System Design and Implementation for Machine Learning and Internet of Things Based Anomaly Detection in Patient Movement

Emerging trends in healthcare have seen an increased need for improved monitoring, and as a result, machine learning (ML) and the Internet of Things have been integrated to develop smart systems that can provide rapid and accurate health assessments. With an inclusive view on mobility anomaly detection in patients, this paper aims to improve hospital patient safety. Patients wear or place these things, and the IoT sensor network then gathers real-time motion data. The information is then moved to the central server. The system was taught the machine learning part using a large number of normal and abnormal patient movement data. It also shows a way to differentiate between common activities and those that might indicate ill health. It comprises an IoT sensor network, a data processing unit, and an ML anomaly discovery module. There are sensors on the Internet of Things that have a wide range of movement data. Then, these are run through an ML model that captures the spatial-temporal dependencies between different sets based upon more sophisticated techniques like the Naive Bayes Classifier. Where there are abnormalities discovered after analyzing them, an alert is sent out; this will help in quick emergency response by saving lives even before diseases become last-stage acute conditions

Patient Safety Culture Analysis at Sultan Agung Islamic Dental Hospital to Improve Patient Safety

A culture of patient safety can improve hospital patient safety. Patient safety is the foundation of quality health services, an indicator of assessment in healthcare facilities, and a mechanism by which health services protect patients from harm. The purpose of this study is to examine the patient safety culture at Sultan Agung Islamic Dental Teaching Hospital Semarang in order to identify ways to improve patient safety. A cross-sectional research of 124 participants was conducted at Sultan Agung Dental Teaching Hospital, Semarang, Indonesia. Distributing questionnaires to collect data, the Dental Office Survey on Patient Safety Culture (DOSOPSC) includes 58 items and 10 dimensions. Out of a total of 124 respondents, the average positive response value exceeded the dependability criterion on four dimensions: teamwork (93.7%), the highest, and work pressure and speed (56.2%), the lowest. There are four areas in which the average positive response value exceeds the dependability threshold, including teamwork, staff training, hospital systems and standards, work pressure, and speed.

Experience of health workers in improving patient safety in hospitals through interprofessional collaboration: study qualitative

Introduction: Patient safety is a global problem because patient safety incidents that occurare estimated to be 134 million incidents due to unsafe care and 2.6 million resulting indeath. Indonesia recorded a patient safety incident rate of 23.5%, so interprofessionalcollaboration is necessary to minimize this condition.Objective: This study aims to identify the experience of health workers in improvingpatient safety in hospitals through interprofessional collaboration. The method in this studyis a qualitative design with an interpretive descriptive approach.Methods: The method in this study is a qualitative design with an interpretive descriptiveapproach. The method in this study is qualitative with a descriptive-interpretative approachdesign. Implementation method with FGD on five professional health workers.Result: The results of the study contained 8 (eight) themes showing the experience ofhealth workers in improving patient safety in hospitals through interprofessionalcollaboration including understanding health workers in improving patient safety,implementation of patient safety for health workers in interprofessional collaboration,hospital management support, obstacles and expectations health workers in improvingpatient safety through interprofessional collaboration.Conclusion: The results of the study contained the experience of health workers on theimplementation of patient safety in hospitals through interprofessional collaboration,namely identifying actions in providing services with uniform documentation and alsocoordinating actions according to Standart Operastional Procedur (SOPs), creatingcommunication according to Situation, Background, Assessment, Recommendation (SBAR),joint commitment in providing services in hospitals, uniform training in hospitals, reducingbarriers in both personal and management services, awareness, interest and motivationfrom each profession, facilities that support the implementation of patient safety open andtransparent system, monitoring evaluation regulary and facilitating staff.

Pentingnya Akreditasi Rumah Sakit Bagi Pelayanan Kesehatan: Hospital Application Study

Hospitals in guaranteeing good service quality and patient safety must be proven through accreditation. Accreditation is carried out by submitting an application to the Hospital Accreditation Commission (KARS). Hospital Accreditation is a way of monitoring the implementation of measuring hospital performance indicators. The purpose of writing this scientific article is to find out the importance of improving the quality of hospitals, especially for hospital structures and health workers. The method in this scientific article is to compare cases from several hospital studies that apply the accreditation process and the literature collected at the editing (re-examination) stage. The data is then processed and analyzed and conclusions drawn. The findings from the study of the application of hospitals in the importance of holding accreditation are to improve the quality of hospital services, improve hospital patient safety, increase protection for patients, the public, human resources for hospitals and hospitals as institutions and support government programs in the health sector. It can be concluded that the importance of hospitals carrying out accreditation is to ensure good service quality and patient safety must be proven through accreditation. Accreditation is carried out by submitting an application to the Hospital Accreditation Commission (KARS). Suggestions made for hospital agencies, namely hospitals that have obtained national accreditation status are required to make strategic improvement plans in accordance with the surveyor's recommendations to meet hospital service standards that have not been achieved.

Croatian Use of The Hospital Survey on Patient Safety Culture

Introduction. Patient safety during hospitalisation is one of the biggest concerns for hospitals worldwide and one obligation of all medical professionals is to create a safe environment for patients and prevent accidents. Approximately 50% of adverse events can be prevented with a systematic approach. Aim. This study aimed to examine the underlying dimensions and psychometric properties of the Hospital Survey on Patient Safety Culture questionnaire in Croatian hospital settings, and to compare the results with the original questionnaire from the United States of America. Methods. The sample consisted of 438 nurses from four Croatian university hospitals. All participants signed an informed consent document, and the questionnaires were provided to the nurses in their units by the head nurses of their departments. Data analysis was performed using IBM SPSS 25.0. Bartlett's test of sphericity and the Kaiser-Meyer-Olkin (KMO) indicator were performed with Cronbach’s alpha test and sample standard deviation. Results. All factors explain the total of 59% of variance of the measured questionnaire. Additionally, the reliability of the entire questionnaire was determined by using the internal consistency coefficient (Cronbach’s alpha) on the Hospital Survey on Patient Safety Culture with 12 of 0.88 particles, which is high internal consistency reliability. Our results show that the particles that make up the questionnaire are very homogeneous according to their object of measurement. Conclusion. The results of our study found that the survey can be applied to Croatian settings and used in hospitals. We also believe that more research on this topic is needed and is crucial for improving patient safety in hospitals. It is necessary to emphasize continuous education of nurses regarding patient safety in hospitals.

The role of hospital characteristics in patient safety: a protocol for a national cohort study.

Background:Substantial expenditures on health care safety programs have been justified by their goal of reducing health care associated–harm (adverse events), but adverse event rates have not changed over the past 4 decades. The objective of this study is to describe hospital-level factors that are relevant to safety in Canadian hospitals and the impact of these factors on hospital adverse events.Methods:This is a protocol for a national cohort study to describe the association between hospital-level factors and adverse events. We will survey at least 90 (35%) Canadian hospitals to describe 4 safety-relevant domains, chosen based on the literature and expert consultation, namely patient safety culture, safety strategies, staffing, and volume and capacity. We will retrospectively identify hospital adverse events from a national data source. We will evaluate organization-level factors using established scales and a survey, codesigned by the study team and hospital leaders. Hospital leaders, clinical unit leaders and front-line staff will complete the surveys once a year for 3 years, with an anticipated start date of winter 2022. We will use national health administrative data to estimate the rate and type of hospital adverse events corresponding to each 1-year survey period.Interpretation:Analysis of data from this project will describe hospital organizational factors that are relevant to safety and help identify organizational initiatives that improve hospital patient safety. In addition to biyearly reports to the leaders of the participating hospitals, we have a multifaceted and tailored dissemination strategy that includes integrating the knowledge users into the study team to increase the likelihood that our study will lead to improved hospital patient safety.

Patient Safety Culture Change over Time-Health Care Staffs’ Perceptions

Rationale and Aim: Patient safety is of great interest in health care organisations, worldwide. In Sweden, a national patient safety initiative was launched in 2011. The aim of this study was to examine and compare patient safety culture change over time from health care staffs’ perspective. A further aim was to examine factors that have had an effect on patient safety culture. Methods: Patient safety culture was assessed in 2009, 2011 and 2013 using the Swedish version of the Hospital Survey of Patient Safety Culture. Respondents in this study were registered nurses (n = 2149), enrolled nurses (n = 959), physicians (n = 355) and managers (n = 159) working in three health care divisions in a Swedish county council. Results: Patient safety culture decreased significantly over time for all but two dimensions. The dimension “Information to Patients/Relatives” was the only dimension to increase significantly over time. Health care staffs’ profession and health care division belonging had significant main effects on 14 and 8 dimensions, respectively. Managers and enrolled nurses scored patient safety culture significantly higher than registered nurses and physicians. Health care staff working in a mixed medical-surgical health care division scored significantly higher than those working in medical or surgical divisions did. Conclusions: Despite efforts for patient safety work in the county council, health care staffs’ perceptions of patient safety culture decreased over time. To improve hospital patient safety culture, managers and staff groups must meet and communicate with each other in order to create a common view of patient safety.

Patient Safety Climate in General Public Hospitals in China: A Multiregion Study.

This study aimed to analyze the potential difference in patient safety climate by region (Shanghai vs Hubei Province vs Gansu Province) and general public hospital level (tertiary vs secondary) in China. Using a stratified sampling method, employees from 54 public general hospitals in Shanghai, Hubei Province, and Gansu Province in China were surveyed in 2015. The Patient Safety Climate in Health Care Organizations tool and the percentage of "problematic responses" (PPRs) were used to measure and analyze the patient safety climate. A χ2 test and hierarchical linear modeling were applied for the analysis. In the study, 4121 valid questionnaires were collected. The psychometric analysis supported the validity and reliability of our Chinese version of the Patient Safety Climate in Health Care Organizations. The overall patient safety climate was relatively good and exhibited no significant differences among the surveyed hospitals by various regions (Shanghai vs Hubei Province vs Gansu Province) and diverse hospital levels (tertiary vs secondary) using hierarchical linear models. "Fear of blame and punishment" and "fear of shame" had the highest PPRs and were prevalent in various types of hospitals. "Provision of safe care" and "organizational resources for safety" also had notably high PPRs. There were 4 dimensions varied by region and hospital level in this survey. Fear of shame and fear of blame are the most important barriers to the improvement of patient safety in the hospitals of China. Facility characteristics contributed somewhat to hospital patient safety climate in some dimensions. The initiatives to improve hospital patient safety climate are necessary and its implementation strategies needs to be shared.

Assuring Rural Hospital Patient Safety: What Should Be the Priorities?

Since reports on patient safety were issued by the Institute of Medicine, a number of interventions have been recommended and standards designed to improve hospital patient safety, including the Leapfrog, evidence-based safety standards. These standards are based on research conducted largely in urban hospitals, and it may not be possible to generalize them to rural hospitals. The absence of rural-relevant patient safety standards and interventions may diminish purchaser and public perceptions of rural hospitals, further undermining the financial stability of rural hospitals. This study sought to assess the current evidence concerning rural hospital patient safety and to identify a set of rural-relevant patient safety interventions that the majority of small rural hospitals could readily implement and that rural hospitals, purchasers, consumers, and others would find relevant and useful. These interventions should help rural hospitals prioritize patient safety efforts. As background, we reviewed literature; interviewed representatives of provider, payer, consumer, and governmental groups in 8 states; and calculated patient safety indicator rates in rural hospitals using the Agency for Healthcare Research and Quality's Health Care Cost and Utilization Project National Inpatient Sample. Based on the research literature and patient safety recommendations from national organizations, we developed a list of potentially important patient safety areas for rural hospitals. The rural relevance of these safety interventions was evaluated by a national expert panel in terms of the frequency of the problem, ability to implement, and the internal and external value to rural providers, purchasers, and consumers. The limited available research suggests that patient safety events and medical errors may be less likely to occur in rural than in urban hospitals. We identified 9 areas of patient safety and 26 priority patient safety interventions relevant to rural hospitals. Many of the identified areas of patient safety and interventions are relevant to all types of hospitals, not just rural hospitals. However, some areas, such as transfers, are especially relevant to rural hospitals. The challenges of implementing some interventions, such as 24/7 pharmacy coverage, are significant given workforce supply and financial problems faced by many small rural hospitals. The results of this study provide an important platform for further work to test the validity and effectiveness of these interventions.
