Predictors of professional help-seeking for emotional problems in Afghan and Iraqi refugees in Australia: findings from the Building a New Life in Australia Database

BackgroundRefugees are particularly vulnerable to poor mental health outcomes due to exposure to pre migration trauma and post migration stressors. Research has demonstrated evidence to suggest that the professional help-seeking among refugee groups is low or problematic. This study seeks to examine help-seeking for emotional problems in two large samples of Iraqi and Afghan refugees in Australia.MethodsThis study uses data from two waves of the Building a New Life in Australia, the longitudinal study of Humanitarian migrants. The data was collected face-to-face between 2013 and 2016, among humanitarian migrants. All participants held a permanent protection visa and had arrived in Australia or been granted their visa between period of May to December 2013. The study sample included 1288 participants born in Iraq and Afghanistan (aged 15 and over). In the Wave 3 interview (2015–2016) participants reported on professional help received to deal with emotional problems.ResultsApproximately 36 and 37% of the Iraqi and Afghan groups respectively, reported seeking help for emotional problems. Within the Iraqi group, associations between mental health status, namely general psychological distress and PTSD and help-seeking were found but this was not present in the Afghan group, where age seemed to play a role in help-seeking. Frequency of help received was low with approximately 47% of the Iraqi and 57% of the Afghan groups reporting having received help 5 times or less in the last 12 months.ConclusionsFindings from this study provide clear directions on areas where culturally tailored mental health promotion programs should target in these two refugee communities. Further, the differences in help-seeking behaviour of these communities should be noted by both clinicians and policy makers as efforts to provide culturally responsive mental health services.

British military women: combat exposure, deployment and mental health.

Internationally, women are about to undertake combat duties alongside their male colleagues. The psychological effect of this policy change is largely unknown. To explore the mental health impact of combat exposure among military women. Self-report, between-subjects survey data were collected in Iraq and Afghanistan on four occasions between 2009 and 2014 (n = 4139). Differences in mental health, stigmatization, deployment experiences, intimate relationship impact, perception of family support levels, unit cohesion, leadership and help-seeking were compared between deployed men and women. Comparisons were repeated with the study sample stratified by level of combat exposure. Outcomes were examined using logistic regression adjusted for socio-demographic, mental health and military factors. Overall, 4.1% of women and 4.3% of men reported post-traumatic stress disorder (PTSD) (odds ratio (OR) 1.31, 95% confidence interval (95% CI) 0.70-2.46); 22% of women and 16% of men reported symptoms of common mental disorder (CMD) (OR 1.52, 95% CI 1.11-2.08). Women were less likely to report mental health-related stigmatization (OR 0.68, 95% CI 0.53-0.87), negative relationship impact from deployment (OR 0.69, 95% CI 0.49-0.98) and subjective unit cohesion (OR 0.69, 95% CI 0.53-0.90). Help-seeking for emotional problems was similar by gender (OR 1.22, 95% CI 0.84-1.77). Overall, outcomes were minimally impacted by level of combat exposure. Although women experienced more CMD symptoms, PTSD symptoms were similar by gender. Subject to confirmation of the study findings, women may not require enhanced mental healthcare during deployment for exposure-based conditions such as PTSD when undertaking the ground close combat role.

Social support in depression: structural and functional factors, perceived control and help-seeking

Aims. This study examined the associations of social support, loneliness and locus of control with depression and help-seeking in persons with major depression. Methods. Twelve-month help-seeking for emotional problems was assessed in a cross-sectional 2006 Estonian Health Survey. Non-institutionalized individuals aged 18-84 years (n=6105) were interviewed. A major depressive episode was assessed using the Mini-International Neuropsychiatric Interview. Factors describing social support, social and emotional loneliness and locus of control were assessed, and their associations with depression were analysed. The associations with reported help-seeking behaviour among people identified as having a major depressive episode (n=343) were explored. Results. Low frequency of contacts with one's friends and parents, emotional loneliness, external locus of control and emotional dissatisfaction with couple relations were significant factors predicting depression in the multivariate model. External locus of control was associated with help-seeking in the depressed sample. Interactions of emotional loneliness, locus of control and frequency of contacts with parents significantly predicted help-seeking in the depressed sample. Conclusions. Depression is associated with structural and functional factors of social support and locus of control. Help-seeking of depressed persons depends on locus of control, interactions of emotional loneliness, locus of control and contacts with the parental family.

The Relationship of Social Support to African American Caregivers’ Help-Seeking for Emotional Problems

This study analyzes whether social support serves as a link to or substitute for formal services among African American female caregivers seeking help with emotional problems. It also analyzes other determinants of help-seeking. It relies on data from the Black Rural and Urban Caregivers Mental Health and Functioning Study and is guided by a modified version of the behavioral model of health services use. Using hierarchical binary logistic regression, analyses reveal that only age, stress, and support from fellow church members are statistically significantly associated with the likelihood of help-seeking. These results support the linking hypothesis, suggesting that the social support received by African American women caregivers in the context of their religious organizations helps to link them to services.

Prevalence and correlates of service utilization and help seeking in a national college sample of female rape victims

This study examines prevalence and correlates of help seeking for emotional problems among undergraduate female rape victims. A national college sample of women endorsing a lifetime history of rape ( N = 228) were interviewed in 2006 to assess demographic characteristics, rape history, rape characteristics, psychopathology, and substance abuse. Participants were asked if they ever sought help for emotional problems, and what type(s) of services were sought (medical professional, religious figure, or mental health professional). Prevalence of help seeking was 52%. Of help-seekers, 93% went to a mental health professional, 48% went to a medical doctor, and 14% sought religious counsel. Only PTSD was related to ever seeking help (OR = 2.35). Findings suggest that university-based mental health and medical facilities should be well prepared to identify and treat PTSD and other rape-related sequelae. Health promotion campaigns are needed to target substance abusing and depressed rape victims, who were less likely to seek help.

Pain and depression in older people: Comorbidity and patterns of help seeking

Background Chronic pain and mood disorders are common in older people. Their relationship however remains unclear. Only a few studies have investigated the role of pain in mental health service use and received psychopharmacological treatment. Methods Cross-sectional study of the 65+ subsample from the European Study on the Epidemiology of Mental Disorders (ESEMeD). 4401 non-institutionalized individuals were interviewed using the third version of the Composite International Diagnostic Interview (CIDI-3.0). Results Painful physical symptoms (PPS) were more likely in people with a 12-month major depressive episode (MDE) than in those without (OR = 2.0). Help seeking for emotional problems was uncommon, but PPS were a significant predictor of help-seeking (OR = 1.7). Respondents with MDE more frequently used benzodiazepines than antidepressants. The presence of PPS in respondents without depression resulted in a significant increase in the use of psychotropic medication. Conclusions PPS were strongly and independently associated with major depressive disorder. Their presence had an influence on help seeking behavior and use of psychotropic medication. Limitations The cross-sectional nature of this study does not allow determination of direction of causality.

Many women with mental health problems do not seek help. Despite substantial research on predictors of help seeking, little is known about factors associated with help seeking in at-risk populations (for example, rape victims). This study examined various forms of help seeking for emotional problems in relation to key variables in a national sample of female rape victims. Telephone interviews were conducted with a representative sample of 3,001 women (aged 18 to 76 years). All interviews were conducted between January 23, 2006, and June 26, 2006. Those endorsing a lifetime history of rape were included in the analyses presented here (N=556). Demographic characteristics, rape history, rape characteristics, psychopathology, and substance abuse were assessed. Help seeking was assessed by asking whether participants ever sought help for emotional problems, and if so, what types of services were sought (services provided through a medical professional, religious figure, or mental health professional). Help seeking was endorsed by 60% of the sample. The final multivariable model showed that ever seeking help was associated with being white (OR=2.61), being married (OR=.42), and having posttraumatic stress disorder (OR=3.45). Specific forms of help seeking revealed unique predictor sets. Although lifetime history of help seeking among rape victims was high, 40% of respondents had never sought help for emotional problems. Odds of help seeking were generally increased by the presence of a mental disorder. The findings suggest that some respondents may have sought help from general medical professionals or religious figures for emotional problems related to being raped. Education and training regarding rape and associated disorders should be available to other professionals to support provision of care. Public policy should be strengthened regarding professionals' service provision and should include reimbursement for mental health services so victims do not have to pay for services.

Clinical factors influencing the prescription of antidepressants and benzodiazepines:: Results from the European study of the epidemiology of mental disorders (ESEMeD)

Objective To examine factors associated with the use of antidepressants (AD) and benzodiazepines (BZD) in 6 European countries. Methods A cross-sectional, population-based study was conducted in: Belgium, France, Germany, Italy, the Netherlands and Spain. 21,425 non-institutionalized individuals aged 18 years and over were interviewed using the third version of the Composite International Interview (CIDI-3.0). Respondents were asked about AD and BZD use, and whether they consulted formal health services for emotional problems in the previous year. Sociodemographic variables, presence of mood/anxiety disorders and of painful physical symptoms were collected. Results 34.38% and 9.17% of the sample reported the use of AD and BZD respectively in the previous 12 months. Only 29.95% of subjects with a 12-month prevalence of major depressive episode (MDE) had been taking antidepressants. After controlling for several clinical and non-clinical factors, help seeking for emotional problems was the most important independent predictor for the use of AD or BZD (OR: 13.58 and 5.17, respectively). Higher age was the second important predictor (OR: 6.52 and 4.86, respectively). A 12-month or lifetime prevalence of MDE or an anxiety disorder were also predictors for AD or BZD use (OR for MDE: 5.00 and 2.82, OR for anxiety disorders: 2.13 and 1.85). Finally, the presence of painful physical symptoms also predicted the use of AD and BZD, while female gender, lower education and higher age predicted only the use of BZD. Conclusion Less than one third of subjects with a 12-month prevalence of MDE had been taking antidepressants. But seeking help for emotional problems was a more important predictor of the use of ADs or BZDs than a formal (DSM-IV) psychiatric diagnosis, suggesting that usage of ADs is not always according to the licensed DSM-IV indication.

Gender differences in factor structure of the Self-Administered Alcoholism Screening Test.

Recent studies have revealed differences between men and women alcoholics in symptoms, consequences, and help-seeking behavior related to alcohol usage. Based on these findings, it was hypothesized that gender differences also would appear on alcohol screening instruments. The Self-Administered Alcoholism Screening Test (Colligan, Davis, & Morse, 1988: SAAST: Swenson & Morse, 1975) of 1,920 men and 1,775 women was subjected to a within-gender, principle-components, factor analysis with a varimax rotation. Gender differences at the component level were revealed. Men endorsed the "help-seeking for alcohol-related problems" component while women endorsed the "help-seeking for emotional problems" component. In addition, men expressed concern about receiving a psychiatric label while women expressed concern about receiving a drinker label. The results suggest that different items need to be used in screening women for alcohol-related problems.

Why the Economy Predicts Help-Seeking: A Test of Competing Explanations

Mental health costs of economic -change are measured in a cross-level analysis that combines aggregate economic conditions (unemployment rate or absolute change in employment levels) with individual self-reports of undesirable job and financial events, psychosomatic symptoms, and help-seeking for emotional problems. The data were collected in Los Angeles County during 1978-80 in 12 quarterly surveys. Data for 4,144 Anglo respondents are analyzed in seven-way contingency tables to test two competing classes of explanations of the reported association between economic change and the use of mental health facilities. One class assumes that economic contraction affects utilization by increasing the incidence of disorder. The other class assumes that economic change affects utilization without increasing the incidence of disorder, but by influencing the decision to seek help for chronic or anticipated disorder. The results support versions of both classes of explanations. The support, however, is qualified by several factors that suggest that the widely-cited association between the economy and mental health is more complex than usually portrayed.
