Hand dermatitis (HD) is a form of occupational dermatitis that frequently occurs . It is c aused by exposure to chemicals and the environment. Hand dermatitis is often related to wor k and pottery industry was one of them. Clay as the main ingredient in the traditional pottery industry contai ns ingredients that potentially irritant and allergenic. The frequency and risk factors that affected the frequency of HD in pottery industry worker was unknown. Objective of this study was to determine frequency and history of atopic, wet work, and contact duration as risk factors that affected th at frequency of HD in traditional pottery industry worker. Analytic observational study with a cross-sectional design. Sixty subjects were includ ed in this study, 53,3% are men. The mean age of subjects was 43.75 years old. Frequency of HD on pottery industral worker was 58,33%. In bivariate analysis, history of atopy, wet work, and contact duration were significantly associated with the incidence of hand dermatitis. Hand dermatitis that appears on the subject is probably caused by calcium oxide, aluminum oxide, and chromium which was the content of clay as the industry's main raw material. Latex Allergy in the Prehospital Setting

Latex allergy first was recognized early in the 20th Century, but was not a matter of concern until the last decade of that Century. The reported incidence of latex allergy in different occupations varies considerably. It has been documented in dental workers, operating theater staff, anesthetists, and laboratory technicians. However, little data specifically related to those involved in patient care in the emergency prehospital setting are available. A questionnaire was distributed to a sample of both volunteer and salaried first responders from St. John Ambulance Australia in South Australia and Western Australia, and the South Australian Ambulance Service. The first responders were surveyed to: (1) determine the incidence of latex allergy; (2) consider possible factors associated with its development; (3) compare characteristics of the surveyed groups; and (4) reinforce the development of an educational program. The study tool had predetermined statistical qualities. Data were collated and processed using standard statistical procedures. Surveys were collected anonymously. Of the 2,716 forms distributed, 1,099 were returned, resulting in an overall response rate of 40.5%. Atopy was identified in 14.9% of participants, hand dermatitis in 9.4%, and latex allergy in 6.4%. In the group of full-time ambulance officers, there was a significantly higher incidence of hand dermatitis and latex allergy. There also was a significant relationship between latex allergies and both dermatitis and glove usage (as measured by frequency and duration). In a group of first responders assessed by an anonymous, voluntary questionnaire, the subset of full-time, salaried ambulance officers was identified as having a higher incidence of hand dermatitis and latex allergy than their volunteer co-workers. These results require further assessment to substantiate the frequency of latex allergy and determine the predisposing factors. All personnel must learn about hand care. Non-powdered, natural rubber latex gloves should be supported for general use in this setting.

Hand dermatitis in a prospectively-followed cohort of hairdressing apprentices: final results of the POSH study. Prevention of occupational skin disease in hairdressers.

The incidence of hand dermatitis is high in the hairdressing trade; the individual disease burden is substantial, as are the costs of treatment, workers compensation and retraining programs. To evaluate the risk of developing occupational irritant hand dermatitis associated with several potential risk factors, namely constitution (atopy) and occupational exposure, an observational prospective population-based cohort study set in vocational training schools in northwest Germany, recruiting 2352 hairdressing apprentices, was performed. Unprotected wet work of more than 2 h per day is the major significant risk factor; low ambient absolute humidity is equally associated with a significantly increased risk. Compared to the youngest age group, risk diminishes with increasing age. Constitutional risk factors are difficult to evaluate because of selective dropout of atopic participants. For primary prevention, a population approach, improving working conditions and skin protection for all exposed, appears to be the most beneficial.

Prevalence and incidence of hand dermatitis in hairdressing apprentices: results of the POSH study. Prevention of occupational skin disease in hairdressers.

Occupational skin changes in hairdressers are very common. Morbidity, however, has not yet been quantified precisely. A cohort of 2,352 hairdressing apprentices (of the 2,570 invited to participate, i.e., 91.5% response) was prospectively followed for the duration of their vocational training (3 years) by three examinations. Three waves (years) were recruited in 1992, 1993, and 1994 from 15 vocational training schools in northwestern Germany. The point prevalence of (mostly slight) irritant skin changes of the hands increased from 35.4% in the initial examination to 47.5% in the intermediate examination and to 55.1% in the final examination. Given a more conservative definition of a case of "hand dermatitis," these estimates were 12.9%, 23.5%, and 23.9%, respectively. Altogether, 34.3 and 15.2 cases of "skin changes (any degree)" and "hand dermatitis," respectively, in 100 person-years were observed during the study period. The incidence rate, i.e., the number of newly diseased study participants in relation to the person-time at risk contributed, decreased in the course of the study. The proportion of dropouts until final follow-up was 51.8%. The present results appear to lie in a range with those found in other, much smaller cohort studies. However, comparison of the results is hampered either by the lack of a clear definition or by a different definition of "person-time at risk" or "a case of hand dermatitis." As compared with an external control group of office apprentices, the incidence was several times higher in hairdressing apprentices, which points to the high risk for skin damage in this occupation.

The role of atopic constitution, contact sensitization, transepidermal water loss, and dry skin in the development of hand dermatitis was investigated in a prospective study of 74 apprentice hairdressers and 111 apprentice nurses. Base-line measurements included a questionnaire on personal characteristics and anamnestic information, examination of hand skin, measurements of transepidermal water loss, patch tests, and prick tests. The condition of the hands, previous exposure, and transepidermal water loss were followed at intervals of four to six weeks. Cox proportional hazard models were used in the statistical analysis. The average incidence rate of hand dermatitis was 32.8 cases per 100 person-years for the hairdressers and 14.5 cases per 100 person-years for the nurses. The rate ratio of having a dry versus normal skin type was 7.3 for the hairdressers [95% confidence interval (95% CI) 2.2-24.3] and 1.7 for the nurses (95% CI 0.5-6.4). Apprentice nurses with a history of (atopic) mucosal symptoms had a 3.4-fold increased incidence rate of hand dermatitis (95% CI 1.05-11.2). The rate ratio of mucosal atopy for the apprentice hairdressers was 2.2 (95% CI 0.7-6.7). Graphic display of the results suggested an increased risk of hand dermatitis among the apprentice hairdressers with transepidermal water loss on the hand greater than 15 g.m-2.h, but the relative risk of increased transepidermal water loss was not statistically significant. The most important endogenous risk factors for hand dermatitis among the apprentice hairdressers and nurses were the presence of dry skin and a history of mucosal atopy. No relationship between increased transepidermal water loss and the risk of hand dermatitis was observed.

A retrospective cohort study on the incidence of hand dermatitis in nurses.

Data on the incidence and induction time of hand dermatitis are not available from the literature. To assist the planning of a prospective study on risk factors for hand dermatitis, a retrospective cohort study was performed in nurses and office employees to obtain a rough estimate of the incidence and induction time of hand dermatitis in these occupational groups. Data were collected by means of a self-administered questionnaire with validated questions on symptoms of hand dermatitis. The cohort consisted of 371 nurses and 111 office employees who were newly employed by a University Hospital between 1 January 1987 and 31 December 1988. Follow-up ended on 30 September 1989. Subjects who reported that symptoms had already occurred before entering the study were excluded for analysis on the incidence, leaving 298 nurses and 101 office employees for further analysis. Due to limitations of the retrospective study design, only half of the cases were able to report the exact month of occurrence of symptoms. Therefore only a rough estimate could be made of the incidence and induction period, assuming that the distribution of person-months of observation in cases with a known incidence date was equal to that of cases with unknown incidence dates. Thus, it was estimated that the overall incidence was 6.5 cases/1000 person-months in nurses and 1 case/1000 person-months in office employees. Hand dermatitis in dental technicians

The incidence of hand dermatitis and contact allergies was analyzed among dental technicians. A survey was carried out by postal questionnaire. It was returned by 106 technicians (88%); 30 had had skin problems, and 20 (19%) had present eruptions. All 20 were invited to a detailed dermatological investigation, but only seven participated. Epicutaneous tests to prosthetic materials were negative in these patients. The hand eruptions in 4 patients appeared to be irritant. 4 other patients (4%), who had had previous hand dermatitis, had a history of positive patch test reactions to methyl methacrylate, but they had become symptomless and refused to participate in further studies.
