Analysis of Age and Gravidity Factors with the Occurrence of Prolonged Parturition in the Midwifery Room of the Viqueque City District Hospital, Timor-Leste

Prolonged or obstructed labor constitutes 8% of the causes of maternal mortality globally and is also the cause of fetal death. One of the causes of prolonged labor is maternal age and gravidity. The purpose of this study was to analyze the factors of age and adolescence with the incidence of prolonged labor in the midwifery room of the Viqueque City Hospital, Tomor Leste.This study used an analytical research design with a cross sectional approach. The population was all women who gave birth for a long time in the midwifery room of the Viqueque City Hospital, Timor Leste, which could be 30 people. The sampling technique used accidental sampling. The research instrument used a questionnaire and observation sheet. The data analysis used Chi Square statistical test.The results showed that more than half of the respondents (66.7%) aged 20-35 years were 20 respondents and more than half (53.3%) were multigravidas as many as 16 respondents, more than half (60%) did not occur during prolonged labor, namely as many as 18 respondents.Based on the results of statistical tests using Chi Square calculations, it is not known that no one knows it is 0.002 <0.05 so it can be shown that there is a relationship between the incidence of prolonged labor.Women with higher parity are at higher risk of prolonged labor because the uterus has weakened the uterine wall muscles due to a previous pregnancy. The solution that needs to be done regarding the results of the above research is to enable health education about the causes and risk factors for the old party to the mother by health workers and early detection of risk factors for prolonged labor must be improved.

Identifikasi Jenis Kegagalan Persalinan Normal pada Ibu Remaja

This study aims to identify the incidence of failure of normal delivery in teenage mothers at the Masyita Women's and Children's Hospital in Makassar City. The method used is quantitative research with a cross-sectional approach. The results of this study showed that there were 46 cases of uterine inertia (25.0%), 70 cases of premature rupture of membranes (38.0%), 24 cases of severe preeclampsia (13.0%), 24 cases of preeclampsia placenta previa as many as 8 people (4.3%). the incidence of prolonged labor was 36 people (19.6%). The conclusion of this study is that there are several findings of failure of normal delivery in teenage mothers including inertia, premature rupture of membranes, severe preeclampsia, placenta previsa and prolonged parturition.

Asuhan Kebidanan Pada Ibu Bersalin Primigravida Dengan Partus Lama Di Klinik Pratama Siti Kholijah JL. Marelan I, Terjun, Kec. Medan Marelan

Prolonged labor is labor that lasts more than 24 hours in primi, and more than 18 hours in multi.(Murnika, 2015) Globally, about 8-11% of pregnant women experience abnormalities in the 1st stage of labor. Delayed labor in the active phase was found in 25% of nulliparous women and 15% of multiparous women. A study in Sweden found that 23% of patients about to go into labor experienced a delay in progress in the latent phase. The study also found that the delay in progress was more common in nulliparous women than multiparous women (29.2% vs. 17). Long parturition is one of the contributors to maternal mortality in the world, based on WHO in 2014 there were cases of prolonged labor in women in the world, namely 289 per 100,000 live births. Meanwhile, in Indonesia, the incidence of prolonged labor is the highest in ASEAN, namely 359 per 100,000 live births, mothers die from prolonged labor (WHO, 2014). Researchers provide comprehensive midwifery care for pregnant women, maternity, postpartum, BBL, neonates and family planning using a midwifery management approach to Mrs "I" with a case of prolonged labor. Methods care in this study by observation and evaluation. The subject of this care is Mrs "I" with a long labor at the Siti Kholijah Primary Clinic. Comprehensive midwifery care for Mrs. "I" during the third trimester of pregnancy, in labor with prolonged labor and spontaneous delivery, during the postpartum period with normal childbirth, BBL with BBLN, neonates with normal neonates, and being an acceptor for condom. This research activity is about handling long parturition in maternity mothers. Researchers conducted observations and evaluations to mothers on a regular basis. Observations and evaluations were carried out not only knowing and assessing contractions and progress of labor but also knowing and assessing emotional conditions, levels of fatigue, and support for mothers. The mother's hydration status also needs to be considered and if possible, the mother can be asked to eat small portions so that she can gather energy for delivery. This research was conducted at the Pratama Siti Kholijah clinic, on April 18, 2022. This observation and evaluation began at 21.30 WIB until the mother carried out a normal delivery process until the fourth stage on April 20, 2022 at 22.30.

The Effect of Fetal Weight and Uterine Contractions on the Duration of Labor

Background: Prolonged labor is defined as labor that has lasted 12 hours or more but is not followed by birth. The incidence of prolonged labor is quite high; it is known that 1 in 5 maternity mothers experience prolonged labor. The case of prolonged labor is included in the list of the 10 highest cases at Srikandi General Hospital Jember, Indonesia. Prolonged labor lasting > 12 hours is associated with severe postpartum hemorrhage, which is the second leading cause of death in East Java and the leading cause of maternal death in Jember Regency. Several factors are thought to affect the duration of labor including uterine contractions and fetal weight. Purpose: This study aims to determine the effect of fetal weight and uterine contractions on the duration of labor at Srikandi IBI General Hospital, Jember, East Java, Indonesia. Methodology: This is an observational analytic study using a cross-sectional design. The population consists of mothers who give birth at Srikandi General Hospital, Jember: 119 mothers who give birth with a sample size according to the Slovin formula of 92 mothers who give birth determined through Systematic Random Sampling. Research variables were assessed through partographs. Data were analyzed by path analysis. Findings: fetal weight has no significant effect on the duration of labor in both the first and second stages (t value = -0.07; 0.91). Uterine contractions had a significant effect on the duration of the second stage (t value = -2.22; -3.65) but did not significantly affect the duration of the first stage of labor (t value = -0.73; -1.13). Conclusion: This study shows that the more adequate uterine contractions, the shorter the duration of the second stage of labor. There may be other factors that influence the duration of labor including age, parity, nutritional status, maternal nutrition, psychology, family assistance, and non-pharmacological care. Further research is needed to examine the effect of these variables on labor.

MIXED METHOD STUDY TENTANG IMPLEMENTASI MODEL ASUHAN KEBIDANAN “PARTNERSHIP CARE” DALAM PROSES PERSALINAN DI PRAKTIK MANDIRI BIDAN KOTA BANJARMASIN

Long parturition is the third largest contributor to maternal mortality in Indonesia, the incidence of prolonged labor in 2007 and 2012 remains the same at 5% (SDKI, 2012). The Partnership care model in childbirth is described by a situation where the midwife and woman become friends, friends, mutually support each other, have an equal position in the delivery process, thus partnership care can facilitate the delivery process and become the solution of choice in cases of prolonged labor.The results of a survey of 10 PMB midwives in Banjarmasin City found that 70% of PMBs did not implement the partnership care model of midwifery care in childbirth.This study aims to determine the implementation of the partnership care model of care in the delivery process by the Independent Practice of Midwives in the City of Banjarmasin quantitatively and qualitatively and to find inhibiting factors from the implementation of the partnership care model of care in the delivery process.The method used in this research is Mixed Method with sequential approach. The sample used is total sampling, namely all PMB in Banjarmasin City. Research results will be presented in quantitative and qualitative forms.

The Relationship of Anemia in 3rd Trimester Pregnant Women with the Length of the 1st Stage of Latent Phase

Prolonged labor is labor that lasts more than 24 hours in primi and more than 18 hours in multigravida. The incidence of prolonged labor in KB Karunia in 2019-2020 is (79.36%). The purpose of this study was to determine the relationship between anemia and the incidence of prolonged labor in the Karunia family. The population studied were 100 patients and found (79.36%) mothers who had anemia and prolonged labor and (40.54%) mothers who were not anemic experienced normal first stage labor. The research design used in this article is cross-sectional, in which only observations and measurements of variables are carried out at one particular moment. The instruments used are medical records and patient partographs or use secondary data by applying the purposive sampling method. The results showed that the results of the chi square test showed P value of 0.056 > 0.05 which indicated that there was a relationship between the length of the first stage and the incidence of anemia. This shows that anemia in pregnancy has a relationship with the length of the 1st stage. The conclusions mostly show that there is a relationship between anemia and the incidence of the duration of the 1st stage of birth. Family Planning Karunia in 2019-2020.

951-P: Diabetes Management Delivery and Pregnancy Outcomes in Women with Gestational Diabetes Mellitus during the First Wave of the 2020 COVID-19 Pandemic: A Single Reference Centre Report

The COVID-19 pandemic has forced a rapid adaptation of healthcare services to secure medical care for many patients’ groups. This includes women with Gestational diabetes mellitus (GDM). We evaluated the impacts of the first COVID-19 wave on parameters such as the GDM treatment, glycemic control and pregnancy outcomes. In this retrospective study from a reference diabetes centre (Krakow, Poland), we compared patient data from two different time periods: the first wave of COVID-19 pandemic (March 2020 - June 2020) and the preceding five months (October 2019 - February 2020). Data was collected from the medical records and telephone surveys. No patient was diagnosed with concomitant COVID-19. We included 155 women - Group 1 N=73 and Group 2 N= 82 from the COVID-19 pandemic period and non-COVID-19 period, respectively. During the COVID-19 pandemic, almost half of all GDM women (N1=36, 49.3%) used telemedicine as a method of contacting their diabetic specialist while this tool was not utilized in the earlier period. Moreover, these patients reported difficulties in performing blood glucose self-control more often (N1=20, 27.4% vs. N2=7, 8.5% p=0.002) and spent less time on diabetes education and training than the control group on average (N1=39, 53.4% vs. N2=9, 9.8% below 2 hours of training; p≤0.001). Glycemic control parameters were very similar and most analysed pregnancy outcomes occurred with comparable frequencies. Differences were found with respect to the incidence of prolonged labour which was more frequent in the COVID-19 period Group 1 (N1=12, 16.4% vs. N2=3, 3.7% p=0.007) whereas no episodes of pre-eclampsia were observed in this group (N1=0 vs. N2=7, 8.5% p=0.01) during this same time period. We report that the first wave of the COVID-19 pandemic did not seem to have a negative impact on glycemic control and pregnancy outcomes in GDM women, in spite of difficulties in diabetes management delivery. Analysis of The Factor That Influence The Incidence of CED In Pregnant Women at Turi Health Center Lamongan Regency

The high incidence of CED in pregnant women at Puskesmas Turi is caused by maternal parity which has an impact on the incidence of prolonged labor and delivery bleeding. The purpose of the study was to analyze the factors of parity, pregnancy spacing, and work status on the incidence of CED in pregnant women in Turi Health Center. The study uses a quantitative approach with cross sectional study design. The total population of 77 pregnant women and taken as many as 64 people with simple random sampling technique. Analysis using chi square and phi. The results showed that pregnant women with multipara parity were at risk of experiencing CED from the statistical test results, namely p-value of 0.012, α = 0.05, p<α, then H1 was accepted and H0 was rejected. This showed the influence between Parity and the incidence of CED in pregnant women at the Turi Health Center. Pregnant women with a pregnancy interval <lt;2 years until 10 years are at risk of experiencing CED from the statistical test results, the p-value of 0.044, α = 0.05, p <α, then H1 is accepted and H0 is rejected, this shows the influence between Distance between Pregnancy and CED events for pregnant women in Turi Health Center. In addition, pregnant women who do not work are at risk of experiencing CEDs from the statistical test results, namely p-value of 0.025, α = 0.05, p <α, then H1 is accepted and H0 is rejected, this shows the influence between occupational status and the incidence of CEDs in the mother pregnant at Turi Health Center. The conclusion of this research is the need for health promotion efforts on the importance of planning the number of children and how to regulate the distance of pregnancy and efforts to increase income for mothers who do not work.

PENGARUH AROMATERAPI LAVENDER TERHADAPPENGURANGAN NYERI PERSALINAN KALA I AKTIF

Labor Pain is caused by uterine contractions through increased secretion of catecholamine and cortisol levels, leading to a decrease in uterine blood flow and decreased uterine activity that will result in prolonged labor. Lampung Province has one maternal mortality case caused by prolonged-labor. The incidence of prolonged labor at midwife private practice Opsi Okta HN in June was 20% (2 cases), July was 27% (3 cases), and in August was 20% (2 cases). Lavender aromatherapy is one of the non-pharmacological useful methods to reduce the pain, relax the mind, reduce tension and anxiety and give peace to the maternity mother. This study aims to determine the influence of lavender aromatherapy on the reduction of labor pain during the active phase I. This study was conducted with the experimental design of the final and post test group. The sample technique used purposive sampling on 15 people. The collection of pain data using the scale of assessment of pain and lavender aromatherapy with the observational method. The univariate analysis of data using mean and bivariate analysis with parametric statistical test dependent Paired t Test. The result of the average intensity of pain in maternal mothers before given lavender aromatherapy was 7.07 (Pain Weight) and after given lavender aromatherapy was 5.53 (moderate pain). The result of Paired t Test is obtained p value 0,000 (p <α 0,05).The Conclusion of the research is the influence of lavender aromatherapy on the reduction of labor pain during the active phase I. It is expected that midwives can apply lavender aromatherapy and non-pharmacological practical method as an alternative in reducing labor pain.

Controlled-release dinoprostone vaginal insert for cervical ripening and labor induction

Labor is induced in approximately 20% of pregnancies in Europe and North America. Labor induction in patients with an unfavorable cervix is associated with a higher incidence of prolonged labor and higher rates of operative and cesarean delivery. Prostaglandins used for cervical ripening lead to shorter labor, less use of oxytocin and a greater likelihood of vaginal delivery within 24 h. Dinoprostone (prostaglandin E2) has been used since the 1970’s and is recognized as an agent that not only results in cervical ripening but also activates myometrial contractility. A proprietary sustained- and controlled-release dinoprostone vaginal insert releases a continuous dose of dinoprostone for 12 h and has been shown to decrease the time to vaginal delivery compared with placebo. This controlled-release insert has been compared with various prostaglandin agents and provides safe and efficacious cervical ripening in women with an unfavorable cervix.

Active management of labour revisited: the first 1000 primiparous labours in 2000

This was a prospective observational study in a tertiary referral obstetric unit in Dublin to ascertain the delivery outcome in the first 1000 nulliparous women in 2000. The initial diagnosis of labour was confirmed in the first 1000 consecutive nulliparous women in spontaneous labour with cephalic presentation at term. All patients underwent active management of labour. Active management included strict criteria for the diagnosis of labour, early amniotomy, 2-hourly vaginal examinations, oxytocin augmentation where progress of labour was slow and the presence of a companion (personal nurse) in labour. Epidural analgesia was freely available. Mode of delivery, duration of labour, analgesia usage and maternal and perinatal complications were the main outcome measures. All patients presented with painful uterine contractions, 75% with show in addition, and 36% had spontaneous rupture of membranes on admission. Eighty per cent presented with a cervical dilatation of h 2 cm; 72.2% had epidural for pain relief. In 81.8% of cases electronic fetal monitoring was used to monitor the baby. Oxytocin was used in 51.8% to augment labour and a further 6.4% required its use for the first time in the second stage of labour. Fetal blood sampling was performed in 20.8%. The spontaneous vaginal delivery rate was 71.8%, 4.2% were delivered by caesarean section and the operative vaginal delivery rate was 24%. The incidence of prolonged labour (labour duration >12 hours) was 4.3%. Postpartum haemorrhage occurred in 3.8% of mothers and 1.6% of babies were admitted to the special care baby unit. Our study suggests that active management of labour is associated with a low incidence of prolonged labour and a low caesarean section rate.

Prolonged spontaneous labour in primigravidae whose labour was actively managed: results of an audit

SummaryThe aim of this observational study was to examine prolonged spontaneous labour in primigravidae with a single, live fetus and cephalic presentation, more than 12 hours duration from the time of admission to the delivery unit to the birth of the baby. All labours were subject to active management as practised in this institution. The incidence of prolonged labour was 1.7 per cent (n = 158). Low cervical dilatation when the diagnosis of labour was made, prolonged pregnancy and increased birth weight (> 3499 g) were more common in prolonged labour than in primigravid labours of less than 12 hours duration. The most common association with prolonged labour was inefficient uterine action (78.5 per cent, n=124). Twenty-seven patients (17.1 per cent) had persistent occipito-posterior position and seven had cephalopelvic disproportion. There were no maternal deaths. Induction of labour postdates in primiparae using vaginal prostaglandin tablets

The management of pregnancy after 40 completed weeks remains controversial because induction of labour has been, in the past, associated with an increase in the incidence of caesarean section. This study examined the use of vaginal prostaglandin tablets for the induction of labour in selected primiparae after forty one weeks of pregnancy. The outcome of prostaglandin induction in 118 consecutive patients was compared with the outcome in 90 consecutive patients who went into labour spontaneously at the same gestation before induction could be undertaken. The active management of labour was applied in both groups and, therefore, the management of labour was strictly standardised. There was no difference between the two groups in the mode of delivery. Nor was prostaglandin induction associated with an increased incidence of prolonged labour. In selected primiparae, the cautious use of vaginal prostaglandin tablets can induce labour successfully postdates without increasing the complications of labour. A larger study is required to examine the effects of the induction of labour at this gestation on the fetal outcome.

The regime outlined in this paper has reduced the Caesarean section rate, the incidence of prolonged labour and the perinatal mortality rate.

SummaryThe reproductive performance of one hundred obese negro women was compared with that of an equivalent number of negro women of average weight. The main hazards of obesity were hypertension and diabetes mellitus. The incidence of prolonged labour was not increased, and there was a low incidence of prematurity. The infants were significantly larger in the obese group but despite this, dispropor‐tion seldom occurred. The obese patients gained less weight antenatally, and lost more in the puerperium, than the controls. There was an age‐related difference in mean antenatal weight‐gain and puerperal weight‐loss in the obese, but not in the controls.
