Systematic Review of the Global Use of Validated Questionnaires in the Management of Stress Urinary Incontinence in Women.

Our goal was to systematically review the most commonly used validated questionnaires in recent global literature on stress urinary incontinence (SUI) treatment. PubMed, Embase, and Ovid databases were queried for manuscripts containing "female stress urinary incontinence" AND "diagnosis" AND "treatment" AND "questionnaire." Two independent reviewers screened studies for randomized controlled trials, prospective, and retrospective studies between 2018 and 2023. Exclusion criteria included male participants, non-SUI incontinence, and articles not originally written in English. The review was registered in PROSPERO [465721] and conformed to PRISMA guidelines. In 117 manuscripts meeting study criteria, the median of the mean ages was 52 years, with a median of 164 participants per study. Most studies originated in Europe (59/117). The International Consultation on Incontinence Questionnaire-Urinary Incontinence Short Form was the most frequently used (52%), followed by the Patient Global Impression of Improvement (31%), the Urinary Distress Inventory 6 Short Form (25%), the Incontinence Quality of Life (20%), and the Incontinence Impact Questionnaire Short Form (19%). These leading questionnaires were short, translated into several languages, and globally addressed important SUI-related domains, including the presence and severity of SUI, additional lower urinary tract symptoms, and the impact of SUI on quality of life, as well as changes perceived after treatment. This systematic review of the validated questionnaires used in contemporary SUI management literature could help guide recommendations for incorporating these favored instruments into future SUI treatment outcome documents.

Effect of Comprehensive Nursing Interventions on Quality of life among Women with Stress Urinary Incontinence

Stress Urinary Incontinence (SUI) is called a silent epidemic which is not a life threatening condition, but a worldwide problem at the same time which contributes to poor quality of life. Aim: Design, implement comprehensive nursing interventions for adult women with stress urinary incontinence and evaluate the effect of these interventions on improving their quality of life. Quazi experimental research design was utilized to conduct this study. This study was carried out in the Urology and Nephrology Assuit University Hospital and its affiliate Clinic. A sample of 50 adult women, the mean age of studied patients was (42.22±11.84). Two tools were used by the researcher namely: Tool I: A structured interview questionnaire sheet it included two parts, part i: demographic data of patient and part ii: medical& urological data assessment. Tool II: Kings Health Questionnaire (KHQ), it is a standardized questionnaire used to assess the impact of stress urinary incontinence on quality of life. The results showed that the quality of life among adult women with stress urinary incontinence were improved after the implementation of comprehensive nursing interventions with a statistically significance difference (p>0.05) in all quality of life domains. . We conclude that provided comprehensive nursing interventions are effective in improving QOL in adult women with SUI. Given the study results, it is recommended that expand of public education efforts through various methods about importance of health educational interventions for appropriate management of stress urinary incontinence among women.

Impact of Stress Urinary Incontinence After Radical Prostatectomy on Time to Intervention, Quality of Life and Work Status

To characterize the incidence of stress urinary incontinence (SUI) after radical prostatectomy (RP), its treatment, and impact on quality of life (QoL) and work status 1year after RP. Prostate cancer patients treated by RP (1998-2016) were selected from CaPSURE. SUI was defined as any pads per day (ppd) 1 year after RP. SUI procedures were tracked by CPT codes (sling and artificial sphincter). Patients reported work status (full-time, part-time, unpaid), UCLA PCa Index urinary function (UF) and bother (UB) and SF36 Index physical function (PF). Associations of incontinence with UF, UB, and PF and work status changes were assessed (ANOVA). Lifetable estimates and Cox proportional hazards regression evaluated risk of undergoing SUI procedures. 664/2989 (22%) men treated with RP reported SUI at 1 year. More men with SUI had ≥GG2, intermediate to high-risk disease and non-nerve-sparing surgery (all P<.01). Cumulative incidence of SUI procedures was 1.4% at 10years after RP. Age (HR 2.68 per 10years, 95% CI 1.41-5.08) and number of ppd at 1 year (HR 3.20, 95% CI 2.27-4.50) were associated with undergoing SUI procedures. UF declined at 1year after RP, while UB and PF remained stable. UF, UB, and PF were inversely associated with number of ppd (all P<.01). Change in work status was not associated with incontinence or QoL scores. Incontinence affected QoL without impacting work status, suggesting that men with SUI after RP may continue working and go under-treated despite impact on QoL.

Neuromodulation effect of laser acupuncture on female stress urinary incontinence: a randomized controlled trial

Although the impact of electroacupuncture or needle acupuncture on stress urinary incontinence (SUI) was previously investigated, there are no previous studies that have investigated the neuromodulatory impact of laser therapy on neurogenic acupoints in women with SUI. The aim of this study was to determine the neuromodulation mechanism of laser therapy on neurogenic acupoints in women with SUI. A prospective, randomized, controlled trial, pre, and post-experimental study was carried out. Thirty women with SUI were equally assigned into two equal groups (control and study groups). The control group received pelvic floor exercises for 6 weeks (n=15), while the study group received the same pelvic floor exercises for 6 weeks in addition to laser acupuncture on neurogenic acupoints (n=15). Before and after rehabilitation, pelvic floor muscle strength (PFMS) was assessed with the XFT-0010 Pelvic Muscle Trainer device, SUI severity was assessed with the Severity Index (SI) for urinary incontinence, and the impact of SUI on quality of life (QOL) was assessed with the International Consultation on Incontinence Questionnaire-Urinary Incontinence Short Form (ICIQ-SF). Comparison between the two groups after treatment showed a significant improvement in PFMS and significant reductions in scores of SI for urinary incontinence and ICIQ-SF (p&lt;0.05). The neuromodulatory impact of laser application on neurogenic acupoints significantly increases the strength of pelvic floor muscles and significantly decreases the incontinence level and its impact on QOL in women with SUI.

Impact of stress urinary incontinence on female sexual activity.

The study aimed to investigate the impact of SUI (Stress Urinary Incontinence) on the sexual activity of women, to assess their sexual functioning, and to show the extent of the problem that SUI poses to the quality of life of women. The study involved 70 women aged 20-48 years. The inclusion criteria included the presence of stress urinary incontinence, the sexual activity of the women, and the history of no urogynecological intervention. The authorial questionnaire and the Polish version of the Female Sexual Function Index (FSFI) were used. SUI contributes to reducing the frequency of intercourse and even complete resignation from sexual intercourse. There is a correlation between the occurrence of urinary leakage during intercourse and the occurrence of sexual dysfunction (p=0.023). The most common factors limiting sexual activity are decreased libido, fatigue, lack of desire, and lack of body acceptance. However, age (p=0.070), marital status (p=0.091), Body Mass Index (BMI) (p=0.436), as well as the duration of stress urinary incontinence (p=0.36) have no effect on women's sexual activity. The most common ways of dealing with the loss of urine during intercourse include micturition before intercourse, intercourse only in safe places, restriction of physical activity during intercourse, and reduction of intercourse frequency and duration. SUI in women has a significant effect on their sexual activity. The cause of this state of affairs is multifactorial. Some women try to cope with the problem and have developed a number of strategies that allow them to be sexually active without unpleasant surprises.

Prevalence of stress urinary incontinence and its impact on quality of life among women in Jordan: a correlational study.

ObjectiveThe study investigated the prevalence and impact of stress urinary incontinence (SUI) among women in Jordan.MethodsA correlational study was conducted to evaluate 500 Jordanian women aged >20 years. Women with symptoms of dementia, delirium, neurodegenerative changes and osteodegenerative changes were excluded. Data were collected using self-administered questionnaires (Arabic version of the Urogenital Distress Inventory-6 and Incontinence Impact Questionnaire (IIQ-7) short forms).ResultsA total of 200 (40%) women reported SUI; 47% reported mild symptoms, 37% reported moderate symptoms and 16% reported severe symptoms. There were positive correlations between SUI prevalence and number of pregnancies, age and obesity. SUI had a substantial impact on all aspects of quality of life (QoL), as assessed by the IIQ-7.ConclusionsA moderate prevalence of SUI was reported. There was a significant association between development of SUI and age, higher body mass index and number of pregnancies. Because SUI is highly prevalent and has a major effect on QoL, healthcare professionals should inquire about this condition and refer patients to related specialties for treatment.

The Impact of Stress Urinary Incontinence on Individual Components of Quality of Life in Malaysian Women

To assess the impact of stress urinary incontinence (SUI) on individual components of quality of life (QoL) using both condition-specific and generic questionnaires, and to compare the results of the 2 instruments with a control group. Women with or without SUI aged ≥21 years old were recruited. Subjects completed the International Consultation of Incontinence-Urinary Incontinence Short Form (ICIQ-UI-SF), International Consultation of Incontinence-Lower Urinary Tract Symptoms Quality of Life (ICIQ-LUTSqol), and EQ-5D questionnaires. A total of 120 women with SUI and 145 controls participated. The ICIQ-LUTSqol total score (mean ± standard deviation) was significantly higher in the SUI group (38.96 ± 10.28) compared with the control group (20.78 ± 2.73) (P <.001). When adjusted for significant confounders, the SUI group continued to have significantly poorer QoL compared with the control group (P <.001). The negative effect of SUI on "physical activities" and "jobs" were the 2 most frequently reported and burdensome components of the ICIQ-LUTSqol, with approximately 50% of women with SUI affected "moderately" or "a lot." When measured using the EQ-5D questionnaire, there were significantly higher percentages of patients with SUI who had problems with usual activities, pain or discomfort, and anxiety or depression (P <.05). Women suffering from SUI have significantly poorer QoL compared with continent women when measured using both condition-specific and generic QoL measures. Clinicians should pay closer attention to the impact of SUI on individual components of QoL, particularly limitations on physical activities and jobs, which were the 2 most impairing and frequently reported components of QoL.

Stress urinary incontinence (SUI) and obesity are problems recognized by the World Health Organization as social diseases. The International Continence Society has estimated that about 10–40% of women have problems with urinary incontinence. Overweight states and obesity are among the biggest health problems in perimenopausal women. The aim of the study was to analyze the impact of SUI on the physical activity (PA) of women in relation to their body weight. The study comprised 54 women aged 41–67 years with SUI. Anthropometric measurements were taken. Physical activity was examined by specially designed questionnaires. The statistical significance was calculated in the Statistica 12 software using normality tests, correlation coefficient, nonparametric tests, and a post -hoc test. The mean age of subjects was 55 ±7 years. Body Mass Index (BMI) in subjects was normal (N1) – 11 (20.4%), or indicated the state of being overweight (N2) – 33 (59.2%) or obese (N3) – 11 (20.4%); 94% (51) patients had the android body type and 6% (3) had the gynoidal body type. PA before the onset of problems with SUI was assessed by patients as a sedentary lifestyle – 12 (22.2%), active – 23 (42.6%) or mixed – 19 (35.2%). Subjects, when asked how strong the impact of SUI on their PA was, answered: minor 13 (24.1%), moderate – 14 (25.9%), very strong – 27 (50%). No statistically signi􀏐icant differences between PA and SUI were found in N1, N2 and N3 groups. Overweight patients reported the strongest impact of SUI on their PA, but the differences between the analysed groups were not signi􀏐icant. Most subjects had the android body type, which was assessed by means of WHR.

Pelvic floor disorders (PFDs) can impact sexual function. This summary provides an overview of the impact of stress urinary incontinence and pelvic organ prolapse and their treatments on sexual function. In general, interventions that successfully address PFDs will generally improve sexual function as well. However, there are patients whose sexual function will remain unchanged despite treatment, and a small but significant minority who will report worsened sexual function following treatment for their pelvic floor dysfunction.

Chinese women's experiences of stress incontinence: a descriptive qualitative study

This study explored the perceptions of 35 Chinese women on the impact of stress urinary incontinence on their life and their adherence to pelvic floor muscle exercise program. Audiotaped interviews were conducted at a time and place convenient to them. Content analysis was used to extract significant categories that described their experience. The five major categories that emerged were: (i) perceptions of stress urinary incontinence; (ii) living with urinary incontinence; (iii) emotions and feelings; (iv) factors facilitating pelvic floor muscle exercise and (v) factors inhibiting pelvic floor muscle exercise program. The women who adhered and those who did not adhere to pelvic floor muscle exercised were similar in the two categories of living with urinary incontinence, and emotions and feelings. However, there were great differences between the perceptions of the two groups in three categories on perceptions of urinary incontinence, factors facilitating and inhibiting the performance of pelvic floor muscle exercise.

Prevalence of Stress Urinary Incontinence in Women with Multiple Sclerosis

PurposeThe purpose of this study was to determine the prevalence of stress urinary incontinence (SUI) in women with multiple sclerosis (MS) and to what degree these women are bothered by their SUI, since there is a paucity of literature regarding the nature of SUI in this unique population of women.MethodsWe conducted a prospective Institutional Review Board approved study. Women scheduled for outpatient follow-up appointments at a dedicated MS center were asked to complete a questionnaire regarding urinary incontinence. Urgency urinary incontinence (UUI) and SUI were defined as an answer of slightly, moderately or greatly to the Urogenital Distress Inventory (UDI-6) question #2 and question #3, respectively. Impact of SUI on physical activity was determined by Incontinence Impact Questionnaire (IIQ-7) question #2.ResultsA total of 55.9% (80/143) women had SUI, 70.6% (101/143) women had UUI, and 44.8% (64/143) women had mixed urinary incontinence. The mean age was 45.8 years old (range, 20 to 72 years). Women with SUI were significantly older (mean, 47.2 vs. 41.9; P=0.023) and there was a trend towards a greater body mass index (mean, 29.3 vs. 26.5; P=0.057). Women with SUI had significantly higher IIQ-7 scores compared to women without SUI (P<0.001). Impact of urinary incontinence on physical activity was also found to be significantly greater in women with SUI (mean IIQ-7 question #2, 0.96 vs. 0.35; P<0.001).ConclusionsThe prevalence of SUI in women with MS is 55.9% and the presence of SUI has a significant impact on their quality of life. A comprehensive urologic evaluation of a woman with MS should include assessment of SUI.

The Impact of Obesity on Urinary Incontinence Symptoms, Severity, Urodynamic Characteristics and Quality of Life

We compared urinary incontinence severity measures and the impact of stress urinary incontinence in normal, overweight and obese women. Baseline characteristics of subjects in the SISTEr (655) and the TOMUS (597) were analyzed. Body mass index was defined as normal (less than 25 kg/m(2)), overweight (25 to less than 30 kg/m(2)) and obese (30 kg/m(2) or greater). Independent urinary incontinence severity measures included a 3-day diary including incontinence episode frequency, Urogenital Distress Inventory scores and Valsalva leak point pressure from urodynamic testing. Impact was measured using the Incontinence Impact Questionnaire. Multivariable regression models were fit for each severity measure (Urogenital Distress Inventory, incontinence episode frequency, Valsalva leak point pressure and Incontinence Impact Questionnaire) on weight category. Covariates included age, race, diabetes and variables significantly associated with body mass index on bivariate analysis. Mean age (SD) of participants was 51.9 (10.3) in SISTEr and 52.9 (11.0) in TOMUS. In each trial 45% of subjects were obese. In SISTEr multivariable regression analyses showed that higher weight category was independently associated with higher mean Urogenital Distress Inventory score (p = 0.003), incontinence episode frequency (p <0.0001), Valsalva leak point pressure (p = 0.003) and Incontinence Impact Questionnaire score (p = 0.0004). In TOMUS higher weight category was not associated with Urogenital Distress Inventory score (p = 0.24) but was associated with higher incontinence episode frequency (p = 0.0003), Valsalva leak point pressure (p = 0.0006) and Incontinence Impact Questionnaire score (p <0.0001). Obese women undergoing surgery for stress urinary incontinence report more incontinence episodes, more symptom distress and worse quality of life despite better measure of urethral function (higher Valsalva leak point pressure) on urodynamics.

Distress and quality of life characteristics associated with seeking surgical treatment for stress urinary incontinence.

BackgroundCurrent research focuses on three variables in evaluating the impact of stress urinary incontinence (SUI) on daily living: severity of incontinence, distress or bother resulting from incontinence, and effect on health related quality of life (HRQoL). Understanding the impact of these variables is important as they are the driving force behind women seeking surgical treatment. Given the importance of HRQoL in determining need for treatment, as well as evaluating treatment success, this review provides an assessment of the degree to which HRQoL is impaired in women seeking surgical treatment.MethodsPubMed searches for the terms "quality of life and distress and urinary incontinence" and "quality of life and bother and urinary incontinence" were performed with limits of English, human and female subjects through May 2008. All studies using validated instruments were included. No time limit was placed on the search.ResultsOf 178 articles retrieved, 21 met the inclusion criteria, and 17 reported methods of scoring. The studies used the Urogenital Distress Inventory (UDI) and the Incontinence Impact Questionnaire (IIQ). Wide ranges of mean and individual levels of severity of symptoms, UDI and IIQ scores were seen among women seeking surgical treatment. Fourteen studies reported baseline and post-surgical treatment distress and QoL data. Statistically significant improvements between baseline and post-surgical UDI and IIQ scores were reported in 12 studies. Reported cure rates ranged from 46% to 97%. Satisfaction with the procedure was reported in 4 studies and ranged from 84% to 91%. A minority of studies reported the relationship between reduction in symptoms and change in HRQoL.ConclusionHRQoL is the main reason women seek surgical treatment for incontinence and surgical treatment leads to a significant improvement in mean HRQoL scores. Assessment of HRQoL has proved less useful in identifying why individual women seek treatment for incontinence. Preliminary work has begun to characterize the interaction between severity of symptoms, distress or bother resulting from these urinary symptoms, impact on HRQoL, and treatment seeking behavior, but further research is needed. Greater standardization in the reporting of results of distress or bother and HRQoL would allow for comparison across studies.

The impact of stress urinary incontinence (SUI) is not limited to physical and psychological consequences classically evaluated for this disease. In fact, some studies emphasize the indisputable existence of sexual disorders directly imputable to SUI. Sexual function is an important evaluation element before and after surgical treatment of SUI. Many validated questionnaires about sexual disorders exist, the most frequently utilized in present literature being the female sexual function index (FSFI). Suburethral slings, in spite of discordant results, do not seem to impair global sexuality of patients, some authors reporting a benefit effect of surgery, related to a decrease of coital incontinence after surgery and a decrease of apprehension before sexual intercourse.

Health‐related quality of life and sexual function in women with stress urinary incontinence and overactive bladder

We evaluated the impact of stress urinary incontinence (SUI) and overactive bladder (OAB) on health-related quality of life (HRQOL) and sexual function. A total of 245 women (SUI; n = 123 and OAB; n = 122) from 21 to 79 years old (mean 50.4) were included in the primary analyses. To obtain HRQOL and sexual function assessments, patients were asked to fill in the 'Bristol Female Lower Urinary Tract Symptoms (BFLUTS)' and the 'Medical Outcomes Study Short Form (SF-36)' questionnaires. Of the eight domains in the SF-36 questionnaire, only 'general health' was significantly different between the groups. Patients with SUI had a better general health than those with OAB (P = 0.016). When comparing the BFLUTS scores in the two groups, the score for 'BFLUTS-filling symptoms' was higher in the OAB group (P = 0.002) but that for 'BFLUTS-incontinence symptoms' was higher in the SUI group (P < 0.001). The score for 'BFLUTS-sex' was higher in the SUI group than in the OAB group but this was not statistically significant (P = 0.096). Of the 169 patients who had a sex life, the SUI group had experienced pain (P = 0.033) and leakage (P = 0.056) more frequently during intercourse than the OAB group. Both SUI and OAB have a detrimental impact on patient HRQOL in Korean women. In addition, our findings suggest that women with SUI had more frequently experienced pain during intercourse and coital incontinence than those with OAB.

Female Urinary Incontinence (UI) is a major public health issue given its high prevalence and its physical, psychological and social impact on women's lives. The aim of this study was to determine the prevalence and impact of Stress Urinary Incontinence (SUI) before and during pregnancy in parturients from the Viana do Castelo district. A cross-sectional study was carried out considering a representative sample of the district comprising 336 women who gave birth in the Santa Luzia Hospital between 15th January and 29th March 2002. A questionnaire was submitted to all the women during the postpartum period at hospital. The prevalence of SUI, defined as Have you ever experienced urine loss while performing an effort? was of 5.4% (IC 95%:3.0-7.8) before pregnancy and 51.5% (IC 95%:46.1-56.9) during the current pregnancy. The factors associated with the occurrence of SUI before pregnancy were multiparity (OR=9.96), diabetes (OR=4.61) and obesity (OR=4.76), and with SUI during pregnancy were multiparity (OR=1.66), diabetes (OR=2.62) and constipation (OR=1.73). The vast majority (88.9%) of women suffering from SUI feel uncomfortable as they feel wet, 48.5% feel nervous or troubled and 57.3% fear that others may notice the smell. During pregnancy, only less than half of the women who experienced urine loss sought help from a healthcare professional, although the majority was interested in having this problem treated. SUI affects a great number of women from the district before and during pregnancy. SUI is reflected in many aspects of health and mostly affects physical and emotional well-being, but only a small percentage of women reveal the burden of SUI to a healthcare professional. In the face of such evidence, it turns out very important for healthcare professionals to be aware of this reality and concerned to give response to this healthcare problem.

Comparison of three disease-specific quality-of-life questionnaires (Bristol Female Lower Urinary Tract Symptoms, Incontinence Quality of Life and King's Health Questionnaire) in women with stress urinary incontinence

To examine the impact of stress urinary incontinence (UI) on quality of life (QOL) using three disease-specific QOL instruments and compare the results obtained with these instruments. A total of 28 women (age range 36-74 years) with stress UI were included in the study. To obtain QOL assessments, patients were asked to complete the Bristol Female Lower Urinary Tract Symptoms (BFLUTS), Incontinence Quality of Life (I-QoL) and King's Health Questionnaire (KHQ) instruments. One domain in the BFLUTS (Incontinence symptoms) and one in the KHQ (Severity measures) correlated inversely with the Valsalva leak point pressure and the cough leak point pressure, respectively; however, other domains in the three questionnaires did not correlate with objective data. Two BFLUTS domains (Incontinence symptoms and QOL) correlated weakly or moderately with six KHQ domains; however, other BFLUTS domains did not correlate with most KHQ domains. Significant negative correlations were noted between two BFLUTS domains (Incontinence symptoms and QOL) and most I-QoL domains; however, other BFLUTS domains did not correlate with the I-QoL. Role limitations, Emotional problems and Severity measures in the KHQ correlated weakly or moderately with the I-QoL; however, General health and Personal relationships did not correlate significantly with the I-QoL. Subjective QOL results of stress UI using condition-specific QOL questionnaires may differ because there are a plethora of measurement instruments that vary in terms of their scope and content. Our findings suggest that, before deciding on an instrument, its contents should be thoroughly reviewed to ensure that a particular aspect of QOL does not need additional assessment.
