Association between Three Heavy Metals and Dry Eye Disease in Korean Adults: Results of the Korean National Health and Nutrition Examination Survey.

PurposeTo investigate the associations between blood heavy metal concentrations and dry eye disease using a Korean population-based survey.MethodsThis study included 23,376 participants >40 years of age who participated in the Korean National Health and Nutrition Examination Survey from 2010 to 2012. Blood concentrations of lead, cadmium, and mercury were measured in all participants. The associations between blood heavy metal concentrations and dry eye disease were assessed using multivariate logistic regression analyses.ResultsAfter adjusting for potential confounders, including age, sex, lifestyle behaviors and sociodemographic factors, the analyses revealed an increased odds ratio (OR) for dry eye disease with higher blood mercury concentrations (tertile 2: OR, 1.22; 95% confidence interval [CI], 0.91 to 1.64; tertile 3: OR, 1.39; 95% CI, 1.02 to 1.89; p = 0.039). The prevalence of dry eye disease was not associated with blood lead (tertile 2: OR, 1.15; 95% CI, 0.87 to 1.51; tertile 3: OR, 0.83; 95% CI, 0.59 to 1.16; p = 0.283) or cadmium (tertile 2: OR, 1.05; 95% CI, 0.77 to 1.44; tertile 3: OR, 1.15; 95% CI, 0.84 to 1.58; p = 0.389) concentrations. There were no significant associations between any of the three heavy metals and dry eye disease in males after adjusting for potential confounding factors, but blood mercury concentrations in females were associated with dry eye disease (tertile 2: OR, 1.18; 95% CI, 0.83 to 1.69; tertile 3: OR, 1.58; 95% CI, 1.12 to 2.24; p = 0.009).ConclusionsMercury concentrations in blood were associated with dry eye disease. Our results suggested that controlling environmental exposure to mercury may be necessary to reduce the incidence of dry eye disease.

Associations between environmental heavy metal exposure and childhood asthma: A population-based study.

The health risks of environmental heavy metals have been of concern are well known. The greater likelihood of heavy metal contamination in the physical environment increases the risk of asthma, especially in children. This cross-sectional, population-based study sought to investigate associations between heavy metal exposure and childhood asthma or wheezing. Data from 5866 subjects, stratified into age groups of 2-5, 6-11, and 12-15 years, from the National Health and Nutrition Examination Survey 2007-2012 conducted by the Centers for Disease Control and Prevention were analyzed retrospectively. The primary outcome was active asthma. Variables included demographics, anthropometric, and clinical data. Univariate and multivariate logistic regression analyses were used to identify associations between blood heavy metal concentrations and adjusted odds (aORs) of active asthma. Higher concentration of blood lead was associated with higher adjusted odds of having asthma (aOR=1.08, 95% CI=1.00-1.16), but no significant effect was shown for current wheezing or whistling. Age-stratified analysis showed that higher blood lead concentration was associated with higher risk for active asthma (aOR=1.24, 95% CI=1.08-1.42) and current wheezing or whistling (aOR=1.19, 95% CI=1.04-1.38) in the 6-11 years age group, while higher blood mercury concentration was associated with lower risk of current wheezing or whistling (aOR=0.95, 95% CI=0.90-0.99). The medium concentration of blood lead was associated with decreased risks of current wheezing or whistling (aOR=0.54, 95% CI=0.30-0.96) in the 2-5 years age group. Higher concentrations of blood lead are associated with higher odds of asthma in children aged 2-15 years.

Tracking Declines in Mercury Exposure in the New York City Adult Population, 2004\u20132014

Mercury is a toxic metal that can be measured in human blood and urine. Population-based biomonitoring from 2004 guided New York City (NYC) Department of Health and Mental Hygiene (DOHMH) efforts to reduce exposures by educating the public about risks and benefits of fish consumption—a predominant source of exposure in the general population—and removing mercury-containing skin-lightening creams and other consumer products from the marketplace. We describe changes in exposures over the past decade in relation to these local public health actions and in the context of national changes by comparing mercury concentrations measured in blood (1201 specimens) and urine (1408 specimens) from the NYC Health and Nutrition Examination Survey (NYC HANES) 2013–2014 with measurements from NYC HANES 2004 and National Health and Nutrition Examination Surveys (NHANES) 2003–2004 and 2013–2014. We found that NYC adult blood and urine geometric mean mercury concentrations decreased 46% and 45%, respectively. Adult New Yorkers with blood mercury concentration ≥ 5 μg/L (the New York State reportable level) declined from 24.8% (95% CL = 22.2%, 27.7%) to 12.0% (95% CL = 10.1%, 14.3%). The decline in blood mercury in NYC was greater than the national decline, while the decline in urine mercury was similar. As in 2004, Asian New Yorkers had higher blood mercury concentrations than other racial/ethnic groups. Foreign-born adults of East or Southeast Asian origin had the highest prevalence of reportable levels (29.7%; 95% CL = 21.0%, 40.1%) across sociodemographic groups, and Asians generally were the most frequent fish consumers, eating on average 11 fish meals in the past month compared with 7 among other groups (p < 0.001). Fish consumption patterns were similar over time, and fish continues to be consumed more frequently in NYC than nationwide (24.7% of NYC adults ate fish ten or more times in the past 30 days vs. 14.7% nationally, p < 0.001). The findings are consistent with the hypothesis that blood mercury levels have declined in part because of local and national efforts to promote consumption of lower mercury fish. Local NYC efforts may have accelerated the reduction in exposure. Having “silver-colored fillings” on five or more teeth was associated with the highest 95th percentile for urine mercury (4.06 μg/L; 95% CL = 3.1, 5.9). An estimated 5.5% of the adult population (95% CL = 4.3%, 7.0%) reported using a skin-lightening cream in the past 30 days, but there was little evidence that use was associated with elevated urine mercury in 2013–14.

The associations between blood mercury levels and shark meat intake among workers in Gyeongsangbuk-do

BackgroundShark meat is used as sacrificial food in Gyeongsangbuk-do, and is a major source of dietary mercury. This study aimed to evaluate the effects of shark meat intake or the ritual of Jesa on blood mercury levels within workers living in Gyeongsangbuk-do.MethodsThis study was conducted from September 2016 to October 2016 in two cities of Gyeongsangbuk-do. To compare the differences between urban and rural areas, two workplaces each in Daegu as the urban area and Yeongcheon as the rural area were selected. General characteristics and characteristics related to shark meat consumption of the workers were acquired by personal interviews during their health examination. Blood mercury concentrations were analyzed by the gold amalgamation method using a direct mercury analyzer (DMA-80; Milestone Inc., Shelton, CT, USA).ResultsThe shark consumption group had a higher blood mercury concentration than the non-consumption group. The levels of blood mercury increased with the frequency, annual intake, as well as most recent date of shark meat consumption. Moreover, the levels of mercury in blood increased according to the annual frequency of participation in Jesa (times per year) and the annual frequency of shark meat consumption during Jesa (times per year).ConclusionsShark meat intake and the ritual of Jesa contributed to an increase in the blood mercury levels of workers in Gyeongsangbuk-do. Therefore, it is necessary to evaluate dietary exposure, occupational as well as other factors that may influence blood mercury concentrations in workers during their health examination, particularly in regions with high mercury exposures.

Neurological and neuropsychological deterioration in artisanal gold miners from the town of Andacollo, Chile

It is widely known that human exposure to mercury vapor can cause neurological and neuropsychological deterioration. We have investigated if a population of Chilean artisanal gold-mining workers heavily exposed to elemental mercury (Hg0) display neurological and neuropsychological impairment. Male volunteers occupationally exposed to Hg0 (“gold miners”, n = 35) were recruited and compared with a group of unexposed workers (n = 40). Blood specimens were obtained from both groups for total mercury quantification. Upon neurological examination, 71% of the “gold miners” group showed abnormalities expressed as frontal impairment, tremor, or simultaneously frontal impairment, parkinsonism, and pyramidal syndrome. In contrast, only 16% of the individuals in the control group displayed neurological abnormalities. The “gold miners” group also showed impairment of the neuropsychological performance and the distribution of abnormal scores for almost all the neuropsychological tests applied was significantly higher in this group compared to the control group. Educational level, alcohol intake, and smoking habits did not influence the outcome of the neurological or neuropsychological examinations. Finally, despite that the higher blood mercury concentrations were found in the “gold miners” group, they were not significantly different from the values obtained in the control group.

Environmental Contaminants in Nestling Bald Eagles Produced in Montana and Wyoming

Abstract Upper-trophic level piscivores, such as Bald Eagles (Haliaeetus leucocephalus), may be at risk of exposure to environmental contaminants in the western United States from coal-fired power plants and coal-bed methane wells. We collected 64 blood samples from nestling Bald Eagles in southeastern Montana and northwestern Wyoming during 2007–08 for analyses of heavy metals, trace elements, and organochlorine compounds. Mercury was detected in all blood samples and averaged (±SD) 0.28 ± 0.20 ppm. Most nestlings (82.8%) had concentrations near expected background levels, but 7.8% had elevated (>0.7 ppm) mercury levels. Nestlings in Wyoming ( = 0.37 ± 0.22 ppm) had higher blood mercury concentrations than nestlings from Montana ( = 0.22 ± 0.17 ppm; t31 = 3.151, P = 0.015). The association of heavy metals in blood, fish consumption frequency, and risk of cardiovascular diseases among Korean adults: The Korean National Health and Nutrition Examination Survey (2008-2010)

Limited studies are available concerning the effect of heavy metal exposure on cardiovascular diseases. As environmental pollution increases, food contamination, including heavy metal contamination of fish, also increases. However, researches based on the intake of heavy metals, cardiovascular disease, and fish intakes are inconclusive. We assessed an association of heavy metal exposure with cardiovascular disease and fish intake in a nationally representative group of general Korean adults. We used data from the combined 2008-2010 Korean National Health and Nutrition Examination Survey (KNHANES), and analyzed the data of 5,139 Koreans who participated in KNHANES. All participants were older than 20 years, and were diagnosed with stroke, ischemic heart disease, or hypertension. The mean blood cadmium, lead concentration, and mercury concentration of subjects were 1.07 ± 0.01 μg/L, 2.49 ± 0.02 μg/dL, and 5.19 ± 0.08 μg/L, respectively. We used the survey logistic regression model to account for the complex sample design of the cardiovascular disease risk in order to estimate the odds ratios (OR). After adjusting for age, education, income, alcohol, smoking, and BMI, the increase of serum cadmium in blood was associated with the increase in the prevalence of hypertension. Further, the increase in blood cadmium concentration was associated with the increase of both systolic blood pressure (SBP) and diastolic blood pressure (DBP). Although higher fish intakes were significantly associated with higher blood mercury concentration (p for trend ＜ 0.0001), fish intakes did not affect either blood cadmium or lead concentration. Comparing the highest fish intake group with the lowest intake group, the OR of hypertension was 0.31 (95% CI: 0.19-0.59) in the crude model for total fish. However, these associations were no longer significant after the adjustment for potential confounding factors. In conclusion, cadmium in blood was associated with an increased risk of hypertension in the general Korean adult population. However, we found no evidence of a clear relationship between cardiovascular disease and frequency of fish consumption.

Infertility, blood mercury concentrations and dietary seafood consumption: a case–control study

Objective To compare blood mercury concentrations of infertile couples with those of fertile couples in Hong Kong, and to examine the relationship between blood mercury concentrations and seafood consumption. Design Case–control study. Setting In vitro fertilisation (IVF) Unit and Antenatal Unit of a university teaching hospital. Sample One hundred fifty-seven infertile couples attending IVF treatment and 26 fertile couples attending antenatal care without known occupational exposure to mercury. Methods Mercury concentrations in whole blood were measured by cold vapour atomic absorption spectrophotometry. A dietitian recorded the quantity of seafood consumption among infertile couples via a food-frequency questionnaire. Blood mercury concentrations and quantity of seafood consumption were compared between infertile and fertile couples. Main outcome measures Whole blood mercury concentrations, quantity of seafood consumption. Results Infertile couples had higher blood mercury concentrations than fertile couples. ‘Infertile males with abnormal semen’ and ‘infertile females with unexplained infertility’ also had higher blood mercury concentrations than their fertile counterparts. Blood mercury concentrations were positively correlated with quantity of seafood consumption. Infertile subjects with elevated blood mercury concentrations consumed a larger amount of seafood. Conclusion Higher blood mercury concentration is associated with male and female infertility. Higher seafood consumption is associated with elevated blood mercury concentrations in our infertile population.

Mercury exposure: current concepts, controversies, and a clinic's experience.

In the context of controversies surrounding fish consumption, amalgams, and commercial hair testing, we reviewed all cases from an occupational and environmental medicine clinic that had undergone mercury testing. Sixty-nine of 71 (97%) patients had no known mercury exposures other than diet or amalgams. Of these 69, 48 had blood mercury tested and 58 had urine testing. Regular-to-heavy fish consumption explained 10 of 11 cases with blood mercury concentrations > 15 micrograms/L (19 to 53 micrograms/L). Six of these 10 individuals reported regular swordfish consumption. For the 31 patients with adequate dietary history, there was a significant relationship between fish consumption and blood mercury concentration (P < 0.001). Higher blood mercury concentrations were, however, not associated with specific patterns of health complaints. Ninety-eight percent (57 of 58) of urine values were < 10 micrograms/L. Fourteen patients were evaluated because they were labeled as mercury toxic by other practitioners after unconventional commercial testing. Using standard tests of blood and urine, we could not document evidence of mercury toxicity in any of these 14 cases. We conclude that consumption of commercially available fish can lead to elevated blood mercury concentrations. A recognized exposure source is a better predictor of significant mercury concentrations in biologic media than any particular symptom constellation. Unconventional commercial panels that test hair or urine for multiple metals have questionable validity. Clinicians should use standard blood and urine tests to evaluate mercury exposure.

Blood Levels of Total and Organic Mercury in Residents of the Upper St. Lawrence River Basin, Québec: Association with Age, Gender, and Fish Consumption

Increased consumption of lake fish was associated with higher blood mercury concentrations among 289 residents of Southwest Quebec living on the southern bank of the Upper St. Lawrence River System. This increase in blood mercury was reflected in the organic fraction and to a lesser extent with total blood mercury. Repeat analyses on a small subpopulation showed a correlation coefficient between time 1 and time 2 of approximately 0.7 for total mercury and for organic mercury. With one exception, blood total mercury concentrations were less than 5 ppb. Blood mercury concentrations were higher among men than women. One individual had much higher total mercury concentrations in blood (i.e., 70 ppb) which were attributed to fish consumption based on medical and dietary history. The values for these subjects are compared with other North American data on blood mercury levels.
