Atypical teratoid/rhabdoid tumors in children treated with multimodal therapies: The necessity of upfront radiotherapy after surgery.

Atypical teratoid/rhabdoid tumor (ATRT) is a rare malignant pediatric brain tumor with a dismal prognosis. We evaluated the efficacy of multimodal therapy in children with ATRT. Nine children diagnosed with cranial ATRT, who received multimodal therapy between 2005 and 2014, including surgical resection followed by radiotherapy (RT), systemic chemotherapy (CT), and high-dose chemotherapy/stem cell transplantation (HDCT/SCT), were analyzed retrospectively. The median age at diagnosis was 32 months. Initial treatment included surgery in eight patients (88%), CT in all (100%), RT in eight (88%), and HDCT/SCT in seven (78%). The median follow-up period for survivors was 21 months. The 2-year progression-free rate was 66.7%. Two patients had progression 4 and 17 months after diagnosis. One received multimodal treatment, including surgery and upfront CT with delayed RT; the other underwent surgery and upfront CT without RT. The 2-year event-free survival and overall survival rates were 46.7% and 62.2%, respectively. Hematologic toxicity of grade 3 or more was observed in six patients treated with HDCT/SCT and two who underwent craniospinal irradiation. Deaths were attributed to progressive disease (n = 2) and treatment-related toxicity (n = 2) from sepsis and acute respiratory failure after CT and HDCT/SCT. Maximal safe resection in conjunction with upfront RT is a reasonable multimodal treatment in patients with ATRT for prolonging progression-free survival. Further research may help determine the optimal parameters for reducing treatment toxicity, such as intensity of HDCT/SCT and the RT field.

Prognostic Parameters of Acute Myeloid Leukaemia at Presentation.

Introduction:The treatment response and outcome in acute myeloid leukaemia (AML) is heterogeneous.Aim:To analyze the prognostic parameters of AML at presentation.Methods:The total sample of 44 AML patients was analyzed on the basis of age <55 and ≥55 years, sex, WBC count <50x10/9/l and ≥50x10/9/l, the Hb concentration <100 g/l and ≥100 g/l, PLT count <100x10/9/l and ≥100x10/9/l, Karnofsky score <60% and >60%, cytogenetics, CD56 expression, morphological type and types of treatment (standard and reduced induction chemotherapy, high–dose chemotherapy/stem cell transplantation – autologous and HLA matched, related, allogeneic, together and separately).Results:The age <55 years, Karnofsky score >60% and standard induction chemotherapy statistically correlated with the higher complete remission (CR) rates, longer relapse free survival (RFS), lower relapse rate (RR), and longer overall survival (OS) (p<0.01). The difference in terms of CR and RR between the sexes were not statistically significant (p<0.05), however women had statistically lower OS comparing to men (9.71±4.54 months vs. 38.03±9.17 months) (p<0.01). WBC count ≥ 50x10/9/l and the Hb concentration <100 g/l statistically correlated with shorter OS (p<0.05), while the WBC count ≥50x10/9/l statistically correlated with shorter RFS (p<0.05). The PLT count <100x10/9/l and ≥100x10/9/l was not found as prognostically significant for CR, RR, RFS, and OS (p<0.05). In comparison to the standard induction chemotherapy, both types of high dose chemotherapy/stem cell transplantation (HDT/SCT) (10/22), together and separately, resulted in longer RFS, lower RR, and longer OS (p<0.05). The frequency of cytogenetic risk was intermediate 81.6%, unfavorable 13.2%, and favorable 5.3%, respectively. CD56 + expression statistically correlated with the lower PLT count, higher RR, shorter RFS, and shorter OS (p<0.05). Statistical analysis of the cytogenetic risk and morphological types of AML were not possible due to the small number of patients in stratified groups.Conclusions:Female sex, the WBC count >50x10/9/l, the concentration of Hb <100 g/l, and CD56 + expression, at presentation of AML, should be considered as parameters of adverse risk, especially in latter decisions considering post-remission treatment with HDT/SCT.

A Prospective Study of Gastric Emptying and Its Relationship to the Development of Nausea, Vomiting, and Anorexia after Autologous Stem Cell Transplantation

Gastric motor dysfunction may be responsible, in some patients, for the nausea and emesis that occur following high-dose chemotherapy (HDT) and autologous stem cell transplantation (SCT). We sought to define the prevalence of gastric emptying abnormalities and their relationship to the development of nausea, vomiting, and anorexia in patients undergoing HDT and autologous SCT. We prospectively studied patients with a variety of malignancies who received standard transplantation doses of chemotherapeutic agents and antiemetics. Gastric emptying was assessed prior to HDT and on Days 0 (day of stem cell infusion), +7, and +14 from SCT. Symptom assessment was obtained daily from initiation of HDT to 28 days after SCT. Twenty-four patients were studied. Prior to HDT, gastric emptying was rapid in two patients. Nausea, emesis, and anorexia occurred in all patients, peaked in severity at Day +7 after SCT and, with the exception of anorexia, had returned toward baseline levels by Day +28. As a group, gastric emptying was significantly slower on Days 0 and +7 and returned to baseline level by Day +14. Twenty-six percent and 44% of patients demonstrated delayed gastric emptying (T(1/2) >90 min) on Days 0 and +7, respectively, while 13% and 31% of patients had rapid gastric emptying (T(1/2) <30 min) on Days 0 and +7, respectively. Thirty-nine percent and 75% of patients had either rapid or delayed gastric emptying on Days 0 and +7, respectively. There was an association between delayed gastric emptying and moderate-severe anorexia on Day +7 and between delayed gastric emptying and at least mild vomiting on Day 0. Additionally, there was an association between rapid gastric emptying and at least mild vomiting on Day +7. Finally, an association was found between either rapid or delayed gastric emptying and at least mild nausea on Day +7. Both delayed and rapid gastric emptying occur commonly during the 2-wk period following HDT and autologous SCT and may be responsible, at least in part, for upper gastrointestinal symptoms that occur in these patients.

The only clinical variable that was predictive of symptom severity was gender. Women had a higher risk of developing anorexia (score > or = 2) at day +14 compared to men (odds ratio = 11.2; 95% CI = 1.7-76.9; p = 0.01). Baseline abnormalities in gastric myoelectrical activity occur frequently in patients who undergo HDT and autologous SCT despite normal gastric emptying scintigraphy and an absence of symptoms. Although slowing of the dominant frequency was seen as symptoms worsened, we failed to identify any EGG parameter at baseline that could predict the severity of nausea, vomiting or anorexia after transplantation.
