Effect of early pulmonary rehabilitation therapy on the pulmonary function of patients with stroke-associated pneumonia and analysis of its effectiveness.

This study aimed to evaluate the clinical effectiveness of early pulmonary rehabilitation (PR) treatment methods for stroke-associated pneumonia (SAP). This is a prospective, randomized controlled intervention study. Eighty SAP patients admitted to the rehabilitation department of Liuzhou People's Hospital from June 2020 to December 2021 were selected and divided into an intervention group (40 cases) and a control group (40 cases) using the random number table approach. Patients in both groups received conventional treatments. Patients in the intervention group received early PR interventions, while patients in the control group received conventional rehabilitation treatment. The clinical effectiveness and pulmonary function indexes were compared. The intervention group showed shorter durations of stay in the intensive care unit, relief from symptoms, and mechanical ventilation compared to the control group following treatment (P < 0.05). The total clinical efficacy after intervention in the treatment group was 82.50%, significantly higher than the control group's rate of 47.50% (P < 0.05). After 21 days of treatment, the forced expiratory volume in one second (FEV1)/predicted value in the intervention group was 64.92 ± 8.53, while it was 52.69 ± 7.08 in the other group. The FEV1/forced vital capacity in the intervention group was 59.73 ± 7.18, compared to 50.36 ± 6.54 in the control group. The intervention group had a clinical pulmonary infection score of 2.62 ± 1.13, while the control group had a score of 4.17 ± 2.08. The intervention group showed a significant improvement in lung function, with statistically significant differences compared to the other group (P < 0.05). Moreover, the intervention group had significantly lower levels of interleukin-4 and interleukin-10 compared to the control group, with statistical significance (P < 0.05). The average National Institute of Health Stroke Scale score of the intervention group was significantly lower than that of the control group in the second and third weeks after treatment (P < 0.05). Early comprehensive pulmonary rehabilitation can significantly enhance the pulmonary function and respiratory status of SAP patients and facilitate the early recovery of brain function. This approach archives significantly better outcomes compared to conventional PR; therefore, it is worth clinical implementation.

Development and psychometric properties of the Balance in Daily Life (BDL) scale in a population of frail older people

BackgroundBalance disorders in older people cause falls, which can have serious functional and economic consequences. No existing scale relates fall risk to daily life situations. This study describes the development, psychometric properties and construct validity of the Balance in Daily Life (BDL) scale, comprising seven routine tasks including answering a phone, carrying a heavy bag, and sitting down and getting up from a chair. MethodsFrail patients aged 65 years or more were prospectively recruited from the geriatric rehabilitation department of a French university hospital. Inclusion criteria included autonomous walking over 20 m and modified Short Emergency Geriatric Assessment score 8–11. Patients with motor skills disorders and comprehension or major memory difficulties were excluded. Patients were assessed on Day 3 and Day 30 with the Balance in Daily Life scale, Timed Up and Go, one-leg stance time, sternal nudge and walking-while-talking tests. The scale was assessed for acceptability, quality, unidimensionality, internal consistency, reliability, temporal stability, responsiveness and construct validity. Results140 patients (83 ± 6 years) were recruited, of whom 139 were assessed at Day 0 and 133 at Day 30. Acceptability was satisfactory (134/139 patients completed the test), quality assessment showed a slight floor effect (6 % of patients with minimal score) and evaluation of item redundancy found no strong correlation (Spearman <0.7). Unidimensionality was verified (Loevinger H coefficient > 0.5 for all items except item 6 = 0.4728). Internal consistency was good (Cronbach alpha = 0.86). Reliability and temporal stability were excellent (ICC = 0.97 and ICC = 0.92). Responsiveness was verified by significant score change p < 0.0001 between Day 0 and Day 30 (decreased by 1 [0; 2] point), in line with other score changes. Construct validity revealed that the Balance in Daily Life scale was convergent with results of the timed up-and-go and one-leg stance time (p < 0.0001 for both) and tended to be higher for participants who had not fallen in the previous 6 months (p = 0.0528). The new questionnaire was divergent to sternal nudge tests (p = 0.0002) and not related to the walking-while-talking test (p = 0.5969). ConclusionThe Balance in Daily Life scale has good psychometric properties for this population. Its simplicity and innovative nature mean that it can be applied in institutions while being easily modifiable to domestic settings.Study registration on clinicaltrials.gov: NCT0334382.

Effects of Thermal Stimulation and Transcutaneous Electrical Nerve Stimulation on Sensory and Motor Function of Upper Extremity in Acute Stroke Survivors: A Randomized Controlled Pilot Study.

Objective Upper-limb coordination is crucial for daily activities, especially among stroke survivors who may encounter obstacles during upper-limb rehabilitation. This study aimed to investigate the effects of thermal stimulation (TS) and transcutaneous electrical nerve stimulation (TENS) on sensory and motor function during recovery in acute stroke patients. Design This is a parallel study with a randomized controlled design. The experiment was conducted in the E-Da Hospital Rehabilitation Department, Kaohsiung, Taiwan. Intervention Thirty participants were in-patients with acute stroke at the E-Da Hospital. Participants were randomly assigned tothree groups for a one-week intervention: exercise combined with TS, exercise combined with TENS, or conventional physical therapy with exercise alone. The Fugl-Meyer upper extremity scale, Brunnstrom stage, minimal current perception (MCP), and modified Ashworth scale were collected for the assessment. Results The outcomes demonstrated considerable improvement in MCP in all the groups after treatment. Specifically, the groups receiving TS and TENS showed significant improvements in the Brunnstrom stage, suggesting that both treatments improved distal motor recovery. Conclusion The results, following a one-week intervention period, suggested that both TS and TENS contributed to the improvement of motor and sensory function, with a significant impact on the Brunnstrom stage in the upper extremity, particularly in the distal region. The inclusion of TS or TENS in rehabilitation protocols improved distal motor function compared to baseline measures, suggesting these treatments as effective components in acute stroke rehabilitation.

Disease spectrum of torticollis in children and diagnostic flowchart: A retrospective, single-centre study.

To describe the disease spectrum of torticollis in Chinese children and to improve its diagnostic flowchart. A retrospective analysis was conducted at the Rehabilitation Department of Beijing Children's Hospital from 2017 to 2021. Patients were diagnosed and referred based on a diagnostic flowchart of torticollis. Detailed patient data were collected from the outpatient electronic medical record system. A total of 2047 patients met the inclusion criteria. The top five conditions were congenital muscular torticollis (CMT) (76.6%), cerebral palsy (5.1%), ocular torticollis (4.7%), brachial plexus injury (1.9%) and atlantoaxial rotary subluxation (1.3%). CMT was most common in 0-2 year olds, cerebral palsy in 3-5 year olds, and atlantoaxial rotary subluxation in 7-12 year olds. The top five referral departments were orthopaedics, ophthalmology, otolaryngology, head and neck surgery, neurology and neurosurgery. The disease spectrum of torticollis in children and the diagnostic flowchart provide important references for diagnosing torticollis, which necessitates multidisciplinary collaboration.

Ultrasound versus palpation-guided corticosteroid injection for de Quervain disease: A randomized controlled trial.

Corticosteroid injection effectively treats de Quervain disease, and due to the high prevalence of the intracompartmental septum in the first extensor compartment, ultrasound guidance improves injection accuracy. To compare the effectiveness, adverse events, and the recurrence rate between ultrasound-guided and palpation-guided injection in patients with de Quervain disease. Prospective, single-blind, randomized controlled trial. Rehabilitation department of a private teaching hospital. We enrolled 49 patients, ≥20 years of age, clinically diagnosed with de Quervain disease based on their medical history and physical examination. Patients were randomized into two groups: ultrasound-guided and palpation-guided injection. Both groups received a mixture of 10 mg triamcinolone acetonide (10 mg/1 mL) and 0.3 mL 1% lidocaine. The primary outcome measure was the Quick Disabilities of the Arm, Shoulder and Hand (QuickDASH) score at 1 week. The secondary outcome measures were visual analog scale for pain (pain VAS) score, patient satisfaction, and adverse events or complications from the interventions at 1 week, 3 months, and 6 months. Both groups showed improvement over time in QuickDASH scores and pain VAS (p < .001); however, no statistically significant differences were noted between the groups for either QuickDASH scores (p = .22) or pain VAS (p = .30). In addition, no statistically significant differences were found between the groups in terms of patient satisfaction (p = .76) and adverse events (p = .47, .33, .58) at the 1-week, 3-month, and 6-month follow-ups. Both ultrasound-guided and palpation-guided injections effectively treated de Quervain disease. During a 6-month follow-up, there were no statistically significant differences between the groups in pain relief, upper limb function, or patient satisfaction. However, the palpation-guided group showed a tendency for more recurrence and skin side effects.

Reliability and validity of a graphical computerized adaptive test Longshi scale for rapid assessment of activities of daily living in stroke survivors

Stroke survivors frequently experience difficulties in daily activities, such as bathing, feeding, and mobility. This study aimed to evaluate the reliability and validity of a computer-adaptive test-Longshi scale (CAT-LS) for assessing activities of daily living (ADL) in stroke survivors. This cross-sectional study collected data using an electronic application. The ADL function of stroke survivors in rehabilitation departments of hospitals was assessed using both the CAT-LS and BI. Correlations between the CAT-LS and Barthel index (BI) and concurrent validity were evaluated using Pearson’s correlation test and multiple linear regression. Interrater reliability was evaluated using the intraclass correlation coefficient based on a two-way random effect. The internal consistency of the CAT-LS was assessed using Cronbach’s coefficient (α) and corrected item-total correlations. Overall, 103 medical institutions in China were used in this study. In total, 7151 patients with stroke were included in this study. The CAT-LS classified patients into three ADL groups (bedridden, domestic, and community) with significantly different BI scores (P < 0.05). The CAT-LS results obtained using the decision-tree scoring model were consistent with the scores for each BI item. A strong correlation was observed between CAT-LS and BI (Pearson’s r: 0.6–0.894, P < 0.001). The CAT-LS demonstrated good internal consistency (Cronbach’s α, 0.803–0.894) and interrater reliability (ICC, 0.928–0.979). CAT-LS is time-efficient and requires < 1 min to administer. The CAT-LS is a reliable and valid tool for assessing ADL function in stroke survivors and can provide rapid and accurate assessments that reduce the burden on healthcare professionals. Community-based group rehabilitation program for stroke patients with dysphagia on quality of life, depression symptoms, and swallowing function: a randomized controlled trial

BackgroundCommunity-based exercise programs have demonstrated potential for implementation in older adults; however, it remains imperative to ascertain whether this strategy will yield comparable benefit in stroke patients with dysphagia.MethodsThis was a single blinded, randomized, matched pairs clinical trial. Sixty-four stroke patients with dysphagia were recruited from patients who had been discharged the Rehabilitation Department of the Third Affiliated Hospital of Sun Yat-sen University. A single blinded, randomized and controlled trial was conducted. Participants were randomly assigned to either the intervention group (n = 32) or the control group (n = 32). Patients in the intervention group received health education followed by swallowing function training in community public spaces for 5 days every week over an eight-week period (60 minutes per day). Patients in the control group received swallowing rehabilitation training, and booster educational information about dysphagia, as well as instructions on how to improve quality of life. Swallowing function (Functional Oral Intake Scale (FOIS) and Standardized Swallowing Assessment (SSA)), depressive symptoms (Geriatric Depression Scale-15), and quality of life (Swallowing-Quality of Life, SWAL-QOL) were assessed before and after all the treatment.ResultsBefore treatment, the two groups did not differ statistically. After the intervention, the swallowing function (SSA and FOIS) showed a significant improvement in both groups (All p < 0.001). But there was no significant difference in Functional Oral Intake Scale change between groups (P = 0.479). Compared with the control group, the intervention group had a significant improvement in depressive symptoms (P = 0.002), with a greater reduction in the number of depressed patients (13 to 6).The control group showed no significant improvements in depressive symptoms or a reduction in the number of depressed patients before and after treatment (P = 0.265, 14 to 12). The Swallowing-Quality of Life scores showed significant improvement in both the intervention and control group (P < 0.001). Specifically within Swallowing-Quality of Life sub-domains, greater changes were observed in symptoms and frequency (P < 0.001), communication (P = 0.012), and sleep (P = 0.006) for participants in the intervention group. And the cost-effectiveness of group rehabilitation surpasses that of rehabilitation training.ConclusionCommunity-based group rehabilitation program is more effective than traditional treatment in improving patients’ depressive symptoms and quality of life, as well as being more cost-effective.

Gender Disparities in Ankylosing Spondylitis: An Algerian Retrospective Analytical Cohort Study

Background: Ankylosing spondylitis (AS) is an inflammatory disease that can impair function and lower quality of life, which causes spinal bone fusion and new bone production. Several studies have been done on the differences in damage between women and men. Objectives: This study aimed to assess the differences between men and women with AS regarding clinical characteristics, structural damage, and treatment used by Algerian patients. Methods: In this study, 292 patients diagnosed with AS at the Hassani Abdelkader Hospital Rehabilitation Department of Sidi Bel Abbes Region between 2018 and 2021 were enrolled. The studied parameters were age, disease duration, age at disease onset, morning stiffness, joint involvements, laboratory data, disease activity, and treatments. All data were processed and analyzed via Excel and SPSS 20.0 (Statistical Package for the Social Sciences, IBM Corporation, Chicago, IL, August 2011). Results: We assessed 166 females and 126 males. The mean age at disease onset was 31.69 ± 10.675 vs. 30.38 ± 10.250 years, and morning stiffness duration was 26.08 ± 26.977 vs. 23.80 ± 26.529 minutes in females and males, respectively. Positive HLA-B27 was noted more in 71.9% of women and 69.6% of men. Also, the female group suffered more from their lumbar (100%) and high inflammation (83.8% elevated erythrocyte sedimentation rate (ESR) and 59.9% positive C-reactive protein (CRP)) rather than females who suffered from a long disease duration (7.28 ± 4.867 vs. 6.74 ± 3.584). Peripheral joints were primarily affected in males than females (knees involvement: 26.4% vs. 3.6% and hips involvement: 69.6% vs. 3%, respectively). In addition, high disease activity was more noted in men (52.8% vs. 41.9%, respectively). The most noted comorbidities, including uveitis, psoriasis, and diabetes, affected more females than males (40% vs. 29.9%, 4.8% vs. 2.4%, and 2.4% vs. 1.2%, respectively). Concerning the treatment, men used more sulfasalazine than women, 86.4% vs. 76%, while women used more Humira than men (88.6% vs. 74.4%, respectively). Conclusions: According to our findings, women had a higher inflammation rate and more damage in their lumbar, requiring more sulfasalazine and Humira treatment than men who suffered from their peripheral joints and higher disease activity.

Association of delivery mode and number of pregnancies with anorectal manometry data in patients with postpartum constipation

ObjectiveTo explore the association of delivery mode and the number of pregnancies with anorectal manometry data in patients with postpartum constipation.MethodsThis retrospective study included women with postpartum constipation treated at the pelvic floor rehabilitation department of Huzhou Maternity & Child Health Care Hospital between January 2018 and December 2019.ResultsAmong 127 patients included, 55 (43.3%) had one pregnancy, 72 (56.7%) had two pregnancies, 96 (75.6%) delivered spontaneously, 25 (16.7%) underwent Cesarean section, and six (4.7%) needed a Cesarean section despite spontaneous labor. The median duration of constipation was 12 months (range, 6–12). There were no differences between the two groups for any manometry parameters (all P > 0.05). The patients with a spontaneous delivery had a lower change in maximal contracting sphincter pressure compared with those with Cesarean section (14.3 (4.5–25.0) vs. 19.6 (13.4–40.0), P = 0.023). Only the delivery mode (Cesarean vs. spontaneous) independently affected the changes in contracting sphincter pressure (B = 10.32, 95%CI: 2.95–17.69, P = 0.006); age (P = 0.201), number of pregnancies (P = 0.190), and constipation duration (P = 0.161) were not associated.ConclusionThe patients with a spontaneous delivery had a lower change in maximal contracting sphincter pressure compared with those with a Cesarean section, suggesting that patients with Cesarean may retain a better “push” function during defecation.

Ergonomic risk factors and work-related musculoskeletal disorders in clinical physiotherapy.

The purpose of this study was to objectively quantify and evaluate the ergonomic risk of clinical physiotherapy practices and evaluate physiotherapists for work-related musculoskeletal disorders and pain. Twenty-nine physiotherapists in the rehabilitation department of a large-scale tertiary hospital were recruited in this study. The sampling period lasted for 2 weeks for each physiotherapist and interval sampling was adopted to avoid duplication of cases. Therapist posture during physiotherapy was captured, tracked and analyzed in real time using structured light sensors with an automated assessment program. The quantification of ergonomic risk was based on REBA (Rapid Entire Body Assessment) and the RPE (perceived physical exertion) scores of the therapists were recorded before and after treatment, respectively. Two hundred and twenty-four clinical physiotherapy cases were recorded, of which 49.6% were high risk and 33% were very high risk, with none of the cases presenting negligible risk. The positioning (p < 0.001) of physiotherapist had a considerable impact on ergonomic risk and pediatric physiotherapy presented a higher risk to physiotherapists than adults (p < 0.001). The RPE score of physiotherapist after performing physiotherapy was greater than before physiotherapy and was positively correlated with the REBA distribution. Our study creates an automatic tool to assess the ergonomic risk of physiotherapy practices and demonstrates unacceptable ergonomic risk in common practices. The high prevalence of musculoskeletal disorders and pains recommends that rehabilitation assistance devices should be optimized and standard ergonomic courses should be included in physiotherapists' training plans.

Breath-holding tests are a simple way to determine the state of the respiratory system in patients with respiratory pathology. The aim of this study was to evaluate voluntary breath-holding time in COVID-19 survivors, taking into account the socio-demographic and clinical characteristics. Materials and methods. The study involved patients aged 46 to 84 years with a complication in the form of bilateral multisegmental pneumonia after COVID-19 who were admitted for aftercare to the second stage respiratory rehabilitation department of the Central City Clinical Hospital in Kaliningrad. The subjects were divided into urban and rural residents, as well as by age and sex, presence of concomitant diseases and degree of damage to the lung tissue (based on computed tomography data). Voluntary inspiratory breath-holding time was estimated. Results. All of the patients after COVID-19 (n = 367) showed decreased voluntary inspiratory breath-holding time (mean value 16.9 ± 6.7 s). The degree of damage to the lung tissue had a significant negative impact on voluntary inspiratory breath-holding time, irrespective of the patients’ sex, age, place of residence and concomitant diseases: patients with less than 50 % lung tissue involvement (n = 164) on average held their breath significantly longer (by 17 %, p &lt; 0.001) than patients with more than 50 % lung tissue involvement (n = 203). Men (n = 125) were able to hold their breath longer (by 23 %, p &lt; 0.05) than women (n = 242), while patients from rural areas (n = 91) longer (by 26 %, p = 0.004) than urban residents (n = 276). Concomitant pathologies had a negative cumulative effect on the test results.

Effect of Auditory Integration Training on Children with Autism Spectrum Disorder and Auditory Attention Disorder

<bold>Objective</bold> To observe the effect of auditory integration training on language, social communication and behavior of Children with autism spectrum disorder (ASD) and auditory attention disorder. <bold>Methods</bold> A total of 83 ASD children with auditory attention disorder treated in the children rehabilitation department of Changsha Maternal and Child Health Hospital from January 2020 to May 2021 were randomly divided into control group and observation group, with 41 cases and 42 cases respectively. The control group received comprehensive rehabilitation training including the language training, behavior therapy, sentiment training and brain bionic electrical stimulation, once a day, five times a week, 20 times for a course of treatment in one month, with a rest interval of 3-5 days, lasting for nine months. In addition to the treatment of the control group, the observation group received auditory integration training for 30 minutes a time, once a day, 20 times for a course of treatment in one month, with a rest interval of two months, lasting for nine months. Before and after treatment, the missing number method was used to evaluate the auditory attention Gesell child development scale was used to evaluate the language and social communication skills; Psycho educational profile-3 (PEP-3) was used to evaluate the communication and behavior skills; Childhood autism rating scale (CARS) and autism behavior checklist (ABC) were used to evaluate the typical symptoms of autism. <bold>Results</bold> 1) Auditory attention: compared with that before treatment, the scores of the auditory attention of both groups significantly increased after treatment (<italic>P</italic>&lt;0.05). The observation group scored higher in the auditory attention than the control group after treatment (<italic>P</italic>&lt;0.05). 2) Language and social communication skills: compared with that before treatment, the scores of language and social communication skills in Gesell scale of both groups significantly increased after treatment (<italic>P</italic>&lt;0.05). Compared with the control group, the scores of language and social communication skills of the observation group after treatment were statistically higher (<italic>P</italic>&lt;0.05). 3) Communication and behavior skills: compared with those before treatment, the scores of communication and behavior skills of PEP-3 scale of both groups significantly increased after treatment (<italic>P</italic>&lt;0.05). The scores of communication and behavior skills of PEP-3 scale of the observation group were statistically higher than those of the control group after treatment (<italic>P</italic>&lt;0.05). 4) Typical symptoms of autism: compared with those before treatment, the scores of ABC and CARS of the observation group after treatment decreased significantly (<italic>P</italic>&lt;0.05). Compared with the control group, the ABC and CARS scores of the observation group after treatment were statistically lower (<italic>P</italic>&lt;0.05). <bold>Conclusion</bold> Auditory integration training can effectively improve the auditory attention, language, social communication, communication and behavior skills of acoustic children with auditory attention disorder, and alleviate the typical symptoms of autism.

Functional status and its related factors among stroke survivors in rehabilitation departments of hospitals in Shenzhen, China: a cross-sectional study

BackgroundMany stroke survivors have multiple chronic diseases and complications coupled with various other factors which may affect their functional status. We aimed to investigate the factors associated with poor functional status in hospitalized patients with stroke in Shenzhen, China.MethodsIn this cross-sectional study, four urban hospitals were selected using convenient sampling, and all stroke patients in these four hospitals were included using cluster sampling. The functional status of stroke survivors was evaluated using Longshi Scale. Explanatory variables (factors affecting functional status comprising age, sex, body mass index, smoking, alcohol consumption, complications, and chronic conditions) were collected. Ordinal logistic regression was used to examine which factors were associated with poor functional status.ResultsStroke survivors with poor functional status accounted for 72.14% and were categorised as the bedridden group based on Longshi scale, 21.67% of patients with moderate functional limitation were categorised as the domestic group, and 6.19% of the patients with mild functional restriction were categorised as the community group. The highest dependence scores were noted for feeding (73.39%), bowel and bladder management (69.74%) and entertainment (69.53%) among the bedridden group, and housework (74.29%) among the domestic group. In the adjusted model, the odds of poor functional status were higher among stroke patients with older age (odds ratio [OR] = 2.39, 95% CI: 1.55–3.80), female sex (OR = 1.73, 95% CI: 1.08–2.77), duration of stroke more than 12 months (OR = 1.94, 95% CI: 1.28–2.95), with pulmonary infection (OR = 10.91, 95% CI: 5.81–20.50), and with deep venous thrombosis (OR = 3.00, 95% CI: 1.28–7.04).ConclusionsOlder adults (age ≥ 60 years) and women were more likely to exhibit poor functional status post-stroke. Pulmonary infection and deep venous thrombosis were related to an increased risk of being dependent on activities of daily living. Therefore, clinical and rehabilitation interventions aimed at preventing or treating these common complications should be addressed to deal with subsequent dysfunction post-stroke. Since all data were obtained in metropolitan areas where the economy is well developed, future studies should be conducted in rural areas and economically less developed cities.

Within the study, we analyzed and evaluated the process of implementing a patient-oriented approach to nursing in the day hospital Rehabilitation Department of the Republic Clinical Oncology Dispensary named after Professor M.Z. Sigal; studied the nursing staff opinions on the role of a nurse in the implementation of a patient-oriented approach in providing medical care to patients; investigated the satisfaction of patients with the nursing care organization from the standpoint of a patient-oriented approach; made recommendations for the development of a patient-oriented approach to providing nursing care to cancer patients in the process of rehabilitation. As criteria for assessing the implementation of a patient-centered approach, we used the results of sociological studies carried out in 1993 by the Picker Institute &amp; Harvard Medical School, which formed the basis of the international concept of patient-oriented approach. The opinion of the nursing staff about the role of a nurse in the implementation of a patient-oriented approach to providing medical care to patients was studied through interviews. Patient satisfaction with the organization of nursing care from the standpoint of a patient-centered approach was assessed using a specially designed questionnaire.

Comparison of interface pressures in two dynamic pressure ulcer prevention supports (NIMBUS 3 and SUMMIT): A randomized controlled trial

BackgroundPressure ulcers are a risk for bedridden patients and various supports exist to prevent them. The Pressure Relief Index (PRI) evaluates pressure relief of dynamic mattresses over time. This study compared the PRI of the SUMMIT mattress (AKS-France) and the NIMBUS 3 (HNE Medical). MethodsIn this non-blinded, randomized, crossover, non-inferiority study, patients aged ≥60 with a BMI of 16–35 kg/m2, predominantly confined to bed, able to walk with aid and with pelvic symmetry were recruited from a hospital rehabilitation department from March–April 2012. Exclusion criteria included past or present pressure ulcers, inability to remain supine and deep vein thrombosis. Peak pressures of the sacrum were recorded at 0.1 Hz during a single complete 10-min inflating cycle on both mattresses, with the order determined via electronic randomization allocation. ResultsThirty-one subjects were included and randomized; with 14 finally analyzed in the SUMMIT-NIBMUS 3 order group and 16 in the NIMBUS 3-SUMMIT group. The difference in PRI <30 mmHg between the two mattresses was 13.2% [0.3–26.1] (p < 0.05), allowing a non-inferiority - superiority switch. The SUMMIT mattress demonstrated a significantly higher percentage of time <30 mmHg (p = 0.0454). No significant difference in mean minimal pressure was seen (p = 0.3231) and mean maximal pressure was in favor of SUMMIT mattress (p = 0.0096). BMI did not affect pressure profile. There were no adverse events. ConclusionsEvaluated by the PRI, the SUMMIT mattress had a better interface pressure profile than the NIMBUS 3 in older patients. The PRI is a promising tool for clinical decision-making and research, warranting validation.

Effect of acupuncture combined with rehabilitation on cognitive and motor functions in poststroke patients.

The aim of the study was to explore the effect of acupuncture combined with rehabilitation on cognitive and motor functions in poststroke patients. All patients were divided into Group A and Group B based on different interventions (Group A: acupuncture + conventional rehabilitation, Group B: conventional rehabilitation alone). Acupuncture was conducted once a day, five times a week for 8 weeks, and rehabilitation (including physical therapy and occupational therapy) was conducted for 2 hr per session, once a day, five times a week for 8 weeks. Mini-mental State Examination (MMSE) and Fugl-Meyer Assessment (FMA) were used to assess the motor and cognitive functions at baseline and the end of 8 weeks. After the intervention, FMA and MMSE scores were improved significantly in the two groups (p <.05), compared with the scores prior to intervention. After 8 weeks of intervention, a statistically significant difference in the FMA and MMSE scores was observed between the Group A and the Group B. The results suggested that the combined intervention is more effective than the conventional rehabilitation alone in improving cognitive and motor functions in poststroke patients.

Effects of Upper Extremity Coordination Exercises Based on Fatigue Prediction on Upper Extremity Sensory-motor Functions in Chronic Stroke Survivors

Objectives: This study aimed to investigate the effect of upper extremity coordination exercises based on fatigue prediction on fine and gross manual dexterity, upper limb motor function, shoulder and elbow proprioception, occupational performance, and activities of daily living in chronic stroke survivors. Methods: In this pilot double-blind randomized clinical trial, 24 chronic strokes were enrolled using the non-probability sampling method. Participants were randomly allocated to the control (received routine occupational therapy) and intervention (received upper extremity coordination exercises based on fatigue prediction using the Kinect) groups. Before and after the interventions (six weeks, three sessions/week, 45 min/session), as well as six weeks after completion of interventions, participants were assessed using the following tools: Box-Block Test, Purdue-Pegboard Test, Wolf-Motor Function Test, Action Research Arm Test, Manual Ability Measure-16, Grip Dynamometer, Shoulder Position Sense Test (SPST), Elbow Position Sense Test (EPST), Canadian Occupational Performance Measure, Shah-Barthel Index (SH-BI), and Multi-dimensional Fatigue Inventory. This project was carried out in the rehabilitation department of Shafa-Yahyaeian Hospital from May 2019 to June 2020. Results: The results showed a significant improvement in all assessments of both control and intervention groups (P≤0.05), except for SH-BI. Further, the improvement of EPST and SPST scores in the intervention group was significantly greater than the control group (P≤0.05). Also, a significant decrease in fatigue severity was observed in both control and intervention groups (P≤0.05). Conclusion: These results suggest that upper extremity coordination exercises based on fatigue prediction in combination with routine occupational therapy could lead to improvement of upper extremity sensory-motor functions and power grip and caused a decrease in fatigue severity in chronic stroke.

Short term outcomes of children with abusive head trauma two years post injury: Aretrospective study.

Abusive head trauma (AHT) can have debilitating sequelae for children who survive. A retrospective medical record review was used to describe short-term developmental outcomes of children with AHT and identify predictors of poorer outcomes. Children with AHT who received follow up by the hospital's rehabilitation department for 12 to 24 months post-injury were included in this review. Data for 85 children were collected on hearing, vision, gross motor, fine motor, speech and language, cognition, play, adaptive functioning, behaviour and personal-social skills. Global assessment found 42% of children had a good recovery, 34% had a moderate disability and 24% had a severe disability. For whom there was data, more than half had abnormal cognition, behaviour and personal-social skills, whilst more than a third had abnormal speech and language, neurological signs on last assessment, vision, play skills, and gross and fine motor skills. Factors that predicted poorer prognosis across all developmental domains included paediatric intensive care unit admission, longer length of hospital stay, breathing difficulty and lower Glasgow Coma Scale on presentation. This study highlights the substantial number of children who have abnormal development in the short-term post-AHT and assists in identifying those who require extensive long-term follow up.

Availability of vocational rehabilitation services for people with acquired brain injury in Dutch rehabilitation institutions

ABSTRACT Objective To provide an overview of the availability of and process involved in vocational rehabilitation (VR) services for people with acquired brain injury (ABI) provided in Dutch rehabilitation institutions. Methods A survey on the availability and characteristics of the VR intervention process for people with ABI was conducted in all Dutch rehabilitation centers and rehabilitation departments of general and academic hospitals (n = 120). Different domains of the VR intervention process were included in the questionnaire, the number (and percentage) of respondents giving a certain answer within a domain was reported. Results Fifty-five institutions replied (response rate 46%). VR services for ABI were provided in 34 (62%) of these, of which 20 (59%) used a VR protocol. The main similarities found among different organizations providing VR services were the involvement of different disciplines, determination of the gap between job requirements and the patient’s abilities, goal-setting for VR, involvement of external partners, timing, and source of funding. The main differences found concerned start and stop criteria, and coordination of the VR process. Conclusions Currently, at least 30% of the Dutch rehabilitation institutions offer VR services for people with ABI; there is variation in processes.

Correlation between three assessment pain tools in subacromial pain syndrome

Objective: To compare the correlation of Visual Analog Scale with pain subsections of Shoulder Pain and Disability Index and Constant-Murley Score in subacromial pain syndrome patients. Design: Single cross-sectional analysis. Setting: Hospital Rehabilitation Department. Methods: The assessment tools were applied at baseline. Correlations between Visual Analog Scale, Shoulder Pain and Disability Index and Constant-Murley Score pain subsections were assessed by Pearson correlation coefficient. Linear regression models were calculated between scales. Statistical significance was set at two-sided p < 0.05. Results: Forty-three patients were included. Pearson’s correlation between assessments was for Visual Analog Scale-Shoulder Pain Disability Index-pain (r = 0.61, p < 0.001) and for Visual Analog Scale-Constant Murley Score-pain were (r = −0.74, p < 0.001). Visual Analog Scale-Shoulder Pain and Disability Index-pain determination coefficient was r2 = 0.37 and r2 = 0.54 for Visual Analog Scale-Constant-Murley Score-pain. Conclusions: Visual Analog Scale showed better correlation with Constant Murley Score-pain than with Shoulder Pain and Disability Index-pain in subacromial pain syndrome patients.
