Analysis of Risk Factors Related to Nurses' Attitudes and Patient Safety Culture Implementation

Patient safety is a critical global concern, with millions of adverse events occurring annually. Nurses' attitudes towards patient safety culture are pivotal in its successful implementation. This study aimed to assess the risk factors related to nurses' attitudes towards patient safety culture and its relationship with the implementation of patient safety culture in a public hospital dr. H. Moch. Ansari Saleh in Banjarmasin, Indonesia. A cross-sectional study was conducted among nurses working in a public hospital dr. H. Moch. Ansari Saleh in Banjarmasin, Indonesia. Data were collected using a questionnaire adapted from the National Patient Safety Guidelines and the AHRQ Hospital Survey on Patient Safety. The questionnaire assessed nurses' attitudes towards patient safety, the implementation of patient safety culture, and potential risk factors such as age, gender, years of experience, and workload. Data were analyzed using descriptive statistics, Spearman's rank correlation coefficient, and multivariate logistic regression. A total of 85 nurses participated in the study. The majority of nurses (81.2%) exhibited a moderate attitude towards patient safety, while 83.5% reported a moderate level of patient safety culture implementation. Spearman's rank correlation analysis revealed a weak positive correlation (rho = 0.417, p = 0.01) between nurses' attitudes and the implementation of patient safety culture. Multivariate logistic regression analysis identified age (OR = 1.12, 95% CI = 1.01-1.24) and workload (OR = 1.18, 95% CI = 1.05-1.33) as significant risk factors associated with a less positive attitude towards patient safety culture. In conclusion, while there is a positive relationship between nurses' attitudes and patient safety culture implementation, the relationship is weak. Age and workload were identified as significant risk factors for a less positive attitude towards patient safety culture. These findings highlight the need for targeted interventions to address these risk factors and enhance nurses' attitudes, thereby strengthening patient safety culture in the hospital.

The Role Of Leader Improve Patient Safety Culture

One of the important things that must be considered by the hospital is patient safety, where patient safety can be improved by implementing a patient safety culture, so that the patient safety culture goes well, it is necessary to increase the role of the leader. This study aims to determine whether there is a relationship between the role of a leader and the implementation of patient safety culture. Method: this research is a quantitative study using cross sectional. Data were collected using a questionnaire of 118 nurses using a total population sampling technique. The statistical test used is the spearmen rank test. Results: the results of this study are that the majority of the role of leaders is in the good category (92.6%), the majority of patient safety culture is in the good category (90.7%) there is a significant relationship between the role of leaders and the implementation of patient safety culture with a p-value of 0.00 , and r : 0.381, conclusion: Increasing the role of a leader in the form of a decision-making role, interpersonal role, and informational role can improve patient safety culture

Persepsi Perawat Terhadap Implementasi Budaya Keselamatan Pasien Di Ruang Rawat Inap Terpadu Rumah Sakit H.Adam Malik Medan

<p><em>Patient safety is a hospital service system that provides safer patient care. WHO (World Health Organization) publications report patient safety incidents that medical errors occur in 8% to 12% of inpatient rooms at H.Adam Malik Hospital Medan. Patient safety incidents that occur include nosocomial infections, patient falls, and medication errors. Each of these incidents exceeded the established standards. The purpose of this study was to evaluate nurses' perceptions of the implementation of patient safety culture at H.Adam Malik Hospital Medan</em></p><p><em>The research design used was qualitative research. The characteristics of the key informants are the head of the nursing field, the head of the room, the head of pharmacy, the nursing committee and the head of the Quality Improvement and Patient Safety (PMKP) section, while the main informants are nurses. Qualitative data collection techniques using the Triangulation method. Data analysis using qualitative descriptive with data source triangulation stages</em></p><p><em>The results showed that patient identification, effective communication, drug safety and risk of infection had gone well, while site marking and the risk of falling patients had not gone well.</em></p><p><em>The conclusion of this study is that patient identification, effective communication, drug safety and risk of infection have gone well, while site marking and the risk of patients falling have not gone well. It is hoped that this can be an illustration of the application of patient safety to improve the quality of health services through the achievement of hospital achievements so that it can be used as the basis for taking a further policy and increasing the satisfaction of each patient who comes.</em></p>

The Implementation of Patient Safety Culture in Hospital: A Qualitative Research

This study explores the implementation of patient safety culture in one of General Hospitals in the central part of Aceh Province, Indonesia. The research used a qualitative method with a phenomenological design, collected data using in-depth interviews with eight participants in inpatient and outpatient rooms. The Collaizi method was used to analyze the data and produced three themes, namely: (1) conditions needed to improve the directing function, (2) leaders’ response and concern, and (3) team support and coordination in implementing patient safety. The results of this study indicate that the problem of implementing a patient safety culture needs to be considered by hospital management to improve patient safety. It is recommended that service leaders at the hospital improve the directing function to improve the implementation of patient safety culture and the quality of hospital services.

Systematic Review : Dampak Penerapan Patient Safety Culture di Rumah Sakit

Efforts to maintain the quality of health services in hospitals are by applying a patient safety culture and making that culture a primary need in maintaining the quality of health services in order to minimize losses and negative impacts on patients both materially and non-materially. Theory. The database was obtained through Google Scholar and Scopus using the keywords for applying patient safety culture and hospitals. Articles that meet the criteria are published in 2016-2021 using Indonesian and English languages. The articles used in this Systematic Review amounted to 15 articles according to the requirements determined by the researcher. The analysis of 15 articles showed that there were three main factors related to the implementation of patient safety culture in hospitals, namely teamwork, communication, and supervision. Its several factors related to the implementation of patient safety culture in hospitals from several previous studies. Factors related to the implementation of a safety culture need to be improved in an effort to implement standardized service quality.

STUDY DESCRIPTION OF PATIENT SAFETY CULTURE AMONG NURSE

Patient safety culture is the main step in achieving patients safety which is formed from the attitudes, perceptions, competencies and behaviours of both a person and the organization where they work. A person’s ability to carry out safety aims to create conditions for patients to be free from injuries that can result in disability and death incident. The purpose of this study was to describe the culture of patient safety in Sawahlunto Hospital in 2022. This research method used a descriptive study with a total sampling technique on nurses in Sawahlunto Hospital so that 52 respondents were obtained. Data collection using the HSOPSC Version 2.0 Questionnaire published by AHRQ in 2019. The Instrument consists of 10 Elements in 6 sections with 32 positive and negative questions. Univariate analysis is presented in a frequency distribution. The results showed that the majory of respondents were in the age group 36-45 years (51.9%), female (88.5%), nurses with working years >10 years (51,9%). Assessment of patient safety culture is in the category of Positive Response (65.4%). Conclusion: The implementation of patients safety by implementing nurses at Sawahlunto Hospital is in the Positive Response Category. Recommendation: Improving the implementation of patient safety culture can be done by all health workers so it needs monitoring and evaluation by the organization as well as providing full support to staff to apply patient safety principles

Obstacles of Patient Safety Culture Implementation to Prevent and Control of Healthcare-Associated Infections (Hais) During Covid-19 Pandemic: A Phenomenological Study

Background: The implementation of patient safety is an effective strategy in preventing and managing Healthcare-Associated Infections (HAIs). Objective: This study aimed to explore the obstacles in implementing the patient safety culture by dental co-assistant (co-ass) on prevention and control of HAIs in the covid-19 pandemic. Method: This study used a qualitative method with a phenomenological study approach. The sample in this study was 16 key informants consisting of 10 dental co-assistant, five dental nurses, and one hospital director. The sampling technique used the purposive sampling technique. Results: The results of this study indicate that there were five obstacles in implementing the patient safety culture to prevent and control HAIs, including 1) the rules were not strict in the implementation of patient safety culture; 2) poor attitude by the dental co-assistant (co-ass); 3) the facilities are not feasible to support the implementation of patient safety culture; 4) inadequate patient safety culture guidelines, and 5) lack of supervision in the implementation of patient safety culture. Conclusion: The results of this study provide important information related to obstacles faced by dental co-assistant in the implementation of patient safety culture to prevent and control HAIs during the covid 19 pandemic. The dental co-assistant should focus on the implementation of patient safety culture to suppress the occurrence of HAIs.

Organizational Factors in Implementation of Patient Safety Culture in Hospitals: Systematic Literature Review

Introduction: Patient safety culture is a program organized to minimize the risk of unexpected events and improve patient safety. This study is aimed to explain the relationship between organizational factors in the implementation of patient safety culture in hospitals based on empirical studies in the last five years.Method: ProQuest, ScienceDirect, Sage, CINAHL, and google scholar databases have been searched using indexed keywords in the Medical Subject Heading (MeSH) on 2015-2020. JBI's review with a cross-sectional study design used to assess research quality. The PRISMA flowchart was used to summarize the study selection process in the literature review with inclusion and exclusion criteria adjusted to the PICOS framework.Results: We found 13 journals that consist of three studies discuss about leadership, seven journals describe about resources, and three journals relate to organizational structure. Every country has a different characteristic in implementation of patient safety culture. The thirteen journals came from countries that divided in to three continents, there are Asia, Europe, and Africa. The results of this studyshow that leadership, resources, and, organizational structure can optimize the application of patient safety culture. Leadership support motivates health workers in the room in implementing a patient safety culture. Adequate resources reduce the risk of accidents at work, and an effective organizational structure creates good and conducive performance.Conclusions: Provide between three and five key words in alphabetical order, which accurately identify the paper’s subject, purpose, method and focus. Don't use words or terms in the title as keywords. These keywords will be used for indexing purposes. Keywords cannot be more than 5 words or phrases in alphabetical order

Analysis of the Implementation of Patient Safety Culture with the AHRQ Model at Mitra Medika Hospital Tanjung Mulia Medan

Safety Culture is a collaborative environment where clinical staff treat one another with respect by involving or empowering patients and families so that each Professional provides patient-focused care. The purpose of this study was to determine the implementation of a patient safety culture with the AHRQ model in Mitra Medika Tanjung Mulia General Hospital Medan. This research uses a mix of methods, namely qualitative and quantitative. The population of this study were all staff on duty in each hospital unit associated with 450 patients at MItra Medika Hospital, so the sample was 75 people using simple random sampling technique. The main research informants were one officer each in each installation.Unit Mitra Medika Tanjung Mulia General Hospital Medan, namely 10 people, one key informant, and one triangulation informant.The results of the study show that the description of expectations and managerial actions of patient safety is 68%, organizational learning is 100%, cooperation in units is 76%, communication open by 80%, feedback about the error of 75%, the dimension of non-punitive response to errors is 55%,Staffing 51%, management support for patient safety efforts by 77%, cooperation between units of 61%, handsoff work and changing patients was 71%,the overall perception of hospital staff about patient safety was 63%, reporting frequency was 63%. With the research results, it is expected that Mitra Media Tanjung Mulia General Hospital Medan is expected to be able to initiate, maintain, and develop an ongoing patient safety program and implement the existing safety culture.

Hasta Güvenliği Kültürünün Tıbbi Hata Tutumları Üzerine Etkisi: Hemşirelerde Bir Uygulama

This study aims to examine the effects of patient safety culture perceptions on their attitudes of medical errors among nurses. The universe of the study is nurses working in a public hospital in Antalya (n = 350). The convenience sampling method was used in the study and the questionnaire was collected from 205 nurses. The data were obtained between October 2-November 3, 2018. The study results demonstrates that the dimensions of patient safety culture explained 28.2% of the total variance in the perception of medical error, 26.4% of the total variance in medical error approach, and 21.8% of the total variance in causes of medical errors. Patient safety culture also explains 32% of the total variance in all subdimensions of medical error. The results of this study highlight the importance of enhancing patient safety culture to protect patients from medical errors in health institutions. This study also revealed important factors that need to be considered for successful implementation of patient safety culture. It is recommended to have a common understanding and increase awareness in reducing medical errors and improving patient safety culture.

Pelaksanaan Budaya Keselamatan Pasien pada Masa Pandemi Covid-19 di Rumah Sakit Umum Daerah Bima

Patient safety is an indicator of the quality of health service facilities, therefore the hospital always strives to improve the quality of service by providing safe and quality care and building a culture of patient safety even during crisis situations such as the Covid-19 pandemic. Aimed at knowing the description of the implementation of patient safety culture in RSUD Bima, this study used a descriptive observational design with a cross-sectional approach, respondents were taken by simple random sampling, data collection used the Hospital Survey on Patient Safety Culture (HSOPSC) questionnaire. In general, the implementation of patient safety culture in most dimensions has been perceived positively by more than 80% of respondents. Meanwhile, the lowest positive perception on the response dimension did not blame the error (47.5%), staff management (57.6%) and the frequency of reporting events / incidents (58.5%). The investigators recommend continuing to improve the patient safety culture with special attention paid to the low mean dimensions to refine and strengthen these dimensions

The relationship between leadership style and nurse's work motivation with the implementation of patient safety culture in hospital, Bone regency

To identify the dominant leadership style and to analyze the relationship between leadership style and nurse's work motivation with the implementation of the patient safety culture of the inpatient installation in the c class hospital of Bone regency. This is a cross-sectional study which involved 104 nurses in total sampling. Data were analyzed using the chi-square test. The dominant leadership style of the inpatient nurse managers is transactional, with a percentage of 69.2%. There was a significant relationship between leadership style (p<0.05) and nurse's work motivation (p<0.05) with the application of patient safety culture, respectively. The transactional leadership has an impact on independent culture, and the combination of extrinsic and intrinsic motivation has an impact on creating public service motivation, which is good to build reporting and learning the culture.

MEDICAL ERRORS AND PATIENT SAFETY CULTURE – SHIFTING THE HEALTHCARE PARADIGM IN ROMANIAN HOSPITALS

Patient safety (PS) is a global concern of national health systems, defined as the patient s right not to be subjected to unnecessary harm potentially associated with the medical act. Reporting medical errors for learning purpose was deeply encouraged by WHO during the last 10 years, as a mandatory step in preventing future errors and increasing patient safety. Aim. The paper aims to analyze the level of implementation of Patient safety culture (PSC) and reporting adverse events in Romanian hospitals, during 2011-2019, in the context of this globally shifting paradigm in healthcare. Materials and methods. The paper is a review of articles published in international databases conducted by the keywords “patient safety” and “medical errors”. A documentation of the statistical data reported by the national health institutions regarding the reports of EA in different national systems was carried out. Results. Passive reporting of the AEs in the national surveillance systems is on an upward trend, but still low when compared to Western European countries. Several prospective studies, on a limited number of hospitalized patients, showed a prevalence of HAIs 10 times higher than that reported (2,8-5% vs. 0.4%). Similar results were found for reported adverse drug reactions and other adverse events. Missing information about who should report, what should be reported, what are the AEs that are useful for learning purposes, what are the consequences of reporting upon individuals and upon institutions are all important causes of a low rate of reporting. Conclusions. Changing the culture in healthcare is a longstanding process, but the results in recent years are encouraging. The popularization of “no blame culture” and “learning by reporting AEs”, increasing communication and teamwork by hospital management and national healthcare institutions will improve patient safety in Romanian hospitals.

Background: Creating a culture of patient safety is something that must be considered. This is because culture contains two important components, namely values and beliefs that can change organizations. Most safety incidents of Yogyakarta District Hospital in 2018 were 21 near miss incidents, incidents in total, then 17 incident, not injured and 5 events in unexpected events, while in potential injured there were no incidents during 2018. In 2018 there were still several months of data that had not yet met patient safety incident targets. From a preliminary study the researcher obtained, data on patient safety incident reporting has not been optimally performed by nurses. The purpose of this study was to determine the implementation of patient safety culture at the outcome level.Methods: This research is mixed methods research with an explanatory sequential design. Primary data obtained from in-depth interviews, a description of the implementation of patient safety culture at the Outcome level data obtained from questionnaires given to 72 nurses.Results: The culture of patient safety Yogyakarta District Hospital has been implemented well. At the level of patient safety culture outcomes related to the frequency of reporting patient safety events have been carried out but related to incidents that have no potential for injury when reporting is not appropriate, the perception of patient safety at the patient safety level, the number of reporting of events at the Yogyakarta District Hospital has been carried out properly.Conclusions: The safety culture of patients at Yogyakarta District Hospital at the outcome level has been implemented well.

Factors that influence the implementation of patient's safety culture by ward nurses in district general hospital

This study aims to determine the influencing factors of patient safety culture implementation by ward nurses in the hospital. This research used descriptive correlation with cross-sectional approach. The study population was 205 respondents. Data obtained by distributing valid and reliable questionnaires. Univariate and bivariate data were analyzed using chi-square test. The results showed that 60.5% of nurses had good patient safety knowledge. A total of 57.5% of nurses had high motivation and stated that leader behaviors supported patient safety implementation (54.6%). In another hand, 51.2% of nurses said they had implemented patient safety culture. In the next analysis, there was a significant relationship between nurses' knowledge, the motivation of patient safety, and the behavior leaders with its implementation of patient safety culture (p<0.05). This study shows the individual factors influence the implementation of patient's safety culture in hospital.

The implementation of patient safety culture in nursing practice

To summarize the implementation of patient safety culture in nursing practice at the hospitals. We did a literature review of the peer-reviewed articles published between 2009 until 2018 from four online databases: Emerald Insight, EBSCO, ScienceDirect, and Wiley Online, using the keywords "patient safety culture", "nurse," and "hospital." For the selection of articles, we used the PRISMA Groove Model with a three-step procedure (i.e. reviewing the titles, abstracts, and full contents of the articles). Selected articles were analyzed based on the 12 dimensions of patient safety culture as proposed by Agency for Healthcare Research and Quality (AHRQ). 11 articles were included in the final review. We categorized the results into three categories: strengths, weaknesses, and predictors of the patient safety culture of the nursing practice. The strengths consisted of four dimensions: (1) teamwork within units, (2) organizational learning and continuous improvement, (3) the support of the hospital management to the safety of patient, and (4) frequency of events reported. On the other side, weaknesses included: (1) assumptions about the safety of the patient as a whole, (2) handover and transfer, (3) open communication, (4) staffing, (5) nonpunitive response to errors, and (6) teamwork across hospital units. Lastly, the predictors of the patient safety culture were: (1) feedback and communication about errors and (2) supervisor/manager hopes and actions that promote safety. Our findings can inform the hospital management to enhance the patient safety culture among nurses. It can also inform the strategy to manage the potential threats and opportunity of the patient safety implementation at the hospital.

Terapi Bermain Clay Terhadap Tingkat Kecemasan Pada Anak Usia Prasekolah (3-6 Tahun)

Kecemasan merupakan perasaan yang paling umum dialami oleh pasien anak yang sedang mengalami hospitalisasi. Terapi bermain adalah suatu kegiatan bermain yang dilakukan untuk membantu penyembuhan anak dan sarana dalam melanjutkan pertumbuhan dan perkembangan anak secara optimal. Dengan terapi bermain, diharapkan kecemasan anak segera menurun, sehingga dapat menjadikan anak lebih bekerjasama pada petugas kesehatan. Permainan yang cocok diterapkan untuk anak usia prasekolah salah satunya adalah permainan membentuk (kontruksi) seperti clay. Tujuan penelitian ini untuk mengetahui pengaruh terapi bermain clay terhadap tingkat kecemasan pada anak usia prasekolah (3-6 tahun) di RSUD Balaraja Tangerang. Metode dalam penelitian ini menggunakan desain penelitian Praeksperimen dengan rancangan One Group Pretest Posttest. Sampel yang diambil 24 responden dengan menggunakan Total Sampling. Instrumen yang digunakan adalah lembar observasi serta uji analisis yang digunakan adalah Wilcoxon Signed Rank Test. Hasil penelitian ini menunjukkan bahwa, nilai (Sig 0,000 &lt;0,05). Kesimpulan Berdasarkan hasil penelitian tersebut maka ada pengaruh terapi bermain clay terhadap tingkat kecemasan pada anak usia prasekolah (3-6 tahun).

The Effect of Leadership, Education and Communication Channel Factors on Patient Safety Culture

Organizational culture has an important role in improving patient safety. In order to improve the implementation of patient safety culture, management and organization culture need to be tranformed. The objectives of this study were to analyze patient safety culture among groups of units of the hospital and to analyze leadership, education, and communication channel related to patient safety culture. This study was based on surveys and data from Quota Random Sampling Method. The data processing for mean analysis of patient safety culture scores among groups of work units was conducted using ANOVA test while the analysis for the relationship of leadership, education, and communication channel with the patient safety culture was performed by using SEM Partial Least Square (PLS) application. The results show that safety patient culture has no significant differences among nursing work unit meanwhile, leadership and education give significant relations on patient safety culture, and communication channel has no significant relations. The patient safety culture that is evenly distributed among the nursing units is a strength for the organization, and the factors that influence the establishment of the patient safety culture on the nurses are leadership and education. PENINGKATAN BUDAYA KESELAMATAN PASIEN MELALUI PEMBERDAYAAN CHAMPION KESELAMATAN PASIEN

&#x0D; Abstract&#x0D; Patient safety culture is a main fondation in implementation of patient safety. The purpose of this study was to know the effects of patient safety champion empowerment on the patient safety culture Rumah Sakit Haji Jakarta. This study was pre-experiment design : one group pretest posttest, with 81 nurses as a samples. Data were analyzed with mcnemar test. Result: implementation of patient safety culture increased not significantly 7,4 % after got a patient safety champion empowering program (p=0,451; CI= 0.084-0.928). Recommendation: Patient safety champion empowering need to developed to increase patient safety’s culture at hospital.
