The Impact of Diabetes Complications on the Physical Function of Maintenance Hemodialysis Patients.

This study investigated the impact of diabetes on the physical function of patients undergoing dialysis.This study included 22 patients undergoing outpatient dialysis with continued exercise therapy during dialysis at our hospital between January 2021 and August 2021. The participants were divided into two groups based on the presence or absence of diabetes, and various parameters were compared between the groups. To compare each physical function assessment and measurement of anterior thigh muscle thickness, repeated-measures analysis of variance was conducted to test for the presence of interactions and main effects. Significant differences were observed in the absence of dyslipidemia (p < 0.01), high-density lipoprotein cholesterol level (p < 0.01), and foot sole skin perfusion pressure (p < 0.02). In terms of physical function, a main effect between the groups was observed in the five-time sit-to-stand test, and anterior thigh muscle thickness showed a main effect over time. Significant differences in the anterior thigh muscle thickness were observed between three and six months after the intervention (p < 0.05). In patients undergoing dialysis with diabetes complications, a decrease in physical activity and lack of exercise can lead to a reduction in overall physical activity levels. Additionally, impairments such as peripheral neuropathy may contribute to an accelerated decrease in skeletal muscle mass.

AN ASSESSMENT OF AWARENESS, ATTITUDE, AND PRACTICE REGARDING RELATIONSHIP BETWEEN DIABETES MELLITUS & PERIODONTITIS AMONG GENERAL DENTAL & MEDICAL PRACTITIONERS IN 3 MAJOR CITIES OF MADHYA PRADESH, INDIA- A SURVEY.

Background: Diabetes mellitus and periodontal diseases evidently have a bidirectional relationship. To apply this into daily practise, an awareness among dentists and physicians of the same is imperative. Thus, a KAP (knowledge, attitude and practice) survey was conducted among dental and medical practitioners about the association between diabetes mellitus and periodontitis. Methodology: A survey was conducted among 563 dental &amp; medical practitioners (263 physicians and 300 dentists) in 3 cities of Madhya Pradesh. Information on knowledge about complications related to periodontal conditions, attitude regarding diabetes and periodontal disease and the relation between the two was collected. Association between categorical was computed using Chi square test, with the α-level set at 0.05. Results: Dentists were more aware about impact of diabetes complications on periodontal health as compared to physicians (p&lt;0.05). Majority of dentists referred their patients to physicians for medical evaluation and were more interested in learning about bidirectional relationship between diabetes mellitus &amp; periodontal health, while fewer physicians referred their patients to dentists. The preferred mode of learning for dental practitioners was through conferences or CDEs, while medical practitioners preferred professional journals. Conclusion: The knowledge, awareness and practice regarding bidirectional association between diabetes and periodontitis of dentists fared better than the physicians.

Impact of Diabetic Complications on Balance and Falls: Contribution of the Vestibular System.

Diabetes causes many complications, including retinopathy and peripheral neuropathy, which are well understood as contributing to gait instability and falls. A less understood complication of diabetes is the effect on the vestibular system. The vestibular system contributes significantly to balance in static and dynamic conditions by providing spatially orienting information. It is noteworthy that diabetes has been reported to affect vestibular function in both animal and clinical studies. Pathophysiological changes in peripheral and central vestibular structures due to diabetes have been noted. Vestibular dysfunction is associated with impaired balance and a higher risk of falls. As the prevalence of diabetes increases, so does the potential for falls due to diabetic complications. The purpose of this perspective article is to present evidence on the pathophysiology of diabetes-related complications and their influence on balance and falls, with specific attention to emerging evidence of vestibular dysfunction due to diabetes. Understanding this relationship may be useful for screening (by physical therapists) for possible vestibular dysfunction in people with diabetes and for further developing and testing the efficacy of interventions to reduce falls in this population.

Insulin Infusion Pumps for Diabetes Mellitus Treatment

Portable insulin pumps have been used for the treatment of diabetes mellitus since the mid 1970s and are available from two manufacturers. Both are programmable syringe pumps and sell for about $3,600. Portable pumps can be a powerful tool for achieving and maintaining precise control of glycemia, but are most likely to improve quality of life in settings where paitents collaborate closely with medical professionals in the management of their disease. Implantable insulin delivery pumps are not yet FDA approved but are available in clinical trial. Decisions regarding which type I diabetic patients are suitable candidates for treatment by implantable pump cannot be meaningfully addressed at this time. Implantable pump therapy is clearly the most expensive therapeutic option; portable pump therapy is intermediate in expense, and conventional therapy is least expensive. Continuous intravenous or intraperitoneal delivery by implantable pump may provide superior metabolic control with fewer episodes of hypoglycemia. If such control retards the development of late complications, the additional cost of pump therapy will provide a favorable benefit-tocost ratio when weighed against the economic impact of diabetic complications.
