Data mining and safety analysis of traditional immunosuppressive drugs: a pharmacovigilance investigation based on the FAERS database

ABSTRACT Objective The purpose of this study aimed to explore the new and serious adverse events(AEs) of Tacrolimus(FK506), cyclosporine(CsA), azathioprine(AZA), mycophenolate mofetil(MMF), cyclophosphamide(CTX) and methotrexate(MTX), which have not been concerned. Methods The FAERS data from January 2016 and December 2022 were selected for disproportionality analysis to discover the potential risks of traditional immunosuppressive drugs. Results Compared with CsA, FK506 has more frequent transplant rejection, and is more related to renal impairment, COVID-19, cytomegalovirus infection and aspergillus infection. However, CsA has a high infection-related fatality rate. In addition, we also found some serious and rare AE in other drugs which were rarely reported in previous studies. For example, AZA is closely related to hepatosplenic T-cell lymphoma with high fatality rate and MTX is strongly related to hypofibrinogenemia. Conclusion The AEs report on this study confirmed that the results were basically consistent with the previous studies, but there were also some important safety signals that were inconsistent with or not mentioned in previous published studies. Expert opinion The opinion section discusses some of the limitations and shortcomings, proposing the areas where more effort should be invested in order to improve the safety of immunosuppressive drugs.

Ethnicity and socio-economic status affects the incidence and survival of hepatosplenic T-cell lymphoma.

To address the lack of contemporary population-based epidemiological studies of hepatosplenic T-cell lymphoma (HSTCL), we undertook a population-based study of ICD-O-3-coded HSTCL in England. We used the National Cancer Registration Dataset and linked datasets on hospital admissions, Systemic Anti-Cancer Therapy, socio-demographics, comorbidities and death, identifying cases from 1 January 2013 to 31 December 2019 with survival data up to 5 January 2021. Crude and directly age-standardised incidence rates per million persons per year were calculated. Crude and adjusted incidence rate ratios compared incidence between groups using Poisson regression. A Cox proportional hazards model estimated mortality risks adjusted for age, sex, ethnicity, deprivation and allogenic stem cell transplant (allo-SCT; time varying). We identified 44 patients, mean age 42 years. Median survival was 11 months, and 1 and 5 year survivals were 48% (95% CI 29%-43%) and 22% (95% CI 12%-42%) respectively. The age-standardised incidence was 0.1 per million/year. Incidence was higher in areas with greater deprivation (0.15 per million/year), and more cases than expected were in non-White patients (39%). Non-Whites had a twofold increased risk of death (adjusted hazard ratio 2.21 [95% CI 1.03-4.78]) even after adjusting for deprivation, younger age and allo-SCT. In conclusion, ethnicity and socio-economic status affect both the incidence and survival of HSTCL.

Flow Cytometry in Mature T- and NK-cell Neoplasms: A Retrospective Descriptive Study from a Tertiary Care Cancer Centre in Kerala, India

Introduction: Mature T- and NK-cell Neoplasms (MTNKN) constitute around 12% of all non-Hodgkin lymphomas. They have a variable clinical course, ranging from indolent to highly aggressive tumours. Flow cytometry immunophenotyping is crucial for the diagnosis, staging, and classification of MTNKN. Aim: To determine the frequency, morphologic, and immunophenotypic profile of various subtypes of MTNKN diagnosed by flow cytometry. Materials and Methods: This was a retrospective descriptive study conducted at the Department of Pathology, Regional Cancer Centre, Thiruvananthapuram, Kerala, India. All cases of MTNKN diagnosed by flow cytometry in peripheral blood, bone marrow aspirates, or body fluids from January 1, 2010 to June 30, 2020 (10.5 years duration) at the cancer centre were studied. The morphology of tumour cells and immunophenotype by flow cytometry were analysed. Clinical parameters (lymphadenopathy, hepatosplenomegaly, skin lesions, effusions, B symptoms) and follow-up details, including Progression-free Survival (PFS) and Overall Survival (OS), were also noted. Descriptive statistics for continuous variables and frequencies and percentages for categorical variables were obtained. The Kaplan-Meier method for survival plots was used. Results: Mature T- and NK-cell neoplasms constituted 83 cases. The median age of patients was 56 years. The majority of patients were males (n=49). Adult T-cell Leukaemia/Lymphoma (ATLL) (n=50) constituted the most common subtype, followed by Mycosis fungoides/Sezary syndrome (MF/SS) (n=14), T-cell Large Granular Lymphocytic Leukaemia (T-LGLL) (n=7), T-cell Prolymphocytic Leukaemia (T-PLL) (n=4), Aggressive NK-cell Leukaemia (ANKL) (n=3), Hepatosplenic T-cell Lymphoma (HSTL) (n=3), and Anaplastic Large Cell Lymphoma (ALCL) (n=2). OS and PFS at three years were 22.3% and 16.6%, respectively. Conclusion: Mature T- and NK-cell neoplasms presenting as leukaemia is rare. ATLL was the most common subtype of MTNKN. Flower cells, Sezary cells, and prolymphocytes are useful morphological clues for diagnosis. Immunophenotyping by flow cytometry, along with clinicopathologic correlation, is crucial for the diagnosis and subclassification of MTNKN. All subtypes except T-LGLL show inferior PFS and OS.

Epidemiology and Independent Prognostic Factors of Patients with Hepatosplenic T-Cell Lymphoma over the Past 2 Decades

Background: Hepatosplenic T-cell lymphoma (HSTCL) is a rare and aggressive peripheral T-cell lymphoma with historically dismal outcomes, representing less than one percent of non-Hodgkin lymphomas. Given its rarity, the true incidence of HSTCL is unknown and most data have been extrapolated through case reports. To the best of our knowledge, the largest and most up to date study addressing the epidemiology and outcomes of patients with HSTCL in the United States covered a period from 1996 to 2014, with a sample size of 122 patients. The aim of this study is to investigate the clinical characteristics, survival outcomes, and independent prognostic factors of patients with neuroblastoma over the past two decades with a larger sample size. Methods: A total of 186 patients diagnosed with HSTCL, between 2000 and 2017, were ultimately enrolled in our study by retrieving data from the Surveillance, Epidemiology, and End Results (SEER) database. We analyzed demographics, clinical characteristics, and overall mortality (OM) as well as cancer-specific mortality (CSM) of HSTCL. Variables with a p value <0.01 in the univariate Cox regression were incorporated into the multivariate Cox model to determine the independent prognostic factors, with a hazard ratio (HR) of greater than 1 representing adverse prognostic factors. Results: Our cohort had a male predominance (68.82%). Most patients were diagnosed between the age of 40- and 59-year-old (36.02%) and HSTCL was least common among patients 80 years old and older (6.45%). Non-Hispanic whites (60.75%) and non-Hispanic blacks (20.97%) were most represented. Univariate cox proportional hazard regression analyses of factors affecting all-cause mortality revealedhigher overall mortality among non-Hispanic black patients (HR= 1.97, 95% CI 1.27-3.07, p<0.01). CSM was higher among non-Hispanic blacks (HR= 2.34, 95% CI 1.47-3.72, p<0.01) and patients with distant metastasis (HR= 2.23, 95% CI 1.24-4.02, p<0.01). Multivariate cox proportional hazard regression analyses of factors affecting CSM revealedhigher mortality in patients aged 80 or older (HR= 4.72, 95% CI 1.14-19.54, p<0.05) and non-Hispanic blacks (HR= 2.02, 95% CI 1.08-3.7, p<0.05). Surgery, radiation and B symptoms did not affect the overall mortality, nor did they affect the CSM. Conclusion: Overall the prognosis of this rare malignancy is very dismal. In this United States population-based retrospective cohort study using the SEER database, we found that non-Hispanic blacks and patients 80 and above had a higher CSM. Furthermore, cancer directed surgery, marital status and distant metastasis did not affect mortality. While one would expect older patients to have a poorer prognosis, it remains unclear why one racial/ethnic group would be at higher risk as well. This data paves the way for larger prospective studies addressing factors associated with worse prognosis in non-Hispanic blacks, such as delay in treatment, which has been shown to worsen mortality in this racial/ethnic group in other malignancies.

Phase II Study of Lenalidomide Maintenance after Salvage Therapy for Relapsed or Refractory Peripheral T-Cell Lymphomas

Introduction Peripheral T-cell lymphomas (PTCLs) are a heterogeneous group of non-Hodgkin lymphomas. Their prognoses are still poor especially after they relapse or become refractory to the first-line treatment. Although salvage chemotherapy followed by autologous stem cell transplantation (ASCT) could be used for those relapsed or refractory patients, a substantial number of patients relapse even after ASCT. Furthermore, ASCT is not always possible because elderly or frail patients are not eligible for high-dose chemotherapy followed by ASCT. Thus, there are limited treatment option for patients who failed after ASCT or ineligible for ASCT because there is no established salvage treatment strategy and the impact of novel therapies in previous trials is unknown. Lenalidomide is widely used for the treatment of B-cell lymphomas as well as multiple myeloma, however, there are a few data about its efficacy in patients with PTCLs. Considering the mode of action is related with the immune modulatory effect on tumor microenvironment, lenalidomide might have a role in the management of relapsed or refractory PTCLs. Thus, we hypothesized that the use of lenalidomide as a maintenance therapy for patients who achieve response after salvage therapy might produce survival benefit comparable to that of ASCT after salvage therapy. Methods This is a prospective, open-labeled, single-arm, multi-center phase II study for relapsed or refractory PTCLs. The inclusion criteria are as follows: 1) Patients should have relapsed or refractory disease after at least one-line of prior chemotherapy; 2) Patients should not be eligible for ASCT; 3) Patients should have any subtypes of PTCLs including PTCL-not otherwise specified (PTCL-NOS), angioimmunoblastic T-cell lymphoma (AITL), anaplastic lymphoma kinase (ALK)-positive or -negative anaplastic large-cell lymphoma (ALCL), extranodal NK/T-cell lymphoma (ENKTL), monomorphic epitheliotropic intestinal T-cell lymphoma (MEITL), hepatosplenic T-cell lymphoma (HSTL), and mycosis fungoides (MF). When patients eligible for the study achieve complete response (CR) or partial response (PR) after four cycles of any kinds of salvage chemotherapy, they can receive lenalidomide as a maintenance therapy. One cycle is 28 days consisting of lenalidomide 25 mg (day 1 - day 21). The treatment can be repeated until progression or the occurrence of intolerable toxicities (up to 2 years). The primary end point is 1-year progression-free survival, and the target rate is 35%. Results A total of 58 patients were enrolled between February 2019 and May 2023. The median age was 62 years (range: 20 - 83 years) and male was dominant (male: 35, female: 23). Patients who failed after ASCT or could not undergo ASCT due to their health status participated in the study. Patients with AITL (n = 23) and PTCL-NOS (15) accounted for 66% of patients whereas other subtypes included ENKTL (n = 4), ALK-negative ALCL (n = 3), MEITL (n = 3), HSTL (n = 2) and MF (n = 7). Prior to the maintenance therapy with lenalidomide, their disease status was CR (n = 22, 38%) and PR (n = 36, 62%). At the median follow-up of 20.7 months (95% CI: 17.7 - 23.7 months), seven patients completed the planned treatment of 2 years, and three patients withdrew their informed consents. Disease progression was documented in 21 patients during the maintenance and the other 20 patients discontinued the treatment due to their adverse events. At the time of analysis, seven patients were maintaining their treatments. Out of 36 patients with PR before maintenance, five patients achieved CR and 13 patients showed PR. However, 18 patients showed disease progression during maintenance. On the other hand, only three patients showed disease progression among 22 patients with CR prior to lenalidomide maintenance. Thus, 1-year PFS after enrollment was 49% and the median OS was (34.1 months, 95% CI: 7.9 - 18.6 months, Figure 1). The most common hematologic toxicities were neutropenia and thrombocytopenia; thus, dose-reduction should be done in patients with grade 4 hematologic toxicity. The most frequent non-hematologic toxicities included skin rash (40%), nausea (35%), and diarrhea (20%) however, they were manageable. Conclusions The lenalidomide maintenance might be a feasible and effective treatment approach for patients with relapsed or refractory PTCLs who are ineligible for ASCT or intensive consolidation chemotherapy.

Pembrolizumab in Relapsed/Refractory Extranodal NK/T Cell Lymphoma and Mature T Cell Lymphoma: A Prospective Phase II Study

Background Pembrolizumab, a monoclonal antibody against programmed death 1 receptor (PD-1) has been shown to be efficacious in relapsed/refractory (R/R) extranodal NK/T cell lymphoma (ENKTL), with an overall response rate (ORR) up to 100% in retrospective case series 1. In R/R mature T cell lymphoma, an overall response rate of 33% has been demonstrated in a phase II study 2. In this prospective phase II study, we aim to evaluate the efficacy of pembrolizumab in patients with R/R mature T- and NK-cell lymphomas. Methods Patients with R/R mature T- and NK-cell lymphomas who failed at least one line of prior therapies were recruited. Pembrolizumab was administered at 200mg intravenously every three weeks for up to two years (35 cycles) unless there was intolerable toxicity or disease progression. PET/CT was performed after at least three cycles of treatment for response evaluation. The primary endpoints were the efficacy and safety of pembrolizumab treatment. Secondary endpoints included progression-free survival (PFS) and overall survival (OS). Response assessment was made according to published criteria 3. Survival was analyzed using Kaplan-Meier method. Statistical calculations were performed with SPSS Statistics version 28. Results ENKTL cohort: Ten women and six men at a median age of 57 (range: 41-83 years) were included. Fourteen patients had been treated with L-asparaginase-containing regimens. Ten patients (62.5%) had relapsed disease while six patients (37.5%) had refractory disease. Other relevant features included staging (I, N=3, 18.8%; II, N=3, 18.8%; IV, N=10, 62.5%). Response assessments were performed in all patients, and their best responses were as below: complete response (CR), N=7, 44%; partial response (PR), N=1, 6%; indeterminate response (IR), N=2, 13%; stable or progressive disease (SD/PD), N=6, 38%. At a median follow-up of 24 months (range 1-51 months), the median PFS was 10 months and the OS was 25 months (Figure 1). Adverse events included immunotherapy-related adverse events (IRAE) in five patients (31%), including two endocrine (one grade 1 and one grade 2 toxicity), two hepatic (both grade 2) and one gastrointestinal (grade 2). Haematological toxicity was mild with one patient having grade 1 anaemia. Mature T-cell lymphomas cohort: Four men and four women at a median age of 60 (range: 22-80 years) were included. Underlying diseases were peripheral T cell lymphoma, not otherwise specified (PTCL-NOS) (N=4, 50%), angioimmunoblastic T-cell lymphoma/PTCL with T-follicular helper phenotype (AITL/PTCL-TFH) (N=2, 25%), hepatosplenic T-cell lymphoma (HSTCL) (N=1, 12.5%) and systemic Epstein-Barr Virus (EBV)-positive T cell lymphoma of childhood (N=1, 12.5%). All patients had advanced-stage diseases (stage 3, N=1, 12.5%; stage 4, N=7, 87.5%). The median line of the prior regimens was 2 (range: 1-3). Response assessment was performed in seven patients, and their best responses were as follows: CR, N=1, 14.2%; IR, N=2, 28.4%; SD/PD, N=4, 57.1%. At a median follow-up period of nine months, the progression-free survival was 4 months and overall survival was 7 months. The only patient with a durable response was the one with systemic EBV-positive T-cell lymphoma of childhood. Toxicity included IRAE in two patients (endocrine, N=1, grade 1; pulmonary, N=1, grade 4) and haematological toxicity in three patients (37.5%) (anaemia, N=1, grade 2; neutropenia, N=1, grade 1; thrombocytopenia, N=1, grade 1). Conclusion Pembrolizumab is a safe and efficacious therapy for R/R ENKTL with potentially durable remission. In R/R mature T-cell lymphomas, the benefit of pembrolizumab treatment was limited. With the durable response observed in EBV-positive T-cell lymphoma of childhood, the use of pembrolizumab in EBV-positive T-cell lymphoma warrants further investigation. Reference: 1. Patterns of Care and Impact of Initial Treatment in Peripheral T-Cell Lymphoma: Outcome Analysis from the Lymphoma Epidemiology of Outcomes (LEO) and Molecular Epidemiology Resource (MER) Prospective Cohort Study

Background Peripheral T-cell lymphomas (PTCLs) comprise a rare, heterogeneous group of non-Hodgkin lymphomas of mature T-cell origin. Most systemic (s) PTCL subtypes typically have poor outcomes with conventional anthracycline-based chemotherapy. Biomarker-driven and subtype-specific treatments are promising but unmet needs. Few benchmark studies exist to characterize the current real-world landscape of clinical and pathologic practice. We report the patterns of care and updated outcome data for PTCL patients enrolled in the LEO-MER multi-center prospective cohort study (ClinicalTrials.gov NCT02736357). Methods Patients aged 18 years or older with newly diagnosed PTCL were prospectively enrolled in the University of Iowa/Mayo Clinic MER cohort (2002-2015) or the expanded LEO cohort (2015-2020). Clinical, pathology, treatment, and outcome data were abstracted from medical records using a standard protocol in both cohorts. Pathology underwent expert re-review based on WHO criteria. Overall survival (OS) was calculated from date of diagnosis to date of death or last follow-up. Event-free survival (EFS) was calculated from date of diagnosis to disease progression, initiation of 2 nd line therapy, or death from any cause. EFS and OS were evaluated using Cox model and Kaplan-Meier estimator. Log-rank test was used to test the significance of difference between groups. Results LEO-MER enrolled 1132 PTCL patients during the study period (462 MER and 670 LEO). Of these, 722 were sPTCLs (281 MER and 441 LEO), including PTCL-NOS (N=253, 35%), anaplastic large cell lymphoma (ALCL; N=180, 24.9% [74 ALK+, 106 ALK-]), angioimmunoblastic T-cell lymphoma (AITL, N=164, 22.7%), extranodal NK-TCL (N=50, 6.9%), adult T-cell leukemia/lymphoma (N=33, 4.6%), enteropathy-associated TCL (N=19, 2.6%), and hepatosplenic TCL (N=13, 1.8%) ( Table 1). Median age at diagnosis was 60 years. M:F ratio was 1.5. Comparing LEO to MER, African American population was 15.4% vs 0.7%, and Hispanic was 10% vs 2.2%. Clinical characteristics were comparable between MER and LEO overall: PS≥2, 20.9%; elevated LDH, 50.7%; stage III-IV, 67.6%; and IPI 2-5, 65.7%. The most common 1 st line chemotherapy regimens overall were CHOP-based (N=506, 70%), including 60% receiving CHOP-like chemotherapy in both MER and LEO cohorts (CHOP [N=268, 37%], CHOEP [N=91, 13%] or EPOCH [N=75, 10%]), and 16% with CHOP-like in combination with novel agents in LEO cohort (BV+ [N=41, 9.3%], azacitidine+ [N=11, 2.5%], pralatrexate+ [N=10, 2.3%], lenalidomide+ [N=8, 1.9%]) ( Table 1). More patients received etoposide in LEO (40.3%) than in MER (20.3%). 74 patients (10.3%) underwent consolidative SCT following induction chemotherapy (71 auto and 3 allo). 65 patients (11.2%) received frontline therapy on clinical trials. At a median follow-up of 9.4 years in MER and 4 years in LEO, 177 (MER) and 269 (LEO) events, and 150 (MER) and 206 (LEO) deaths were observed, respectively. The 3-year EFS and 4-year OS estimates for sPTCL were 46.3% and 59.1% in MER, and 40.4% and 51.3% in LEO. EFS and OS correlated with IPI scores and differed by sPTCL subtypes ( Figure 1). ALCL had superior survival, with 3-year EFS and 4-year OS of 78.2% and 90.5% for ALK+, and 65.9% and 76.9% for ALK-. Non-ALCL subtypes including PTCL-NOS and AITL had inferior outcomes, with 3-year EFS and 4-year OS of 31.4% and 42.8% for PTCL-NOS, and 33.4% and 50.7% for AITL. In subset analysis, the addition of etoposide to CHOP chemotherapy as either CHOEP or EPOCH was associated with improved OS for ALK- ALCL (p=0.038, log-rank), but not for ALK+ ALCL (p=0.75) or non-ALCL subtypes of PTCL-NOS and AITL (p=0.88). Conclusion The LEO-MER study is the largest prospective cohort study of PTCL to date. Patterns of care in the LEO cohort begin to incorporate novel agents in frontline therapy. Outcomes continue to mature with longitudinal follow-up and ongoing accrual, which poise to shape benchmarks in the contemporary era. The lack of benefit of etoposide adding to CHOP induction and poor overall survival of non-ALCL subtypes underscores the unmet need of therapeutic breakthrough for non-ALCL frontline treatment, particularly through clinical trials with biomarker-guided approaches incorporating novel agents.

Intent to Treat Allogeneic Stem Cell Transplantation Outcomes in Relapsed/Refractory T-Cell Lymphomas

Introduction T-cell lymphomas (TCL) are a heterogeneous group of lymphoproliferative malignancies with an overall poor prognosis. Despite aggressive first-line therapy, most patients experience disease relapse or progression. Allogeneic stem cell transplant (AlloSCT) is a standard of care for relapsed/refractory (R/R) TCL. However, many patients do not receive this potentially curative therapy. There is a paucity of data regarding the frequency of and barriers to undergoing AlloSCT in R/R TCL. To address this knowledge gap, we conducted a retrospective analysis of R/R TCL patients intending to undergo AlloSCT and hypothesized the primary barrier to AlloSCT to be lack of effective disease control. Methods We identified patients with a primary diagnosis of R/R TCL seen at the University of Washington/Fred Hutchinson Cancer Center (UW/FHCC) between 1/2008 and 6/2023 and linked these patients with the UW/FHCC bone marrow transplant (BMT) database to identify those who were intended for transplant. Intention to transplant (ITT) was defined as undergoing human leukocyte antigen (HLA) typing for purposes of AlloSCT. Patients with T-cell lymphoblastic leukemia/lymphoma were excluded. The primary endpoint was the rate of patients who underwent AlloSCT. Secondary endpoints included progression-free (PFS) and overall survival (OS) in patients who underwent AlloSCT and specific barriers to AlloSCT. Results Overall, 163 patients met ITT criteria. Males comprised 66% of patients. Median age was 65 (range 34-81) years and the median number of treatment regimens prior to HLA typing was 3 (range 1-10). The majority of patients (60%) were non-Hispanic whites (NHW), with Asians/Pacific Islanders (API), Hispanics/Latinos (HL), African Americans (AA) and Alaskan Natives/American Indians (ANAI) contributing 17%, 12%, 7% and 4%, respectively. Sixty-four patients (39%) had peripheral T-cell lymphoma, not otherwise specified (PTCL-NOS), 23 (14%) had angioimmunoblastic TCL (AITL), 17 (10%) had anaplastic large cell lymphoma (ALCL; 6 with ALK+), 16 (10%) had NK/TCL, 11 (7%) had hepatosplenic TCL, 12 (7%) had T-cell prolymphocytic leukemia, 5 (3%) had adult T-cell leukemia/lymphoma and 12 (7%) had primary cutaneous TCL. Twenty-two (13%) patients had secondary CNS involvement and 27 (17%) had previous autologous stem cell transplant. Of all patients who received HLA typing, 154 (94%) had a transplant consult, 54 (33%) initiated pre-AlloSCT workup and 51 (31%) underwent AlloSCT. Barriers to AlloSCT included progressive disease, patient preference, inability to identify a donor, indolent disease, poor functional status/multiple comorbidities and lack of a caregiver in 55%, 9%, 8%, 5%, 3% and 1%, respectively (Figure 1A). Six (5%) patients are currently receiving salvage therapy with plans for subsequent AlloSCT. AlloSCT occurred in NHW, API, HL, AA and ANAI patients at rates of 35%, 11%, 26%, 36% and 50%, respectively. All patients who were unable to undergo transplant due to lack of a donor were from groups other than NHW. AlloSCT rates were 30%, 39% and 17% in PTCL-NOS, AITL and ALCL, respectively. At a median follow-up time of 24.2 months, 101 (63%) patients are deceased with 83 (82%) due to disease. Twenty (39%), 17 (33%), 11 (22%) and 3 (6%) patients received an AlloSCT with a matched unrelated, matched related, mismatched unrelated and haploidentical donor, respectively. Fifteen (29%) patients underwent a myeloablative while 36 (71%) underwent a reduced intensity conditioning regimen. None of the 51 patients who underwent AlloSCT had pre-transplant measurable disease. Of transplanted patients, median PFS was 55.7 months and OS was 62.4 months (Figure 1B). Sixteen (31%) patients had eventual disease relapse. Eighty percent of relapses occurred within 3 months after AlloSCT. Seven (13%) and 6 (12%) of the transplanted patients were deceased within 12 months after AlloSCT due to disease relapse and complications of AlloSCT, respectively. Conclusions Although AlloSCT may be highly effective for R/R TCL achieving a CR, only a small minority of patients are able to undergo AlloSCT and the outcomes for the ITT population is dismal. Common barriers to undergoing AlloSCT include progressive disease and inability to identify suitable donors, particularly in minority populations. More effective pre-transplant salvage therapies and alternative AlloSCT donors are greatly needed.

Epidemiology and prognostic factors of hepatosplenic T-cell lymphoma (HSTL): An analysis of NCDB (National Cancer Database).

e19544 Background: Hepatosplenic T-cell lymphoma (HSTCL) is a rare T-cell malignancy, associated with rapid disease progression and generally poor outcomes. To date, few studies have been performed that examine outcomes expected survival outcomes and clinical features of this disease. We aimed to perform a large retrospective study with a focus on disease characteristics and overall survival outcomes. Methods: Utilizing the National Cancer Database (NCDB), we identified 434 patients with HSTCL 2004-2020. Patients were selected by ICD-0-3 histology code 9716. Kaplan-Meier curves and survival tables were created to calculate 1-, 5-year survival probabilities for variables including ages, race, sex, facility type, age, insurance, primary site, stage, and adjuvant therapy. Log-rank and cox regression analysis was used to compare the groups within each variable. Results: The cohort consisted of 68.9% male patients and 67.4% Caucasian patients with a median age of diagnosis of 50 years old. The most common type of insurance was private insurance 49.5% then Medicare 24%, Medicaid 16.4% and not insured 5.8% with median distance of 16.1 miles from hospital where they received care. Most patients (56.8%) were treated at academic/ research facilities, followed by comprehensive community cancer programs 22.8%. Mostly located in metro areas (85.5%) with median household income > $ 46000 (43.5%). More cases diagnosed since 2013. The most common primary anatomical site was spleen 91.7% followed by lymph nodes. Stage IV was the most common stage 77.2%t. Overall, 1-, 5- year survival probabilities for patients with HSTCL were 47.4% and 25.4% respectively. Median overall survival time is 11.24 months (95% CI 9.592-12.888). Female patients had better survival outcomes than males with median survival 20.6 months and 9.5 months, respectively. Academic/research centers (12.65 months) and Medicaid (14.98 months) followed by private insurance (11.79 months) had better survival outcomes. As stage increased, overall survival probabilities decreased; stage 1 median survival 12.71 months, stage 4 (11.24 months). Spleen (12.35 months) had better survival outcomes compared to lymph nodes. With regards to treatment modalities, patient who underwent bone marrow transplant (81.64 months) followed by surgery of primary site (14.92 months) followed by multiple agent chemotherapy (11.27 months) had better survival rates compared to all other treatment modalities. Conclusions: This is the largest and most comprehensive study to date on the epidemiology and prognostic factors of Hepatosplenic T cell Lymphoma. The result of our analysis brings up several interesting trends including females had significantly longer survival when compared to males with the same disease. Other major factors influencing survival are facility type, age, sex, race, insurance status, stage, adjuvant therapy and primary site.

A randomized phase II trial of CHOP versus ICED and autologous stem cell transplantation in newly diagnosed T‐cell lymphoma

Purpose: Peripheral T-cell lymphomas (PTCLs) are a heterogeneous group of non-Hodgkin lymphomas, and their prognoses are still poor because of frequent relapses and the absence of optimal standard therapy. Cyclophosphamide, doxorubicin, vincristine, and prednisone (CHOP) has been the backbone of frontline chemotherapy for PTCL for many years. However, it is not clear whether anthracycline-based chemotherapies such as CHOP could be standard induction therapy for PTCL. Thus, we conducted a randomized phase II study to compare the efficacy of CHOP with fractionated ifosfamide, carboplatin, etoposide, and dexamethasone (ICED). Patients and Methods: This study was a phase II, multicenter, open-label randomized trial at 21 hospitals that belonged to the Consortium for Improving Survival of Lymphoma (CISL) in Korea (CISL-1504/ROSE study). Eligible participants were patients aged 20–65 years with previously untreated histologically confirmed PTCLs based on the World Health Organization classification 2008 including PTCL-not otherwise specified (NOS), angioimmunoblastic T-cell lymphoma (AITL), ALK-negative anaplastic large-cell lymphoma (ALCL), enteropathy-associated T-cell lymphoma (EATL), and hepatosplenic T-cell lymphoma (HSTL). Patients with ALK-positive ALCL, extranodal NK/T-cell lymphoma, and mycosis fungoides/sezary syndrome were not included in the study. Patients were randomized at a 1:1 ratio to receive either CHOP or ICED every 3 weeks for 6 cycles. Upfront autologous stem cell transplantation (ASCT) was done for patients achieving complete or partial response and the primary end point was progression-free survival (PFS). Results: Between September 2015 and March 2021, 145 patients were screened, and 138 patients were enrolled. The characteristics were not different between CHOP (n = 69) and ICED (n = 66), and PTCL-NOS (n = 60) and AITL (n = 53) were dominant. The objective response rate was not different between CHOP (41/69, 59.4%) and ICED (37/66, 56.1%), and the 3-year PFS was not different between CHOP (36.7%) and ICED (33.1%, P = 0.709). Around 80% of patients who completed either CHOP or ICED followed by upfront ASCT showed 3-year overall survival (Figure 1). There was no statistically significant difference in PFS between the CHOP and ICED arms for any subgroup analyzed except AITL. CHOP was favored over ICED in AITL patients, whereas ICED was favored over CHOP in EATL/HSTL patients. ICED was associated with more anemia, neutropenia, and thrombocytopenia of all grades and grade 3 or worse, and the frequency of febrile neutropenia was higher in the ICED arm. Retrospective analysis of diagnosis and therapeutic strategies for patients with hepatosplenic T cell lymphoma.

Hepatosplenic T cell lymphoma (HSTCL) is a rare and aggressive lymphoma with no standard treatment and poor treatment response. From 2001-2021, 20 from a lymphoma cohort of 7247 patients (0.27%) were diagnosed with HSTCL at Samsung Medical Center. The median age at the time of diagnosis was 37.5 (range, 17-72) years, and 75.0% of patients were male. Most patients had B symptoms, hepatomegaly, and splenomegaly. Lymphadenopathy was found in only 31.6% of patients, and increased PET-CT uptake was found in 21.1% of patients. Thirteen patients (68.4%) expressed T cell receptor (TCR) γδ, and 6 patients (31.6%) expressed TCRαβ. The median progression-free survival (PFS) for the entire cohort was 7.2 months (95% CI, 2.9-12.8), and the median overall survival (OS) was 25.7 months (95% CI, not calculated). In subgroup analysis, the overall response rate (ORR) was 100.0% in the ICE/Dexa group and 53.8% in the anthracycline-based group, and the complete response rate was 83.3% in the ICE/Dexa group and 38.5% in the anthracycline-based group. The ORR was 50.0% in the TCRαβ group and 83.3% in the TCRγδ group. The OS was not reached in the autologous hematopoietic stem cell transplantation (HSCT) group and was 16.0 months (95% CI, 15.1-16.9) in the non-transplant group at the data cutoff time (P value 0.015). In conclusion, HSTCL is rare but has a very poor prognosis. The optimal treatment strategy is not defined. More genetic and biological information is needed.

Central Pontine Myelinolysis in a Patient with Hepatosplenic T Cell Lymphoma and Hemophagocytic Lymphohistiocytosis (P6-4.007)

<h3>Objective:</h3> To discuss a rare case of central pontine myelinolysis (CPM) in a patient with hepatosplenic T cell lymphoma (HSTL) and hemophagocytic lymphohistiocytosis (HLH). <h3>Background:</h3> HSTL is a rare disease characterized by infiltration of mature T cells within the sinusoids of the spleen, liver, and bone marrow. Hyponatremia prior to orthotopic liver transplant for liver failure has previously been identified as a risk factor for the development of CPM due to possible disruption of astrocyte metabolism and impaired capacity to regulate osmotic changes leading to secondary dysfunction of the blood brain barrier. CPM has also previously been discovered in EBV positive HLH. <h3>Design/Methods:</h3> We conducted a case report and literature review with PubMed search terms: 1) “central pontine myelinolysis,” “hyponatremia,” and “liver” 2) “central pontine myelinolysis,” “hemophagocytic lymphohistiocytosis,” and “hemophagocytic syndrome” 3) “central pontine myelinolysis” and “hepatosplenic T cell lymphoma.” <h3>Results:</h3> A 20-year-old EBV negative male presented with fever, splenomegaly, and blurry vision with bilateral retinal hemorrhages. Workup was significant for pancytopenia, mild hypovolemic hyponatremia secondary to emesis (initially 132 mmol/L), triglycerides 377, INR 1.2, ALT 55, and ALP 402. HSTL and HLH were confirmed on bone marrow biopsy. He was found to have subclinical CPM on MRI brain without contrast which resolved on 2 month follow-up imaging after 3 rounds of ifosfamide, carboplatin, and etoposide (ICE) intrathecal chemotherapy. We found no prior literature describing HSTL and CPM, 22 articles regarding CPM and liver transplant, and 1 article describing CPM and HLH. <h3>Conclusions:</h3> CPM should be considered as a neurologic complication in patients with HSTL and HLH. We report such a case, despite only mild levels of hyponatremia, the absence of liver failure, and negative EBV status. Future research should explore the connection between these factors to further our understanding of osmotic regulation of the blood brain barrier leading to CPM. <b>Disclosure:</b> Dr. Avila has nothing to disclose. An integrated cytomorphology and sequencing analysis for hepatosplenic alpha/beta T-cell lymphoma.

A 47-year-old male with a past medical history of diabetes mellitus under the control of oral hypoglycemic medications developed nausea and intermittent fever for weeks. Physical examination showed splenomegaly (4 cm below the costal margin) without significant lymphadenopathy. Laboratory data showed pancytopenia (Hemoglobin: 11 g/dl, White blood cells: 1.53 k/μl, Platelet: 47 k/μl), elevations of liver function tests (totalbilirubin: 1.38 mg/dl, direct bilirubin: 0.65 mg/dl, alanine transaminase (ALT): 56 U/L, aspartate transaminase (AST): 103 U/L, gamma-glutamyl transferase (GGT): 515 U/L, alkaline phosphatase (ALP): 260 U/L) and serum lactate dehydrogense (LDH) level (311 U/L, 1.1X). Flow cytometry of peripheral blood revealed that the abnormal lymphoid cells accounted for 4.3% of total nucleated cells, larger-than-normal lymphocytes, with positive expression of CD2, CD3, CD7, CD8, CD56 and negative for CD5 and CD45. Bone marrow (BM) aspirate revealed abundant lymphoma cells, med- to large-sized, with pleomorphic nuclei, prominent nucleoli, and coarse cytoplasmic granules (Figure 1). BM biopsy showed hypercellularity and the CD3+ lymphoma cells were negative for CD45, CD34, CD117, TdT, CD79a, and Epstein-Barr virus-encoded RNA (EBER) staining, with the pattern of sinusoidal infiltration (Figure S1). BM flow cytometry gating on the lymphoma cells confirmed the positive expression of CD2, CD3, CD7, CD8, CD56, T-cell receptor (TCR) alpha-beta chains, perforin, granzyme B, and negative for CD34, CD4, CD5, and TCR gamma-delta chains (Figure 2), consistent with natural killer (NK)/T lymphoma involvement. Cytogenetic analysis of the bone marrow was normo-karyotypic. Fluorodeoxyglucose-positron emission tomography (FDG-PET) scans revealed hypermetabolism in hepatosplenomegaly (maximum standardized uptake value (SUVmax) = 7.72 in spleen), bilateral cervical nodes, right supraclavicular nodes, right paratracheal nodes (SUVmax = 4.64), L4 (SUVmax = 5.03) and T9 vertebral bodies (SUVmax = 6.3) (Figure 3). Next-generation sequencing of BM TCR gamma gene VJ-recombination was indicative of monoclonality of T cells (59.52% of total reads) (Figure 4). This hepatosplenic T-cell lymphoma, alpha-beta type, Ann Arbor stage IV was thus diagnosed. He received three cycles of chemotherapy L-CHOEP (L-asparaginase, cyclophosphamide, doxorubicin, vincristine, etoposide, prednisolone) followed by a myeloablative matched sibling peripheral stem cell transplant. He remained free from relapse 14 months post-transplant with a normal hemogram and normal-metabolic FDG-PET scans. Genomic and phylogenetic characterization of Elizabethkingia anophelis strains: The first two cases of life-threatening infection in Japan

ObjectiveElizabethkingia anophelis causes meningitis, bloodstream infections, and respiratory infections in immunocompromised individuals. We examined two E. anophelis strains isolated from the first life-threatening cases caused by this species in Japan to determine the phylogenetic origin and genomic features of them. MethodsWe performed whole genome-based analysis to clarify the genetic relationship for the two strains (EK0004 and EK0079) and Elizabethkingia sp. strains isolated from worldwide and to characterize the genomic features such as the prevalence of virulence- and antimicrobial resistance (AMR)-related genes. PatientsA 29-year-old man with hepatosplenic T-cell lymphoma and a 52-year-old man with systemic lupus erythematosus developed fatal bacteremia and meningitis due to E. anophelis, respectively. ResultsTwo strains, EK0004 and EK0079, were genetically different but most closely related to the strains isolated from the largest outbreak in Wisconsin, USA from 2015 to 2016, and the strain isolated from cerebrospinal fluid of a patient in Florida, USA in 1982, respectively. The two strains contained AMR-related genes such as those encoding for an extended-spectrum β-lactamase and multiple metallo-β-lactamases and several virulence-related genes such as capsular polysaccharide synthesis gene clusters. ConclusionsAlthough further functional analyses are required to understand the virulence of these clones, these finding suggests that enough caution of E. anophelis infection in immunocompromised patients is required since the number of infections by this species is increasing outside Japan.

HLA Fully-Mismatched Sibling-Derived CD7 CAR-T Therapy Bridging to Haploidentical Hematopoietic Stem Cell Transplantation for Hepatosplenic γδ T-cell Lymphoma.

While chimeric antigen receptor (CAR)-T-cell therapy has demonstrated remarkable effectiveness in the treatment of B-cell lymphomas and leukemias, research on T-cell malignancies is still limited. Here, we reported a patient with hepatosplenic γδ T-cell lymphoma refractory to multiple lines of chemotherapy, who eventually achieved first complete remission with flow cytometry-confirmed minimal residual disease negativity after human leukocyte antigen (HLA) fully-mismatched sibling-derived CD7 CAR-T therapy. However, given the allogeneic nature, CAR-T cells dropped rapidly after a peak of 83.4% of circulating T-cells. Cytokine release syndrome, cytopenia, and infections occurred but were manageable after treatments. After the consolidative haploidentical hematopoietic stem cell transplantation (HSCT), the patient remained in remission at the end of the follow-up (13 months post-CAR-T infusion). This is the first case of relapsed/refractory hepatosplenic γδ T-cell lymphoma who achieved lasting CR after HLA fully-mismatched sibling-derived CD7 CAR-T therapy bridging to haploidentical HSCT.
