An interdisciplinary treatment to reshape upper anterior displaced teeth by using dynamic occlusal recording with an intraoral scanner.

This article describes an interdisciplinary treatment that helped a patient with displaced upper anterior teeth. A gingivectomy, root canal therapies, digital smile design, digital wax-up, and guided tooth preparations were applied. A patient with pathologically migrated teeth asked for treatment without orthodontic involvement due to a primary failed orthodontic treatment history. A smile photo was taken and superimposed with the dentition in a CAD software to accomplish a digital smile design. The jaw movements were recorded with two different methods, a mechanical articulator and an intraoral scanner with Patient-Specific-Motion function. The occlusal contacts during protrusive and lateral movements were compared and the digital wax-up was designed according to the later occlusal data. An aesthetic crown lengthening and pre-op root canal treatment were carried out in advance accordingly. After guided tooth preparation with a silicone index, the final fixed restorations were manufactured and cemented. A 2-year follow-up showed that our prosthesis functions well. This clinical report revealed that an intraoral scanner with Patient-Specific-Motion function can effectively record individual dynamic occlusal patterns and these data can be integrated into the CAD/CAM process to enhance the fulfillment of clinical requirements. This clinical procedure with a 2-year follow-up demonstrated that a prosthodontic-based interdisciplinary treatment of pathologically migrated teeth using dynamic occlusal recording with an intraoral scanner could achieve satisfactory esthetics in a relatively short treatment period. The Patient Specific Motion module may be used to record a personalized functional movement and the data can be integrated into the design process of the final restorations.

Factors Affecting Patients' Atttendance for Periodontal Follow-up Visits after Crown Lengthening Surgery.

To assess adherence to follow-up maintenance visits among patients who had previously undergone crown-lengthening surgery and investigate the different factors impacting their compliance. A total of 314 patients were identified for follow-up appointments. Based on their responses, participants were categorised into four groups: attendees, non-attendees, refusals, and unreachable. Furthermore, data on sociodemographic factors (age, sex, nationality, marital status, education, occupation, and residential area), medical history, dental history (including missing teeth, implants, or orthodontic treatment history), and past appointment attendance (average yearly appointments, missed appointment percentage, and last appointment date) were collected and analysed to understand their influence on patient compliance. In a sample of 314 patients, 102 (32.5%) attended the appointments successfully. Improved attendance rates were significantly associated with being female, Saudi Arabian, married, and employed (p < 0.05). Moreover, patients with a high frequency of annual appointments and a recent history of appointments exhibited better compliance. None of the analysed dental factors affected the attendance rates. About one-third of patients who had undergone crown lengthening surgery were compliant with the follow-up visits. Different factors influenced this compliance pattern to varying extents, with more efforts needed to enhance patients' commitment to these visits.

Bruxism, Lifestyle, Anxiety, and Sleep Impairment in Dental Students

AbstractThe aim of this study was to verify the correlation of self-reported sleep and awake bruxism with demographic characteristics, oral behaviors, anxiety, temporomandibular disorder (TMD) signs and symptoms, sleep quality, and orthodontic treatment history in dental students. A total of 104 students of Dentistry located in Paraná (South Brazilian State) answered the following self-administered questionnaires: Oral Behavior Checklist, State Anxiety Inventory, TMD signs and symptoms questionnaire, and the Pittsburgh Sleep Quality Index. Associations between possible awake bruxism (AB) and sleep bruxism (SB) with sleep quality, anxiety, and TMD were analyzed by Poisson Regression with robust variance. The significance level adopted was 5%. The frequency of AB and SB was 76% and 55.8%, respectively. A statistically higher frequency of AB was observed in students who had the following conditions: use of psychotropic medication, physical activity practitioners, moderate or high level of anxiety, more oral behaviors, sleep disorders, and sleep bruxism. Sleep bruxism was diagnosed more frequently in students who presented moderate or high levels of anxiety, oral behaviors, and sleep disorders. The frequency of TMD reported was higher in students with SB, as well as in those with AB. In conclusion, lifestyle, moderate and high anxiety levels, and sleep disorders are associated with a higher prevalence of AB and SB. Furthermore, AB and SB are associated with a higher frequency of TMD reporting.

Expectations of Patients Receiving Orthodontic Treatment from Saidu College of Dentistry Swat: A Cross Sectional Study

OBJECTIVE: The aim of this study was to assess the anticipations of orthodontic treatment held by both patients and parents. This would enable the provision of optimal services for expectations that are rational, while also facilitating discussions or guidance for expectations that are impractical. One of the key goals of healthcare systems today is to meet patient expectations. The declaration of a patient’s desires is significant because healthcare providers frequently undervalue their need for treatment. When a clinician is aware of patient demands, they are better equipped to meet the patient’s realistic expectations while explaining the least possibility to fulfil arbitrary expectations, thus results in more rewarding therapeutic interventions. METHODOLOGY: The data collection process involved the utilization of a non-randomized sampling technique, specifically selecting 300 participants as determined through Epi info using a structured form. The study focused on recruiting new patients who exhibited internal motivation, along with anatomical or morphological issues related to their teeth or jaws. A prior history of orthodontic treatment was an exclusion criterion. Participants with craniofacial syndromes or malformations, as well as those dealing with mental or psychological health conditions, were excluded from the research. RESULTS: In total, 300 individuals were examined, with 170 females and 130 males. Improvement in facial appearance was the most popular reason for seeking orthodontic treatment (180 patients out of 300), although other functions like mastication, better oral health and communication were mentioned by 120 patients. 70% (210) had a thought, since they would receive treatment, they would be more accepted by society and thus improve their social standing, while the remaining 30% (90) thought that it would have no difference. CONCLUSION: Essentially, expectations from orthodontic interventions among patients are quite uniform, although males generally display more practical perceptions regarding treatment duration and the initial appointment. However, when it comes to dietary and beverage restrictions during orthodontic treatment, patient expectations differ from those held by their parents. Frequency and Pattern of Anterior Crossbite With Primary and Mixed Dentition in School Children

OBJECTIVES To determine the frequency and pattern of anterior crossbite with primary and mixed dentition in School Children. METHODOLOGY A descriptive cross-sectional study was conducted at Sharif Medical and Dental College, Lahore. This study included 296 participants having either deciduous or mixed dentition, no history of orthodontic treatment, aged between 3-11 years, both genders and Pakistani nationals. Participants with a history of trauma, cleft lip/palate, or any craniofacial syndrome and systemic disease were excluded. Participant’s age, gender, skeletal class, and anterior crossbite (ACB) were recorded. The Chi-square/Fisher exact test was run to compare ACB and their pattern among gender, age group, and skeletal class. RESULTSThere is a relatively high rate of anterior crossbite in this population, which is about 10%. The females were 169(57.09%) and males were 127(42.91%). The mean age was 6.92 ± 1.68 years. Overall, the ACB was present in 31(10.47%). The most common pattern of ACB was single incisor involvement (n=11, 35.48%) followed by two incisors (n=9, 29.03%), and the least was four incisors (n=5, 16.13%). The difference for ACB was statistically significant among skeletal classes (p&lt;0.001). The frequency of ACB was higher in skeletal class 1 (n=17, 54.84%) and in skeletal class 3 (n=13, 41.94%) than in class 2 (n=1, 3.23%). CONCLUSION The frequency of anterior crossbite is about 10%, which is relatively higher than in other populations. Most anterior cross bites are dental due to one or two incisor involvement, which can be corrected easily at the mixed dentition stage.

Cervical headgear effectiveness in distalizing molars in relation to patient compliance.

The purpose of this study was to measure the effectiveness of the cervical headgear for distalizing first permanent maxillary molars in relation to hours of use. This was a one-centre, prospective, clinical study conducted at the Department of Orthodontics, School of Dentistry, National and Kapodistrian University of Athens, Athens, Greece. Participants (N = 26; 17 females, 9 males) were patients with no history of orthodontic treatment, no syndromes or clefts, and Angle's Class II malocclusion, where the treatment plan included a cervical headgear. They were instructed to wear the appliance for at least 12 h per day. A TheraMon® microsensor was embedded in the headgear's strap to objectively measure wear-time. To measure tooth movement, pre- and post-treatment digital models were superimposed, using the palate as a reference area; translation and rotation were measured along three axes. Superimposition and movement measurements were made with the Viewbox 4 software. Average treatment time and headgear wear were 130 days and 55 days, respectively, i.e. 10.1 h/day. During this period, distal movement averaged 1.75 mm with high variability (min 0.2 mm, max 4.5 mm). Distal tipping and rotation had an average of approximately 5 °C. Cumulative headgear wear was significantly correlated with distal movement (r2 = 0.32, P < .002), distal tipping (r2 = 0.27, P < .01), and distal rotation around the long axis of the tooth (r2 = 0.20, P < .05). Compliance is critical for having a successful clinical outcome. Distalization of the molar with a cervical headgear is correlated with the cumulative hours of appliance use, with hours per day being a weaker predictor.

Validation of Machine Learning Models for Craniofacial Growth Prediction.

This study identified the most accurate model for predicting longitudinal craniofacial growth in a Japanese population using statistical methods and machine learning. Longitudinal lateral cephalometric radiographs were collected from 59 children (27 boys and 32 girls) with no history of orthodontic treatment. Multiple regression analysis, least absolute shrinkage and selection operator, radial basis function network, multilayer perceptron, and gradient-boosted decision tree were used. The independent variables included 26 coordinated values of skeletal landmarks, 13 linear skeletal parameters, and 17 angular skeletal parameters in children ages 6 to 12 years. The dependent variables were the values of the 26 coordinated skeletal landmarks, 13 skeletal linear parameters, and 17 skeletal angular parameters at 13 years of age. The difference between the predicted and actual measured values was calculated using the root-mean-square error. The prediction model for craniofacial growth using the least absolute shrinkage and selection operator had the smallest average error for all values of skeletal landmarks, linear parameters, and angular parameters. The highest prediction accuracies when predicting skeletal linear and angular parameters for 13-year-olds were 97.87% and 94.45%, respectively. This model incorporates several independent variables and is useful for future orthodontic treatment because it can predict individual growth.

Dental caries and associated factors in 7-, 12- and 15-year-old schoolchildren in the canton of Basel-Landschaft, Switzerland: Changes in caries experience from 1992 to 2021.

Epidemiological surveys in schoolchildren are used to assess the current status of oral health. To investigate the changes in caries experience among schoolchildren in the canton of Basel-Landschaft, Switzerland, over a period of three decades. Secondary objectives were to evaluate the impact of various personal and demographic factors such as age group, place of residence or dental hygiene awareness on caries prevalence as well as the history of orthodontic treatment in the year 2021. A random sampling of school classes from first, sixth and ninth grades, that is schoolchildren aged 7, 12 and 15 years, was performed. Children's dmft and DMFT scores were determined according to the WHO methodology while information on oral hygiene habits and dental prophylaxis awareness was collected by means of a questionnaire directed to the legal guardians of the children. Individual logistic regressions were performed to identify possible influencing factors for caries. A total of 1357 schoolchildren could be included in the study. In the year 2021, the youngest age group had an average of 0.68 primary teeth that needed treatment, whereas the 12- and 15-year-olds each had approximately 0.3 permanent teeth requiring treatment. While these numbers remained constant over the examination period of three decades, most of the other caries indices improved. Younger children (p = .001) and children with a migrant background (p < .001) were found to be risk groups. Orthodontic treatment was more frequent in females, schoolchildren of Swiss nationality and children attending higher secondary schools at ninth grade. This study demonstrated that even in a country with a low prevalence of caries experience, untreated carious lesions remain a problem as their prevalence remained unchanged over the examination period of three decades.

Development of craniometry-based predictive model to determine occlusal vertical dimension by using the eye-ear distance in a French population

Techniques for determining occlusal vertical dimension (OVD) have limitations, including the lack of reproducibility or invasiveness. Recently, a craniometry-based predictive model comparing OVD with eye-ear distance (EED) was developed in Chile. However, this study included a specific population and excluded patients with a history of orthodontics. For verification, studies on other populations are required. The purpose of this clinical study was to follow the previously described protocol to obtain an equation for determining OVD in a French cohort (mostly White with an orthodontic history). Dentate adults with a stable occlusion and no known maxillofacial, otolaryngeal, or temporomandibular problems were included in this study. Demographic information, including participant age, sex, and history of orthodontic treatment, was collected. Facial height and width were measured with digital calipers, and the left EED and OVD were recorded with a craniometer. The facial index was calculated to classify participants into euryprosopic, mesoprosopic, or leptoprosopic types. Of the 300 included participants (28 ±11 years), 60% were women, and 67% reported a history of orthodontic treatment. Euryprosopic represented 17% of participants, mesoprosopic 48%, and leptoposopic 35%. A positive correlation was found between the left EED and OVD in all facial types, but it was more important in women. The following equation was obtained: OVD=44.58+(0.45×left EED)+sex (women=-4.57; men=0)+facial type (leptoprosopic=0; mesoprosopic=-3.35; euryprosopic=-7.27). The occlusal vertical dimension is correlated with sex, left EED, and facial type. This straightforward method can be applied in conjunction with other techniques to determine the OVD in the French population.

The prevalence, characteristics, and risk factors of external cervical resorption: a retrospective practice-based study.

External cervical root resorption (ECR) is a poorly understood and aggressive form of resorption. The purpose of this study was to examine the prevalence, characteristics, and risk factors associated with the occurrence of ECR in patients seeking endodontic care from private practice settings. Records of 343 patients with 390 teeth diagnosed with ECR were identified from 3 private endodontic practices from 2008 to 2022. The patients' demographic information, systemic conditions, and dental history were recorded. The characteristics of the cases including Heithersay classification, pulpal and periapical status, and their management were documented. The association between case severity and potential predisposing factors was examined using chi-square analysis. The overall prevalence of ECR among patients seeking endodontic care was low (< 1%). However, there was a greater than twofold increase in the pooled prevalence from 2016 to 2021 (0.99%) compared to the data from 2010 to 2015 (0.46%). The most commonly affected teeth were anterior teeth (48.7%). Class II (30.0%) and class III (45.4%) defects were the most often identified. Patients with a history of trauma or orthodontic treatment were significantly more likely to be diagnosed with severe cervical resorption (class III or IV) (p < 0.05). There has been an increase in the prevalence of ECR in patients seeking endodontic care. A history of orthodontic treatment and traumatic dental mechanical injuries may predict the severity of resorption. The upward trend in the occurrence of ECR warrants close monitoring of the patients at high risk of developing the condition to facilitate early detection and management.

Dental crowding and spacing measurements of the tooth and arch on patients visiting UMDC

A stable and aesthetic set of teeth is ensured by proper alignment. Additionally, a correct tooth position offers excellent conditions for good health and the best possible dental care. However, harmony amongst tooth and arch proportions leads to the continuity and integrity of the dental arch. Any imbalance amongst these components puts a person at risk for dental crowding and spacing, both of which are bad for the function and health of the teeth. Objectives: The goal of the current study was to compare the tooth and arch diameters amongst dental arches that are normal, crowded, and spaced. Methodology: This cross-sectional study was carried out at the UMDC Faisalabad orthodontic department. Patients and/or parents gave their informed consent before the start of therapy so that their records might be utilised for research. 120 dental casts of Pakistani subjects older than 13 years old were gathered as a sample. Dental casts that met the following inclusion criteria were chosen from the files of patients seeking orthodontic treatment: Pretreatment casts of patients with no past orthodontic treatment history, a permanent dentition that has fully emerged and allows accurate measurements of the crown dimensions, and excellent orthodontic casts.

The peripheral ossifying fibroma (POF) is a non-neoplastic reactive proliferative lesion with slow growth and often unclear etiology, normally associated with local irritants. This study described a case of a 64-year-old woman presenting an asymptomatic anterosuperior gingiva lesion, with a 21-year progression. The patient reported previous history of orthodontic treatment in the last five years, with no response. In the intraoral exam, there was an exophytic lesion in the anterior gingiva of the maxilla of approximately 2 cm in its greatest extent, which was non-tender and firm in consistency. Clinical examination, panoramic X-ray and cone-beam computed tomography suggested a reactive proliferative lesion. An excisional biopsy, associated with local periodontal treatment, was performed, and the material was submitted to histopathologic analysis, confirming the diagnosis of POF. After 12 months of follow-up, no recurrence and complaints were observed, emphasizing the benign features and the importance of surgical and periodontal treatment. 0808 Comparison of Maxillary Transverse Dimension with Pediatric Obstructive Sleep Apnea Presence and Severity

Abstract Introduction Obstructive Sleep Apnea (OSA) in children and adolescents is commonly discussed amongst dental specialists. Physical attributes such as a high and narrow palate, dolichofacial appearance, and retrognathic mandible are reported to increase pediatric risk for OSA. Previous research states rapid maxillary expansion decreases AHI in children with OSA and may be an effective treatment option. Current literature does not report a consistent method of measuring a maxillary transverse deficiency nor does it report a numerical maxillary threshold deficiency that requires expansion in pediatric patients with OSA. Our aims are to compare two different orthodontic diagnostic methods for measuring the maxillary transverse dimension and to assess whether patients diagnosed with mild, moderate, or severe OSA have a numerical threshold for maxillary deficiency. Methods Participants were ages 7-17 years old (average 11.7 years) with no history of orthodontic treatment, craniofacial syndromes, craniosynostosis forms, or nasal abnormalities. Those with and without adenoids and tonsils were included. Participants presented to the Mayo Clinic Sleep Center for their polysomnogram. Their dentition was scanned, and a digital model was rendered. The maxillary transverse dimension was measured via two different methods: McNamara’s Analysis and Andrew’s Six Elements of Orofacial Harmony. Results There were 65 participants: 31 diagnosed with OSA (13 Mild, 4 Moderate, 5 Severe, and 9 Other) and 34 without (controls). Patients in the Other category were diagnosed based on clinical findings. No statistical difference was found between the maxillary transverse measurements of patients with versus without OSA or between the OSA categories. Patients with OSA were more likely to have permanent teeth (61.3% vs 32.4%, p=0.019), tonsils removed (54.8% vs 26.5%, p=0.020), and to have snored during the PSG (83.9% vs 55.9%, p=0.048). Conclusion Conclusions based on this pilot study should be interpreted carefully due to limited sample sizes. No statistically significant differences of the maxillary transverse dimension were found between those with and without OSA using either diagnostic method. No numerical maxillary deficiency threshold was found. Maxillary expansion should not be recommended based on OSA diagnosis alone. Longitudinal trends in temporomandibular joint disorder symptoms, the impact of malocclusion and orthodontic treatment: A 20-year prospective study.

Studies on the association between malocclusion and temporomandibular joint disorder (TMD) have reported conflicting results. To determine the impact of malocclusion and orthodontic treatment on symptoms of TMD. At 12 years, 195 subjects fulfilled a questionnaire regarding TMD symptoms and participated in an oral examination including preparation of dental casts. The study was repeated at ages 15 and 32. The occlusions were assessed by applying the Peer Assessment Rating (PAR) Index. Associations between the changes in PAR scores and TMD symptoms were analysed with the chi-square test. A multivariable logistic regression was used to calculate the odds ratios (OR) and 95% confidence intervals (CI) of TMD symptoms at 32 years predicted by sex, occlusal traits and orthodontic treatment history. One in three subjects (29%) was orthodontically treated. Sex was associated with more self-reported headaches by females at 32 years (OR 2.4, 95% CI 1.05-5.4; p = .038). At all time points, any crossbite was significantly associated with greater odds for self-reported temporomandibular joint (TMJ) sounds at 32 years (OR 3.5, 95% CI 1.1-11.6; p = .037). More specifically, association occurred with posterior crossbite (OR 3.3, 95% CI 1.1-9.9; p = .030). At 12 and 15 years, boys whose PAR score increased were more likely to develop TMD symptoms (p = .039). Orthodontic treatment had no impact on the number of symptoms. Presence of crossbite may increase the risk of self-reported TMJ sounds. Also, longitudinal changes in occlusion may have an association with TMD symptoms while orthodontic treatment is not associated with the number of symptoms.

Covariance patterns between ramus morphology and the rest of the face: A geometric morphometric study

The growth and development of the mandible strongly depend on modeling changes occurring at its ramus. Here, we investigated covariance patterns between the morphology of the ramus and the rest of the face. Lateral cephalograms of 159 adults (55 males and 104 females) with no history of orthodontic treatment were collected. Geometric morphometrics with sliding semi-landmarks was used. The covariance between the ramus and face was investigated using a two-block partial least squares analysis (PLS). Sexual dimorphism and allometry were also assessed. Differences in the divergence of the face and anteroposterior relationship of the jaws accounted for 24.1% and 21.6% of shape variation in the sample, respectively. Shape variation was greater in the sagittal plane for males than for females (30.7% vs. 17.4%), whereas variation in the vertical plane was similar for both sexes (23.7% for males and 25.4% for females). Size-related allometric differences between the sexes accounted for the shape variation to a maximum of 6% regarding the face. Regarding the covariation between the shapes of the ramus and the rest of the face, wider and shorter rami were associated with a decreased lower anterior facial height as well as a prognathic mandible and maxilla (PLS 1, 45.5% of the covariance). Additionally, a more posteriorly inclined ramus in the lower region was correlated with a Class II pattern and flat mandibular plane. The width, height, and inclination of the ramus were correlated with facial shape changes in the vertical and sagittal planes.

Stomatognathic Risk Factors and Clinical Manifestations of Temporomandibular Disorders in Indian Population: A Case-control Study.

This study was aimed to compare the risk factors and associated clinical manifestations of patients with temporomandibular disorders (TMDs) in the Indian population. A total of 52 patients were explored according to the Diagnostic Criteria for Temporomandibular Disorders (DC/TMD) and compared with 48 controls. The mean age of the study group was 30.96 ± 11.60 years, 41% were males and 59% were females. The mean age in the control group was 31.5 ± 9.9 years (37.5% males), measuring differences in TMD risk factors (self-perceived stress, parafunctions, occlusal interferences, history of orthodontic treatment, and/or extraction). The most common sign observed in the selected subjects was the joint sound (clicking) (42%), followed by mandibular deviation 39% as the second most common sign. Myofascial pain (MFP) (single or multiple diagnoses) was the most frequent diagnosis (n = 40, 76%), followed by disc displacement with reduction (DDWR) (32.1%), arthralgia (30%), and disc displacement without reduction (DDWoR) (7.6%). More than one DC/TMD diagnosis was established in 63.2% of the patients. The risk factors like presence of clenching, stress perception, parafunctions, tooth wear, and occlusal interferences were significantly more frequent in all the cases than in the respective controls. In the studied population, the presence of TMD was found to be positively related to factors namely female gender, parafunction, occlusal interferences, psychological stress, and tooth wear. Myofascial pain is the commonest diagnosis (either single or in combination). It is followed by DDWR (either single or in combination with others). Double diagnosis is also common. Temporomandibular disorders showed a female predilection and were found to be positively related to factors, viz., parafunction, occlusal interferences, stress, and tooth wear. Clinicians should also be aware of common clinical manifestations of TMDs and their related stomatognathic risk factors to provide comprehensive oral care and to identify such factors for disease prevention.

Comparison of Maxillary Canine Retraction Using Split-Mouth Design With Dual Force Cuspid Retractor and T-loop Segmental Arch: A Split-Mouth Randomized Clinical Trial.

Introduction This study was designed to explore the differences between two frictionless mechanics for canine retraction i.e., dual force cuspid retractor and T-loop segmental arch. T-loop for canine retraction creates a biomechanical system to deliver a predetermined force and a relatively constant moment-to-force ratio whereas dual force cuspid retractor uses power arms on buccal as well as palatal aspects for canine retraction. Bodily tooth movement can be achieved by both methods, but in this study, our main focus was to reduce the canine retraction timing with better three-dimensional control. Method This split-mouth study was conducted on a total of 20 cuspids of ten patients (five male and five female). Where one side of the arch was selected for T-loop and the other side for dual force cuspid retractor, randomly. Inclusion criteria for this study were; no congenitally missing teeth (excluding third molar), class I or class II molar relationship, no previous history of orthodontic treatment, good oral periodontal status, patients in whom extraction of maxillary first premolar during treatment was indicated. Both groups were compared for the duration of canine retraction, anchorage loss; tipping, and rotation of cuspid and molar, individually, after retraction. Result The result of this study showed that the duration of canine retraction was significantly less in group one, i.e., dual force cuspid retractor 73.8 ± 12.38 days, than in group two, i.e., T-loop 109.4 ± 16.71 days. The anchorage loss in group one was 0.60 ± 0.61 mm and that in group two was 2.40 ± 0.87 mm. Also, the amount of tipping and rotation of the cuspid and molar individually was significantly lesser in group one than in group two. Conclusion In this study, the dual force cuspid retractor shortens the duration of canine retraction with better three-dimensional control and better anchorage preservation when compared to T-loop.

Prevalence of malocclusion and orthodontic treatment needs among adolescents in Najran City, Saudi Arabia.

To determine the prevalence of malocclusions and the need for orthodontic treatment in a sample of school-going adolescents in the Najran city, Kingdom of Saudi Arabia (KSA). This cross-sectional study was conducted among 1094 school-going Saudi male adolescents. The age range of the sampled adolescents was 13-18 years, and none of them had any history of orthodontic treatment. A survey chart related to malocclusion was designed and filled up after clinical examination by a single experienced and calibrated examiner. The need for orthodontic treatment was assessed using the Index of Orthodontic Treatment Need (IOTN). The present study's findings demonstrated 61.2% of the samples with Angle's Class I malocclusions, 27.5% with Class II, and 11.4% of samples with Class III malocclusion. A significant difference was observed between the different classes of malocclusion (P < 0.001). Most samples presented normal overjet, crossbite, and no reverse overjet, deep bite, or open bite. The number of samples with no treatment need was 573 (52.37%), with slight treatment in 185 (16.91%) samples, moderate treatment needs in 123 (11.24%) samples, and severe and extreme treatment needs were 109 (9.96%) and 104 (9.50%), respectively. A significant difference was observed between the samples with no or slight treatment need (grades 1 and 2), moderate need (grade 3), and definite treatment need (grades 4 and 5) (p < 0.001). The prevalence of malocclusion and IOTNs among the sample of school-going adolescents in the Najran city was 47.63%. However, 9.63% of those samples required immediate attention for orthodontic treatment.

Orthodontic retreatment need and related self-perceived factors among contemporary college freshmen in China.

Given that an increasing number of patients who received orthodontic treatment during their childhood or adolescence are seeking retreatment in contemporary China, it is of great necessity to comprehensively understand their motivations. A valid and reliable self-designed questionnaire, based on the Index of complexity, outcome and need (ICON), was distributed online to college freshmen who received orthodontic treatment during their childhood or adolescence. After collecting their basic information and orthodontic retreatment needs data from the survey, the participants' general self-perception of front facial appearance, lateral facial appearance and tooth alignment, as well as their self-perceived dental alignment, occlusal status, oral function and psychological status, were assessed. Correlation analysis, Chi-square test, Kruskal-Wallis test and logistic regression were performed. Reliability was evaluated for 20 paired questionnaires, and all questions were found to be reliable (intraclass correlation coefficient, >0.70). Among the 1609 participants with a history of orthodontic treatment, 45.56% were males and 54.44% were females. Their mean age was 18.48 ± 0.91 years. Our results showed that self-perceived front facial appearance, lateral facial profile, tooth alignment, occlusal status, oral function and psychological status were significantly correlated with orthodontic retreatment needs. Both appearance and psychological status affected their self-perceived dental alignment and occlusal status. In conclusion, patients who received orthodontic treatment during their childhood or adolescence in contemporary China seek orthodontic retreatment because they desire better aesthetics of their front facial appearance and tooth alignment, especially the anterior region of the tooth, the lower part of the face and a decent pronunciation. Additionally, psychological concerns should be viewed as an urge, while intraoral factors should be viewed as the foundation during future clinical practice regarding orthodontic retreatment in this age group.

Perceived esthetics and value of clear aligner therapy systems: A survey among dental school instructors and undergraduate students.

To investigate the attractiveness, acceptability, visibility and willingness-to-pay for clear aligner therapy (CAT) systems in first-year and final-year dental students and instructors. A questionnaire designed to collect information regarding esthetic preferences and intentions related to seven CAT systems was handed out to 120 undergraduate students and instructors at the Academic Centre for Dentistry Amsterdam (ACTA). Proportional odds models and population average generalized estimating equation models were used to examine potential association between participant characteristics, esthetic perceptions and CAT systems. Overall, the examined CAT systems received favorable esthetic ratings. Expertise status was significantly associated with willingness-to-pay additionally for CAT, compared to fixed orthodontic appliances. There was no association between sex, previous orthodontic treatment history, satisfaction with own dental appearance and potential interest in treatment and aligner visibility and willingness-to-pay. CAT system was significantly associated with the perceived aligner visibility, acceptability and attractiveness by students and instructors. CAT systems were considered to a great extent attractive and acceptable for future treatment by dental school instructors and students. Willingness-to-pay for CAT systems was significantly associated with expertise status, with instructors appearing more reluctant to pay for CAT.
