Influence of health information management practices on the quality of healthcare service delivery in OAUTHC Ile-Ife, Osun State Nigeri

Introduction: Medical records are crucial and vital in the continuity of patient care because physicians needs them to attend to patients each day and it is impossible for them to recall the details of each separate case at the same time. This study is therefore necessitated by the need to have a change of orientation about health records management practice and the profession and to have better work productivity and improved quality of care. The study aims at assessing the influence of health information management practices on the quality of health information management service delivery in OAUTHC Ile-Ife, Osun State Nigeria. Methods and Materials: A descriptive survey research design was conducted. Stratified random sampling technique was used to select 317 respondents for the study. The respondents includes; Health information managers, Doctors, Nurses, Medical laboratory scientists and Pharmacists. Results: Findings from the study shows that majority of the respondents assented that the quality of health information management practice in this institution is satisfactory (28.43%) and very satisfactory (68.30%). Study also shows that the greatest HIM service quality the patients and other services seekers enjoyed in the hospital is service reliability with the average response rate of mean 2.69. Conclusion: It is concluded from the study that health information management (HIM) practices have a great influence on the overall quality of healthcare services.

College students and patient work: Health information management by emerging young adults

College students with a life experience of chronic illness and impairments grow into young adulthood as health information monitors as well as communicators with healthcare professionals. This interview study focused on the health information management practices of college and graduate students living with disabilities and chronic illness. SEIPS (Systems Engineering Initiative for Patient Safety), a work system model drawn from human factors engineering, was used as a conceptual framework for qualitative analysis. This revealed the importance of other people, particularly parents, in patients' information worlds. Digital technologies were ubiquitous; analog tools from business cards to binders were also key components of students' information systems. Using the SEIPS structure to interrogate the data allowed an understanding of how health experiences are integrated into everyday life. The findings of this research may provide health information technology designers and information science researchers insight into how to meet the needs of this population.

Differences in Knowledge and Practice of Health Information Management among Health Care Managers in Urban and Rural Districts of Ghana: A Comparative Study

Health care managers in urban settings have a greater advantage over their rural counterparts when it comes to the needed essential training and logistics such as access to constant information technology and communication media to ensure effective health information management among health facilities. This assumption perceived health care managers in urban settings to have adequate knowledge and practice of health information management compared to their rural counterparts. This study, compared means scores of knowledge and practice of health information management among rural and urban health care managers in 67 conveniently selected government health facilities from 1st February to 10th March 2022 in the Ashanti region of Ghana. The study consisted of 37 (55.2%) urban and 30 (44.8%) rural managers of government health facilities (N= 67). Adequate health information management knowledge levels for rural and urban settings were 36.7% and 34.3% respectively. The level of adherence to good health information management practices for rural and urban were 53.3% and 43.2% respectively. The study revealed no statistical difference in the mean scores for knowledge t(65) = -0.08, p = 0.94 and practice t(65) = -0.59, p = 0.56 of health information management between rural and urban settings. Continuous capacity building tailored to health information management is paramount to ensure adequate knowledge and practice to improve health information practice among health facility managers in both rural and urban settings.

Findings from the 2021 Yearbook Section on Health Information Management.

SummaryObjectives: To summarize the recent literature and research and present a selection of the best papers published in 2020 in the field of Health Information Management (HIM) and Health Informatics. Methods: A systematic review of the literature for the IMIA Yearbook HIM section was performed by the two section editors with the help of a medical librarian. We searched bibliographic databases for HIM-related papers using both MeSH headings and keywords in titles and abstracts. A shortlist of the fifteen best candidate papers was first selected by section editors before being peer-reviewed by independent external reviewers. Results: The three major themes of Health Information Exchange (transmitting, sharing, and accessing patient health-related data and information) (HIE), Data Quality, and Privacy and Security make up 80% of the fifteen papers, with individual papers on personal health records, information governance and the professionalism of the HIM field. Conclusions: Traditional HIM concerns about HIM practice and workforce as well as issues about the data in electronic health records (EHRs) including data quality, coding, health information exchange among entities within the healthcare systems and privacy and confidentiality continue to be a large part of the HIM research literature. Although there was little research applying these themes to pandemic concerns, HIM professionals have the expertise to make ccontributions to public health informatics research and this research would benefit from their involvement.

TECHNOLOGY APPLICATION IN HEALTH INFORMATION MANAGEMENT IN OBAFEMI AWOLOWO UNIVERSITY TEACHING HOSPITALS COMPLEX (OAUTHC) ILE-IFE, OSUN STATE

This study focused on technology application in Health Information Management in Obafemi Awolowo University Hospitals Complex Ile-Ife. A simple random sampling technique was employed for the selection of respondents for the study. A total of Seventy eight (78) respondents across the department was randomly selected for the study. Descriptive statistics was employed in the analysis of data collected for the study. The reliability test of the instrument shows a (?= 0.9 for staff level of awareness of technology use in health information management practice), (?= 0.8 for areas of technology application in health information practice), and for (?= 0.9 for barriers of technology application in health information practice).The findings showed the majority of the respondent had good knowledge /aware of technology use in health information practices and the need for its application in the day-to-day administration of the hospital activities. The finding shows areas of technology application include: coding and indexing of clinical diagnosis, maintenance and records keeping, information processing and decision making, reduction of medical cost. The findings showed that majority of the respondents (80.8%) identified such barriers as militating technology used in health information management in OAUTHC. The study concluded that technology has the potential of enhancing job satisfaction of health information management professionals. Lastly the study recommended that other departments in the hospital should also embark on similar study so as to have an overview of the impact of technology application in other departments of the hospitals for a generalized assessment

Findings from the Health Information Management Section of the 2020 International Medical Informatics Association Yearbook.

Summary Objectives : To summarize the recent literature and research and present a selection of the best papers published in 2019 in the field of Health Information Management (HIM) and Health Informatics. Methods : A systematic review of the literature was performed by the two section editors with the help of a medical librarian. A shortlist of 15 candidate best papers was first selected by section editors before being peer-reviewed by independent external reviewers. Results : Over half of the 15 papers addressed the issue of data quality in the electronic health record (EHR). In addition to the focus on data quality, there were papers on other topics of long-standing interest to the field of HIM. These topics include privacy, security, and confidentiality of health information, comparability of different coding vocabularies, classifications and terminologies, and the HIM workforce. Finally, there were papers on newer topics for the HIM field, including mobile Health (mHealth), EHR use by public health departments, and usability of different strategies for displaying information in the EHR. Conclusions : Traditional HIM concerns about HIM practice and workforce as well as issues about data in the EHR including data quality, coding, and privacy and confidentiality continue to be a large part of the HIM research literature. However, newer topics which reflect innovative and emerging technologies, usability assessments, and the application of the EHR outside the traditional clinical setting are starting to appear and more research is needed on these newer areas.

A false promise of COVID-19 'big' health data? Health data integrity and the ethics and realities of Australia's health information management practice.

Context: Coronavirus disease (COVID-19) caused by severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) has precipitated an unprecedented volume of medical research. Articles reporting two studies were recently retracted from prestigious journals for reasons including the (thus far) unverifiable provenance of data. This commentary adopts a health information management lens to focus on aspects of data in one of the studies (investigating the use of hydroxychloroquine or chloroquine with or without a macrolide for treatment of COVID-19). The issues: Referencing the Australian context, the current article considers some of the study's reported hospital administrative and coded data categories within the context of Australian hospitals' health information management practices. It highlights potential risks associated with the collection and interpretation of 'big' health data. Implications: This article identifies pitfalls that confront researchers undertaking multi-country studies and the need to consider country-specific: (i) collected administrative data items; (ii) health information-related ethical, legal and management policy constraints on the use of confidential hospital records and derived data; and (iii) differences in health classification systems and versions used in the coding of diagnoses and related procedures, interventions and health behaviours. Conclusions: The article concludes that the inclusion of a qualified, senior Health Information Manager in research teams and on institutional Human Research Ethics Committees would help to prevent potential problems.

Personal Health Information Management Practices of Older Adults: One Size Does Not Fit All.

Older adults are the largest consumers of health care, have the greatest number of chronic conditions, and generate the greatest amount of health data. Yet, information systems designed to aid health information management do not align with their needs and practices. We describe a process of identifying the personal health information management (PHIM) activities and objectives of older adults (60 years and older) from different residential settings.

Health Information Management: Implications of Artificial Intelligence on Healthcare Data and Information Management.

Summary Objective: This paper explores the implications of artificial intelligence (AI) on the management of healthcare data and information and how AI technologies will affect the responsibilities and work of health information management (HIM) professionals. Methods: A literature review was conducted of both peer-reviewed literature and published opinions on current and future use of AI technology to collect, store, and use healthcare data. The authors also sought insights from key HIM leaders via semi-structured interviews conducted both on the phone and by email. Results: The following HIM practices are impacted by AI technologies: 1) Automated medical coding and capturing AI-based information; 2) Healthcare data management and data governance; 3) Fbtient privacy and confidentiality; and 4) HIM workforce training and education. Discussion: HIM professionals must focus on improving the quality of coded data that is being used to develop AI applications. HIM professional’s ability to identify data patterns will be an important skill as automation advances, though additional skills in data analysis tools and techniques are needed. In addition, HIM professionals should consider how current patient privacy practices apply to AI application, development, and use. Conclusions: AI technology will continue to evolve as will the role of HIM professionals who are in a unique position to take on emerging roles with their depth of knowledge on the sources and origins of healthcare data. The challenge for HIM professionals is to identify leading practices for the management of healthcare data and information in an AI-enabled world.

Understanding health information management practices in public hospitals in Kuwait.

Health information technology (IT) solutions can aid healthcare reform efforts, but without proper information management, these efforts are futile. In this study, we used Kuwait as an example of a high per-capita gross domestic product country that faces information management challenges to draw insights that can be generalised to other developed countries. (i) to uncover the status quo of information management practices in public hospitals and (ii) to offer recommendations to improve them. This study analysed qualitative and quantitative accreditation-related data pertaining to the compliance with the information management standard at all secondary care public hospitals over two accreditation cycles. Overall, public hospitals had made positive progress in their compliance with the information management standard. However, issues still existed with (i) developing and implementing an information management plan, (ii) involving the appropriate stakeholders in selecting health IT solutions and (iii) access to the Internet by staff and patients. Evidence underscored the importance of proper information management driven by clear centralised strategic plans. With the rapid adoption of digital health systems, the role of health information management leaders should not be undervalued. Embracing health IT solutions with strong information management practices can aid healthcare reform efforts.

Health information management practices in informal caregiving: An artifacts analysis and implications for IT design

IntroductionUnpaid informal caregivers of adult care recipients, including persons with dementia, experience multiple unmet information needs and information management challenges. ObjectivesTo understand the current personal health information management (PHIM) practices in informal caregiving for adults with and without dementia. MethodsSemi-structured interviews were performed with ten informal caregivers—half of whom were caring for persons with dementia—and four formal caregivers at an adult day service. Interviews centered on a paper-based tool distributed by the day service, the CARE Kit, permitting an artifacts analysis of the tools used by participants for PHIM. Qualitative thematic analysis was applied to interview data. ResultsCaregivers’ PHIM practices aimed to support daily care management and decision-making on behalf of care recipients, through: 1) information acquisition and integration across multiple sources and records; 2) information maintenance, updating, and use over time; and 3) information sharing and communication with healthcare professionals and other family caregivers. Participants reported advantages and challenges of their PHIM practices and tools, including fitting PHIM into their daily lives, managing PHIM-related cognitive workload, the functionality of PHIM tools, and the dynamic, longitudinal nature of PHIM. ConclusionThe study produced a number of implications for caregiver health information management information technology (CHIM IT), based on findings about the nature of caregivers’ practices for managing information for adult care recipients. We present CHIM IT requirements related to privacy and security, customization and flexibility, ease of use, credibility and sensitivity, situation awareness, information integration, delegation and shared use, updating and maintenance, archiving and versioning, communication, agency and information access, and validation.

Repositioning health information management practice in Nigeria: Suggestions for Africa.

The significance of health information management practice to the effectiveness of a healthcare delivery system cannot be overemphasised. A well-structured and coordinated health information management system has been known to generate the information needed for decision-making at all levels of healthcare delivery. However, the state of health information management in Nigeria, as is the case in most African countries, is a cause for concern. Observation and past studies have highlighted challenges facing the practice of health information management in Africa to be centred around the quality of professional training, inadequately qualified practitioners, disgruntled practitioners, government's indifference towards the practice, lack of policies and inadequate technological infrastructure among others. This article examines some of the challenges facing health information management practice in Nigeria and makes recommendations that may uplift the profession.

Improving the Curriculum with Ethics: Gaps between Perceived Ethical Challenges of Practitioners and Educators

Health information management (HIM) professionals must address ethical challenges in their role as guardians ofpatients’ personal information and organizations’ proprietary information. Because of this need, HIM educatorsstrive prepare their students to address these challenges. Unfortunately, little evidence exists about specific areas ofapplied ethics that should be part of the HIM curriculum. The purpose of this exploratory study was to determine theethical issues relevant to the current practice in Health Information Management. In particular, the study aimed todetermine if the perceptions of HIM educators and practitioners regarding HIM workplace ethical issues wereconsistent, and to identify gaps in perceptions. The researchers distributed a survey to HIM educators and HIMpractitioners, 12 educators and 25 practitioners completed their surveys. Thematic content analysis indicated adivergence of themes among these groups. Overall, educators' responses reflected general knowledge areas whereaspractitioners’ comments were more specific in terms of ethics concerns. The study findings indicated a gap inperceptions signalling a need for increased communication regarding applied ethics in HIM practice so thatcurriculums could reflect both theory and application.

Health@Home: The Work of Health Information Management in the Household (HIMH): Implications for Consumer Health Informatics (CHI) Innovations

ObjectiveContemporary health care places enormous health information management demands on laypeople. Insights into their skills and habits complements current developments in consumer health innovations, including personal health records. Using a five-element human factors model of work, health information management in the household (HIMH) is characterized by the tasks completed by individuals within household organizations, using certain tools and technologies in a given physical environment. DesignWe conducted a descriptive-exploratory study of the work of HIMH, involving 49 community-dwelling volunteers from a rural Midwestern community. MeasurementsDuring in-person interviews, we collected data using semistructured questionnaires and photographs of artifacts used for HIMH. ResultsThe work of HIMH is largely the responsibility of a single individual, primarily engaged in the tasks of acquiring, managing, and organizing a diverse set of health information. Paper-based tools are most common, and residents develop strategies for storing information in the household environment aligned with anticipated use. Affiliative relationships, e.g., parent-child or spousal, within the household serve as the organization that gives rise to health information management practices. Synthesis of these findings led to identification of several storage strategies employed in HIMH. These strategies are labeled “just-in-time,” “just-because,” “just-in-case,” and “just-at-hand,” reflecting location of the artifacts of health information and anticipated urgency in the need to retrieve it. ConclusionLaypeople develop and employ robust, complex strategies for managing health information in the home. Capitalizing on these strategies will complement and extend current consumer health innovations to provide functional support to people who face increasing demands to manage personal health information.

Health Information Management Practice in Sri Lanka.

Health information system development in Sri Lanka has generally been a low priority. Weaknesses exist in many areas, including vital registration, and hospital information such as inpatient and outpatient morbidity and mortality data. Lapses in hospital information systems are mainly due to incomplete patient medical records; non-availability, inaccuracy and illegibility of the final diagnoses on the front sheets; shortage of trained statistical staff; lack of supervision at all levels; and lack of facilities at medical record departments. WHO's South-East Asian Regional Organization, in collaboration with the Australian National Centre in Classification in Health, has developed a group of trainers who conduct MR/HIM and ICD-10 training courses within the country.

Health information management in Singapore.

This paper provides an overview of health information management practices in Singapore, based on personal observation and interviews with health information managers in five Singaporean hospitals. In Singapore borrowing a medical record often requires a formal request and a permanent tracer is maintained for each record. Medical officers generally have more responsibility than in Australia for coding, which is completed within three days of discharge. A Singapore-wide centralised patient database has been developed which is an important source of information about patient drug allergies. Record design and assembly, release of information and centralised registries are also described.
