (single transcript of 1.8 kilobases) rose from 0.9 +/- 0.2 to 3.6 +/- 1.6 arbitrary units after 96 h of DEX treatment. Fetal DEX treatment produced a significant increase (7.1 +/- 1.2% to 13.1 +/- 1.4%) in the Concanavalin-A-binding forms of CBG that predominate in adult sheep plasma. There was negligible transfer of purified [125I]CBG from the ewe to fetal plasma, urine, or amniotic fluid. We also injected adult sheep with DEX (10 mg/day for 4 days) and demonstrated a significant decrease in plasma corticosteroid-binding capacity by 24 h, which remained suppressed for the duration of the study. After 96 h of DEX treatment, there was also a significant decrease in adult hepatic CBG mRNA abundance. We conclude that glucocorticoids increase fetal plasma CBG in part by increased hepatic biosynthesis. It may also be accentuated by a change in the glycosylation of CBG, but cannot be attributed to transplacental transfer. Furthermore, glucocorticoid treatment exerts opposite effects on CBG biosynthesis in fetal and adult sheep.

Effect of adrenalectomy at different pregnancy stages on maternal and fetal serum corticosteroid binding globulin and corticosterone in the rat

The response of pregnant rat corticosteroid binding globulin to maternal adrenalectomy was studied as a function of the stage of pregnancy. Non-pregnant or pregnant rats were deprived of their adrenal glands during 4 days. In non-pregnant animals, adrenalectomy led to undetectable corticosterone levels and to the doubling of corticosteroid binding globulin. In pregnant rats adrenalectomized at 12 days and studied at 16 days, the serum corticosterone was likewise undetectable and the corticosteroid binding globulin was doubled as compared with pregnant rats of the corresponding age. In contrast, adrenalectomy from day 14 to 18 or from day 16 to 20 did not deplete the maternal serum corticosterone and the corticosteroid binding globulin remained unchanged. Under these conditions neither fetal corticosteroid binding globulin nor fetal corticosterone were modified. However, when the pregnant rats adrenalectomized from day 16 to 20 also received an injection of 30 mg of metyrapone on days 19 and 20 in order to inhibit fetal adrenal secretion, the maternal response was again a depletion of serum corticosterone together with an increase in corticosteroid binding globulin. Under these conditions, the fetus also reacted by a fall of corticosterone and a rise of corticosteroid binding globulin. Our results suggest that the maternal response of corticosteroid binding globulin to adrenalectomy depends on the pregnancy stage inasmuch as it may be influenced by a supply of corticosterone from the fetus during late pregnancy. Moreover, they show that in this late period, fetal corticosteroid binding globulin is regulated independently.

Thyroid hormones and plasma corticosteroid binding globulin capacity in fetal and newborn lambs.

Although the increase in plasma corticosteroid binding globulin (CBG) capacity during late gestation in the fetal sheep is dependent on an intact fetal pituitary, the possible role of thyroid hormones is unknown. In rats, thyroid hormones control the postnatal increase in CBG. In the present study we investigated the effect of thyroidectomy and T3 treatment on CBG capacity in both the fetal and newborn lamb. Fifteen fetal lambs were thyroidectomized in utero at 119-121 days gestation and then infused with either vehicle or T3 (8-50 micrograms/h) for 8 days. Thyroidectomy alone did not prevent the expected increase in CBG capacity. T3 treatment of thyroidectomized fetuses caused a dose-dependent reduction in CBG capacity (maximum decrease = 31% of control). By contrast, plasma concentrations of total protein, albumin, and beta-globulins were unaltered. T3 treatment reduced plasma total corticoid concentrations, but the unbound level was unchanged. Administration of T3 (maximum dose 8 micrograms/h) to one intact fetus did not affect CBG capacity. Eleven newborn lambs were thyroidectomized between 2 and 8 days of age and infused with either vehicle or T3 (25 micrograms/h) for 8 days. Thyroidectomy alone did not prevent the normal decrease in CBG capacity after birth. T3 administration to the thyroidectomized lambs resulted in even lower plasma CBG capacity. We conclude that the prenatal increase in CBG capacity of the fetal sheep is not regulated by thyroid hormones, and that pharmacological concentrations of T3 decrease plasma CBG capacity and secondarily plasma corticoids. These findings indicate that CBG in the fetal sheep is apparently controlled by a different hormone(s) than in other species which have been studied.

Postnatal development of corticosteroid-binding globulin: effects of thyroxine.

The aim of this study was to examine the temporal and dose characteristics of the corticosteroid-binding globulin (CBG) response to T4 and to determine whether this response is due to stimulation of hepatic biosynthesis of CBG. When n-propylthiouracil (PTU)-induced hypothyroid pups were given a single injection of T4 (0.1 microgram/g BW) on postnatal day 5, 6, or 7, only pups treated on day 7 showed a significant increase in CBG. In a T4 dose-response study conducted with 5- and 8-day-old pups, older pups exhibited maximum CBG concentrations (Rmax) which were 2.5-fold higher than those of younger pups. The D1/2 (dose required to elicit half the maximum response) values were similar at both ages. The effect of T4 withdrawal on serum CBG was also studied in PTU-treated pups. T4 injection on postnatal days 5-19 resulted in a progressive rise in CBG. In pups treated with T4 on days 5-9 and then withdrawn from treatment through day 20, serum CBG showed no further increase but was maintained at an elevated level. Using a liver slice system to assess CBG production in vitro, livers from 14-day-old hyperthyroid pups produced 4.77 ng corticosterone bound/g liver, while livers from euthyroid pups produced no CBG. We conclude: 1) the response of CBG to T4 is a function of the age of the animal; between days 5 and 8 this is due to increased Rmax without any change in sensitivity to T4 (D1/2); 2) T4 is required not only to initiate but also to sustain the developmental increase in CBG; and 3) T4 elicits an increase in circulating CBG by stimulating its synthesis.

Role of thyroxine in coordinate control of corticosterone and CBG in postnatal development

The role of thyroxine (T4) in the ontogeny of serum corticosteroid-binding globulin (CBG) and corticosterone has been studied in the rat. Daily injection of T4 (0.1 micrograms/g body wt) resulted in the precocious appearance of both CBG and corticosterone. A dose-response study revealed an increase in both CBG and corticosterone at thyroxine doses greater than 0.02 micrograms/g body wt. In propylthiouracil-induced hypothyroid pups, the developmental rise of both CBG and corticosterone was suppressed. To determine whether T4 elevation of CBG was mediated by corticosterone, serum CBG was measured in corticosterone-injected pups (10 micrograms/g body wt). No increase in CBG was detected on days 8 and 10. A slight increase, only one-eighth that elicited by T4, occurred on day 12. There was also no synergistic effect between T4 and corticosterone. We conclude that the postnatal increase in serum T4 concentrations cues the developmental rise of both CBG and corticosterone in the rat.

Hormonal control of postnatal development of corticosteroid-binding globulin.

The hormonal regulation of the ontogenic rise in serum corticosteroid-binding globulin (CBG) has been studied in the rat. Pups received daily injections of estrogens (either estradiol, estrone, or diethylstilbesterol, each at 0.05 microgram/g body wt) or thyroxine (0.1 microgram/g body wt) on postnatal days 2-7. When CBG binding capacity was determined on day 8, only the thyroxine-injected pups were found to have elevated CBG. This effect of thyroxine on CBG binding capacity was further studied by daily administration of the hormone (0.1 microgram.g body wt-1.day-1) between postnatal days 5-12. This caused a precocious rise in serum CBG, with CBG values 6- to 38-fold higher than euthyroid controls on days 8, 10, and 12. Conversely, in pups made hypothyroid by administration of propylthiouracil, the normal ontogenic increase in CBG was suppressed. Thyroxine replacement resulted in the reappearance of the CBG rise. These results suggest that the developmental rise in the binding capacity of CBG is independent of estradiol and estrone and instead is elicited by the rising concentrations of circulating thyroxine that normally occur in the early postnatal period.

MEASUREMENT OF CORTICOSTEROID-BINDING GLOBULIN IN MAN.

A method for quantitation of corticosteroid-binding globulin (CBG) has been developed utilizing gel filtration and 4-14C cortisol. With this method it was demonstrated that exchange equilibrium was essentially instantaneous at 37 C but that 2–4 hr were required at 4 C. Cortisol was demonstrated to exert a protective effect against destruction of binding by heating to 60 C. Increase in binding with decrease in assay temperature was again demonstrated for normal and pregnancy serum utilizing the present method. Values in 19 normal males ranged from 23 to 45 mg/l of plasma (35±3.6). No significant difference was found in 16 normal females (mean 33±4.2 mg/l), and these values were in close agreement with the results reported by us previously after direct isolationof CBG from normal plasma. Pregnant females had a significant increase in CBG by the second month and reached an elevated plateau value of 73 mg/l by the sixth lunar month.
