Risk factors for cough after pulmonary resection

BackgroundTo investigate the risk factors for cough after pulmonary resection.MethodsThe PubMed, Embase, Web of Science, ClinicalTrials.gov, and China National Knowledge Network databases were searched from inception to November 2022. The Q tests and I2 statistic were used to evaluate the heterogeneity. Odds ratios (OR) were combined using the inverse variance method. All statistical analyses were performed by RevMan 5.4.1.ResultsNineteen studies with 4755 patients were included, the incidence of postoperative cough was 21.1%-55.8%. The results showed that young age [OR = 0.66, 95% CI (0.46, 0.96), p = 0.03], female sex [OR = 1.69, 95% CI (1.07, 2.66), p = 0.02], preoperative cough [OR = 5.96, 95% CI (2.58, 13.73), p < 0.01], right lobe operation [OR = 2.14, 95% CI (1.44, 3.19), p < 0.01], lobectomy [OR = 3.70, 95% CI (1.73, 7.90), p < 0.01], subcarinal lymph node dissection [OR = 3.45, 95% CI (1.86, 6.39), p < 0.01], mediastinal lymph node removal [OR = 3.49, 95% CI (2.07, 5.89), p < 0.01], closure of bronchial stump with stapler [OR = 5.19, 95% CI (1.79, 15.07), p < 0.01], peritracheal lymph node resection [OR = 3.05, 95%CI (1.40,6.64), p < 0.01], postoperative acid reflux [OR = 11.07, 95%CI (4.38,28.02), p < 0.01] were independent risk factors for cough after pulmonary resection.ConclusionsYoung age, female sex, preoperative cough, right lobe operation, lobectomy, subcarinal lymph node dissection, mediastinal lymph node removal, closure of bronchial stump with stapler, peritracheal lymph node resection, and postoperative acid reflux are independent risk factors for cough after pulmonary resection.

Compound lidocaine/prilocaine cream combined with tetracaine prevents cough caused by extubation after general anaesthesia: a randomised controlled trial

BackgroundCoughing caused by tracheal extubation is common following general anaesthesia. Heavy aerosol production by coughing during recovery from general anaesthesia in patients with respiratory infections (especially COVID-19) may be one of the highest risk factors for infection in healthcare workers. The application of local anaesthetics to the endotracheal tube is an effective method to reduce coughing. The most commonly used anaesthetics are compound lidocaine/prilocaine cream and tetracaine spray. However, coughing still occurs when the two anaesthetics are used alone. We speculated that the application of compound lidocaine/prilocaine combined with tetracaine spray would better prevent coughing caused by tracheal extubation.MethodsPatients scheduled for laparoscopic cholecystectomy or cholecystectomy combined with common bile duct exploration under general anaesthesia were randomly assigned to Group C (saline spray), Group L (2 g compound lidocaine/prilocaine cream contains 5 mg of lidocaine and 5 mg prilocaine)), Group T (tetracaine) and Group F (compound lidocaine/prilocaine cream combined with tetracaine). The incidence of coughing, the endotracheal tube tolerance assessment, the incidence of agitation, the active extubation rate, the incidence of postoperative pharyngeal pain and the incidence of postoperative cough were recorded and analysed. Systolic blood pressure (SBP), diastolic blood pressure (DBP), heart rate (HR), and the plasma concentrations of epinephrine and norepinephrine were measured immediately before extubation and 1 min after extubation.ResultsA total of 211 patients were randomly assigned to Group C (53 cases), Group L (52 cases), Group T (52 cases) and Group F (54 cases). The primary result is assessment of the incidence of cough. The patients emerged from general anaesthesia, 96% of Group C had cough, which was significantly reduced in Group L (61.5%, P < 0.001), Group T (75%, P < 0.05) and Group F (22.2%, P < 0.001). Group F had a significantly reduced incidence of cough compared to Group L and Group T (P < 0.05 or P < 0.01, respectively). The secondary results were assessed. The endotracheal tube tolerance score in Group C ((1, 3) 4, P < 0.001) was higher than Group L ((0, 1) 2), Group T ((0, 1.25) 3) and Group F ((0, 0) 1). Group F had a significantly lower score than Group L and Group T (P < 0.05, P < 0.01, respectively). The incidence of agitation and the active extubation rate were also higher in Group C (96.2% and 71.7%, respectively, P < 0.001) than Group L (48.1% and 15.4%, respectively), Group T (61.5% and 26.9%, respectively) and Group F (17.3% and 7.7%, respectively). Blood pressure, HR and plasma concentrations of epinephrine and norepinephrine were significantly higher in Group C than in all other groups at the time of extubation and 1 min after extubation (P < 0.001). Group F exhibited significantly reduced blood pressure, heart rate and plasma concentrations of epinephrine and norepinephrine compared to Group L and Group T (P < 0.05, P < 0.01 or P < 0.001, respectively). The incidence of postoperative pharyngeal pain and the incidence of postoperative cough were not significantly different among the groups.ConclusionsCompound lidocaine/prilocaine cream combined with tetracaine may be a more effective approach for preventing coughing and stabilising circulation during extubation following general anaesthesia. This may play an important role in preventing medical staff from contracting respiratory infectious diseases.Trial registrationChinese Clinical Trial Registry: ChiCTR2200058429 (registration date: 09–04-2022) “retrospectively registered”.

Evaluation of effect of dexamethasone and bicarbonate as adjuvants to intracuff lignocaine on endotracheal tube tolerance during emergence and incidence of postoperative cough and sore throat

Background: Smooth uneventful extubation and prevention of postoperative complications such as cough and sore throat adds to the quality of perioperative care and patient outcome. This study was designed to compare tube tolerance and hemodynamics during extubation, as well as the incidence of postoperative cough and sore throat, in patients operated under general anesthesia of more than 2-h duration.Materials and Methods: This prospective double-blinded, randomized controlled study included 100 patients posted for elective surgical procedures with expected duration of more than 2 h, under general anesthesia. Patients were divided randomly into two equal groups, namely Group A and Group B. In Group A, endotracheal tube (ETT) cuff was filled with lignocaine with sodium bicarbonate as an adjuvant, while in Group B, dexamethasone was added as an adjuvant to intracuff lignocaine. ETT tolerance at the time of extubation was recorded and compared. Other parameters such as rate pressure product, volume of drug diffused through cuff, and incidence of postoperative cough and sore throat were also compared. Data analysis was done using unpaired t-test and Chi-square test as appropriate, where P < 0.05 was considered statistically significant.Results: Eighty-two percent of patients in Group A while 78% of patients in Group B were categorized as tube tolerant during extubation (P = 0.62). The incidence of postextubation sore throat at 1 h was 30% versus 16% (P = 0.09) while at 24 h was 20% versus 6% (P = 0.03) in Group A and Group B, respectively. The incidence of cough postextubation at 1 h was 6% versus 0 (P = 0.07) while at 24 h was 18% versus 4% (P = 0.02) in Group A and Group B, respectively. The volume of intracuff drug diffused during anesthesia was 0.71 ± 0.27 (mean ± standard deviation [SD]) ml in Group A and 0.5 ± 0.23 (mean ± SD) ml in Group B (P = 0.02). Majority of patients (>85%) in both the groups remained hemodynamic stable during emergence with no statistical difference between the groups.Conclusion: Both bicarbonate and dexamethasone as an adjuvant to 2% intracuff lignocaine offer smooth emergence in terms of tube tolerance and hemodynamic stability during extubation. However, dexamethasone when used as an adjuvant has a better effect on prevention of development of postoperative cough and sore throat.

To Compare the Efficacy of Betamethasone Gel and Lignocaine Gel Applied over Endotracheal Tube to Reduce Post Operative Sore Throat and Cough

Background: Post-operative sore throat, cough and hoarseness of the voice are often common, uncomfortable sequelae after tracheal intubation with inflammation being the most common pathogenesis. Post operative sore throat and cough has a multifactorial aetiology that includes patient-related factors such as age, sex, and smoking, as well as intubation factors such as technique, duration, tube size, intracuff pressure, cuff design, trauma to the pharyngolaryngeal mucosa and various factors.&#x0D; Objectives: To compare the incidence of post operative sore throat, cough after endotracheal tube intubation when applying betamethasone gel and lignocaine jelly.&#x0D; Materials and Methods: At Saveetha Medical College and Hospital in Thandalam, Chennai, a prospective controlled double blinded study was done among patients who were scheduled for elective procedures under general anaesthesia with orotracheal intubation. The study comprised 60 patients who met the study's inclusion and exclusion criteria. The 60 patients were divided into two study groups each enrolled with 30 patients. Before the study could begin, approval from the institutional research board was required. Before the study began, an informed, written consent was obtained. The method used was Qualitative observational randomised double blind study by using a computer-generated random number table and the sealed envelope approach, patients were assigned to one of two equal groups.&#x0D; Following a pre-anaesthetic evaluation, 60 patients of either sex, aged 18 to 60 years, with an ASA physical status of I or II, who were undergoing elective surgery (likely to last up to 240 minutes) under general anaesthesia with orotracheal intubation and met the above inclusion criteria were included in the study. The differences between the study groups were analysed by chi square test and the “p” value used as a cut off for estimating statistical significance between groups is 0.05.&#x0D; Results: The incidence and severity of post operative sore throat and cough after endotracheal intubation during 6 and 24 hours was found to be statistically significantly in patients in whom betamethasone was used. The results are significant at p&lt;0.05.&#x0D; At 6 hours the incidence of post operative sore throat for betamethasone and lignocaine was 12% and 30% respectively. At 24 hours the incidence of post operative sore throat for betamethasone and lignocaine was 9.36% and 25.2% respectively.&#x0D; At 6 hours the incidence of post operative cough for betamethasone and lignocaine was 8.4% and 24%.At 24 hours the incidence of post operative cough for betamethasone and lignocaine was 7.2% and 21.6% respectively.&#x0D; Conclusion: The use of 0.05 percent betamethasone gel to lubricate the endotracheal tube before intubation helps to reduce the time it takes for symptoms to resolve.

Comparison Between the Efficacy of Saline, Alkalinised Lignocaine 2% and Alkalinised Lignocaine 4% in the Cuff of Endotracheal Tube to Reduce the Incidence of Cough and Sorethroat During Extubation

Background: To compare the efficacy of saline, lignocaine 2% and Lignocaine 4% in the endotracheal tube cuff to decrease the incidence of post operative cough and sore throat. A unique, efficient and easy method designed to decrease cough and sore throat during emergence. Hence there was a need for the study. Methods: After approval from institutional Ethical Committee, Kasturba Medical college, Mangaluru and written informed consent from 120 patients of A.S.A class I &amp; II, aged between 18-60 years posted for various surgeries were included. Randomization was done into three groups of 40 patients each. Group 1 – Normal saline, group 2 - 2% Lignocaine and group 3 - 4% Lignocaine. Postoperatively patients were assessed for incidence of cough and sore throat pain based on visual analogue scale (VAS) Results: 65% of patients in group 2 and 72% of patients in group 3 were extubated smoothly, whereas only 20% of patients in group 1 had smooth extubation, p&lt;0.001. There was significant difference in sore throat pain and cough in group 2 and 3 when compared to group 1. Conclusion: The incidence of sore throat and cough was significantly reduced with Lignocaine in the endotracheal tube cuff when compared to saline. Lignocaine 4% was found to be far more better than 2% in reducing sore throat.

Effects of preserving the bronchial artery on cough after thoracoscopic lobectomy.

BackgroundThe purpose of this prospective study was to explore the influence of both preoperative three‐dimensional (3D) reconstruction and intraoperative preservation of the bronchial artery (BA) on postoperative cough after thoracoscopic lobectomy.MethodsA total of 60 patients who had received a combination of thoracoscopic lobectomy and systematic lymph node dissection were included in this study. They were divided into two groups, namely the BA preservation group (Group A), and conventional surgical treatment group (Group B). In group A, we used Exoview software for 3D reconstruction of the BA before the operation and the BA was preserved during the operation. 3D reconstruction of the BA was not performed before surgery in group B. The incidence of postoperative cough, the Mandarin Chinese version of the Leicester cough questionnaire (LCQ‐MC), physiological, psychological and social dimensions and total score of the two groups were compared and analyzed.ResultsThe scores and total scores of LCQ‐MC in group B were lower than those in group A one and two months after surgery. There were significant differences between the two groups in physiological and psychological dimensions and total scores (p < 0.05), but there was no significant difference in social dimension between the two groups (p > 0.05). The incidence of postoperative cough in group A (16.7%) was lower than that in group B (30%), while the difference was not statistically significant (p = 0.222).ConclusionsPreoperative 3D reconstruction and intraoperative preservation of the BA can reduce the severity of postoperative cough.

Predictive factors of cough after uniportal video-assisted thoracoscopic pulmonary resection.

BackgroundCough is one of the shared complications after lung surgery. In this study, a prospective analysis was conducted for exploring the risk factors of persistent cough after uniportal video-assisted thoracoscopic pulmonary resection.MethodsOne hundred thirty-five patients with pulmonary nodules who underwent surgical treatment in the same surgical group from November 2019 to January 2020 were enrolled in this prospective study. The severity of cough and its impact on patients’ quality of life before and after surgery were assessed by the Mandarin Chinese version of the Leicester cough questionnaire (LCQ-MC), and postoperative cough was tested by the cough visual analog scale (VAS) and cough symptom score (CSS). Risk factors of cough after pulmonary resection (CAP) were determined by univariate and multivariate logistic regression analysis.ResultsThe incidence of postoperative cough was 24.4% (33 of 135 patients). Univariate analysis showed that gender (female), the surgical site (upper right), the resection (lobectomy), subcarinal lymph node dissection, postoperative acid reflux, length of hospitalization contributed to the development of CAP resection. Multivariate logistic regression analysis showed that the resection (lobectomy) (OR 3.590, 95% CI: 0.637–20.300, P=0.017), subcarinal lymph node dissection (OR 4.420, 95% CI: 1.342–14.554, P=0.001), postoperative acid reflux (OR 13.55, 95% CI: 3.186–57.633, P<0.001) and duration of anesthesia (over 153 minutes, OR 0.987, 95% CI: 0.978–0.997, P=0.011) were independent risk factors for postoperative cough.ConclusionsThe application of uniportal video-assisted thoracoscopic techniques to several types of lung surgery are conducive to enhanced recovery after surgery (ERAS). Postoperative cough is related to an ocean of factors, the resection (lobectomy), subcarinal lymph node dissection, postoperative acid reflux, and duration of anesthesia (over 153 minutes) are independent high-risk factors for CAP resection.Trial registrationThis study was registered on ClinicalTrials.gov (NCT04204148).

Incidence and risk factor for short term postoperative cough after thyroidectomy

BackgroundThe prevalence of potential risk factors for postoperative cough after thyroidectomy remain unknown. The current study aimed to research postoperative cough in patients undergoing thyroid surgery prospectively.MethodsAdult patients who underwent primary thyroid surgery were selected prospectively. Data regarding age, sex, BMI, pathology and surgical procedure were collected and analyzed. The Leicester Cugh Questionnaire (LCQ) was required to be completed by all patients before operation, 2 weeks and 4 weeks after operation.ResultsThere were 1264 patients enrolled in total. Eleven patients with vocal cord paralysis were excluded. In patients with benign disease, postoperative cough occurred in 61 patients, with an prevalence rate of 17. 0% compared to an prevalence rate of 33.1% in patients with malignant disease; the difference was significant. For benign patients, the factors of smoking and operation time were independently related to the occurrence of postoperative cough. For malignant patients, the factors of smoking, operation time, operation extent, and the number of positive nodes at level 6 were independently related to the occurrence of postoperative cough. There was no significant difference regarding the LCQ score in patients with benign or malignant disease at the preoperative and the postoperative 4-week time periods. Patients with malignant disease had a significantly lower LCQ score than patients with benign disease at the postoperative 2-week time point (p = 0.004).ConclusionsPatients undergoing thyroid cancer surgery had a higher incidence of postoperative cough and were also associated with a decreased cough-related quality of life. The factors of smoking and operation time were the most important predictors for postoperative cough after thyroidectomy.

Preemptive Effect of Two Different (Intracuff and Intravenous) Routes of Dexamethasone Administration on Postoperative Sore Throat and Cough

Introduction: The most common complications following anesthesia intubation include postoperative sore throat, post-extubation cough, and hoarse voice. Currently, the use of preoperative Dexamethasone has been effective in controlling these complications. Therefore, the present study aimed to compare the preemptive effect of intracuff and intravenous routes of Dexamethasone administration for postoperative sore throat and cough. Materials and Methods: In the present clinical trial, 96 patients who underwent general anesthesia intubation were randomly assigned to three groups. The first group received 4 mg intracuff Dexamethasone, the second group received 4 mg intravenous Dexamethasone, and the third group was administered a placebo. Cough frequency and severity of a sore throat after extubation were investigated in the three groups. Results: Any significant difference was not observed among the three groups regarding hemodynamic variables (P&gt;0.01); however, the incidence of cough was significantly lower in all the three groups (P&lt;0.01). Also, the average severity of a sore throat in PAR and 1 h after surgery in the three groups was significantly lower (P&lt;0.05). Moreover, no difference was detected in the groups regarding the severity of a sore throat within 24 h post-surgery (P&gt;0.01). Conclusion: The use of two methods of intravenous and intracuff Dexamethasone administration before surgery reduced the incidence of postoperative cough and the severity of sore throat in patients.

Spinal Anaesthesia for Quicker Recovery in Fast-Track Abdominal Hysterectomy

Introduction: Fast-track approach during hysterectomy has revolutionized the postoperative recovery. In this study we tried to analyse the effect of one more component (spinal anaesthesia versus general anaesthesia) during hysterectomy for benign gynaecological conditions to further hasten the postoperative recovery and thus evolve the concept. Aim: To compare recovery among those who receive spinal anaesthesia versus those who receive general anaesthesia during fast tract abdominal hysterectomy. Materials and Methods: A total of 97 women were recruited who were planned for hysterectomy for benign conditions and agreed to follow the fast-track protocol. Forty six consented for spinal anaesthesia (three cases were excluded later) and 51 for general anaesthesia. Fast-track protocol was followed that included preoperative counselling, and no preoperative sedation. Postoperatively all patients were monitored for pain, vomiting, drowsiness and fatigue. Early oral intake and ambulation was encouraged. Postoperative events and complications as well as duration of hospital stay were compared among the two groups. Differences in continuous variables were analysed with student’s t-test for normally distributed data and the Mann-Whitney U Test for skewed data. Pain score was analysed by repeated measures of ANOVA. Results: Mean operating time in spinal anaesthesia group, was much less (92.72+23.61 minutes) than in general anaesthesia group (124.20+33.61 minutes), the difference being statistically significant (p<0.001). Mean blood loss was also less in spinal anaesthesia group (298.14+61.34 ml versus 404.90+110.57 ml; p<0.001). Women in this group could be started on oral fluids earlier, had less vomiting and fatigue postoperatively, and passed motion earlier. Duration of hospital stay was not found to be much different between the groups. However, patients who were in the spinal anaesthesia resumed their routine activities earlier (15.47+2.77 versus 18.55+4.25 days; p<0.001). We also noted an interesting finding yet undiscussed in the literature that general anaesthesia group had more than 2.5 times higher incidence of postoperative cough which has the potential to influence postoperative recovery following any abdominal surgery. Conclusion: Spinal anaesthesia should be considered in cases that are planned for hysterectomy under fast-track setting to optimise the results.
