Dental health status of community-dwelling adults aged 50 years and over in Ireland. A cross-sectional analysis of the Wave 3 TILDA cohort.

Background: Little is known about the current oral health status of adults in Ireland. The aim of this study was to assess the dental health of community-dwelling adults aged 50 years and over in Ireland and to compare the current status to previous national surveys. Methods: The Irish Longitudinal Study on Ageing (TILDA) Wave 3 assessed the dental health of a subset of participants. Respondents attending for health assessments were offered a dental examination. The World Health Organization examination criteria were used. Results: Of the 3111 people who were offered the dental assessment, 2525 were examined. Adults below 50 years of age and respondents whose dental health data were unavailable at the time of analysis were omitted, giving a final sample of 2504. Among the dental assessment sample, 9.9% (249) were edentate. Of those aged 65 years and older, 15.6% were edentate while for the same age group 40.9% were edentate in the 2000-02 national survey. The mean number of teeth present in those aged 65 years or older was 14.9 for males and 14.2 for females, whereas in 2000-02 it was 9.9 and 7.4, respectively. 56.8% of the dentate sample had 10 or more tooth contacts. The mean DMFT of those aged 50 years or more was 18.5 and the Root Caries Index was 6.3. Between 2000-02 and 2014-5 (this study) in adults aged 65 years and over, the mean DMFT decreased from 25.9 to 20.1 and the Root Caries Index decreased from 11.6 to 9.1. Conclusion: The results indicate the dental health of community-dwelling adults aged 50 years and over in Ireland improved since the previous survey of 2000-02. These improvements mean a change in the treatment needs of this age group and will require policy and service adjustments to meet these needs.

Rehabilitation Interventions to modify endocrine-metabolic disease risk in Individuals with chronic Spinal cord injury living in the Community (RIISC): A systematic review and scoping perspective

Context: Endocrine-metabolic disease (EMD) risk following spinal cord injury (SCI) is associated with significant multi-morbidity (i.e. fracture, diabetes, heart disease), mortality, and economic burden. It is unclear to what extent rehabilitation interventions can modify EMD risk and improve health status in community-dwelling adults with chronic SCI. Objectives: To characterize rehabilitation interventions and summarize evidence on their efficacy/effectiveness to modify precursors to EMD risk in community-dwelling adults with chronic SCI. Methods: Systematic searches of MEDLINE PubMed, EMBASE Ovid, CINAHL, CDSR, and PsychInfo were completed. All randomized, quasi-experimental, and prospective controlled trials comparing rehabilitation/therapeutic interventions with control/placebo interventions in adults with chronic SCI were eligible. Two authors independently selected studies and abstracted data. Mean differences of change from baseline were reported for EMD risk outcomes. The GRADE approach was used to rate the quality of evidence. Results: Of 489 articles identified, 16 articles (11 studies; n=396) were eligible for inclusion. No studies assessed the effects of rehabilitation interventions on incident fragility fractures, heart disease, and/or diabetes. Individual studies reported that exercise and/or nutrition interventions could improve anthropometric indices, body composition/adiposity, and biomarkers. However, there were also reports of non-statistically significant between-group differences. Conclusions: There was very low-quality evidence that rehabilitation interventions can improve precursors to EMD risk in community-dwelling adults with chronic SCI. The small number of studies, imprecise estimates, and inconsistency across studies limited our ability to make conclusions. A high-quality longitudinal intervention trial is needed to inform community-based rehabilitation strategies for EMD risk after chronic SCI.

Acceptance of seniors towards automatic in home fall detection devices

Purpose Senior citizen falls are one of the highest-cost factors of healthcare within this population group. Various approaches for automatic fall detection exist. However, little is known about the seniors’ acceptance of these systems. The purpose of this paper is to investigate the acceptance of automatic fall detection devices as well as the technological commitment and the health status in community-dwelling adults with a predefined risk of falling. Design/methodology/approach Seniors with a risk of falling were equipped with either an accelerometer or an accelerometer with an additional visual and optical fall detection system in a sub-group of the study population for a period of eight weeks. Pre- and post-study questionnaires were used to assess attitudes and acceptance toward technology. Findings In total, 14 subjects with a mean age of 75.1 years completed the study. Acceptance toward all sensors was high and subjects were confident in their ability to handle technology. Medical assessments showed only very mild physical and no mental impairments. Measures that assured subjects privacy protection were welcomed. Sensor technology should be as unobtrusive as possible. Originality/value Privacy protection and uncomplicated use of the fall detection equipment led to high acceptance in seniors with high-technical commitment and good health status. Issues to further improve acceptance could be identified. Future research on different populations is necessary.

Oral health status in elders from South Brazil: a population‐based study

To assess the oral health status of community-dwelling adults aged 60 years and older from southern Brazil and to determine demographics, socioeconomic, behavioural and dental risk indicators. This cross-sectional study used a multistage, probability sampling method to draw a representative sample of the metropolitan area of Porto Alegre, Brazil. A subsample of 217 subjects was included in this analysis. Oral mucosal lesions, dental caries, tooth loss and periodontal status (full-mouth, six sites per tooth exam) were assessed by calibrated examiners. Prevalence of edentulism was 39.5%, and mean tooth loss was 20.2 (SE = 0.6). Older individuals [Odds Ratio (OR) = 2.2], women (OR = 2.3), white people (OR = 5.9), individuals of lower socioeconomic status (OR = 5.6) and smokers (OR = 3.5) had higher likelihood of being edentulous. Approximately 36% of dentate individuals had caries and/or restoration affecting, in average, 5.0 teeth. Periodontitis affected 79% of subjects, and it was associated with older age (OR = 4.0), men (OR = 3.4) and large amounts of supragingival plaque (OR = 3.0). Poor oral health was observed in this elderly population from South Brazil. Sociodemographic disparities accounted for most of the burden of disease and treatment needs.
