Evaluation of hospital information system acceptance from the point of view of users of teaching hospitals of Ahvaz Jundishapur University of Medical Sciences according to the information technology usage model

Introduction: The hospital information system is an electronic tool that collects, classifies, stores and retrieves the financial, administrative and clinical information of patients using computer capabilities and provides them to decision makers at any place and time. The present study was conducted with the aim of evaluating the acceptance of the hospital information system from the point of view of the users of teaching hospitals of Ahvaz University of Medical Sciences. Method: The present study is descriptive observational. The samples included 270 users of teaching hospitals in Ahvaz. Sampling was done randomly and samples were available. The data collection tool was a standardized questionnaire.An average of less than 2.5 indicates a low acceptance level, an average of 2.5 to less than 3.75 is considered an average acceptance level, and an average greater than 3.75 to 5 indicates a favorable level of HIS acceptance by users Results: The average variables of the questionnaire in terms of task support dimension, user interface, support for interdisciplinary cooperation and support service dimension are 4.42, 4.05, 4.64, 4.25, respectively, which indicates the level of acceptable acceptance. And in the dimensions of adaptability to tasks, learning ability, and accessibility, respectively, 3.73, 3.07, and 3.59, which indicate the average level of acceptance Conclusion: Officials can increase user satisfaction by providing appropriate platforms in terms of accessibility and learning capabilities, and also by improving hospital information systems, they can significantly save time and increase the quality of services provided in the health care sector

A Case Study on Acceptance of Hospital Information Systems (HIS), among Nurses in a Tertiary Care Hospital

Purpose: To improve the quality of care, information technology must be implemented in the healthcare field. In any healthcare system, nurses are the primary users of the hospital information system (HIS), making them the Centre of care. Therefore, understanding their views and the way they use these systems will help improve hospital information systems. Methodology: Data were collected from 389 nurses in hospital wards, intensive care units, and emergency departments using a structured questionnaire. There were two sections of questions on the questionnaire. In the first section demographic details were included. In the second session, 15 statements about nurse’s acceptability of the hospital information system in the hospital are also included. It was suggested to include one more open-ended question in the study to get feedback from the nurses about ways to increase HIS acceptance. Finding/Result: The study found that the majority were female nurses with a bachelor degree and 3-4 years of computer experience. The majority of nurses accepted that the HIS is integrated into their daily work. As a result of the study, the nursing staff will be more likely to accept the HIS. Originality/value: This study will provide a comprehensive overview of understanding information technology and the acceptance of using these systems, which will reduce the workload of nurses while improving the quality of care and patient satisfaction. Analysis Of Factors Affecting The Acceptance Of Hospital Information System Using Utaut Method In Budi Kemuliaan Hospital Batam Year 2021

The acceptance of the hospital information system using the UTAUT method at the Budi Kemuliaan Hospital Batam in 2021 is still a complaint and the computer-based SIMRS is not running optimally. The acceptance of the hospital information system using the UTAUT method is related to individual characteristics, performance expectations, business expectations, social and facilities. This study aims to determine the effect of receiving hospital information systems with the UTAUT method at Budi Kemuliaan Hospital Batam. This type of quantitative research with correlational techniques. The population in this study were users of the SIMRS application at Budi Kemuliaan Hospital Batam and a sample of 120 people. The collection of data with primary and secondary data and the data were analyzed by Multiple Regression test. The results showed a positive and significant influence between Hospital Information System Acceptance (individual characteristics, performance expectations/performance expectations, business expectations/business expectations, social influences/social influences, and facilitating conditions/facilitating conditions) on usage behavior/behavior. implementing SIMRS with the UTAUT method at Budi Kemuliaan Hospital Batam and it is recommended that the implementation of SIMRS then need to pay attention to the ease of users in using SIMRS and developers from SIMRS and the hospital need to work together in developing to create a SIMRS with business processes and needs the one in the hospital.

MODEL OF HOSPITAL INFORMATION SYSTEM ACCEPTANCE IN HOSPITALS OWNED BY THE SOCIAL SECURITY ORGANIZATION

Purpose: The purpose of this study is to provide a model of hospital information system acceptance owned by the Social Security Organization.
 Methodology: This study was performed Cross-sectional – applied and descriptively and analytically. This study had two stages. The study population was in the first stage (qualitative), information technology experts in the Social Security Organization (6 people) and the second stage (quantitative), users of the hospital information system in medical centres owned by the Social Security Organization (375 people). The first stage of this study was based on interviews with IT experts. The second stage of the study was based on the results of interviews with experts using the design of a researcher-made questionnaire. The validity of the questionnaire was measured by using the opinion of experts in the field of information technology and health information technology and by calculating the CVR index. Cronbach's alpha coefficient (0.941) was used to assess the reliability of the questionnaire. The questionnaire consisted of 12 axis and 46 questions with a five-point Likert scale with a score of 5-1. The interviews were analyzed qualitatively (content analysis) and the questionnaires were analyzed descriptively, analytically (correlation) and structural equation method is done with the help of SPSS software.
 Main Findings: Evaluation of direct relationships between model factors and hypotheses showed that the relationships between habit with intent to use, intention to use with actual use, increase motivation to use, increase management support, meet user expectations in achieving the expected performance of the system, value the cost to the system and the social impact of co-workers significantly increases the intention to use the system. Also, the coefficient of determination R2 for the target variables, which is the intention of use and actual use, were equal to 0.774 and 0.448, respectively. The goodness of fit (GoF) was equal to 0.671. The study of the effect of demographic factors on the target variables (intention to use and actual use) showed that none of the demographic variables has a significant effect on the intention to use and actual use. By considering these factors, the managers of the Social Security Organization can increase the rate of HIS admission in the hospitals and clinics of this organization.
 Application of Study: One of the technologies used in the healthcare sector is the hospital information system. It is less developed than other information systems that use information technology and apply quality standards for customer satisfaction. The IT acceptance model is one of the most valid models available that examines the acceptance factors of technology at different levels, including the individual level.
 Novelty/Originality: The novelty of this research is giving a model of clinic data framework acknowledgement claimed by the Social Security Organization.

Hospital information system acceptance among the educational hospitals

Context: Acceptance of individuals and the use of technology by users are an important factor. Aims: This study is aimed to investigate the acceptance of the hospital information system (HIS) among the educational hospitals. Setting and Design: Educational hospitals of Mazandaran University of Medical Sciences, cross-sectional study. Materials and Methods: This study conducted in 2018. Census sampling method was used, and the sample of this study was 400 nurses, physicians, and paramedical staff. In this study, the unified theory of the acceptance and use of technology (UTAUT) questionnaire was used for data collection. Statistical Analysis Used: The present study has been analyzed using structural equation modeling. Results: The results demonstrated that the behavioral intention (BI) to use HIS was predicted by performance expectancy (β = 2.08, P Conclusions: The results of this study showed that the acceptance of HIS, directly and significantly influenced by the UTAUT model.

Factors Affecting Acceptance of Hospital Information Systems in Public Hospitals of Zahedan University of Medical Sciences: A Cross-Sectional Study.

A hospital information system is used to support a wide range of operations and activities in the hospital. This study was conducted to determine the factors affecting hospital information system acceptance by users.A cross-sectional, descriptive, analytic study was performed in 2018. The study population included 550 users of the system. The data were collected using a questionnaire and analyzed using the SPSS software.A significant moderate positive correlation was found between hospital information system acceptance and perceived usefulness (r = 0.54, P < 0.01), perceived ease of use (r = 0.41, P < 0.01), human factors (r = 0.46, P < 0.01) and technological factors (r = 0.54, P < 0.01). A significant weak positive correlation was detected between the acceptance of the hospital information system and organizational factors (r = 0.35, P < 0.01).Perceived usefulness of the system, social influence, system quality, perceived ease of use of the system, and top managers’ supports had the most substantial influence on the users’ intention to accept a hospital information system. User education, preparation of guidelines suited to the user specialty or department, incorporating users’ work needs into the capabilities of the hospital information system, and improving the system to an ideal level are important considerations.

Designing a model of hospital information system acceptance: Organizational culture approach.

Background: The significance and influence of organizational culture on Information Technology acceptance, especially in healthcare field, has been recognized as a source of organizational inertia. This study aimed at developing a model of Hospital Information System (HIS) acceptance for non-teaching hospitals of Iran University of Medical Sciences to encourage the authorities to promote organizational culture and successful application of HIS. Methods: The proposed model was developed according to Michigan Organizational Assessment Questionnaire (MOAQ), Harrison, Hofstede models, and Comparative Values Framework (CVF). The questionnaires were designed based on the model and distributed among 400 HIS users in the hospitals under study, who were selected using stratified random sampling. The structural equation modeling method was used for data analysis in LISREL software. Results: According to the final model, the influences of developmental culture on perceived usefulness, the relationship of 4 types of organizational culture with mandatoriness according to CVF, and the relationships of hierarchical and developmental culture with system use were attested. The relationships between supervision and 4 variables of HIS acceptance were confirmed. Furthermore, the influence of process/ result oriented culture on user satisfaction was demonstrated. The normed chi square index (2.60) revealed that the final model was fitted to the data. The indices were as follow: GFI= 0.95, CFI= 0.97, AGFI= 0.88, RMSEA= 0.064. Conclusion: The components and structural relationships in the model of this study are applicable in the related hospitals, and using this model can promote organizational culture and acceptance of HIS by the users.

Understanding the Impact of the Cultural Factors on Hospital Information System Acceptance: A Literature Review and Proposal of a New Model

The behavioural intention to accept information system technologies remains an interesting research field for information system management specialists and researchers. However, the cultural dimension has rarely been treated by researchers in the healthcare context. This paper focuses on identifying the impact of national culture on users’ intention to accept the Hospital Information System in public healthcare establishments in Morocco. Thus, we propose a research model which is based on a strong literature review, taking into consideration the previous studies’ recommendations and limitations. The proposed model was developed through the extension of the Technology Acceptance Model (TAM) (Davis, 1989), by Hofstede’s cultural dimensions.

Modeling the Acceptance of Hospital Information Systems among Nurses: An Extended Technology Acceptance Model

Introduction: The perceived usefulness and perceived ease of use have been considered as the main factors affecting the acceptance of the new technologies since last few decades. However, it appears that these only two factors cannot describe the users’ behavior in the environments like the Hospital Information System. From the technology acceptance standpoint at the individual acceptance level, the present paper tends to develop a Technology Acceptance Model with introducing some external factors.Material and Methods: This study was conducted in 2017. The research population was included 185 nurses who works in Health Information Management (HIM) departments of Tehran University of Medical Sciences. A questionnaire was developed in order to gather the required data. The validity was obtained by panel of experts and the reliability was examined and then confirmed in a 50 person sample using the Cronbach’s Alpha (a=0.93). The Likert’s five item scale was applied. The data were analyzed using descriptive statistics, exploratory factor analysis, and path analysis.Results: The behavioral intention was affected significantly and positively by the factors of perceived usefulness, perceived ease of use, self-efficacy, end user support, social norm, trust, job relevance, and training, with trust having the highest level of effects. Also perceived ease of use had a significant effect on perceived usefulness along with an indirect effect on behavioral intention through perceived usefulness. The factors of anxiety, voluntariness, and facilitating conditions showed no significant effects on behavioral intention.Conclusion: The factors of trust, perceived usefulness, social norm, end user support, and self-efficacy have an impact on the behavioral intention of the users utilizing the Hospital Information System in the concerning hospitals. These factors could explain 72% of the changes of behavioral intention. Concentrating on them would lead to the improvement of the acceptance and Hospital Information System efficiency.

Factors Affecting Acceptance of Hospital Information Systems Based on Extended Technology Acceptance Model: A Case Study in Three Paraclinical Departments.

Regardless of the acceptance of users, information and communication systems can be considered as a health intervention designed to improve the care delivered to patients. This study aimed to determine the adoption and use of the extended Technology Acceptance Model (TAM2) by the users of hospital information system (HIS) in paraclinical departments including laboratory, radiology, and nutrition and to investigate the key factors of adoption and use of these systems. A standard questionnaire was used to collect the data from nearly 253 users of these systems in paraclinical departments of eight university hospitals in two different cities of Iran. A total of 202 questionnaires including valid responses were used in this study (105 in Urmia and 97 in Khorramabad). The data were processed using LISREL and SPSS software and statistical analysis technique was based on the structural equation modeling (SEM). It was found that the original TAM constructs had a significant impact on the staffs' behavioral intention to adopt HIS in paraclinical departments. The results of this study indicated that cognitive instrumental processes (job relevance, output quality, result demonstrability, and perceived ease of use), except for result demonstrability, were significant predictors of intention to use, whereas the result revealed no significant relationship between social influence processes (subjective norm, voluntariness, and image) and the users' behavioral intention to use the system. The results confirmed that several factors in the TAM2 that were important in previous studies were not significant in paraclinical departments and in government-owned hospitals. The users' behavior factors are essential for successful usage of the system and should be considered. It provides valuable information for hospital system providers and policy makers in understanding the adoption challenges as well as practical guidance for the successful implementation of information systems in paraclinical departments.

Hospital information system institutionalization processes in indonesian public, government-owned and privately owned hospitals

The Hospital Information System (HIS) could help hospitals as a public entity to provide optimal health services. One of the main challenges of HIS implementation is an institutional change. Using institutional theory as the analytical lens, this study aims to explain the institutionalization of HIS as an instance of e-health initiatives in Indonesia. Furthermore, this paper aims for hospital management and researchers to improve the understanding of the social forces that influence hospital personnel's HIS acceptance within an organizational context. We use case studies from four public, government-owned hospitals and four privately owned (public and specialty) hospitals to explain the HIS institutionalization process by exploring the three concepts of institutional theory: institutional isomorphism, institutional logic, and institutional entrepreneurship. This study reveals that differences exist between public, government-owned and private hospitals with regard to the institutionalization process: public, government-owned hospitals’ management is more motivated to implement HIS to comply with the regulations, while private hospitals’ management views HIS as an urgent requirement that must be achieved. The study findings also reveal that various institutional isomorphism mechanisms and forms of institutional logic emerge during the process. Finally, three factors—self-efficacy, social influence, and management support—have a significant influence on the individual acceptance of HIS.

The Impact of National Cultural Differences on Nurses' Acceptance of Hospital Information Systems.

This study aims to explore the influence of national cultural differences on nurses' perceptions of their acceptance of hospital information systems. This study uses the perspective of Technology Acceptance Model; national cultural differences in terms of masculinity/femininity, individualism/collectivism, power distance, and uncertainty avoidance are incorporated into the Technology Acceptance Model as moderators, whereas time orientation is a control variable on hospital information system acceptance. A quantitative research design was used in this study; 261 participants, US and Taiwan RNs, all had hospital information system experience. Data were collected from November 2013 to February 2014 and analyzed using a t test to compare the coefficients for each moderator. The results show that individualism/collectivism, power distance, and uncertainty avoidance all exhibit significant difference on hospital information system acceptance; however, both masculinity/femininity and time orientation factors did not show significance. This study verifies that national cultural differences have significant influence on nurses' behavioral intention to use hospital information systems. Therefore, hospital information system providers should emphasize the way in which to integrate different technological functions to meet the needs of nurses from various cultural backgrounds.

The Role of Organizational Culture According to Hofstede Model on Information Technology Acceptance in Hospital Environment

The effect of information systems on organizational performance is dependent on the users’ acceptance. Some researchers suppose that organizational culture can be one of the involving factors in successful implementation of information systems. The purpose of this study was to examine the role of Hofstede organizational culture on the acceptance of hospital information system by its users in non-teaching hospitals of Iran University of Medical Sciences. This research was applicable and was done by using descriptive and analytical methods. The conceptual model was proposed according to the related literature. The questionnaire was developed based on the model variables; then it was validated by expert panel and content validity ratio and its reliability was tested by calculating Cronbach’s alpha. The questionnaires were delivered among 400 users of HIS in the studied hospitals. To test the model fitness, structural equation modelling method was used by applying LISREL software. The normed chi-square index (2.01) showed that the research model has fitness with the gathered data. The indices included: GFI = 0.99, CFI = 0.99, AGFI = 0.96, RMSEA = 0.051. According to the research model, the relationship between two aspects of Hofstede organizational culture (process/results-oriented and open/closed systems) and HIS acceptance variables was statistically significant. The results showed that process/results-oriented culture was related to user satisfaction, and also open/closed system was correlated with three variables, including perceived ease of use, subjective norms and user satisfaction.

Factors influencing nurses' acceptance of hospital information systems in Iran: application of the Unified Theory of Acceptance and Use of Technology.

User acceptance is a precondition for successful implementation of hospital information systems (HISs). Increasing investment in information technology by healthcare organisations internationally has made user acceptance an important issue in technology implementation and management. Despite the increased focus on hospital information systems, there continues to be user resistance. The present study aimed to investigate the factors affecting hospital information systems nurse-user acceptance of HISs, based on the Unified Theory of Acceptance and Use of Technology (UTAUT), in the Shiraz University of Medical Sciences teaching hospitals. A descriptive-analytical research design was employed to study nurses' adoption and use of HISs. Data collection was undertaken using a cross-sectional survey of nurses (n=303). The research model was examined using the LISREL path confirmatory modeling. The results demonstrated that the nurses' behavioural intention (BI) to use hospital information systems was predicted by Performance Expectancy (PE) (β= 2.34, p<0.01), Effort Expectancy (EE) (β= 2.21, p<0.01), Social Influence (SI) (β= 2.63, p<0.01) and Facilitating Conditions (FC) (β= 2.84, p<0.01). The effects of these antecedents of BI explained 72.8% of the variance in nurses' intention to use hospital information systems (R2 = 0.728). Application of the research model suggested that nurses' acceptance of HISs was influenced by performance expectancy, effort expectancy, social influence and facilitating conditions, with performance expectancy having the strongest effect on user intention.

An investigation on physicians’ acceptance of hospital information systems: A case study

PurposeInformation technology is used to support a wide range of highly specified healthcare tasks and services. There is, therefore, a need to understand the factors affecting the acceptance of this technology by healthcare professionals. Physicians are key providers of healthcare services and are among the principal users of hospital information systems. Their acceptance of hospital information systems is hence of great significance when evaluating the success of those systems. MethodThe survey methodology was employed to targeted physicians in the selected case hospital for investigating factors affecting physicians’ acceptance of hospital information systems. A total of 202 questionnaires were sent out, with 124 completed copies returned, indicating a valid response rate of 61.4%. We used structural equation modeling to analyze the data. ResultsThe results indicated that top management support (γ=0.431, p<0.001) had a significant impact on perceived usefulness. Project team competency (γ=0.381, p<0.001) and system quality (γ=0.369, p<0.001) had a significant impact on physicians’ perceived ease of use of hospital information systems. Physicians’ perceptions of the usefulness (β=0.132, p<0.05, R2=0.296) and ease of use (β=0.952, p<0.001, R2=0.784) of hospital information systems had a significant impact on the acceptance of the systems, accounting for 81.4% of total explained variance. ConclusionsThrough the understanding of the identified critical factors affecting physicians’ HIS acceptance, the planners and managers should ensure that hospital information systems to be introduced into a hospital are useful and ease to use. Effort should be focuses on providing sufficient top management support, selecting qualified project team members, and delivering higher system quality in addressing physicians’ clinical needs. Thus, our research results can help planners and managers understand key considerations affecting HIS development and use, and may be used as a reference for system design, development and implementation.

Factors Determining Nurse Acceptance of Hospital Information Systems

This study proposed an evaluation model, derived from the Technology Acceptance Model and Information System Success Model, to explore factors influencing the acceptance of hospital information systems by nurses. Although many healthcare institutions have applied hospital information systems, relatively few studies have investigated the perceptions of nurses regarding the usefulness, ease of use, or acceptance of these systems. This study recruited the nursing staff of a regional hospital in Taiwan. A total of 297 questionnaires were sent, and 277 were returned, for a response rate of 93.3%. The results indicated that system quality, information quality, and service quality were positively correlated with the perceived ease of use (R=0.69) and perceived usefulness (R=0.72). Information quality has the greatest influence on perceived usefulness (γ3=0.57, P<.001) and ease of use (γ4=0.61, P<.001). Perceived usefulness and ease of use have a significant influence on system acceptance (R=0.75). Perceived usefulness (β1=0.61, P<.001) has a significant influence on system acceptance. These findings indicated that nursing care requires high-quality healthcare information to support the daily activities of nursing professionals. The results of this study also provide a valuable reference for hospital administrators in developing hospital information systems.

An Empirical Study of Physicians' Acceptance of Hospital Information Systems in Taiwan

This study investigates the key factors affecting the acceptance of hospital information systems (HIS) by physicians based on an extensive technology acceptance model. This model consists of six constructs: System quality, information quality, service quality, perceived usefulness, perceived ease of use, and HIS acceptance. Survey research targeted physicians in the selected case hospital as subjects. Of 131 questionnaires sent out, 81 completed forms were returned (a valid response rate of 64.8%). The partial least squares approach was used to analyze the collected data. This study shows that system quality and information quality are significant factors influencing perceived ease of use of HIS, while information quality and service quality are key factors affecting perceived usefulness of HIS. Results indicate that perceived usefulness and perceived ease of use significantly affect HIS acceptance (R(2)=0.545) from the physician's perspective. The findings of this study can help managers understand the key considerations affecting HIS development and use and provide important reference material for system improvement.

A Study of Factors Affecting Acceptance of Hospital Information Systems

Hospital information systems (HISs) are widely used in Taiwan, and HIS performance must be carefully evaluated. Nursing personnel are the largest group of staff in a hospital and are the center of care delivery; thus, they play an important role in the adoption and evaluation of HISs. The primary objective of this study was to explore the critical factors affecting the acceptance of HISs in Taiwan from a nursing perspective. On the basis of the technology acceptance model, we used six exogenous variables (system quality, information quality, user self-efficacy, compatibility, top management support, and project team competency) as investigation factors. Survey research targeted nursing personnel in the selected case hospital as participants. A total of 545 questionnaires were sent out, and 501 were returned, indicating a valid response rate of 91.9%. Collected data were analyzed using multiple regression analysis. : Results indicate that user self-efficacy, top management support, compatibility, and information quality have significant impacts on perceived ease of use. In addition, top management support, compatibility, and information quality were identified as having significant impacts on perceived usefulness. Furthermore, nurses' perceived ease of use and perceived usefulness on HISs was found to impact significantly on system acceptance, with 45.1% of the total explained variance. Results can help managers understand key considerations affecting HIS development and use and may be applied as a reference for system development and improvement.
