True hermaphroditism: A nightmare for an adult

Abstract The disorder of sex development is a rare disorder that usually occurs in early childhood. As adults, those with disorder of sexual development present with gynecomastia, primary amenorrhea, and primary infertility, which often causes great psychological impact. We report a unique case of a male adult hermaphrodite presenting with hematometra and hematosalpinx. Early management including psychiatry counseling, gender reassignment, and surgery is essential. Our patient underwent müllerian tissue removal with phallus reconstruction.

The gametogenic cycle and spawning of the mussel Brachidontes pharaonis (Fischer, 1876) (Bivalvia: Mytilidae) from Alexandria Coast, Egypt

The reproductive cycle and sexuality studies of the mussel Brachidontes pharaonis were carried out during the period from March 2016 to February 2017 along Alexandria Coast, Egypt. The gametogenic development of the gonads and the spawning duration under natural conditions were also studied. Histological preparations of a total of 307 individuals were examined monthly and the results showed that 44% were males, 51.8% females, 3.9% were sexually undifferentiated individuals and only one individual was hermaphrodite presenting 0.33% of the examined population, which indicates that B. pharaonis is gonochoristic. Histological studies showed that the gametogenic cycle could be divided into five stages of maturation. Also, an extended period of spawning starting from April to October was detected. Spawning activity of the studied species was in May to July and October. Gonad index reached its maximum in May, August and October when most of the individuals were in ripe and spawning activity. Sexes were distinct all over the studied size classes except those individuals in resting condition (stage 0). The smallest ripe gonads of both sexes were observed at 8.0–12.0 mm shell lengths. A case of true hermaphroditism presenting as a testicular tumour.

True hermaphroditism represents only 5% cases of all of disorders of sexual differentiation (DSD) and usually present in early childhood with ambiguous genitalia. Occasionally, cases might present later on in adolescence with problems of sexual maturation. Our case report presents a true hermaphrodite with normal male phenotype that presented as a left testicular mass, two years after being diagnosed with Sertoli cell only syndrome in the contralateral testis. Histological examination of the left testis showed ovarian, fallopian tube, myometrial, endometrial, and epididymal tissue. This combination of findings is found in approximately one-third of true hermaphrodites, but it is very rare to present clinically as an inguinoscrotal mass.

True hermaphrodite presenting as primary amenorrhea

True hermaphrodite is one of the rare variety of disorders of sex development. Most of them are genotypically females (46 XX) and present as under virilized males. Features of hyperandrogenism are present in those reared as females. The commonest histological variety of gonad found in them is ovotestis, which is at risk for malignancy. We report a girl with 46 XY true hermaphrodite without ovotestis presenting as primary amenorrhea and isolated clitoromegaly in the absence of any other features of virilization.

A 21-year-old patient with cryptorchidism was found to have a left inguinal mass on physical examination. The patient was operated with a diagnosis of bilateral cryptorchidism and left inguinal hernia. Besides bilateral inguinal undescended testicles, female genital organs like fallopian tubes, uterus and ovary were found on the exploration.

True hermaphroditism is a rare intersex disorder in which individuals possess both testicular and ovarian gonadal tissue. A case of true unilateral hermaphroditism presenting with ambiguous external genitalia, right scrotal testis and left pelvic ovotestis is herein outlined Phallic, gonadal and genetic factors were considered before male gender was assigned. Gender assignment procedures have been questioned by intersex activists opposed to early genital surgery. Western societies have a binary perspective on gender and this leads to a stigma being placed on intersex cases. A multidisciplinary approach to this problem involving paediatric specialists in the field, of endocrinology, surgery and psychiatry is necessary, along with educational programmes that promote tolerance in society to variations in gender.

To report a rare case of true hermaphroditism presenting in adolescence as bilateral gynecomastia in a phenotypic male. Case report. Academic medical center. A 19-year-old phenotypic male who presented with pronounced bilateral gynecomastia and was ultimately diagnosed with true hermaphroditism. Endoscopic removal of the internal female genital tract (hemiuterus and ovary), reduction mammoplasty, repair of penile chordee, and left testicular prosthesis placement. Diagnosis and treatment of true hermaphroditism. Bilateral mammoplasties, repair of a penile chordee, placement of a left testicular prosthesis, and excision of the left hemiuterus and ovary resulted in return to testicular function in this true hermaphrodite. Although rare, true hermaphroditism should be suspected in male patients presenting with bilateral breast enlargement in adolescence.

True hermaphroditism presenting as intermittent hematuria and groin pain

True hermaphroditism was diagnosed when a male was evaluated for inguinal swelling and hematuria. A true hermaphrodite rarely has bilaterally descended gonads. This patient had intrascrotal ovotestes. Diagnosis and management of the true hermaphrodite are reviewed.
