Using Machine Learning and Feature Importance to Identify Risk Factors for Mortality in Pediatric Heart Surgery.

Background: The objective of this IRB-approved retrospective monocentric study was to identify risk factors for mortality after surgery for congenital heart defects (CHDs) in pediatric patients using machine learning (ML). CHD belongs to the most common congenital malformations, and remains the leading mortality cause from birth defects. Methods: The most recent available hospital encounter for each patient with an age <18 years hospitalized for CHD-related cardiac surgery between the years 2011 and 2020 was included in this study. The cohort consisted of 1302 eligible patients (mean age [SD]: 402.92 [±562.31] days), who were categorized into four disease groups. A random survival forest (RSF) and the 'eXtreme Gradient Boosting' algorithm (XGB) were applied to model mortality (incidence: 5.6% [n = 73 events]). All models were then applied to predict the outcome in an independent holdout test dataset (40% of the cohort). Results: RSF and XGB achieved average C-indices of 0.85 (±0.01) and 0.79 (±0.03), respectively. Feature importance was assessed with 'SHapley Additive exPlanations' (SHAP) and 'Time-dependent explanations of machine learning survival models' (SurvSHAP(t)), both of which revealed high importance of the maximum values of serum creatinine observed within 72 h post-surgery for both ML methods. Conclusions: ML methods, along with model explainability tools, can reveal interesting insights into mortality risk after surgery for CHD. The proposed analytical workflow can serve as a blueprint for translating the analysis into a federated setting that builds upon the infrastructure of the German Medical Informatics Initiative.

Paediatric acute liver failure: early outcome and evaluation of transplantation criteria in a Belgian transplantation centre

BackgroundOne of the challenges when caring for children admitted for acute liver failure (ALF) is to quickly identify those who will improve spontaneously and those for whom liver transplantation (LT) is the only therapeutic option.MethodsRetrospective study to review our experience, identify mortality risk factors and update our LT criteria in case of paediatric ALF.Results111 children were admitted between March 1989 and May 2021 (mean age 59.1 months). 28 children never met our LT criteria; 17 had contraindication to LT; 66 were registered on the LT waitlist. 14 of these 66 were subsequently withdrawn because of spontaneous liver function recovery; 11 died before having received a liver; 41 were transplanted. Hospital survival rate was 63% for the whole series, 98% for the children without LT criteria and 71% for the transplanted children. Univariate analysis identified cardiovascular and respiratory failures on admission, and grade 4–5 hepatic encephalopathy (HE) during stay significantly associated with death. Non-survivors also had, on admission and during their stay, significantly higher levels of lactate, ammonia and bilirubin and, during their stay, significantly more frequent prothrombin time ≤ 25% or international normalized ratio ≥ 4.0 than the survivors. Multivariate analysis identified grade 4–5 HE, lactate level on admission and ammonia peak level as significant mortality risk factors.ConclusionOur criteria identified almost all children who had the capacity to spontaneously recover their liver function and suggest that updated criteria should combine biological tests and signs of multiple organ failure.

Early Period Survival and Neurologic Prognosis in Newborns with Perinatal Asphyxia: A Tertiary Center Experience and a Mortality Chart

Background: Clinicians require more data regarding mortality and brain damage risk factors in perinatal asphyxia. Objectives: To assess early term outcomes and identify mortality risk factors in perinatal asphyxia. Methods: This study was conducted in a referral-center tertiary intensive care unit in Istanbul, Turkey, between 2016 and 2023. We included all patients who underwent therapeutic hypothermia treatment due to perinatal asphyxia. We recorded laboratory follow-up data, magnetic resonance imaging (MRI) findings, amplitude-integrated electroencephalograms (aEEG) results, mortality, and clinical outcomes. Both conventional frequentist statistical methods and Bayesian methods were used for analysis. Results: A total of 164 patients were included in the study, with an overall mortality rate of 9.8%. Risk factors for mortality included LDH, troponin I, INR, lactate, 2nd day creatinine, voltage anomalies, seizures, and male gender, as well as APGAR scores. A basic chart for mortality prediction was developed. The Sarnat score showed strong evidence, and APGAR 1 showed anecdotal evidence for association with brain damage, although brain damage was independent of laboratory results and other clinical findings, based on moderate and anecdotal evidence from Bayesian calculations. Cranial MRI findings revealed profound damage in 14.8% of Sarnat 1, 21.8% of Sarnat 2, and 50% of Sarnat 3 patients. Conclusions: This study presents prognostic factors for survival and brain damage in perinatal asphyxia. We recommend obtaining cranial MRI for all patients diagnosed with asphyxia, as most laboratory tests were independent of brain damage. Given that profound brain damage can occur even in Sarnat stage I patients, we emphasize the importance of therapeutic hypothermia for these patients.

Higher Morbidity and Mortality in Women With Fournier Gangrene Compared With Men: Insights From National Inpatient Sample Data.

To compare Fournier gangrene in female and male patients and identify mortality-associated characteristics in both. We used National Inpatient Sample data (2016-2020) to identify Fournier gangrene cases and extracted demographic, comorbidity, and procedural variables. Multivariable regression models were used to identify mortality risk factors for both cohorts. We identified 2875 female (31%) and 6451 male patients (69%) with Fournier gangrene corresponding to an estimated 14,375 (95% CI, 13,784-14,966) and 32,255 (95% CI, 31,390-33,120) cases, respectively. Female patients were more likely to die than male patients (7.1% vs 5.7%, P < .0001, respectively). The median incidence rates were 1.7 (IQR, 1.5-1.8) and 4 (IQR, 3.6-4.3) cases per 100,000 person-years for female and male patients, respectively. Female patients had higher median age, longer hospital stays, more charges, procedures, and fecal diversion rates, but lower routine discharges than male patients (P < .05). Non-White female patients had increased mortality odds compared with White female patients (odds ratio [OR], 1.49; 95% CI, 1.07-2.07; P = .019). Prolonged interval until initial perineal debridement correlated with higher mortality odds in both female and male patients (OR, 1.02; 95% CI, 1-1.04; P = .034 vs OR, 1.03; 95% CI, 1.01-1.05; P < .0001). Diabetes lowered mortality odds in female and male patients (OR, 0.68; 95% CI, 0.47-0.99; P = .046 vs OR, 0.54; 95% CI, 0.41-0.7; P < .0001). In female patients, Fournier gangrene incidence surpasses previous reports, with slightly worse outcomes compared with male patients, emphasizing the need for precise clinical assessment and early intensive interventions.

Assessing the Predictive Utility of the C-Reactive Protein-to-Lymphocyte Ratio for Mortality in Isolated Traumatic Brain Injury: A Single-Center Retrospective Analysis.

Introduction: Early identification of high-risk traumatic brain injury (TBI) patients is crucial for optimizing treatment strategies and improving outcomes. The C-reactive protein-to-lymphocyte ratio (CLR) reflects systemic immunology and inflammation function and serves as a new biomarker for patient stratification. This study aimed to assess the predictive value of the CLR for mortality in patients with isolated moderate to severe TBI. Methods: A retrospective analysis of trauma registry data from 2009 to 2022 was conducted, including 1641 adult patients with isolated moderate to severe TBI. Patient demographics, the CLR, injury characteristics, and outcomes were compared between deceased and surviving patients. Univariate and multivariate analyses were performed to identify mortality risk factors. The optimal CLR cut-off value for predicting mortality was determined using receiver operating characteristic (ROC) curve analysis. Results: The CLR was significantly higher in deceased patients compared to survivors (60.1 vs. 33.9, p < 0.001). The optimal CLR cut-off value for predicting mortality was 54.5, with a sensitivity of 0.328 and a specificity of 0.812. The area under the ROC curve was 0.566, indicating poor discriminative ability. In the multivariate analysis, the CLR was not a significant independent predictor of mortality (OR 1.03, p = 0.051). After propensity score matching to attenuate the difference in baseline characteristics, including sex, age, comorbidities, conscious level, and injury severity, the high-CLR group (CLR ≥ 54.5) did not have significantly higher mortality compared to the low-CLR group (CLR < 54.5). Conclusion: While the CLR was associated with mortality in TBI patients, it demonstrated poor discriminative ability as a standalone predictor. The association between a high CLR and worse outcomes may be primarily due to other baseline patient and injury characteristics, rather than the CLR itself.

Intense light and conspecific density increase seedling mortality across age groups in a typhoon‐disturbed tropical forest

AbstractQuestionsMaintaining forest species diversity, regenerative function and ecosystem resilience begins with successful seedling establishment. Despite their critical role, seedlings face the highest mortality risks among plant life‐history stages. Our research aims to answer the following questions: In a tropical forest disturbed by typhoons, what is the median survival time for seedlings, to what extent do light levels contribute to mortality risks and what are the crucial mortality risks across seedling age groups?LocationA tropical mountain zonal foothill evergreen broad‐leaved forest in Nanjenshan Nature Reserve, Taiwan, frequently visited by typhoons.MethodsWe investigated woody seedlings in 75 quadrats (each 1 m × 1 m) every 3 months from October 2009 to January 2018, spanning 8.3 years with 34 censuses. We used the Kaplan–Meier method to estimate seedlings’ median survival time and Generalized Linear Models to identify mortality risk factors for all seedlings and those that persisted beyond the median survival time.ResultsThe median survival time for all seedlings was 0.5 years, ranging from 0.25 years for shade‐intolerant species to 1.25 years for shade‐tolerant species. Seedlings germinated in high‐light microhabitats and high conspecific seedling density exhibited higher mortality probabilities, regardless of age.ConclusionsThis study reveals the short survival time and key mortality risks of seedlings in a typhoon‐disturbed forest. The negative impact of light levels on seedling survival highlights the potential consequences of increased light levels resulting from typhoon disturbances in the study region.

Impact of Statin or Angiotensin Converting Enzyme Inhibitor/Angiotensin Receptor Blocker on In-Hospital Mortality of COVID-19 Patients

Background: It is important to determine the risk factors that contribute to the mortality of a disease and take measures to prevent or alleviate it. In the case of COVID-19, old age, male gender, and comorbidities such as diabetes (DM) and hypertension (HTN) have been identified as potential risk factors. However, there is conflicting information on the effects of statins and ACEI/ARB in COVID-19 patients admitted to the ICU, particularly those with diabetes or hypertension. This study aims to investigate the effects of these drugs on the in-hospital prognosis of ICU COVID-19 patients, with a focus on patients with DM or HTN. Methods: During 18 months, we conducted a descriptive-analytical observational analysis on 391 patients who were admitted to the ICU. The study focused on COVID-19 patients and aimed to identify mortality risk factors by assessing their demographic, clinical, pharmaceutical, laboratory, and imaging data. We statistically analyzed the data to achieve this goal. Results: Out of 391 patients, 83 received statins and 89 received ACEI/ARBs. The research has revealed that the use of ACEI/ARBs in COVID-19 patients admitted to the ICU may increase the risk of endotracheal intubation (P&lt;0.0001) and mortality (P&lt;0.0001). Additionally, patients treated with these drugs are more likely to experience secondary bacterial infections (P=0.007) and venous thromboembolism events (P=0.015). The results of a recent study analyzing diabetic and hypertensive patients hospitalized in ICU showed that there is no significant difference in clinical outcomes between COVID-19 patients who used ACEI/ARB and those who did not. Our study has found that the use of statins in diabetic patients is linked to a reduction in mortality rate (0.008) and secondary bacterial infections (P=0.035) of COVID-19 patients admitted to the ICU. In multivariate logistic regression, the use of statin or ACE/ARB was not identified as the mortality prediction factor. Conclusion: Statins can help reduce mortality rates among COVID-19 patients, especially in diabetic patients, hospitalized in the ICU. So, they should be used to manage cardiovascular risk factors and lower the mortality risk. Statins and ACEI/ARB drugs were not predictors of mortality and did not decrease survival rates during ICU hospitalization.

Predictors of Mortality among Intensive Care Unit (ICU) Hospitalized Diabetic Patients with COVID-19

Objective: Diabetic patients with COVID-19 are at the higher risk of clinical complications and intensive care unit (ICU) admission. There is limited information available on the risk factors of mortality in diabetic patients with COVID-19 admitted to the ICUs. The aim of this study was identifying the mortality risk factors in diabetic patients with COVID-19 who are admitted to the ICU. Materials and Methods: In this study, we conducted a descriptive-analytical observational analysis on 391 patients admitted to the ICU for 18 months. We assessed the demographic, clinical, pharmaceutical, laboratory and imaging data of diabetic patients and statistically analyzed them to identify mortality risk factors. Results: The study found 156 (39.89%) diabetic out of 391 patients. The group of diabetic patients had significantly higher rates of endotracheal intubation (P&lt; 0.001), mortality (P&lt; 0.001), and complications during hospitalization due to COVID-19, including secondary bacterial infections (P = 0.005), venous thrombosis (P = 0.008), and gastrointestinal bleeding (P = 0.011), compared to the nondiabetic patient. Conclusion: Patients with diabetes who also have COVID-19 tend to experience more severe clinical outcomes and a higher mortality rate when admitted to the intensive care unit. The likelihood of mortality in these patients is closely associated with factors such as stroke occurrence, oxygenation levels, and the presence of secondary infections at the time of admission.

Risk Factors Associated with Mortality in Hospitalized COVID-19 Patients: A Case Study from Southeast Iran

Background: This study aimed to identify mortality risk factors among hospitalized COVID-19 patients in southeast Iran. Methods: This cross-sectional study used data from the COVID-19 patients admitted to Afzalipour teaching hospital in Kerman, Iran, from February 2020 to September 2021. The demographic and clinical data of 6,057 patients were analyzed using Bayesian network and logistic regression models. Results: Out of 6,057 patients, 333 (5.5%) died. The most important risk factors for COVID-19 mortality were age, gender, fever, headache, decreased level of consciousness (LoC), chronic liver disease, blood oxygen level (BOL), admission season, and length of stay (LoS). Fever, headache, and longer LoS were protective and mortality-reducing variables. Conclusion: Following model estimation results, it is recommended that older male patients with low oxygen levels and a lower LoC, as well as patients with chronic liver disease, receive additional medical care and not be discharged prematurely. Early medical interventions for high-risk patients may reduce COVID-19 mortality risk.

Hospital Outcomes Among Children With Congenital Heart Disease and Adenovirus Pneumonia.

The aim of the study is to evaluate the mortality risk factors and hospitalization outcomes of adenovirus pneumonia in pediatric patients with congenital heart disease. In this retrospective multicenter cohort study utilizing the Pediatric Health Information System database, we analyzed congenital heart disease patients with adenovirus pneumonia from January 2004 to September 2018, categorizing them into shunts, obstructive lesions, cyanotic lesions and mixing lesions. Multivariate logistic regression analysis was employed to identify mortality risk factors with 2 distinct models to mitigate collinearity issues and the Mann-Whitney U test was used to compare the hospital length of stay between survivors and nonsurvivors across these variables. Among 381 patients with a mean age of 3.2 years (range: 0-4 years), we observed an overall mortality rate of 12.1%, with the highest mortality of 15.1% noted in patients with shunts. Model 1 identified independent factors associated with increased mortality, including age 0-30 days (OR: 8.13, 95% CI: 2.57-25.67, P < 0.005), sepsis/shock (OR: 3.34, 95% CI: 1.42-7.83, P = 0.006), acute kidney failure (OR: 4.25, 95% CI: 2.05-13.43, P = 0.0005), shunts (OR: 2.95, 95% CI: 1.14-7.67, P = 0.03) and cardiac catheterization (OR: 6.04, 95% CI: 1.46-24.94, P = 0.01), and Model 2, extracorporeal membrane oxygenation (OR: 3.26, 95% CI: 1.35-7.87, P = 0.008). Nonsurvivors had a median hospital stay of 47 days compared to 15 days for survivors. The study revealed a 12.1% mortality rate in adenoviral pneumonia among children with congenital heart disease, attributed to risk factors such as neonates, sepsis, acute kidney failure, shunts, cardiac catheterization, extracorporeal membrane oxygenation use and a 3-fold longer hospital stay for nonsurvivors compared to survivors.

Infective Endocarditis in the Colombian Caribbean Region: Clinical Profile, Microbiological Insights, and Risk Factors for Mortality

Background: Infective endocarditis (IE) is defined as the infectious and inflammatory process of the heart’s internal structures. It can be caused by a broad group of bacteria and, rarely, fungi, with potentially life-threatening consequences. Objective: To profile bacterial resistance and identify mortality risk factors in IE patients. Methods: This cross-sectional study included clinically diagnosed IE patients. Sociodemographic, comorbidity, clinical, and microbiological data were recorded. Descriptive analyses, Chi-Square/Fisher’s exact tests, and Student’s t-tests examined variables in relation to IE outcomes (survival vs. mortality). Multivariate logistic regression calculated odds ratios and confidence intervals.

Longitudinal Worsening of Left and Right Sided Cardiac Function in Patients with Sickle Cell Disease

Rationale: Sickle cell disease (SCD) is a genetic hemoglobinopathy characterized by hypercoagulability and accelerated mortality. Venous thromboembolism (VTE) occurs in 11-12% of SCD patients by age of 40 and increases mortality risk two to three-fold. Transthoracic echocardiography (TTE) can identify mortality risk factors in SCD including an elevated tricuspid regurgitant jet velocity (TRV) and diastolic dysfunction. We sought to characterize longitudinal changes in left and right sided cardiac function and to identify the impact of VTE in this process. Methods: We conducted a retrospective review of 402 consecutive SCD adults, 18 years of age and older, treated at our institution between 2003 and 2021. Deep venous thrombosis and/or pulmonary embolism with confirmatory diagnostic imaging defined VTE. Demographics, clinical history, laboratory data, and diagnostic evaluation were abstracted from the medical record. For echocardiographic data, clinical echocardiograms were individually re-analyzed with extensive coding of left ventricular (LV) and right ventricular (RV) systolic and diastolic function. Data were recorded at time of first contact or prior to VTE if applicable and were repeated at five years post-VTE. In the non-VTE group, baseline and five-year longitudinal data were obtained. Results: Within our cohort, 62% were HbSS/HbSβ 0, 56% were female, and 19% had a prior VTE. Prior history of acute chest syndrome, stroke, surgical splenectomy, and avascular necrosis was more common in the VTE group (p&amp;lt;0.01 for each comparison). Mortality was significantly higher in the VTE group compared to the non-VTE group (13% v 6%, p=0.04). In our cohort, 146 patients had initial and 5-year follow-up echocardiography (107/329 of non-VTE group, 39/75 of VTE group). Longitudinally, there were significant declines in both medial e ‘ and lateral e’ velocity and the E/A ratio, reflective of worsening left ventricular diastolic dysfunction. Right-sided cardiac function also worsened with a significantly increased TRV and right ventricular base dimension in follow-up echocardiograms. Of note, the baseline TRV was higher in the VTE group compared to the non-VTE group. In the VTE group, E/A ratio significantly decreased and RV base dimension significantly increased on follow-up study. In the non-VTE group, significant increases in TRV and RV base dimension were observed along decreases in left atrial ejection fraction, medial and lateral e', and E/A ratio on follow-up study (Table 1). Conclusions: Patients with SCD and VTE have increased mortality compared to those without VTE. Echocardiography demonstrates longitudinal worsening of left ventricular diastolic and right-sided cardiac function. These changes were more pronounced in the non-VTE group, and the lack of changes in the VTE group may reflect missing data. Our findings need to be confirmed prospectively but suggest the importance of a deeper evaluation of echocardiography to better understand the progression of SCD related cardiac dysfunction and pulmonary hypertension.

Survival of Heart Transplant Patients with Chagas' Disease Under Different Antiproliferative Immunosuppressive Regimens.

Chagas' disease (CD) is an important cause of heart transplantation (HT). The main obstacle is Chagas' disease reactivation (CDR), usually associated to high doses of immunosuppressants. Previous studies have suggested an association of mycophenolate mofetil with increased CDR. However, mortality predictors are unknown. To identify mortality risk factors in heart transplant patients with CD and the impact of antiproliferative regimen on survival. Retrospective study with CD patients who underwent HT between January 2004 and September 2020, under immunosuppression protocol that prioritized azathioprine and change to mycophenolate mofetil in case of rejection. We performed univariate regression to identify mortality predictors; and compared survival, rejection and evidence of CDR between who received azathioprine, mycophenolate mofetil and those who changed from azathioprine to mycophenolate mofetil after discharge ("Change" group). A p-value < 0.05 was considered statistically significant. Eighty-five patients were included, 54.1% men, median age 49 (39-57) years, and 91.8% were given priority in waiting list. Nineteen (22.4%) used azathioprine, 37 (43.5%) mycophenolate mofetil and 29 (34.1%) switched therapy; survival was not different between groups, 2.9 (1.6-5.0) x 2.9 (1.8-4.8) x 4.2 (2.0-5.0) years, respectively; p=0.4. There was no difference in rejection (42%, 73% and 59% respectively; p=0.08) or in CDR (T. cruzi positive by endomyocardial biopsy 5% x 11% x 7%; p=0.7; benznidazole use 58% x 65% x 69%; p=0.8; positive PCR for T. cruzi 20% x 68% x 42% respectively; p=0.1) rates. This retrospective study did not show difference in survival in heart transplant patients with CD receiving different antiproliferative regimens. Mycophenolate mofetil was not associated with statistically higher rates of CDR or graft rejection in this cohort. New randomized clinical trials are necessary to address this issue.

Hospital Admission Factors Independently Affecting the Risk of Mortality of COVID-19 Patients.

COVID-19 is a disease characterized by high in-hospital mortality, which seems to be dependent on many predisposing factors. The aim of this study was to analyze the clinical symptoms, abnormalities in the results of laboratory tests, and coexisting chronic diseases that independently affected the risk of in-hospital mortality in patients with COVID-19. We analyzed the records of patients with COVID-19 who were hospitalized from 6 March 2020 to 30 November 2021. Out of the entire group of 2138 patients who were analyzed, 12.82% died during hospitalization. In-hospital mortality was independently associated with older age (OR 1.53, 95% CI 1.20-1.97); lower arterial blood oxygen saturation (OR 0.95, 95% CI 0.92-0.99); the presence of a neoplasm (OR 4.45, 95% CI 2.01-9.62), a stomach ulcer (OR 3.35, 95% CI 0.94-11.31), and dementia (OR 3.40, 95% CI 1.36-8.26); a higher score on the SOFA scale (OR 1.73, 95% CI 1.52-1.99); higher lactate dehydrogenase (LDH) (OR 1.08, 95% CI 1.05-1.12); higher N-terminal pro-brain natriuretic peptide (NT pro BNP) (OR 1.06, 95% CI 1.01-1.11); and lower total bilirubin in blood concentration (OR 0.94, 95% CI 0.90-0.99). We found that low oxygen saturation, old age, and the coexistence of cancer, gastric ulcers, and dementia syndrome were variables that independently increased mortality during hospitalization due to COVID-19. Moreover, we found that decreased platelet count and bilirubin concentration and increased levels of LDH and NT-proBNP were laboratory test results that independently indicated a higher risk of mortality. We also confirmed the usefulness of the SOFA scale in predicting treatment results. The ability to identify mortality risk factors on admission to hospital will facilitate both adjusting the intensity of treatment and the monitoring of patients infected with SARS-CoV-2.

Survival Analysis and Contributing Factors among PCR-Confirmed Adult Inpatients during the Endemic Phase of COVID-19.

In May 2023, the global health emergency status of COVID-19 concluded, marking the onset of an endemic era. This study assessed survival rates among PCR-confirmed adult inpatients during this phase and determined contributing factors. Employing a survival analysis approach, this investigation utilized a nationwide Mexican cohort encompassing 152 adult inpatients. Survival rates were computed using the Kaplan-Meier method, and a proportional Cox model identified mortality risk factors. Survival rates remained above 65% on day 14 after admission. Vaccination status, including the number of doses administered, was not significantly associated with fatal outcomes. Chronic kidney disease or a history of immunosuppression (due to any cause) increased mortality risk. Our findings underscore the persistent severity of COVID-19 beyond the global health emergency, emphasizing the necessity for tailored interventions for vulnerable patients.

ASSESSMENT OF OUTCOMES OF MALIGNANCY PATIENTS ADMITTED IN INTENSIVE CARE UNITS

Background: The decision to admit advanced cancer patients to the intensive care unit (ICU) is multifaceted, considering factors such as prognosis and quality of life. This study aimed to identify mortality risk factors in critically ill advanced cancer patients admitted to the ICU. Objectives: The objective of this study was to identify mortality risk factors in critically ill advanced cancer patients admitted to the ICU. Study Design: This was an observational study. Setting: The study was conducted at Bahria International Hospital, Lahore. Duration of Study: The study was conducted between March 2022 and March 2023. Material and Methods: A total of 65 adult cancer patients admitted non-electively to the ICU were enrolled in the study. Data on demographics, clinical conditions, and outcomes were collected. Independent medical decisions were made by ICU staff. Results: The ICU mortality rate was 34.6%, rising to 59.4% within thirty days post-discharge. Patients spent an average of 15.2 days in the ICU. Common admission reasons included respiratory failure (36.2%) and sepsis/septic shock (59.2%). Cox regression analysis revealed six significant indicators of poor outcomes: acute kidney injury (AKI), sepsis with multiple organ failure (MOF), acute respiratory distress syndrome (ARDS), uncontrolled malignancy, mechanical ventilation, and vasopressor use. Conclusion: Our findings highlight AKI, sepsis, MOF, ARDS, and uncontrolled malignancy as prognostic predictors of early death in critically ill cancer patients admitted to the ICU. Additionally, mechanical ventilation and vasopressor use are associated with increased mortality risk. These insights can aid clinicians in optimizing care strategies for this vulnerable patient population.

ASSESSMENT OF OUTCOMES OF MALIGNANCY PATIENTS ADMITTED IN INTENSIVE CARE UNITS

Background: The decision to admit advanced cancer patients to the intensive care unit (ICU) is multifaceted, considering factors such as prognosis and quality of life. Objectives: The objective of this study was to identify mortality risk factors in critically ill advanced cancer patients admitted to the ICU. Study Design: This was an observational study. Setting: The study was conducted at Bahria International Hospital, Lahore. Duration of Study: The study was conducted between March 2022 and March 2023.Material and Methods: A total of 65 adult cancer patients admitted non-electively to the ICU were enrolled in the study. Data on demographics, clinical conditions, and outcomes were collected. Independent medical decisions were collected by ICU staff. Mortality risk factors in primary Sjögren syndrome: areal-world, retrospective, cohort study.

What baseline predictors would be involved in mortality in people with primary Sjögren syndrome (SjS) remains uncertain. This study aimed to investigate the baseline characteristics collected at the time of diagnosis of SjS associated with mortality and to identify mortality risk factors for all-cause death and deaths related to systemic SjS activity measured by the ESSDAI score. In this international, real-world, retrospective, cohort study, we retrospectively collected data from 27 countries on mortality and causes of death from the Big Data Sjögren Registry. Inclusion criteria consisted of fulfilling 2002/2016 SjS classification criteria, and exclusion criteria included chronic HCV/HIV infections and associated systemic autoimmune diseases. A statistical approach based on a directed acyclic graph was used, with all-cause and Sjögren-related mortality as primary endpoints. The key determinants that defined the disease phenotype at diagnosis (glandular, systemic, and immunological) were analysed as independent variables. Between January 1st, 2014 and December 31, 2023, data from 11,372 patients with primary SjS (93.5% women, 78.4% classified as White, mean age at diagnosis of 51.1 years) included in the Registry were analysed. 876 (7.7%) deaths were recorded after a mean follow-up of 8.6 years (SD 7.12). Univariate analysis of prognostic factors for all-cause death identified eight Sjögren-related variables (ocular and oral tests, salivary biopsy, ESSDAI, ANA, anti-Ro, anti-La, and cryoglobulins). The multivariate CPH model adjusted for these variables and the epidemiological features showed that DAS-ESSDAI (high vs no high: HR=1.68; 95% CI, 1.27-2.22) and cryoglobulins (positive vs negative: HR=1.72; 95% CI, 1.22-2.42) were independent predictors of all-cause death. Of the 640 deaths with available information detailing the specific cause of death, 14% were due to systemic SjS. Univariate analysis of prognostic factors for Sjögren-cause death identified five Sjögren-related variables (oral tests, clinESSDAI, DAS-ESSDAI, ANA, and cryoglobulins). The multivariate competing risks CPH model adjusted for these variables and the epidemiological features showed that oral tests (abnormal vs normal results: HR=1.38; 95% CI, 1.01-1.87), DAS-ESSDAI (high vs no high: HR=1.55; 95% CI, 1.22-1.96) and cryoglobulins (positive vs negative: HR=1.52; 95% CI, 1.16-2) were independent predictors of SjS-related death. The key mortality risk factors at the time of SjS diagnosis were positive cryoglobulins and a high systemic activity scored using the ESSDAI, conferring a 2-times increased risk of all-cause and SjS-related death. ESSDAI measurement and cryoglobulin testing should be considered mandatory when an individual is diagnosed with SjS. Novartis.

Tracking a Fatal Disease: Lower Limb Gas Gangrene's Mortality-Related Factors

Introduction Because of its rarity compared to dry and wet gangrene of the lower limb, the gaseous form has often been considered as the most severe form of gangrenous disease. We aimed to report clinical, paraclinical, and therapeutic features, and to identify mortality risk factors of gas gangrene of the lower limb. Patients and Methods We conducted a one-center case series retrospective study over a period of 13 years from January 2006 to December 2018. Results A total of 19 patients were included: 12 men, 7 women. Mean age was 59.3 ± 11.3 years. There were 17 diabetic patients and one patient was on radio-chemotherapy for lower rectal cancer. Septic shock was initially present in 4 cases. Hyperleukocytosis was noted in 17 patients. C-reactive protein was elevated in 11 patients. All patients were put on antibiotics then operated on. A total of 8 patients died postoperatively (42%) in an average time of 9.6 days after surgery. On univariate analysis, factors associated with mortality were: Septic shock on admission (p = 0.02); local signs limited to foot (p = 0.05) or extended above the knee (p = 0.02); leukocytosis (p = 0.005); glycemia level (p = 0.02); antibiotic therapy duration (p = 0.04); antibiotic association of Penicillin G, metronidazole, and gentamicin (p = 0.02); amputation procedure solely (p = 0.04) or debridement procedure without amputation (p = 0.05); intraoperative transfusion (p = 0.006); and hospital stay (p = 0.01). Conclusion Identifying gas gangrene mortality factors is fundamental to standardize management. Our study was able to build on the small size of our series, but further prospective and large-scale studies are required.

In-hospital mortality risk stratification in children aged under 5 years with pneumonia with or without pulse oximetry: A secondary analysis of the Pneumonia REsearch Partnership to Assess WHO REcommendations (PREPARE) dataset

ObjectivesWe determined the pulse oximetry benefit in pediatric pneumonia mortality risk stratification and chest-indrawing pneumonia in-hospital mortality risk factors. MethodsWe report the characteristics and in-hospital pneumonia-related mortality of children aged 2-59 months who were included in the Pneumonia Research Partnership to Assess WHO Recommendations dataset. We developed multivariable logistic regression models of chest-indrawing pneumonia to identify mortality risk factors. ResultsAmong 285,839 children, 164,244 (57.5%) from hospital-based studies were included. Pneumonia case fatality risk (CFR) without pulse oximetry measurement was higher than with measurement (5.8%, 95% confidence interval [CI] 5.6-5.9% vs 2.1%, 95% CI 1.9-2.4%). One in five children with chest-indrawing pneumonia was hypoxemic (19.7%, 95% CI 19.0-20.4%), and the hypoxemic CFR was 10.3% (95% CI 9.1-11.5%). Other mortality risk factors were younger age (either 2-5 months [adjusted odds ratio (aOR) 9.94, 95% CI 6.67-14.84] or 6-11 months [aOR 2.67, 95% CI 1.71-4.16]), moderate malnutrition (aOR 2.41, 95% CI 1.87-3.09), and female sex (aOR 1.82, 95% CI 1.43-2.32). ConclusionChildren with a pulse oximetry measurement had a lower CFR. Many children hospitalized with chest-indrawing pneumonia were hypoxemic and one in 10 died. Young age and moderate malnutrition were risk factors for in-hospital chest-indrawing pneumonia-related mortality. Pulse oximetry should be integrated in pneumonia hospital care for children under 5 years.
