The primary aim of this research is to assess how the involvement of clinical pharmacists can influence home health care patient’s outcomes, including medication management, adherence to prescribed therapies, and overall patient satisfaction. To achieve this, the study adopted a descriptive and quantitative approach. Data gathered through an electronic questionnaire specifically targeted at home health care patients, totaling 112 individuals, aiming to capture their experiences and perceptions regarding the contributions of clinical pharmacists to their health care. This questionnaire included questions that focus on the effectiveness, accessibility, and patient-centered care provided by clinical pharmacists.
