Professional values, cultural competence, and moral sensitivity of surgical nurses: Mediation analysis and structural equation modeling.

Developing a framework that illustrates causal relationships and an in-depth comprehension of contextual elements is essential for steering the development of ethical interventions to enhance nurses' ethical decision-making. To examine the relationship between cultural competence, professional nursing values, and moral sensitivities of surgical nurses with a mediation analysis and structural equation model. This study is descriptive and correlational. and research context: This study was conducted with a total of 201 surgical nurses from two university hospitals in Konya, Türkiye. Data were gathered face-to-face between June and October 2023 with the Moral Sensitivity Questionnaire (MSQ), Nurses Professional Values Scale-Revised (NPVS-R), and Nurse Cultural Competence Scale (NCCS). Ethical approval from X University Ethics Committee was obtained (Number: 2023/419). In this study, the mean scores for the surgical nurses were as follows: 89.3 ± 19.33 on the MSQ, 113.1 ± 20.74 on the NPVS-R, and 72.06 ± 18.61 on the NCCS. Nurses' cultural competence level significantly affected their professional nursing values (β = 0.192; p = .007; R2 = 0.04), and their professional nursing values, in turn, had a significant effect on their moral sensitivities (β = -0.363; p < .001; R2 = 0.16). However, it was determined that the direct effect of nurses' cultural competence level on their moral sensitivity was not statistically significant. In contrast, the indirect effect of nurses' cultural competence level on their moral sensitivity, mediated by their professional nursing values was seen to be statistically significant (B = -0.070; p = .008). This study showed that there is a significant positive relationship between the professional nursing values and cultural competence levels of surgical nurses and that as professional nursing values increase, their moral sensitivity also rises. Sensitivity with higher professional nursing values. Additionally, it was found that nurses' professional values served as a mediating factor between their levels of cultural competence and moral sensitivity. Therefore, it is crucial to enhance the cultural competence, professional nursing values, and moral sensitivity of nursing students and registered surgical nurses and to improve their reasoning and decision-making skills in ethical dilemmas.

Safety culture among operating room healthcare workers: still a long way to go. An analytical cross-sectional study from Turkey

Introduction: safety culture attitudes of health workers are still not at the desired level. Although the creation of patient safety culture is important for all health care environments, it is more vital for critical units. Objective: to determine the patient safety culture levels of those working in the operating room environment and compare them with the 2008 results of the same hospitals. Materials and Methods: an analytical cross-sectional study was conducted in 2017-2018. The Turkish version of the Hospital Survey on Patient Safety Culture was administered to nurses, anesthesia technicians, assistant physicians, and specialist physicians working in the Operating Rooms (n=258) of two university hospitals in Konya, a large city in Anatolian region of Turkey. Results: average percent positive response to the 42 items was low (41%, n=258). While there was no change in one dimension of the questionnaire compared to 2008; there was a positive change in 8 dimensions and a negative change in 3 dimensions. All 12 dimensions were lower than the Agency for Healthcare Research and Quality score. Discussion: Despite many studies, policy developments and interventions on patient safety, the improvement of a patient safety culture is very slowly in Turkey as in other countries. Conclusion: non-reporting of errors and a punitive approach in case of errors are still considered the most important problems.

Family-Centered Care Scale in Neonatal Intensive Care Unit (FCCS-NICU): Development and psychometrics evaluation

PurposeThe study was conducted to develop FCC Scale in Neonatal Intensive Care Units (FCCS-NICU) and to test its validity and reliability. MethodsThis methodological study was conducted with 484 mothers whose neonates were hospitalized in the NICU of four different hospitals in Konya, Turkey. Data were collected with the mother and neonate information form and FCCS-NICU, and analyzed with Items Analysis, Cronbach's alpha, Exploratory Factor Analysis, and Confirmatory Factor Analysis tests. ResultsA 48-item pool was created for the draft FCCS-NICU based on the family-centered care model. The Content Validity Index of the scale was found to be popular opinion 0.95. In the Exploratory Factor Analysis, a four-factor structure with 29 items emerged. These four factors explained 53.23% of the total variance. The subscales were named dignity and respect (7 items), information sharing (9 items), participation in care (5 items), and collaboration with the family (8 items). The parallel form correlation coefficient was 0.80, the test-retest correlation coefficient was 0.76 (p < .001). The Cronbach's Alpha was found 0.94 for a total scale. The Confirmatory factor analysis confirmed the four-factor structure. ConclusionsWe provide a valid and reliable scale to evaluate FCC and associated factors in NICU. FCCS-NICU can be used as a tool in descriptive research to assess FCC and identify associated factors.Practice implication.It can be used as a tool in experimental studies to develop FCC.

Albumin haemoglobin index: A novel pre-operative marker for predicting mortality and hospital stay in patients under one-year undergoing gastrointestinal surgeries.

Aim:The aim of this study was to evaluate the mortality and morbidity of infants <1 year of age with intestinal obstruction requiring surgical intervention and to investigate the factors affecting mortality and hospital length of stay in paediatric surgery, including albumin-haemoglobin index.Patients and Methods:The records of gastrointestinal paediatric surgeries in the past 10 years of patients who were <1-year-old at Baskent University Konya Hospital were obtained from the hospital and retrospectively studied. Patient characteristics, especially the relationship between albumin haemoglobin index (AHI) and hospital duration and mortality, were examined. According to the surgical areas, it also subjected this relationship to further analysed in subgroups.Results:There were 144 cases who fulfilled the inclusion criteria. Pre-operative serum AHI was analysed using receiver operating characteristics (ROC) curve analyzes. In the ROC analysis, AHI had a diagnostic value in predicting case discharge rates (area under the curve: 0.755, P = 0.001). When the cut-off point was set at 46.18, the sensitivity of the test was 57.5% and the sensitivity for predicting survival was 84%. In the logistic regression model to estimate survival, the odds ratio of AHI was 1.063 (confidence interval: 1.020–1.108, P = 0.004). In subgroup analyzes, AHI positively predicted survival in the NEC group and in the other group. In a linear regression model analysing the effect of AHI on hospital stay of length, AHI explained 10% of the variance in the hospital stay of length variable and significantly and negatively influenced the hospital length variable (β = −0.319, P = 0.05). In the linear regression model for subgroup analyzes, AHI significantly and negatively predicted hospital length of stay in the NEC and pyloric surgery groups, but positively predicted hospital length of stay in the perforation group.Conclusion:The AHI can be used as a valuable marker to predict the likelihood of discharge and length of hospital stay in paediatric surgical cases <1-year-old.

COVID-19 Pandemi Döneminde İş Stresinin İşte Kalma Niyetine Etkisinde İş Tatmininin Aracılık Etkisi(The Effect of Work Stress With Medıatıon Effect of Job Satisfaction on Intention to Stay at Work in the COVID-19 Pandemic Period)

Purpose – This study aims to determine whether the job stress experienced by healthcare workers during the Covid-19 Pandemic affects their intention to stay at work through job satisfaction and to determine which factors lead to job stress. Design/methodology/approach - The mixed analysis method, which includes both quantitative and qualitative analysis methods, was used for the study, and the questionnaire method was preferred among the primary data collection methods for quantitative analysis. For qualitative analysis, semi-standardized interview technique, one of the data collection methods, was used. The sample of the study consists of healthcare professionals in private hospitals in Konya. 633 health workers participated in the study for quantitative analysis and 13 for qualitative analysis. Findings - As a result of the quantitative research, it was determined that the job stress experienced by healthcare workers negatively affected their job satisfaction, in addition job satisfaction positively affected the intention to stay at work, and job stress negatively affected the intention to stay at work through mediation effect of job satisfaction. In qualitative analysis, 13 factors that affect work stress positively and negatively were found. While the only factor that reduces job stress by negatively affecting it is team spirit and work, the first factor that positively affects and increases it (according to the frequency of coding) is mobbing. Discussion – The health sector is one of the sectors where work stress is intense. It is an undeniable reality that especially during the pandemic period, the sector employees were affected by work stress. Increasing work stress negatively affects job satisfaction and employees want to leave their jobs due to decreased job satisfaction. However, as stated in the results of the study, although the intention of employees to remain at work is negatively affected due to the increase in job stress and decreased satisfaction, it is a reality that should not be overlooked that employees do not want to leave their jobs due to economic reasons.

The Association Between Platelet Mass Index and Mechanical Ventilation in Pediatric Surgery Cases

Platelet mass index (PMI) is an indicator of platelet functionality. This study aimed to examine the relationship between PMI and mechanical ventilation need in infants less than 1 year of age who underwent gastrointestinal system surgery. We retrospectively reviewed the medical records of cases that underwent gastrointestinal surgery in the last 10 years (2010-2020) at Baskent University Konya Hospital. Demographic data, indications for surgery, age at surgery, preoperative blood tests, hospitalization times, discharge status, mechanical ventilation need and duration, sepsis, accompanying anomalies, recurrent surgery requirements were collected from patient records. The study group (n: 143) was divided into 2 groups as the need for mechanical ventilation (MV; n: 73) and the group that did not need mechanical ventilation (n-MV; n: 70). Data were analyzed with SPSS version 25. PMI was significantly lower in the MV group (1999 Vs 2798, p&lt; .001). 65% of the cases were newborns. Mean gestational age was 34.5 ±4 weeks, and birth weight was 2100 ± 820 g. Most of the cases that need surgical intervention consist of small bowel atresia, esophageal atresia, surgeries due to necrotizing enterocolitis, and anal atresia. Ileus was present in 54% of the patients. The recurrent surgery requirement was 34%. The frequency of accompanying cardiac anomalies was 30.6%, and the rate of proven sepsis was 38.3%. Preoperative 2163 PMI value can discriminate not needing MV with 72% sensitivity and 69% specificity (AUC: .699, p&lt;.001). When the basic characteristics of the groups were evaluated, 83% of the patients who underwent MV were in the newborn group. The birth weight, gestational age, hospital duration, oxygen exposure, culture-positive sepsis rates, total parenteral nutrition (TPN) rates, and duration were found to be significantly higher in patients who underwent MV. While there was no difference in the number of white blood cells in the preoperative blood analysis, the neutrophil-lymphocyte (N-L) ratio, platelet number, albumin values were significantly lower and CRP values were significantly higher in the MV group. There was no difference in the platelet –lymphocyte (PL) ratio. High PMI values can be used as a parameter to discriminate the need for mechanical ventilation

Reliability and Validity Assessment of the Turkish Version of the Leuven Questionnaire on Patient Knowledge of Chemotherapy

Amaç: Araştırma, Leuven Kemoterapi Hasta Bilgi Ölçeği (L-KHBÖ)'nin geçerlik ve güvenirliğini değerlendirmek amacıyla gerçekleştirildi. Gereç ve Yöntem: Metodolojik tipte olan araştırmanın evrenini Konya ilindeki iki hastanenin ayaktan kemoterapi ünitelerinde kemoterapi uygulanan tüm hastalar, örneklemini ise Mart-Temmuz 2014 tarihleri arasında kemoterapi tedavisi uygulanan 239 hasta oluşturdu. Araştırmanın verileri Hasta Bilgi Formu, L-KHBÖ kullanılarak toplandı. Bulgular: L-KHBÖ’nin geçerlik-güvenirlik çalışması doğrultusunda Cronbach Alpha’sı 0.86; maddelerin toplam puan korelasyonu rs=0.25 ile 0.58 arasında; uyum indeksleri Ki-kare:151.83 (p≤0.001), Serbestlik Derecesi:100. RMSEA:0.047, SRMR:0.061, CFI:0.90, NNFI:0.88, GFI:0.93, AGFI: 0.90 olarak; test ve tekrar test korelasyonu r:0.71, p≤ 0.001 olarak bulundu. Sonuç: L-KHBÖ’nin Türkçe formunun geçerli ve güvenilir bir ölçek olduğu belirlendi.

Evaluation of parental sociocultural background and education level in response to pediatric testis torsion

Acute scrotal pain (ASP) remains one of the most common male urologic emergencies in the pediatric age group. The most concerning outcome of testicular torsion (TT) is the need for orchiectomy, which has a negative impact on the child's development in general and on sexual development and psychology in particular. Time is the most important factor in the treatment of TT. Parental awareness of ASP indicating the possibility of TT is a significant factor in children's hospital admission time. Sociocultural background may be an indicator in parental awareness. This study sought to determine if parental sociocultural and education level is associated with delayed treatment for TT. This retrospective study evaluated data for patients with scrotal or abdominal pain and TT at two hospitals in Konya, Turkey from 2012 to 2020. Study participants were the parents of the patients treated for TT. Participants were contacted by telephone and asked about their educational background. The study population was divided into 2 groups based on parent characteristics. Group 1 parents had an education level less than high school, had no health insurance, were in need of state aid, and had a low sociocultural background. Group 2 parents had an education level of at least high school or higher and had health insurance. Symptom duration (time between symptom onset and hospital admission) and surgical procedures for the patients were compared between the 2 parent groups. Of the 140 patients who received a diagnosis of TT, 77 were in Group 1 and 63 in Group 2. Mean patient age was 12.7±2.7 (5-16) years Median symptom duration was 7 (1-120) hours. Symptom duration was higher in Group 1, but no statistically significant differences were noted between groups (Group 1 duration was 8h vs. Group 2 duration of 6h; p=0.331). Orchiectomy was performed for 62 (44.3%) patients and testicular-sparing surgery for 78 (55.7%). Orchiectomy rates between groups were statistically significant and higher in Group 1 with 41 (53.2%) versus 21 (33.3%) in Group 2. Factors such as low sociocultural family background and low parental education level increase the risk of orchiectomy for their children. Awareness of the symptoms of TT may minimize the possibility of testicular loss.

Social Media Usage as A Dialogical Communication Instrument of Hospitals: A Research on Public Hospitals and Private Hospitals in Konya

Teknoloji beraberinde gelişen internet, günümüzde birçok alan gibi kurumların dijital alanda temsilini de etkilemekte ve bireyleri medyanın sosyalleşmiş haliyle yeniden yönlendirmektedir. Bireylerin her an her yerde sınırsızca ulaşabileceği bir kanal olarak sosyal medya ise internetle birlikte bireylere sağladığı dijital katılım olanağıyla, bilgiye erişimin de rehberliğini yapmaktadır. Diyalojik olarak iki yönlü iletişimsel eylem sürecine katılan birey, salt bilgiyi alan değil, aktaran konumuna da taşınmaktadır.Çalışma temelde sosyal medyanın bir diyalojik iletişim aracı olarak hastanelerde kullanımını ele almak amacıyla oluşturulmuştur. Nitel araştırma yöntemi kapsamında yer alan içerik analizi tekniği kullanılan çalışmada, Konya’da kamu ve özel hastaneler kategorisinde yer alan iki hastanenin sosyal medya hesapları 13.11.2019-13.12.2019 tarihlerinde incelenmektedir. Hastanelerin Twitter, Instagram ve Facebook paylaşımlarının gözlendiği süreçte, paylaşım konuları, sayıları, beğenileri, görseller, niteliksel olarak kategorilere ayrılmakta, sosyal medya sayfa ve arayüz farkları ortaya konmaya çalışılmaktadır. Araştırmada hastanelerin, bilgi paylaşımında, diyalojik döngüye katkıda bulundukları, enformasyon anlamında, kamuya ait bilgilendirme içeriklerini kapsayan paylaşımların olduğu, ziyaretçilerin yorum yapma konusunda herhangi bir engelle karşılaşmadıkları, paylaşımların diyalojik iletişim doğrultusunda gerçekleştiği görülürken, paylaşımlara yapılan yorumlar ve mesajlar kapsamında ise hastanelerin sosyal medya hesaplarının ziyaretçilerle sohbet ve bilgilenme süreci dolayısıyla diyalojik olarak yeterli görülmediği sonucuna ulaşılmıştır.

The Number of Patients Diagnosed with Brain Death and Organ Donation Rates at the Başkent University Konya Hospital: 6-Year Experience

Kidney transplant started at Başkent University Konya Hospital in July 2016. In this study, we compared the causes of brain death and the organ donation rates in our center between 2013 and 2016 versus between 2016 and 2019. Patient files and records were analyzed retrospectively. Age, biologic sex, cause of brain death, and organ donation rates of patients diagnosed with brain death were examined and compared. The number of patients who were diagnosed with brain death and became deceased donors at our center increased 4-fold during the period from 2016 to 2019 compared with that shown from 2013 to 2016. In addition, organ donation rates increased to 71.4%, which is much higher than the average in Turkey (24%-28%). Between 2013 and 2016, trauma was the leading cause of brain death (42.8%), whereas between 2016 and 2019 cerebral hemorrhages rose to first place with a rate of 89.3%. In 2018 and 2019, there were 199 and 62 brain deaths, respectively, reported in our organ donation coordination region with an approximate organ donation rate of 25%. In our center, 12 patients were diagnosed with brain death in 2018 and 8 of these patients (66.7%) became donors; 6 brain deaths were diagnosed in 2019, and 4 patients (66.7%) became donors. Since we started kidney transplant at our center in 2016, the number of patients diagnosed with brain death has increased significantly. Establishing a healthy communication with relatives of patients and having a dedicated organ transplant center are important reasons for our much higher organ donation rates compared with the general rate in Turkey. A healthy communication is the most effective way to establish trust with next-of-kin and the general public.

How blameless are hospitals in climate change? An example of a province in Turkey

Abstract Starting from the 1970s, the discussion about the negative effects of human activity on the world has accelerated and with a increasingly raised voice and it has been noted that the natural balance of our world was being altered. The World Health Organization has focused its policies and directives on strategies aimed on dealing with climate change (and its impact on human health), and diseases related to air pollution and implementing health-related sustainable development goals in climate friendly-hospitals. Hospitals exist to treat patients, but they also pollute the environment because hospitals consume a lot of energy and water and produce hazardous waste. These organizations need to work hard to improve their carbon footprints. The study investigated practices at 21 public hospitals in Konya, Turkey. Results show that domestic waste was on average 54.83 tons per year, medical waste was 33.59 tons per year and packing waste was 24.36 tons per year. It was determined that medical waste disposal costs on average of €26,800 per annum, and the amount of medical waste per bed was 1.15 kilograms per annum. According to 2014 medical waste data the average medical waste per bed of these hospitals in Konya province is less than the average in Turkish public hospitals, in which it is 1.18 kilograms per bed. The hospitals in our study were found to be especially inadequate at water management and did not pay much attention to green practices.

The Family-Centered Care Assessment Scale: Development and Psychometric Evaluation in a Turkish Sample

PurposeThe purpose of this study was to develop a family-centered care assessment scale for the parents of hospitalized children and to evaluate the psychometric characteristics of the new scale. MethodThis is a methodological study carried out in three phases. The study was conducted between on September 2017 and February 2018 with the parents of 360 children treated at the pediatric clinics of two medical faculty hospitals in Konya. Design/MethodsThe study was conducted with parents (n = 360). The data was collected via the Socio-demographic Information Form, the Family-Centered Care Scale (parallel form) and the draft scale developed by the researcher. Data was analyzed by construct validity index, exploratory and confirmatory factor analyses. End of the exploratory factor analysis FCCAS consisted of 21 items and three sub-dimensions. The content validity index was 0.92. The internal consistency coefficient (Cronbach's Alpha) was 0.94 for the total scale. The strong correlations was found between test and re-test (r = 0.90, p < .001). Confirmatory factor analysis has confirmed the three-factor structure. ConclusionIn this study developed family-centered care assessment scale (FCCAS) is a valid and reliable measurement tool. Practice implicationsThis scale can be used to evaluate family-centered care in pediatric clinics (excluding neonatal care units). It can be used as a measurement tool in descriptive and intervention studies examining family centered care.

SAĞLIK HİZMETLERİ ARZINDA DEFANSİF TIP VE HEKİMLERİN KARAR VERME ALGISI

Amaç: Bu çalışmada, sağlık hizmetleri arzında defansif tıp ve hekimlerin karar verme algıları arasındaki ilişkinin incelenmesi amaçlanmıştır.&#x0D; Gereç ve Yöntem: Araştırma, Konya ilinde Özel Akademi Meram Hastanesi, Özel Akademi Konya Hastanesi, Farabi Hastanesi ve Konya Göz Hastanesi’nde görev yapmakta olan hekimler üzerinde uygulanmıştır. Nicel araştırma deseni kullanılan araştırmaya 32 kadın 70 erkek olmak üzere toplam 102 hekim katılmıştır. Araştırmada örneklem grubunun oluşturulmasında kolayda örneklem yöntemi, verilerin toplanmasında ise anket tekniği kullanılmıştır. Çalışmada demografik bilgileri içeren “kişisel bilgi formu” kullanılmıştır. Hekimlerin karar verme algılarını ölçmek için Scott ve Bruce (1995) tarafından geliştirilen Taşdelen (2002) tarafından Türkçeye uyarlanan Karar Verme Stilleri Ölçeği ile birlikte hekimlerin defansif tıp tutumlarını ölçmek amacıyla Başer ve Ark. (2014) tarafından geliştirilen Defansif Tıp Tutum Ölçeği kullanılmıştır.&#x0D; Bulgular: Araştırmaya katılan hekimlerin %31,4’i kadın, %68,6’sı erkektir. Katılımcıların %39,2’si 35-44 yaş aralığındadır. Çalışılan birimlere baktığımızda hekimlerin çoğu %44,1 ile dahili birimlerde çalışmaktadır. Katılımcıların %60,8’i 11 yıl ve üstü süre hekimlik yaptığı görülmüştür. Çocuk sayısı incelendiğinde %39,2’inin 2 çocuk sahibi olduğu görülmüştür. Karar verme ile defansif tıp arasında negatif yönde düşük düzeyde anlamlı bir ilişki tespit edilmiştir. Hekimlerin karar verme ile defansif tıp tutumları arasındaki ilişki incelenmiş ve sonucunda karar verme stilleri ile defansif tıp arasında negatif yönde düşük düzeyde anlamlı bir ilişki tespit edilmiştir (r=-,229; p&lt;0,05). Defansif tıp ile karar verme puan ortalamaları ile cinsiyet ve medeni durum değişkenleri arasında bağımsız gruplarda t testi analizi yapılmış ve anlamlı farklılık görülmemiştir. Çalışmamıza katılan hekimlerden “Hekimlik mesleğiniz sürecinde malpraktis nedeni ile dava açıldı mı” sorusuna 13 hekim (%2,7) evet cevabını verirken, 89 hekim (%87,3) hayır cevabı vermiştir. Defansif tıp uygulamaları kavramının içeriği konusunda yeterli bilgiye sahip misiniz? sorusuna 56 hekim (%54,9) “hayır” cevabını vermiştir. &#x0D; Sonuç: Araştırmamızın sonuçlarına göre hekimler defansif tıp konusunda yeterli bilgiye sahip değildir ve defansif tıp davranışı gösterme eğilimleri fazladır.

Vitamin D receptor gene TaqI single nucleotide polymorphism is not associated with lead levels in maternal and umbilical cord blood

Purpose: We aimed to investigate the association of vitamin D receptor (VDR) gene TaqI single nucleotide polymorphism (SNPs) with serum lead (Pb) levels in maternal and umbilical cord blood.Materials and methods: Eighty-one patients who lived in Konya, Turkey for the last 3 years and had delivery at Başkent University Konya Hospital in 2016 were included in this study. Venous blood samples were drawn from each volunteer immediately before giving birth to determine the maternal Pb levels and VDR SNPs. Additionally, umbilical cord blood samples were collected from the umbilical vein into tube with EDTA as an anticoagulant immediately after birth to determine Pb levels of the fetus.Results: The median level of Pb in the maternal blood was 29.00 (Interquartile Range (IQR) = 16.35) μg/L and the median Pb level in the cord blood was 22.50 (IQR = 9.75) μg/L. Blood Pb level of women living in the urban area was significantly higher than in those living in the rural area (Z = 2.118; p = .034). There was a very strong positive correlation between the Pb levels in the maternal blood and in the umbilical cord blood (ρ = 0.825, p < .001, respectively). Regarding VDR SNPs, “TT”, “TC”, and “CC” VDR TaqI genotypes were observed in 28 (34.6%), 45 (55.5%), and eight samples (9.9%), respectively. Pb levels in maternal and cord blood were higher in women with the “CC” VDR TaqI genotype; however, there was no statistically significant difference (p > .05).Conclusions: Although women with the “CC” VDR TaqI genotype had higher maternal and cord blood Pb levels, this was statistically insignificant and therefore, VDR TaqI SNPs did not significantly affect maternal and umbilical cord blood Pb levels.

Although the number of surgical emergencies continues to increase, comprehensive data on emergency surgical admissions are scarce. The aim of this multicenter study was to evaluate the causes, management, and outcomes of the general surgical emergencies in the city of Konya, Turkey. The relevant details of the cases admitted and considered to be general surgical emergencies in Konya over a nine-year period (January 2003-January 2012) were analyzed. All demographic data were analyzed statistically. The study group comprised 21954 cases from 4 hospitals in Konya: 7154 from Konya Numune Hospital, 6,654 from Konya Education and Research Hospital, 6,400 from Necmettin Erbakan University Meram Medical Faculty, and 1,390 from Başkent University Konya Education and Research Hospital. Their mean age was 59.6 years, and the average hospitalization time was 3.3 days. The diagnoses of the admitted patients were as follows: acute appendicitis (59.57%), bowel obstruction (11.12%), trauma (7.97%), strangulated inguinal hernia (5.46%), acute cholecystitis (4.87%), peptic ulcer perforation (4.09%), mesenteric ischemia (2.73%), necrotizing fasciitis (2.73%), gastrointestinal system bleeding (1.79%), and others (1.1%). The findings of the study indicate a steady increase in surgical admissions to emergency units. Non-traumatic acute abdomen was the most common reason for general surgical emergencies. Although the number of elderly patients increased, the hospital stay and mortality rates decreased over the study period.

Correlation between the Fetal Liver Length and Crown-Rump Length During the First Trimester Screening

Objective: The aim of the present study was to determine the correlation between fetal liver length (FLL) at the 11+0–13+6 week of pregnancy and crown-rump length (CRL). Methods: Pregnant women (n=63) referring to the Baskent University Konya Hospital Obstetrics and Gynecology Clinic between January and March 2014 for first trimester screening were included in the study. Patients were excluded for the following reasons: two for type II DM, one for metabolic disease, and three because they refused to share their information. FLL was measured in patients at the 11+0–13+6 week of gestation and in those who had normal ultrasonographic findings. Exclusion criteria were as follows: nuchal translucency of ≥3.5 mm, antenatal first trimester combined test results of ≥1/250, reverse flow of the ductus venosus and tricuspid valve, absence of nasal bone (NB), and fetal malformation in second trimester. Fiftyseven women who fulfilled inclusion criteria were included in the study. Statistical Package for the Social Sciences version 15.0 (SPSS, Inc., Chicago, IL, USA) was used for statistical analysis. Descriptive statistics and Pearson’s correlation test was used for correlation between FLL and CRL. Results: Mean age of patients was 28.18±4.7 (19–39). Mean gravida and parity were 2.05±1.2 (1–6) and 1±0.8 (0–5), respectively. Mean gestational age was 12.4 weeks (min: 11.1 – max: 13.6), and mean FLL of the study group was 11.2±1.1 mm (8.2–13.7 mm). Mean NT was 1.1±0.3 (0.5–2.4) mm. In the present study, it was established that FLL was correlated with CRL (r (57): 0.840; p<0.001). Conclusion: This study showed that there was a significant, strong, positive correlation between FLL and CRL at the 11 th –13 th week of pregnancy. (JAREM 2015; 5: 60-3)
