Exposure to Ionizing Radiation from Initial Brain Computed Tomography for Injuries in the South-West Region of Cameroon

The increasing use of computed tomography (CT) in injury management is a growing concern as ionizing radiation (IR) is associated with cancer-related risks especially in young persons, who are most affected by injuries. This study aimed to assess ionizing radiation exposure from head CT in patients with injuries and to propose CT dose age-specific diagnostic reference levels (DRLs). Data was extracted from two prospectively collected CT registries over a period of 5 years at two community-based university-affiliated hospitals from 2019 to 2024. The linear relationship between CT dose (dose-length product; DLP) and age was assessed using Pearson’s correlation whilst linear regression was used to determine the strength of the relationship. The 75th percentiles and 95% confidence intervals (CIs) of the DLP were determined. Eligible initial head CT scans for patients with injuries were 1,155. There were 685 (59.31%) males (sex ratio of 1.5:1) and overall median age of 33 years (interquartile range: 21 to 46). Road traffic injuries were the source of injury in 1090 individuals (94.37%; 95% CI: 92.88 - 95.63%), followed by falls (50 cases; 4.33%; 95% CI: 3.23 - 5.67%) and assaults (10 cases; 0.87%; 95% CI: 0.42 – 1.59). The median DLP was 1,062 mGy.cm (range: 264 to 1,954 mGy.cm). There was a positive linear relationship between the DLP and the age of the patients (Pearson’s rho = 0.38, p<0.001). DRLs were comparable to international values. Measures to curb the rising incidence of injuries and the continuous implementation of CT optimization techniques will reduce IR exposure.

Association Between Head Computed Tomography Findings and In-Hospital Mortality in COVID-19 Patients.

The present study investigated the association between head computed tomography (CT) findings and mortality in patients with COVID-19. Specifically, we focused on low-density lesions identified on head CT screenings. We performed a single-center, retrospective cohort study based on data obtained from the medical charts of inpatients admitted to the Tokyo Metropolitan Hiroo Hospital between January 1 and December 31, 2021. We focused on the basal ganglia--a representative anatomical region for assessing routine head CT in patients with COVID-19. Patients were divided into two groups based on the presence or absence of low-density lesions in the basal ganglia. The primary outcome was all-cause in-hospital mortality, and the secondary outcome was the length of hospital stay. We performed multivariable regression analyses for outcomes to adjust for patients' background and disease severity. During the study period, 1,906 COVID-19 patients were admitted to our facility. Among them, 1,203 patients underwent head CT evaluations and were included in this study. The median age was 56 years (interquartile range: 43-76 years) and 725 patients (60.3%) were male. A total of 235 (19.5%) patients required oxygen therapy on admission and 1,051 (87.4%) patients had pneumonia. Crude in-hospital mortality was 6.1% and the median length of hospital stay was 10 days (interquartile range: 8-14 days). The multivariate regression analyses showed that low-density lesions in the basal ganglia were significantly associated with increased in-hospital mortality and prolonged hospital stay. The presence of ischemic changes in the basal ganglia denoted by low-density findings may be a promising prognostic factor in patients with COVID-19.

Lowering head computed tomography radiation dose for craniosynostosis: An institutional change and review of literature

Definitive diagnosis of Craniosynostosis (CS) with computed tomography (CT) is readily available, however, exposure to ionizing radiation is often a hard stop for parents and practitioners. Lowering head CT radiation exposure helps mitigate risks and improves diagnostic utilization. The purpose of the study is to quantify radiation exposure from head CT in patients with CS using a ‘new’ (ultra-low dose) protocol; compare prior standard CT protocol; summarize published reports on cumulative radiation doses from pediatric head CT scans utilizing other low-dose protocols. A retrospective study was conducted on patients undergoing surgical correction of CS, aged less than 2 years, between August 2014 and February 2022. Cumulative effective dose (CED) in mSv was calculated, descriptive statistics were performed, and mean ± SD was reported. A literature search was conducted describing cumulative radiation exposure from head CT in pediatric patients and analyzed for ionizing radiation measurements. Forty-four patients met inclusion criteria: 17 females and 27 males. Patients who obtained head CT using the ‘New’ protocol resulted in lower CED exposure of 0.32 mSv ± 0.07 compared to the prior standard protocol at 5.25 mSv ± 2.79 (p < 0.0001). Five studies specifically investigated the reduction of ionizing radiation from CT scans in patients with CS via the utilization of low-dose CT protocols. These studies displayed overall CED values ranging from 0.015 mSv to 0.77 mSv. Our new CT protocol resulted in 94% reduction of ionizing radiation. Ultra-low dose CT protocols provide similar diagnostic data without loss of bone differentiation in CS and can be easily incorporated into the workflow of a children’s hospital.

The independent risk factors for abnormal head computed tomography in patients with hepatic encephalopathy

It's known that head computed tomography (CT) is used excessively to exclude intracranial hemorrhage in patients with hepatic encephalopathy (HE) in the emergency department. However, the independent risk factors for abnormal head CT in patients with HE have not been studied extensively to date. In this retrospective study, patients with an ammonia level of >90 U/L who were clinically considered HE and had head CT were included. The characteristics of patients with abnormal head CT and independent risk factors for abnormal CT were investigated. Three hundred seventy-eight patients were included in the study. CT findings of 18 (4.8%) of the patients were abnormal: 12 had intracranial hemorrhage, 1 had an ischemic stroke, and 5 had an intracranial mass. Intracranial hemorrhage (odds ratio [OR] 12.5), history of recent trauma (OR 23.4), history of active malignancy (OR 10.3), thrombocyte count <100.000/μL (OR 4.3), and international normalized ratio ≥1.5 (OR 3.2) were found to be independent risk factors for abnormal head CT. Head CT scan may be considered in patients with HE if any of the following are present: intracranial bleeding history, recent trauma history, active malignancy, platelet count <100,000/μL, and international normalized ratio >1.5.

Repeat CT after blunt head trauma and Glasgow Coma Scale score 13-15 without neurological deterioration is very low yield for intervention.

Due to the increase in accessibility of computed tomography (CT), repeat head CT scans are routinely ordered for patients with minor head injuries. The aim of this study is to evaluate the necessity and outcomes of routine repeat head CT in patients with GCS score of 13-15 who presented to the emergency department (ED) of Antalya University Hospital in Turkey with blunt head trauma. We retrospectively reviewed the charts of patients with minor head trauma that received initial and repeat head CT results from July 1, 2013 to June 30, 2015. Clinical characteristics of patients were compared for two groups of patients: those with neurological deterioration, and those who had routine head CT not required by change in neurological status. Repeat head CT results were analyzed for radiological worsening and the necessity of a surgical or medical intervention such as craniotomy, ICP monitoring, VP shunt and mannitol or hypertonic saline administration. Of 3578 patients with blunt head trauma, 656 (18.3%) patients had repeat head CT; 449 of these (68.4%) had a GCS score of 13-15. We analyzed 441 patients for CT and clinical changes. Eight patients were excluded because of poor image quality and/or penetrating injury. Neurological deterioration was the reason for repeat head CT in 73 (16.5%) patients Rates of medical (mannitol treatment) or surgical (craniotomy) intervention in this group were 26% (95% Confidence Interval [95% CI], 15.7-36.3%) in contrast to 0.8% (95% CI 0.1-1.7%) in the group of patients with routinely ordered head CT but without clinical deterioration. The following factors were statistically associated with need for intervention: use of anticoagulant or antithrombotic medication, fracture in middle meningeal artery territory, even a single point decrease in GCS score, increased headache, recurrent vomiting, neurological deficit, and finally, changes in repeat head CT. In patients with minor head injuries, those without neurological deterioration have a very low risk of need for medical or surgical intervention. Routinely ordering repeat head CT scans in this group may not be routinely indicated.

HEMORRHAGIC PROGRESSION OF CONTUSION IN PATIENTS WITH MILD TRAUMATIC BRAIN INJURY ON THE ROUTINE REPEAT HEAD COMPUTED TOMOGRAPHY

Computed tomography (CT) scan is a standard for the diagnosis of intracranial pathology after traumatic brain injury (TBI). Hemorrhagic progression of contusion (HPC) is frequently seen on repeat CT, but its clinical and radiological significance in case of mild TBI is not well define. The aim of the study: to evaluate the result of routine repeat head CT in patients with mild TBI and brain contusions. Materials and methods: retrospective analysis of management of patients with mild TBI (Glasgow Coma Scale (GCS) score – 13 to 15) and cerebral contusion. All patients were treated at the Kyiv City Clinical Emergency Hospital between 2016 and 2017. Results: within 202 patients with mild TBI, 87 (43.1 %) met the inclusion criteria and were selected for detailed analysis. There were 69 (79.3 %) men and 18 (20.7 %) women. The mean age of the patients was 43.8±12.7 years (17–82 years). The average time between trauma and CT was 3.3 hours. The average volume of contusion on the initial CT was 1.9±0.6 cm3 (0.2–9.6 cm3). The average time of routine CT was 6.8 hours (range 4–24 hours) after the initial scans. HPC was found in 24 (30.7 %) of 87 cases. The average volume of brain contusion on the repeat CT was 2.3±0.5 cm3 (0.2-17.1 cm3). In 3 (3.4 %) patients the size of the hematoma increased. Clinical deterioration occurred in 10 (11.5 %) patients. Six (6.9 %) patients were operated after a CT scan due to HPC with midline shift in 4 cases and increasing of subdural hematoma – 2 cases. Four of these patients had clinical deterioration, and 2 patients were neurological stable. Patients with HPC at admission had lower points of GCS, fractures of the skull (both p &lt;0.001), subdural blood collection (p=0.002), a higher average duration of treatment 8.1±4.2 vs 14.3±5.2 days (p=0.0001), and mortality rate 0 vs. 3 (12.5 %) (p=0.02). Conclusions: Routine repeat CT in patients with mild TBI with brain contusions is aimed to find a patients with s high risk for clinical deterioration. One third of patients with mild TBI and brain contusion experienced HPC. Patients with HPC often have unfavorable clinical course with higher average duration of treatment, delay surgical treatment and mortality rate.

Estimation of Absorbed Dose of the Thyroid Gland in Patients Undergoing 64-Slice Head Computed Tomography and Comparison the Results with ImPACT Software and Computed Tomography Scan Dose Index.

Thyroid exposure to radiation in brain computed tomography (CT) scan is of great value since it is considered as a vital organ. This study aimed to investigate the absorbed dose of thyroid by various protocols of head CT in patients referring to 64-slice CT scan center and to compare the values with the calculated dose by imaging performance and assessment of CT (ImPACT) method. Also, the values of CT scan dose index (CTDI) were calculated with semiconductor detector. In this cross-sectional study, 120 outpatients including three groups of forty individuals over 40 years old referring to the hospital radiology centers in Tehran for head CT were chosen and 3 thermo-luminescence dosimeter (TLD-GR200) were applied on thyroid gland of each patient. For brain CT, Absorbed and effective doses of thyroid gland were calculated by ImPACT software. In addition, semiconductor detector in head CTDI phantom calculated CTDI for the applied protocols. Mean effective dose of thyroid gland in brain scan group was calculated by TLD and ImPACT software which showed no significant difference (P < 0.001). Mean effective dose of thyroid gland in unidirectional and bi-directional sinus scan by TLD and ImPACT software were different significantly (P < 0.001). Also, the differences between CTDI values shown by brain and sinus scan protocol with semiconductor detector and those CTDI were significant (P < 0.001). The calculated values of absorbed dose and effective doses of thyroid by TLD and ImPACT software were not significantly different. Mean effective dose calculated for thyroid gland in head scans by TLD and ImPACT was less than the annual permissive level for thyroid gland suggested by International Committee on Radiological Protection. In this study, calculated values of thyroid effective dose in brain scan with 64-slice scanner were less than the calculated values in a similar study.

Application of the Canadian Computed Tomography Head Rule to Patients With Minimal Head Injury

Two clinical decision rules, the Canadian CT Head Rule and the New Orleans Criteria, set the standard to guide clinicians in determining which patients with minor head trauma need computed tomography (CT) imaging. Both rules were derived with patients with minor head injury who had had a loss of consciousness or witnessed disorientation. No evidence exists for evaluating patients and need for CT imaging with minimal head injury; that is, patients who had a head injury but no loss of consciousness or disorientation and therefore would have been excluded from the Canadian CT Head Rule and New Orleans Criteria trials. We evaluate the Canadian CT Head Rule in patients with head injury without loss of consciousness or witnessed disorientation (minimal head injury). We studied a prospective convenience sample of patients with minimal head injury who received head CTs as part of their evaluations in the emergency department (ED). Participants were enrolled after head CT was ordered, but before the physician received the imaging results. Physicians were surveyed on their clinical reasoning for ordering imaging in this low-risk cohort of patients. Physicians surveyed consisted of ED attending physicians and senior-level emergency medicine residents. Final patient disposition was recorded when it became available. Patients with positive CT findings had their medical records reviewed for specific disposition, admission length of stay, ICU stay, and any operative or procedural interventions. Two hundred forty patients with minimal head injury were enrolled. Five patients (2.1%) had head CTs that were positive for intracranial hemorrhage. All instances of intracranial hemorrhage occurred in patients who were at high or moderate risk by the Canadian CT Head Rule (2 high risk [age], 3 moderate risk [mechanism]). No patient with intracranial hemorrhage went to the ICU or underwent any intervention; the average hospital length of stay was 1.25 days. The Canadian CT Head Rule was 100% sensitive (95% confidence interval 40% to 100%) and 29% specific (95% confidence interval 23% to 35%) for the presence of intracranial hemorrhage. Physicians listed their own reassurance (24.6%), patient reassurance (24.2%), patient expectation (14.6%), and reduction of legal liability (11.7%) as the rationale for ordering head CT in patients with minimal head injury. Shared decisionmaking was used in 51% of cases. Risk of intracranial hemorrhage in patients with minimal head injury was very low, and even in patients found to have an intracranial hemorrhage, none had any serious adverse outcome (eg, death, intubation, prolonged hospitalization, surgical procedure). The Canadian CT Head Rule was 100% sensitive in this small cohort of patients with minimal head injury. Among our study cohort, which specifically included only patients who had CT scanning, applying the Canadian CT Head Rule may have reduced the need for CT, potentially saving costs and resources. However, because many patients with minimal head injury who present to the ED may not have CTs, it is unclear what effect the broad application of this rule would have on overall CT use. Providers' rationale for obtaining CT was multifactorial. These represent barriers that may need to be overcome before physicians are comfortable changing CT ordering patterns in this group of head injury patients.

Prevalence of Intracranial Hemorrhage in Outside Hospital Cardiac Arrest Patients

SESSION TITLE: ICU Management SESSION TYPE: Original Investigation Poster PRESENTED ON: Wednesday, November 1, 2017 at 01:30 PM - 02:30 PM PURPOSE: The purpose of this study is to evaluate patients with out-of-hospital cardiac arrest (OHCA) who have intracranial hemorrhage (ICH), and to determine if a protocol could be established in the emergency department (ED) to have head computed tomography (CT) performed prior to cardiac catheterization in certain patient populations. This is a retrospective study with the following specific aims: Aim 1) Determine the proportion of patients suffering an OHCA with return of spontaneous circulation (ROSC) diagnosed as having ICH. Aim 2) Compare demographic characteristics and co-morbidities of patients with and without ICH to determine which patient population is prone to OHCA and ICH, excluding those related to trauma. METHODS: This is a retrospective, multi-center (three institutions: Level I and II) study where 1987 patients over the age of 18 who had an OHCA were screened for obtaining ROSC either in the field or in the ED between January 1, 2010 and December 31, 2015. Out of 1987, 293 patients obtained ROSC. Information regarding pre-existing co-morbities; history of cerebrovascular diease, hypertension, diabetes mellitus, chronic kidney disease, tobacco use, anticoagulation therapy was collected. Presenting cardiac rhythm and whether head CT was obtained and presence of ICH on head CT was recorded. RESULTS: Of the 293 patients who experienced OHCA with ROSC, 50 (17.1%) patients had a head CT, 237 (80.9%) did not have a head CT, and presence/absence of a head CT was not documented for 6 (2.0%) patients. Patients who had a head CT were younger (mean: 59.3 years; SD: 18.5) than patients who did not have a head CT (mean: 65.9; SD: 18.0) (t: 2.34; df: 285; p=0.020). A smaller proportion of patients with hypertension, CKD had a head CT (14.7%, 6.0%) than patients without hypertension, CKD (27.9%, 22.7%) (χ2(1): 5.52; p=0.019), (χ2(1): 7.02; p=0.008). There was no difference in obtaining or not obtaining a head CT based on sex (χ2(1): 0.60; p=0.455), smoking status (χ2(1): 1.40; p=0.238), diabetes (χ2(1): 3.40; p=0.065), cerebrovascular disease (p=0.199), or anticoagulation therapy (p=0.158). Comparing presenting rhythm who had a CT; ventricular fibrillation/tachycardia (29.5%) , pulseless electrical activity/asystole (13.4%) (χ2(1): 7.78; p=0.005). Among the 50 patients who experienced OHCA with ROSC and had a head CT, 12 (24.0%) were diagnosed with ICH. Among patients who had a head CT, a greater proportion of patients with pulseless electrical activity/asystole had a diagnosis of ICH (42.9%) than patients with ventricular fibrillation/tachycardia (5.6%) (p=0.018). CONCLUSIONS: One fourth of the population presenting with OHCA who obtained ROSC were diagnosed with intracranial hemorrhage and presenting rhythm was PEA/asystole about 43% of the time. CLINICAL IMPLICATIONS: A substantial number of patients presenting with PEA/asytole who obtained ROSC had a diagnosis of ICH.Prompt neurosurgical intervention and transfer to appropriate facility may improve survival rate and functional outcome. Further pilot protocol can be established which triggers clinicians to obtain a head CT in patients with PEA/asystole who obtain return of spontaneous circulation. Complicated Mild Traumatic Brain Injury and the Need for Imaging Surveillance.

To evaluate the need for repeat head computed tomography (CT) in patients with complicated mild traumatic brain injury (TBI) determined nonoperative after the first head CT. A total of 380 patients with mild TBI and a positive head CT not needing surgery were included. Changes between first and second head CT were categorized as decreased, increased, or stable. Three patients required neurosurgical intervention (0.8%) after the second CT. There were no significant differences in demographics including age, gender, alcohol consumption, anticoagulation status, time between first and second CT, Glasgow Coma Scale score at admission and discharge, and incidence of subarachnoid hemorrhage, epidural hematoma, contusion, or skull fractures between the operated and nonoperated groups. All patients in the operated group had subdural hematoma compared with 40.8% in the nonoperated group (P= 0.07). All operated patients showed symptoms of neurologic worsening after initial head CT, compared with 2.7% in the nonoperated group (P < 0.001). Moreover, patients who showed neurologic worsening were more likely to show increased intracranial bleeding on repeat head CT, whereas patients who did not show neurologic worsening were more likely to show decreased or stable intracranial bleeding (P= 0.04). Routine repeat head CT in patients with complicated mild TBI is very low yield to predict need for delayed surgical intervention. Instead, serial neurologic examination and observation over the first 8 hours after the injury is recommended. A second CT scan should be obtained only in patients who have neurologic worsening.

Cranial Imaging Before Lumbar Puncture in Adults With Community-Acquired Meningitis: Clinical Utility and Adherence to the Infectious Diseases Society of America Guidelines.

The Infectious Diseases Society of America (IDSA) guidelines delineate criteria for the use of computed tomography (CT) scan of the head before lumbar puncture (LP) in adults with community-acquired meningitis (CAM). There are limited data to document adherence to these guidelines and assess the clinical utility of brain imaging. This was a retrospective analysis from January 2005 to January 2010 in Houston, Texas. Among 614 adults with CAM, 407 patients (66.3%) did not have an indication for a head CT scan and 207 (33.7%) did. Patients with a CT scan indication were older, had more comorbidities, were sicker, and had more urgent treatable etiologies and adverse clinical outcomes (P < .001). A CT scan was ordered before the LP in 549 patients (89%). Overall, clinicians did not adhere to clinical guidelines in 368 of 614 (60%) subjects. A CT of the head was ordered when not indicated in 355 of 549 patients (64%), and not done when indicated in 13 of 207 patients (0.6%). CT of the head revealed intracranial abnormalities in 35 of 193 patients (18.1%) with an indication for brain imaging, compared with only 2 of 356 (0.05%) with no indication (P < .05). Major intracranial findings were seen in only 15 of 549 (2.7%) patients, all with an indication for brain imaging. Furthermore, only 8 patients had abnormalities that affected clinical management. Most clinicians do not adhere to IDSA guidelines, delaying diagnostic LP and increasing costs. Usefulness of head CT in patients with CAM without an indication for imaging is limited and has no impact in clinical management.

Privately insured medical patients are more likely to have a head CT.

Previous studies suggest overuse disparity of head computed tomography (CT) in white pediatric trauma patients with minor head injuries. Our study is meant to determine if race or insurance status impacts the probability of obtaining head CT in patients with a Glasgow Coma Scale (GCS) = 15. Using the 2008-2010 National Hospital Ambulatory Medical Care Survey for Emergency Departments (NHAMCS) database, the following variables were analyzed: race, emergency medical services (EMS) arrival, triage category, admission status, gender, age, and insurance status. Patients with injuries were excluded. All patients included had GCS = 15. In univariate analysis, head CT is more likely to be obtained for patients in the following categories: Medicare insured, private insurance, Medicaid insured, and self-pay, EMS arrival, triage category immediate, and age >75years. In logistic regression, race (white vs. black) was no longer significant, but there was disparity based on insurance status with privately insured patients more likely to receive a head CT (OR = 1.683, 95% CI = 1.255-2.259). After controlling for the above inclusion variables and focusing on patients less likely to need CT (non-traumatic with GCS = 15), privately insured patients were more likely to receive a head CT compared with uninsured. Race alone was not associated with an increased probability of receiving a head CT.

BackgroundProlonged sedation is common in mechanically ventilated patients and is associated with increased morbidity and mortality. We sought to determine the diagnostic value of head computed tomography (CT) in mechanically ventilated patients who remain unresponsive after discontinuation of sedation.MethodsA retrospective review of adult (age >18 years of age) patients consecutively admitted to the medical intensive care unit of a tertiary care medical center. Patients requiring mechanical ventilation for management of respiratory failure for longer than 72 hours were included in the study group. A group that did not have difficulty with awakening was included as a control.ResultsThe median time after sedation was discontinued until a head CT was performed was 2 days (interquartile range 1.375–2 days). Majority (80%) of patients underwent head CT evaluation within the first 48 hours after discontinuation of sedation. Head CT was non-diagnostic in all but one patient who had a small subarachnoid hemorrhage. Twenty-five patients (60%) had a normal head CT. Head CT findings did not alter the management of any of the patients. The control group was similar to the experimental group with respect to demographics, etiology of respiratory failure and type of sedation used. However, while 37% of subjects in the control group had daily interruption of sedation, only 19% in the patient group had daily interruption of sedation (p < 0.05).ConclusionIn patients on mechanical ventilation for at least 72 hours and who remain unresponsive after sedative discontinuation and with a non-focal neurologic examination, head CT is performed early and is of very limited diagnostic utility. Routine use of daily interruption of sedation is used in a minority of patients outside of a clinical trial setting though it may decrease the frequency of unresponsiveness from prolonged sedation and the need for head CT in patients mechanically ventilated for a prolonged period.

Does Routine Serial Computed Tomography of the Head Influence Management of Traumatic Brain Injury? A Prospective Evaluation

Computed tomography (CT) of the head is the current standard for diagnosing intracranial pathology following blunt head trauma. It is common practice to repeat the head CT to evaluate any progression of injury. Recent retrospective reviews have challenged the need for serial head CT after traumatic brain injury (TBI). This study intends to prospectively examine the value of routine serial head CT after TBI. Consecutive adult blunt trauma patients with an abnormal head CT admitted to an urban, Level I trauma center from January 2003 to September 2003 were prospectively studied. Variables collected included: initial head CT results, indication for repeat head CT (routine versus neurologic change), number and results of repeat head CT scans, and clinical interventions following repeat head CT. Over the 9-month period, there were 128 patients admitted with an abnormal head CT after sustaining blunt trauma. The 16 patients who died within 24 hours and the 12 patients who went directly to craniotomy were excluded. The remaining 100 patients make up the study population. Abnormal head CT findings were subarachnoid hemorrhage (47%), intraparenchymal hemorrhage (37%), subdural hematoma (28%), contusion (14%), epidural hematoma (11%), intraventricular hemorrhage (3%), and diffuse axonal injury (2%). Overall, 32 patients (32%) had only the admission head CT, while 68 patients (68%) underwent 90 repeat CT scans. Of the repeat head CT scans, 81 (90%) were performed on a routine basis without neurologic change. The remaining 9 (10%) were performed for a change in Glasgow Coma Scale (n = 5), change in intracranial pressure (n = 1), change in Glasgow Coma Scale and intracranial pressure (n = 1), change in pupil size (n = 1), or sudden appearance of a headache (n = 1). Three patients had their care altered after repeat head CT: two underwent craniotomy and one was started on barbiturate therapy. All three patients had their repeat head CT after neurologic change (decrease in Glasgow Coma Scale in 2 and increase in intracranial pressure in 1). Serial head CT is common after TBI. Most repeat head CT scans are performed on a routine basis without neurologic change. Few patients with TBI have their management altered after repeat head CT, and these patients have neurologic deterioration before the repeat head CT. The use of routine serial head CT in patients without neurologic deterioration is not supported by the findings of this study.
