Analisis Risiko Keselamatan dan Kesehatan Kerja Menggunakan Metode Hazard Identification Risk Assessment Determining Control (HIRADC) di Pt Bumi Sarana Beton

The background of this research focuses on the importance of OHS in the manufacture field, especially considering the high number of work accidents reported by BPJS Ketenagakerjaan. This research aims to identify and assess the risk of hazards at PT Bumi Sarana Beton using Hazard Identification Risk Assessment Determining Control (HIRADC) and provide solutions to reduce work accidents. The HIRADC method is used to identify hazards, assess risks, and determine appropriate controls in accordance with the AS/NZS 4360: 2004 standard. The results showed that the production work area has several high risks such as pinching, respiratory distress, and slipping. Effective risk control implementation is expected to improve work safety and productivity. These findings have important implications for the management of PT Bumi Sarana Beton to improve OHS policies and provide adequate training and protective equipment for workers. In addition, this research is also expected to be a reference for other industries in implementing OHS optimally to reduce the risk of work accidents and improve workers' welfare

Risk Management in Palm Fruit Harvesting Activities using the Hirarc Method PT. Prima Artha Dynamics

Risk Management in Palm Oil Fruit Harvesting Activities Using the Hirarc PT method. Prima Artha Dynamics. This research is motivated by a source of danger in oil palm fruit harvesting activities with the risk of work accidents in oil palm fruit harvesting activities. Therefore, the aim of this research is to analyze the main causes of the risk of work accidents in oil palm fruit harvesting activities, to analyze the severity of the risk of work accidents in oil palm fruit harvesting activities, To determine the steps taken to control the risk of work accidents in oil palm fruit harvesting activities. This research uses the HIRARC (Hazard Identification Risk Assessment Risk Control) method, namely by identifying sources of danger, analyze the level of risk and risk control efforts. Data was obtained through interviews with 18 harvest employees. From the results of this research, there are main causes that are a source of danger and risk for oil palm fruit harvesting activities, namely slippery land, palm fruit bunches, sharp objects, palm fronds, snake droppings/powder, centipedes and wasps. The main causes that are the source of danger and the occurrence of risks have a mild level of risk, namely falling/slipping, being hit by fruit when removing fruit to TPH, being hit by fronds during harvest, falling into the eyes of droppings, being stung by centipedes and being stung by wasps and for moderate risks, namely being injured/injured by sharp objects. Hazard Identification Risk Assessment Onsite Survey for External Human-Induced Events Aspect of the Nuclear Power Plant in West Kalimantan, Indonesia

Every stage of the West Kalimantan Nuclear Power Plant (NPP) building process prioritizes safety. The external, human-induced events (HIE) aspect is one part of the NPP site survey, as some risks in the site evaluation procedure could compromise the safety of the activity implementers. As a result, a risk assessment study relating to occupational safety and health (OSH) is necessary. This study aims to determine the quantity of risk from NPP site survey activities in terms of the HIE aspect. The risk assessment method involves a literature review and collecting data from field verification using a quantitative method that refers to the risk assessment based on BATAN Standard 006.1:2019 to support the implementation of the OSH management system. Following that, the analysis begins with a hazard risk identification risk analysis to determine the risk probability scale and consequence scale acquisition that may result from the hazards. The research findings from the conducted assessment of risk related to the nonroutine HIE site survey show ergonomic, mechanical, and biological hazards that were developed during the preparation, field review, and reporting processes. The risk rating is category A, with a risk scale of 2, indicating acceptable risk and effective control measures. These data and information support site evaluation license requirements for Indonesia as it plans for the construction of NPPs.

HUBUNGAN POTENSI BAHAYA KERJA PERAWAT DENGAN UPAYA PENCEGAHAN BERBASIS HAZARD IDENTIFICATION RISK ASSESSMENT DETERMINING CONTROL

The quality of health services needs to be maintained and improved, while the number of work accidents still has the potential to occur through the phenomenon of potential work hazards for nurses and low prevention efforts. Information found potential work hazards for nurses include physical, biological, ergonomic, chemical and psychosocial hazards as well as the low prevention efforts of nurses in the Dialysis Room. This study aims to determine the relationship between the potential work hazards of nurses and prevention efforts based on hazard identification risk assessment determining control (HIRADC) in the Dialysis Room at Jasa Kartini Hospital, Tasikmalaya (RSJKT). This research method is a quantitative research type descriptive correlation cross sectional approach. The number of samples was 30 nurses in the Dialysis Room at Jasa Kartini Tasikmalaya Hospital with the sampling technique using total sampling. The instrument used was a valid and reliable questionnaire including a questionnaire on the potential hazards of work for nurses (Cronbach Alpha value 0.953) and HIRADC-based prevention efforts (Cronbach Alpha value 0.952). Data analysis used the Chi-Square test. The results showed that most of the potential work hazards for nurses were in the unsafe category (63.3%). Most of the HIRADC-based prevention efforts are in the low category of 56.7%. There is a significant relationship (p-value = 0.000) between potential work hazards for nurses and HIRADC-based prevention efforts. The results of this study are expected that hospitals hold workshops and training on HIRADC-based prevention efforts regularly at least once a year, nurses improve the patient safety culture implementation program in an effort to minimize potential occupational hazards and further research can explore potential work hazards for nurses and HIRADC-based prevention efforts with the method qualitative research and identify other factors related to HIRADC-based prevention efforts.

Risk Analysis and Occupational Health and Safety Assessment Series (OHSAS) at Hospital X Surabaya

A hospital is a facility that generally raises the degree of public health. More and more human involvement makes hospitals have great potential to cause harm. To control the risk of accidents, it is necessary to identify hazards that exist in the workplace and evaluate the level of risk and control. One of the first stages in risk management is HIRARC (Hazard Identification Risk Assessment and Risk Control). The existence of HIRARC makes it easier to compile a risk register or hospital risk register in each work unit. This study aims to analyze the application of the Occupational Health and Safety Management System (OHSAS) at Hospital X Surabaya based on HIRARC. This type of research uses qualitative research with a research sample of 78 respondents. Qualitative variables in this research used for analyze risk management and identify risk control through the process of observation and interviews. Qualitative data analysis is carried out through identification of initial conditions, exposure of interview results and qualitative content analysis. The results of the study stated that the implementation of occupational safety and health management has been carried out at Hospital X Surabaya. OHS risk management is very important in preventing workplace accidents and improving performance. In this study, the HIRARC approach (Hazard Identification, Risk Assessment and Risk Control) found three levels of risk in hospitals, namely high risk, moderate risk and low risk. The results showed that the implementation of OHS risk management was used to identify, evaluate, and control OHS risk. The thing that cannot be avoided from work activities is a risk. Based on this, it is expected that there will be an increase in understanding and awareness related to OHS in hospitals, the application of risk control, routine evaluations and good cooperation and coordination between the OHS team and health workers for the implementation of OHSAS

Hazard Identification Risk Assessment and Risk Control Measures in Micro, Small, and Medium Enterprises Cuanki in Kasemen District, Serang City

Micro, Small, and Medium Enterprises (MSMEs) Cuanki Bakti Mulia is a company engaged in the food industry that produces cuanki in Indonesia. It is at high risk due to the severity and possibility of accidents causing severe injuries. This study aims to identify hazards, assess risks, and control at MSMEs Cuanki Bakti Mulia, Kasemen District, Serang City. The method used is Hazard Identification Risk Assessment and Control (HIRAC). The object of study is the work of MSMEs Cuanki Bakti Mulia, namely all potentially hazardous activities. Data from the company, interviews, direct surveys of production equipment, and machines in the production process were collected for this study. The results of the study showed that many risks were identified. The process of making and rolling meatball dough, and fried dumpling dough had a high risk rating compared to others. In dough making, ingredients in flour that come into direct touch with the breath might irritate the nasal and mouth mucosa and create respiratory problems. In dough rolling, hands are exposed to vibration for a long time, and noise causes hearing loss. Several protection and prevention measures have been taken, mainly to protect workers from workplace accidents and maintain the hygiene of the food produced. The proposed hazard controls are replacing firewood with gas stoves when frying to make tofu and fried dumplings, setting restrictions on workers' working hours, and ensuring adequate rest by reducing working hours. The company needs to provide and equip with Personal Protective Equipment (PPE).

OSH risk analysis with hazard identification risk assessment and control (HIRARC) method at NPK Fertilizer Production Department of PT X

According to Menurut BPJS Ketenagakerjaan (Social Security Management Agency of Manpower), there has been a constant increase in occupational accidents in the last 7 years, where by 2022 it reached 265.334. The NPK factory of PT X is a petrochemical factory that produces NPK fertilizer. In it's production process, it needs nitrogen, phosphate, kalium and clay as it's raw materials. In addition to the chemical materials for it's production purposes, PT X utilizes modern machineries to make the process easier. From this description, it can be concluded that the production of NPK fertilizer would create a number of hazards which may lead to occupational accidents (OA) as well as occupational diseases (OD). This study aims to analyze the OSH risks which are present at the NPK factory of PT X using the method of Hazard Identification Risk Assessment and Risk Control. This is a quantitative study with observational descriptive approaches. It was found out that before the implementation of risk control, the NPK factory of PT X had the extreme risk rate of 75%, high risk rate of 14%, and medium risk rate of 3% from the total of 28 existing hazards (100%). Upon the implementation of control on the 28 hazards, PT X experienced the decrease in risk rates as follows: high risk 11%, medium risk 23%, and low risk 7%. Occupational Health and Safety Analysis Using Job Safety Analysis and Hazard Identification Risk Assessment and Risk Control Methods

Occupational Safety and Health is an important factor in the smooth production of a company that has a relationship with machinery, work tools, work environment, raw materials and processing. The purpose of this research is to minimize the number of work accidents in the company by identifying hazards, risk assessment and risk control. The research method used is the Job Safety Analysis method to classify work activities and identify hazards descriptively while the Hazard Identification Risk Assessment and Risk Control method is used to conduct risk assessment and risk control in each sequence of work processes. The results of the research conducted showed 57 potential hazards with 17 potential hazards in the unloading process, 3 potential hazards in the measurement process, 11 types of hazards in the sawing process and 26 potential hazards in the sorting process. Based on the results obtained, the risk categories can be classified as 16 (28%) potential hazards in the lowrisk category, 28 (49%) potential hazards in the medium risk category, 13 (23%) potential hazards in the highrisk category and 0 (0%) potential hazards in the extreme risk category. The risk control carried out is by using elimination, substitution, administrative and PPE.

Work accident risk analysis using HIRARC and FTA methods (Case study: Suwarno Meubel)

Suwarno Meubel is a business engaged in furniture production, which has risks related to occupational health and safety including workers carrying heavy wood manually, cutting wood with a table saw, painting wood with a spray gun, and so on. The purpose of this study is to find out the potential hazards that exist in each job and find the root causes of work accidents and then provide suggestions for improvements. The research method used is Hazard Identification Risk Assessment and Risk Control (HIRARC) to identify hazard risks and risk assessments for each job, after that, jobs with extreme values will be further analyzed using the Fault Tree Analysis method (FTA). The results of this study found 9 low categories of hazard risks, 12 moderate hazard categories, 4 high hazard categories, and 1 extreme hazard category. Risks in the extreme category were analyzed and found that there were 7 basic events, namely fatigue, lack of focus, lack of training, a limited number of PPE (goggles), no safety equipment on the machine, and no work procedures. The proposed controls are in the form of administrative control, PPE, and engineering.

Analysis of Occupational Safety and Health Cost Plan Construction on The Project “Preservasi Jalan dan Jembatan Surabaya – Sidoarjo”

Indonesia is one of the developing countries which currently has a lot of development underway. Problems related to occupational health and safety are still often ignored, especially in East Java province, this is demonstrated by the still high number of construction work accidents in 2021 in East Java province, namely 26% of the total construction work accident cases in Indonesia. This research was carried out on the project “Preservasi Jalan dan Jembatan Surabaya – Sidoarjo”. The method of this research is hazard identification risk assessment and risk control (HIRARC) and quantitative descriptive method. The results of this research show that the amount of occupational health and safety costs is based on the owner's estimate which is guided by the Minister for Public Works and Housing Regulation Number 10 of 2021 concerning Construction Safety Management Systems is IDR. 664,667,000.00 where The value of 4,04 % of occupational safety and health costs has met the ideal standard from circular letter number 10/SE/M/2018 of the Minister for Public Works and Housing that is between 1.5% - 2.5% of the total project value.

The effect of activated carbon granules and KOH solution on welding particulates and gases in air filtration

Hazard Identification Risk Assessment Risk Control (HIRARC) is the process of identifying hazards that can occur in routine or non-routine activities in a company or workplace, then carrying out a risk assessment of these hazards.And from the risk assessment, the hazards in the welding workshop that have a moderate risk and are interesting for researchers to innovate are from sources of danger caused by smoke resulting from welding activities, causing workers to experience respiratory problems and eye irritation. Researchers created a smoke filtration device generated from welding with a system where the welding smoke is channeled into the KOH solution to reduce the number of air particles and reduce the levels of toxic substances in the welding smoke, then to keep the KOH solution always clean, a circulation system was created through the carbon to removes dirt and keeps the solution water from being dirty and colorless.Once all the tools are ready, measurements can be made to measure several chemical elements in the welding smoke from the point before it enters the filter and after going through the filtration process. Among the elements measured are Carbon Dioxide (CO 2), Carbon Monoxide (CO), Particulate PM 2.5 (2.5 Microns), and Particulate PM 10 (10 Microns). Based on research and data testing carried out using the Anova Test and Fisher Pairwise Comparisons, it can be concluded that to produce the best and cleanest air results, the composition is K4 C1 or KOH 10% with Cabon Active 2-3 Mesh. Because if activated carbon with a mesh size larger than 2-3 mesh will affect the quality of the KOH solution because the granules are too small and contaminate the KOH solution. And the higher the KOH solution level, the more effective it is at reducing the dangerous elements in welding fumes.

So the purpose of this study was to analyze the effect of the level of occupational risk on the suitability of the safety sign at PT. Air Mancur Karanganyar. Hazard Identification Risk Assessment Determinant Control (HIRADC) method is a way of identifying hazards, risks and determining control of hazards. This study used a cross sectional quantitative, the sampling technique used was Porportionate Statified Random Sampling is if the population had members or elements that had proportional strata with the samples being 38 people. The inclusion criteria for this study were workers in engineering rooms, Solid External Medicines (OLP), and cosmetics at PT. Air Mancur Karanganyar. Data analysis of a spearmen correlation test. The results that there is a significant influence between the Level of Occupational Risk on the Compliance of the Safety Sign at PT. Air Mancur Karanganyar with a significance value of 0.012. The recommendations is are the placement of safety signs such as the Caution for Pinched Hands sign, routine socialization twice a week, and review HIRADC once a year

Manual Material Handling Training for Workers in The Furniture Industry

The work processes in the furniture industry starts from wood transportation, construction, sanding, assembling, and finishing. One of the potential hazards that always exist in every work process in the furniture industry is ergonomic hazards. The risks of Manual Material Handling (MMH) that includes lifting and lowering, pushing and pulling, rotating, carrying, and holding against an object adversely affect both the company and workers. This study aims to determine potential ergonomic hazards and control efforts to reduce risk of ergonomic hazards and injuries to furniture industry workers by providing training Occupational Safety and Health and proposed work system during Manual Material Handling activities. The method used to identify is the Hazard Identification Risk Assessment Control (HIRAC) technique. HIRAC used to help provide an understanding of the hazards that exist in each work process to minimize the occurrence of adverse Occupational Diseases. This research is qualitative research with observation and interview data collection techniques. Analysis techniques in this research included data reduction, data presentation, conclusions, and verification. Results of the evaluation showed that the identification of ergonomic hazards in the furniture industry was found in all furniture work processes from wood cutting to finishing.

Identifikasi Bahaya dan Penilaian Risiko Keselamatan dan Kesehatan Kerja

The high number of work accidents in construction projects shows that there is still a lack of understanding and application related to occupational safety and health (OSH), so that every party involved in project implementation needs to understand and apply OSH to minimize the occurrence of work accidents. This study aims to identify OSH in the construction project of the Rectorate building of Universitas Muhammadiyah Kendari (UMKendari) to provide educational office facilities as a center for management and academic services based on high technology. The research method is to identify potential hazards that cause work accidents and apply OHS standards using the Hazard Identification Risk Assessment and Determining Control (HIRADC) method. Specifically, this research identifies hazards, assesses risk levels, and provides risk control efforts in the construction of the Rectorate building. Based on this research, 4 stages of construction were obtained, namely preliminary work, structural work, architectural work, and mechanical, electrical, and plumbing (MEP) work. There are 112 potential hazards with 13 potential hazards are at low risk level, 36 potential hazards are moderate, 57 potential hazards are high, and 6 potential hazards are very high. We suggest for OHS implementation where potential risks and assessments are needed to reduce work accidents by administrative control, elimination, substitution, control techniques, and personal protective equipment (PPE).

Risk Identification Of Roll Forming Machine Area With The (HIRARC) Approach at PT. Bumi Lestrai Karya Perkasa

PT. Bumi Lestari Karya Perkasa experiences 25 workplace accidents in a year. The purpose of this study is to estimate the amount of potential work accident risks in the field of roll forming machines and to control risks with appropriate Personal Protective Equipment (PPE) to reduce occupational risks. The Hazard Identification Risk Assessment and Risk Control (HIRARC) method was used in this study. This research is a qualitative research. Field observations, literature study of company documents, and in-depth interviews with the SMK3 division at PT. Bumi Lestari Karya Perkasa is used to collect data. The risk score is calculated first in the analysis. This research resulted in identification of companies in an effort to improve the safety of all work in the roll forming machine area to avoid unsafe actions and unsafe conditions, as well as the use of Personal Protective Equipment (PPE) by adjusting the type of work being carried out and the lack of awareness about the importance of tools. ANALISIS RISIKO KERJA PADA PROSES PEMBUBUTAN MENGGUNAKAN METODE HIRARC

Poor management of occupational safety and health (OSH) has an impact on occupational accidents. This results in losses for both the individual worker and the company. Therefore, the occupational hazard accident risk analysis was carried out using the HIRARC (Hazard Identification Risk Assessment and Risk Control) method. Data collection was conducted from the business owners of SSBC (Sanggar Sarana Baja Cakra) Welding and Turning Shop, located in Cakranegara, Mataram, West Nusa Tenggara. There are 3 work activities for risk identification, namely the clamping process, the engraving process and the drilling process. The results of the analysis show that the highest level of occupational risk occurs during clamping and engraving process. For drilling, it's in low level of risk. As for risk management, it is mainly done for high-risk level. Immediate improvement is needed by providing personal protective equipment (PPE) to workers to reduce these risks. Based on these results, business owners can do evaluation on his business so occupational accidents can be in the workplace.

Identifikasi Risiko Keselamatan dan Kesehatan Kerja Pada Proses Perbaikan Kapal Riset Geomarin III dengan Metode HIRADC

The application of K3 according to standards will help workers feel safe at work and can avoid potential work accidents. Lack of awareness of the application of K3 during work can result in the emergence of hazard risks for workers. The purpose of this study is to determine the types of work activities that are at risk and determine the application of work accident control efforts in the repair process on the GEOMARIN III research ship. The method used is HIRADC (Hazard Identification Risk Assessment and Determining Control). The identification results from the deck department and machinery department obtained 13 potential hazards with risk analysis where 4 risks included in the low risk category and 9 risks included in the medium risk category. Risk control that can be done to minimize work accidents is to inspect and maintain work equipment regularly and the use of personal protective equipment (PPE) according to standards and complete consisting of safety helmets, safety shoes, safety googles, safety gloves, ear plugs and masks. This research provides concrete recommendations on how to identify and mitigate K3 risks in the repair of research vessels, as well as contributes to the development of knowledge on the application of the HIRADC method in the shipping industry.

HIRARC is an effort to prevent and reduce the potential for work accidents, avoiding and minimizing risks that occur appropriately by avoiding and minimizing the risk of work accidents and controlling them in order to carry out the process of activities so that the process becomes safe.
