Low-volume high-intensity interval training for cardiometabolic health.

High-intensity interval training (HIIT) is characterised by short bouts of high-intensity submaximal exercise interspersed with rest periods. Low-volume HIIT, typically involving less than 15min of high-intensity exercise per session, is being increasingly investigated in healthy and clinical populations due to its time-efficient nature and purported health benefits. The findings from recent trials suggest that low-volume HIIT can induce similar, and at times greater, improvements in cardiorespiratory fitness, glucose control, blood pressure, and cardiac function when compared to more traditional forms of aerobic exercise training including high-volume HIIT and moderate intensity continuous training, despite requiring less time commitment and lower energy expenditure. Although further studies are required to elucidate the precise mechanisms of action, metabolic improvements appear to be driven, in part, by enhanced mitochondrial function and insulin sensitivity, whereas certain cardiovascular improvements are linked to increased left ventricular function as well as greater central and peripheral arterial compliance. Beyond the purported health benefits, low-volume HIIT appears to be safe and well-tolerated in adults, with high rates of reported exercise adherence and low adverse effects.

Influence of the Lower Body on Seated Arm Cranking Performance.

During upper-body tasks, use of the lower body is important for minimizing physiological strain and maximizing performance. The lower body has an integral role during standing upper-body tasks, however, it is less clear if it is also important during seated upper-body tasks. We determined the extent to which the lower body influenced seated arm cranking performance. Eleven males performed incremental (40+20 W·3 min-1) and short-duration maximal effort (5 s, 120 rpm) arm cranking trials with and without lower-body restriction. The lower body was restricted by securing the legs to the seat and suspending them off the floor. Upper-body peak oxygen consumption (V̇O2peak) and maximal power were determined. At the end of the incremental protocol, lower-body restriction reduced V̇O2peak by 14±12% (P<0.01) compared to normal arm cranking. At greater submaximal stages (60-100% isotime) heart rate, ventilation, RER, and arm-specific exertion increased to a greater extent (all P<0.05) with lower-body restriction. During short duration maximal arm cranking, lower-body restriction decreased maximal power by 23±9% (P<0.01). Results indicated that lower-body restriction limited aerobic capacity, increased physiological strain during high-intensity submaximal exercise, and compromised maximal power generating capacity. These results imply that use of the lower body is critical when performing seated arm cranking. Our findings have implications for exercise testing, training and rehabilitation.

Skeletal muscle fatigue and decreased efficiency: two sides of the same coin?

During high-intensity submaximal exercise, muscle fatigue and decreased efficiency are intertwined closely, and each contributes to exercise intolerance. Fatigue and muscle inefficiency share common mechanisms, for example, decreased "metabolic stability," muscle metabolite accumulation, decreased free energy of adenosine triphosphate breakdown, limited O2 or substrate availability, increased glycolysis, pH disturbance, increased muscle temperature, reactive oxygen species production, and altered motor unit recruitment patterns.

Exercise in Patients With Breast Cancer and Healthy Controls: Energy Substrate Oxidation and Blood Lactate Responses

The aim of this study was to compare select aspects of exercise energy metabolism in patients with breast cancer with that of healthy controls across a variety of exercise intensities. Posttreated patients with breast cancer were matched with healthy women based on age, physical fitness level, and menopausal status. Subjects participated in low-, moderate-, and high-intensity submaximal exercise sessions that corresponded with 40% of maximal oxygen consumption (Vo(2max)), 60% Vo(2max), and 70% Vo(2max). Oxygen consumption and respiratory exchange ratio were taken during submaximal exercise sessions to determine substrate oxidation rates for carbohydrate (CHO) and fat. Blood lactate and blood glucose were also measured before and after each of the submaximal exercise sessions as indices of CHO metabolism. Results indicate that the patients with breast cancer had a significantly (P≤.05) lower CHO oxidation rate and higher fat oxidation rate at all exercise intensities compared with healthy women. The patients with breast cancer had a significantly (P≤.05) lower blood lactate response to exercise across all intensities compared with the healthy women. Glucose responses tended (P<.08) to be more elevated in patients with cancer both before and after the exercise sessions. The findings indicate that posttreated patients with breast cancer have augmented fat metabolism and a reduced CHO-based energy metabolism during submaximal exercise. It is unclear whether these changes are the result of the patient's cancer or their treatment regimen for the cancer.

Oral creatine supplementation decreases plasma markers of adenine nucleotide degradation during a 1-h cycle test

We investigated the effect of oral creatine supplementation (20 g d(-1) for 7 days) on metabolism during a 1-h cycling performance trial. Twenty endurance-trained cyclists participated in this double-blind placebo controlled study. Five days after familiarization with the exercise test, the subjects underwent a baseline muscle biopsy. Thereafter, a cannula was inserted into a forearm vein before performing the baseline maximal 1-h cycle (test 1 (T1)). Blood samples were drawn at regular intervals during exercise and recovery. After creatine (Cr) loading, the muscle biopsy, 1-h cycling test (test 2 (T2)) and blood sampling were repeated. Resting muscle total creatine (TCr), measured by high performance liquid chromatography, was increased (P < 0.001) in the creatine group from 123.0 +/- 3.8 - 159.8 +/- 7.9 mmol kg(-1) dry wt, but was unchanged in the placebo group (126.7 +/- 4.7 - 127.5 +/- 3.6 mmol kg(-1) dry wt). The extent of Cr loading was unrelated to baseline Cr levels (r=0.33, not significant). Supplementation did not significantly improve exercise performance (Cr group: 39.1 +/- 0.9 vs. 39.8 +/- 0.8 km and placebo group: 39.3 +/- 0.8 vs. 39.2 +/- 1.1 km) or change plasma lactate concentrations. Plasma concentrations of ammonia (NH(3)) (P < 0.05) and hypoxanthine (Hx) (P < 0.01) were lower in the Cr group from T1 to T2. Our results indicate that Cr supplementation alters the metabolic response during sustained high-intensity submaximal exercise. Plasma data suggest that nett intramuscular adenine nucleotide degradation may be decreased in the presence of enhanced intramuscular TCr concentration even during submaximal exercise.

New insights concerning the role of carnitine in the regulation of fuel metabolism in skeletal muscle

In skeletal muscle, carnitine plays an essential role in the translocation of long-chain fatty-acids into the mitochondrial matrix for subsequent beta-oxidation, and in the regulation of the mitochondrial acetyl-CoA/CoASH ratio. Interest in these vital metabolic roles of carnitine in skeletal muscle appears to have waned over the past 25 years. However, recent research has shed new light on the importance of carnitine as a regulator of muscle fuel selection. It has been established that muscle free carnitine availability may be limiting to fat oxidation during high intensity submaximal exercise. Furthermore, increasing muscle total carnitine content in resting healthy humans (via insulin-mediated stimulation of muscle carnitine transport) reduces muscle glycolysis, increases glycogen storage and is accompanied by an apparent increase in fat oxidation. By increasing muscle pyruvate dehydrogenase complex (PDC) activity and acetylcarnitine content at rest, it has also been established that PDC flux and acetyl group availability limits aerobic ATP re-synthesis at the onset of exercise (the acetyl group deficit). Thus, carnitine plays a vital role in the regulation of muscle fuel metabolism. The demonstration that its availability can be readily manipulated in humans, and impacts on physiological function, will result in renewed business and scientific interest in this compound.

Encouraged 6-min Walking Test Indicates Maximum Sustainable Exercise in COPD Patients

In patients with moderate-to-severe COPD, an encouraged 6-min walking test (6MWT) is a high-intensity submaximal exercise protocol that shows an oxygen uptake (Vo(2)) plateau after the third minute of the test. This last feature prompted the hypothesis that self-paced walking speed is set to achieve "maximal" sustainable Vo(2), namely "critical power" or "critical speed." Eight patients with moderate-to-severe COPD (mean age, 68 +/- 7 years [+/- SD]; FEV(1), 50 +/- 13% predicted; Pao(2), 69 +/- 8 mm Hg) underwent the following tests on different days in order: (1) encouraged 6MWT; (2) standard incremental shuttle test to identify peak walking speed; (3) four different high-intensity, constant walking speed tests to exhaustion to calculate critical walking speed; and (4) timed walking test at critical walking speed (CWS) to examine sustainability of the exercise. 6MWT and CWS showed similar results (mean of last 3 min): Vo(2) (1,605 +/- 304 mL/min vs 1,584 +/- 319 mL/min), minute ventilation (47 +/- 12 L/min vs 48 +/- 11 L/min), respiratory exchange ratio (0.89 +/- 0.1 vs 0.90 +/- 0.1), heart rate (130 +/- 18 beats/min vs 131 +/- 16 beats/min), Borg dyspnea score (5.4 +/- 1.3 vs 5.5 +/- 2.4), and walking speed (1.49 +/- 0.1 m/s vs 1.44 +/- 0.1 m/s, respectively). This study supports that 6MWT indicates maximum sustainable exercise that might be related with its predictive value in COPD patients.

EFFECT OF HIGH INTENSITY SUBMAXIMAL WORK, WITH OR WITHOUT A REST PERIOD, ON SUBSEQUENT VO2MAX

A common protocol to evaulate endurance athletes is a two-step lactate profile-VO2max test. A typical procedure provides 20–30 min rest between profile and max. The effect of the profile and/or the rest period on the subsequent VO2max value is not known. PURPOSE: To examine effects of preceding high intensity submaximal exercise, with or without rest, on a subsequent rapid ramp VO2max test. METHODS: Ten endurance-trained males completed three separate exercise trails: an incremental run to fatigue (MAX), a continuous lactate profile-VO2max test (C), and a discontinuous lactate profile-VO2max test (DC). Protocol for C and DC were identical except for a 20-minute rest period between the lactate profile and VO2max test during DC. All trials were separated by 24 hours. MAX was always completed first; C and DC were randomized between the second and third day. RESULTS: No differences in VO2max between trials were evident (MAX = 62.67 ml/kg/min, C = 61.20 ml/kg/min, DC = 58.29 ml/kg/min; p = .059). Subjects with a ± 7.5% fall in VO2max from MAX (Responders) were compared to those with < 7.5% fall (Non-Responders). DC Responders (n = 5) had higher VO2max during MAX than DC Non-Responders (n = 5) (Responders = 67.64 ml/kg/min, Non-Responders = 57.70 ml/kg/min; p = .008). C Responders (n = 2) achieved a VO2max of 71.05 ml/kg/min during MAX; C Non-Responders (n = 8) achieved a VO2max of 60.58 ml/kg/min during MAX. CONCLUSIONS: The trend for lower DC VO2max values, coupled with the DC Responder results, suggests a reduction in VO2max in some athletes if maximal exercise is preceded by high intensity submaximal exercise and a rest period. The effects of preceding high intensity exercise without rest on the achievement of VO2max are still unclear. Supported by the United States Olympic Committee.

Responses of plasma human atrial natriuretic factor to high intensity submaximal exercise in the heat.

No data exists regarding responses of human atrial natriuretic factor (ANF) to exercise in the heat. The purpose of this study was to examine the responses of plasma ANF to high intensity submaximal (71% +/- 0.9 VO2max) exercise in the heat over an eight day acclimation period. Fourteen healthy males volunteered to participate in the study. Subjects performed intermittent exercises on a treadmill (0% grade) during 50 min of each 100 min trial in an environmental chamber maintained at 41.2 +/- 0.5 degrees C, 39.0 +/- 1.7% relative humidity. Blood was obtained from an antecubital vein after standing 20 min in the heat prior to exercise, and immediately after exercise. Measures were compared on days 1, 4 and 8. ANF did not change pre- to post-exercise nor did it change over the eight day heat acclimation period despite other heat acclimation adaptations. Conversely, plasma aldosterone (ALDO), renin activity (PRA) and cortisol (COR) all increased (p less than 0.05) pre- to post-exercise on each day but again no changes were observed over the eight day period. These data support that ANF may not increase when ALDO and PRA increases are observed.

Effects of naloxone on exercise performance.

This study was designed to investigate the effects of naloxone on athletic performance in humans. Two groups of elite middle-distance runners performed a maximal or a submaximal exercise protocol following the double-blind intravenous injection of either naloxone (0.15 mg X kg body wt-1) or saline. The maximal test (group M) was comprised of a short-duration treadmill run to maximal intensity; the submaximal test (group S), a prolonged submaximal treadmill run to exhaustion. O2 uptake, heart rate, ventilation, and perceived exertion were determined during each test. Perception of pain was assessed after exercise by use of a modified McGill pain questionnaire. No significant differences between placebo and naloxone treatments were found in any of the measured variables at the usually accepted 5% (P = 0.05) confidence level; however, evidence suggesting differences (i.e., P = 0.1 to 0.05) in these important respects was observed. In group M, maximal exercise performance measured by maximal O2 consumption was not different between placebo and naloxone; results suggest that VE was increased (P = 0.08) following naloxone, but only at the final work stage. In group S, exercise performance time was reduced following naloxone (P = 0.09), whereas the affective component of pain was increased (P = 0.06); no differences in the measured physiological variables were observed. These results suggest the following: 1) the opiate receptor-endorphin system may alter the perception of pain associated with prolonged high-intensity submaximal exercise with a resultant significant effect on performance; and 2) it may play a role in the control of ventilation during maximal exercise.
