Subcutaneous chest wall hamartoma: Case report

chest wall hamartoma is one of the rare findings in the practice of pediatrics surgery. The usual origin of chest wall hamartoma in literature are the ribs. Here we report a very rare case of hamartoma arising from the subcutaneous tissue of the chest wall. a 3 years old female child presented with painless chest wall swelling of 1 year duration. On examination, the mass was soft-firm, nontender and mobile measuring 5cm by 7 cm and located on the posterolateral chest. Chest x-ray showed soft tissue mass otherwise no rib abnormality. She was operated with a finding of solid mass with multi cystic component in the subcutaneous tissue with no extension to the ribs or the underlying muscles with clear margin of demarcation. Mass was completely excised and sent for pathology examination which showed hamartomatous lesion. Our patient has smooth post-operative course and discharged on her 5th post-operative day. Although chest wall hamartoma not arising from the rib is a very rare finding, it can mimic several benign and malignant lesions arising from the chest wall. Hence, preoperative imaging and proper pathologic evaluation are important to differentiate it from other masses.

Multifocal Chest Wall Hamartoma: A Rare Congenital Anomaly

Chest wall hamartoma is a very rare tumour with benign course and distinct clinical, radiological, and histopathological characteristics. The lesion develops during foetal life and is present at or shortly after birth. It should be kept in differential diagnosis of complex chest wall masses diagnosed during antenatal ultrasound. CT thorax is useful for appreciation of detailed anatomy, characterization of the congenital abnormality and for surgical planning. Histopathology is used for the confirmation. Accurate diagnosis of mesenchymal hamartoma is important since many chest wall masses in children are malignant. We report a rare case of multifocal chest wall hamartoma in an infant who presented with multiple bilateral complex chest wall masses in which characteristic radiological and histopathological features led to the diagnosis of hamartoma, which was successfully treated with surgery.

Mesenchymal hamartoma of chest wall (MHCW) is a rare benign chest wall tumor mostly presenting at birth or in the first year of life with typical clinical, radiological as well as histopathological features. Here we report a case of this rare entity which can be easily mistaken for a malignant tumor. In this case preoperative probable diagnosis of some malignant tumor was made on clinicoradiological features and the final diagnosis of MHCW was given histopathologically.

Cytohistomorphological identification of congenital bilateral mesenchymal hamartoma of chest wall

Cases of Congenital hamartoma of the chest wall in infants are rare and their bilateral involvement is extremely rare. An 18 month-old child presented with bilateral chest wall swellings since birth which were gradually increasing in size. The X-Ray of the chest wall revealed bilateral involvement of 1st, 2nd and 3rd ribs with involvement of extra-pleural soft tissues. The cytological diagnosis given was round cell tumor of the chest wall keeping in view the presence of round cells in a hemorrhagic background. The biopsy revealed areas showing mature cartilage, bone, fibrous tissues along with hemorrhagic cystic cavities. Basing on the biopsy report, final diagnosis of chest wall hamartoma was given. Following complete excision, the child was doing well and there was no recurrence of the lesion as observed in the follow- up sessions.

A mesenchymal hamartoma of the chest wall is a rare benign tumor. Each case has a different clinical course as well as different radiological imaging findings, and various histopathological diagnoses. In addition, mesenchymal hamartoma is sometimes mistaken for a malignant tumor. This report presents a neonatal case of mesenchymal hamartoma of the bilateral chest wall.

Bilateral Mesenchymal Hamartoma of the Chest Wall in an Infant Boy

Mesenchymal chest wall hamartoma is an extremely rare tumor striking neonates and infants. Histologically, the tumor is composed of islands of hyaline cartilage intermixed with mesenchymal-like stroma and hemorrhagic cysts. We present a case of a congenital bilateral mesenchymal chest wall hamartoma (MCWH) in an infant boy. This extremely rare benign entity may be misdiagnosed as malignant tumor and shares features with another tumor of childhood–fibrocartilagenous mesenchymoma of bone.

Introduction: Thoracoscopic-assisted resection of thoracic aneurysmal bone cyst has never been described in the literature. Infantile cartilaginous hamartoma of the rib is a large tumor characterized by cartilaginous, vascular, and mesenchymal elements. Most cases are diagnosed before the age of one year due to their rapid rate of growth and expansion. Aneurysmal bone cysts usually arise secondary to a pre-existing tumor, and are known for their hypervascularity. Use of preoperative embolization is one technique that has been described in the literature to facilitate resection of aneurysmal bone cysts and to avoid complications from hemorrhage. Mesenchymal hamartoma of the chest wall

A 33-week gestational male infant with mild respiratory distress after triplet birth was found on physical examination to have a firm, fixed 2-cm mass arising from the lateral right chest wall. Chest x-ray confirmed a spherical soft tissue mass distorting the right fourth and fifth ribs (A). Fine needle biopsy was consistent with a benign chest wall hamartoma (mesenchymoma). The child was treated expectantly until four months of age, when rapid enlargement of the lesion prompted CT examination. CT evaluation revealed progressive destruction of the right third, fourth, and fifth ribs with leftward mediastinal displacement by the mass (B). Heterogeneous areas within the lesion were consistent with hemorrhage and likely accounted for the rapid expansion of the mass seen clinically. The infant had primary resection of the lesion with Gore-Tex (WL Gore & Assoc, Flagstaff, AZ) patch reconstruction of the chest wall. The recovery period was uneventful. Histologic examination showed a benign mesenchymoma with no evidence for malignancy. The characteristic elements of this benign tumor are primitiveappearing mesenchymal cells, stromal elements, and cartilage. Hypercellular cartilage with enchondral ossification at its periphery is present (C1). Woven bone with a suggestion of osteoblastic rimming appears to arise from the primitive mesenchymal cells (C2). In the upper left corner and center of the slide is a dense cellular mixture of primitive mesenchymal cells, osteoclasts, and vascular spaces. At 1-year followup, the child is asymptomatic with a normal chest CT.

Chest wall hamartoma with Wiedemann-Beckwith syndrome: clinical report and brief review of chromosome 11p15.5-related tumors.

A girl born with a left chest wall hamartoma, macroglossia, nevus flammeus of the middle forehead, and a small umbilical hernia developed left lower extremity hemihypertrophy by 1 year of age and is assumed to have Wiedemann-Beckwith syndrome. Hamartoma of the bladder and a cardiac fibrous hamartoma have been reported previously in association with Wiedemann-Beckwith syndrome. Infantile hamartomas are exceedingly rare and add to the spectrum of tumor formation in the syndrome.

Chest Wall HamartomaReport of Two Cases with Secondary Aneurysmal Bone Cysts

Chest wall hamartoma is a rare non-neoplastic benign lesion occurring in the neonatal period with particular clinical, radiological and pathological features. Histologically, it is composed of a mixture of bone trabeculae with spindle-cell stroma, chondroblast-like cells, and mature and immature hyaline cartilage. Aneurysmal bone cyst is a benign lesion that may arise secondary to several bone processes, such as giant cell tumor, chondrosarcoma, non-ossifying fibroma or osteosarcoma. We present two new cases of chest wall hamartoma with secondary aneurysmatic bone cyst formation studied with histological and immunocytochemical methods.

Mesenchymal hamartomas of the chest wall in infancy: radiologic and pathologic correlation.

Mesenchymal hamartoma of the chest wall is a rare tumor with about 53 reported cases in the English literature. We reviewed six chest wall mesenchymal hamartomas in four patients, including two cases with multiple lesions, with specific focus on the radiologic and pathologic correlation. All cases occurred in neonates or infants with ages ranging from seven hours to seven months. They were diagnosed with plain chest radiographs (n=6), ultrasonography (n=2), chest CT scan (n=6), whole body bone scan (n=2) and MRI (n=3). All cases except a small one without cystic change showed the typical features of mesenchymal hamartoma radiographically and pathologically. Radiologically they were well-circumscribed masses with solid and cystic components with multiple fluid-fluid levels in association with single or multiple rib destruction or change. The CT scan showed the typical findings of chest wall hamartoma, and the MR showed heterogeneous signal intensities of the mass on T1- and T2-weighted images. The MR also revealed more concisely a secondary aneurysmal bone cyst formation with multiple fluid-fluid levels on the T2-weighted image. Microscopically, they showed alternating areas of cartilaginous islands and primitive appearing mesenchymal proliferation, which corresponded well with the solid component on the radiologic findings. The areas of bone formation and blood-filled cystic spaces matched the calcified or ossified densities and the cystic components, respectively. A small case without cystic change showed peculiar radiological and pathological findings resembling an osteochondroma. In conclusion, mesenchymal hamartoma of the chest wall in infancy is quite rare and sometimes can be misdiagnosed as malignancy due to the bone-destroying radiographic appearance and the highly cellular and mitotically active microscopic features, unless the radiologists and pathologists are aware of the characteristic clinical, radiological, and pathological findings. Imaging studies can usually make a correct diagnosis with good correlation to the pathologic findings.

Chest wall hamartoma: An alarming chest lesion with a benign course

Chest wall hamartoma is a very rare mass lesion of the chest wall that is manifest at birth or in early infancy with deformity of the thoracic wall and/or varying grades of respiratory distress. The authors report on an 11-month-old infant presenting with chest wall hamartoma who was treated by en bloc excision of the lesion including the involved ribs. They emphasize the importance of early and complete excision of the lesion even in minimally symptomatic patients to avoid lethal respiratory complications, more severe postoperative orthopedic problems, and malignant transformation.

Nasal chondromesenchymal hamartoma: an upper respiratory tract analogue of the chest wall mesenchymal hamartoma.

Nasal chondromesenchymal hamartoma is the suggested appellation for a tumefactive process of the nasal passages and contiguous paranasal sinuses in seven children with a detectable mass in the nose. With the exception of one patient who was 7 years of age at diagnosis, the others were 3 months of age or less upon recognition of the mass. Two children were diagnosed in the first 2 weeks of life. Imaging studies showed a complex solid and cystic mass or masses filling the nasal cavity and extending into the ethmoid sinuses in most cases. Erosion of the surrounding bone, including the cribriform plate, resulted in an intracranial component in the four cases. Surgical resection was the treatment of choice despite its technical difficulties that often necessitated a combined intranasal and intracranial approach. Residual disease with continued growth in one case was the clinical outcome in two children, and the remaining five patients have not experienced any further difficulties. The piecemeal fragments of tissue disclosed a collage of histologic features, but the basic morphologic elements were well-demarcated nodules of cartilage with some variation in the cellular density and maturation of the chondrocytes, a myxoid to spindle cell stroma, focal osteoclastlike giant cells in the stroma, and erythrocyte-filled spaces resembling those of the aneurysmal bone cyst. Two of the tumors were less polymorphous or complex in their spectrum of histologic features. These nasal masses have similarities to the so-called chest wall hamartoma or mesenchymal hamartoma of the chest wall in terms of the clinical presentation in infancy and the basic cartilaginous character of both entities. There is a degree of presumption in the designation of these nasal and chest wall tumors as hamartomas because the pathogenesis has not been established for either entity.
