Penile Hair Tourniquet Syndrome (HTS) is a rare event but a serious syndrome characterized by the presence of a hair coiled around the coronal sulcus of penis which causes a range of pathologies from glandular swelling to necrosis and amputation of penis. If diagnosis is delayed, long-term complications such as urethral fistula, urethral transection, and penile gangrene or amputation develop. Penile HTS is almost exclusively described in circumcised boys. Prevention of such major complications depends on awareness of the etiology and presence of a high index of suspicion. Herein, we aimed to present a boy who was 4 years old with a penile HTS caused by wrapping of his mother's hair coil.

Delayed presentation of Penile tourniquet syndrome due to hair coil as complete urethral transection: A devastating complication

Penile tourniquet syndrome (PTS) is a condition in which a hair coil is wrapped around the penis causing damage to the urethra and penis. The spectrum of complications due to PTS may range from simple oedema of the penis to complete penile necrosis. We are reporting an 8-year-old boy who presented late to us with complete urethral transection having history of hair coil around penis 3 years back. Examination of the external genitalia revealed complete urethral transection at the coronal sulcus and deeply grooved cut mark at the dorsal part of the glans. The coronal sulcus was shallow and the glans overhanging. The urethral defect was about 0.5 cm. Following evaluation the patient underwent urethral repair by end to end anastamosis and was able to pass urine through the native urethral meatus postoperatively. Most of the cases reported in literature presented soon after the urethral injury unlike the index case which was discovered after 3 years.

A non-destructive technological study of three fresco fragments from Iklaina, Pylos, Greece

ABSTRACTThe objective of the present study is to conduct a non-destructive characterisation analysis of three fresco fragments from Iklaina, southern Peloponnese, Greece, in order to identify their manufacture techniques. Furthermore, this study aims at using the results of the scientific and analytical analyses to produce accurate replicas of these fragments (use of similar composition mortars, pigments, and manufacture techniques), which can be used for the restoration program of the site. These replicas will be exhibited alongside the originals in the new Pylos Archaeological Museum.Due to the high archaeological value of the objects, the analytical approach we followed was completely non-destructive and was based on the parallel use of optical microscopy, p-XRF and SEM/EDS. The analyses suggest the use of Egyptian blue pigment for the backgrounds. The wavy black coil of hair was painted with an organic black pigment. The use of inorganic pigments has been revealed on the upper layers on the basis of the identification of minerals and rocks. The substrate of the wall paintings is made of aluminosilicate lime mortars. The results corroborate the suggestions of previous studies for the pigments comprising the Mycenaean artistic palette and provides further insight on the artistic and technological choices made by Mycenaean artists.

Penile Hair Tourniquet Syndrome Due to a Coil of Hair: First Report in Iran

: Penile hair tourniquet syndrome (PHTS) is a rare clinical condition characterized by progressive penile strangulation caused by a piece of hair curled around sulcus corona. This clinical condition may result in some clinical and urgent complications including ischemia and gangrene of glans penis, urethra-cutaneous fistulae, and urethral transection. The case report is about a four year and six month old circumcised boy with a hair coil penile strangulation caused by strands of hair being wrapped around the sulcus corona.

Surgical Management of Hair-coil Penile Injury: Anatomical Insights and Grading System

To report our experience in the surgical management of a group of patients with varying degrees of hair-coil penile injury, while applying a more detailed grading scale for the severity of injury. The study included 16 boys with hair-coil penile injury who were managed during the period 2010 through 2016. In the acute state, the constricting hair coil was removed under general anesthesia. The repair was planned 6 months later after the acute injury. In 1 case presenting with severe form (near total glanular amputation), the glans was resutured to the penile shaft immediately after removal of the hair coil; the urethroplasty was performed six months later. The patients' age ranged from 2 to 17 years (mean 4.8 years). The urethroplasty was always covered by a protective layer, which was derived from the ventral dartos fascia in all but 1 case. In the latter, we used a tunica vaginalis flap for covering the urethroplasty. A successful outcome (regarding both urethral reconstruction and cosmesis) was achieved in all cases. Hair-coil penile injury is an uncommon condition with insidious course. Unawareness of such condition can lead to progression to serious complications. Delayed and staged surgical repair is associated with successful outcome.

Hair coil penile tourniquet syndrome in an unusual age.

Penile tourniquet syndrome (PTS), a rare urologic emergency, may lead to undesirable results including necrosis and amputation of penis, if not diagnosed and treated appropriately. Sometimes these injuries may be accepted as a forensic case. Miscellaneous objects used for strangulation can be metallic or nonmetallic. Of all ages, the most vulnerable period is infancy. Telogen effluvium is the most common cause of PTS in infants who are 0–6 years old. In the literature, telogen effluvium as a reason of PTS was not found except for this age group. Therefore, we aimed to present a boy who is 8 years old diagnosed as PTS because of his mother's hair coil.

Can an “Aesthetic” Intervention (Braided Hair Coil) Cause Hair Loss After an Aesthetic Operation?

Postoperative pressure alopecia (PPA), defined as hair loss caused by prolonged pressure on the patient's scalp during surgery, is an uncommon condition after aesthetic surgery. Originally, it was described for patients who underwent lengthy cardiovascular and gynecologic operations. The authors present a rare case, in which hair loss occurred after secondary breast augmentation (replacement of breast implants). The PPA appeared in the occipitoparietal region of the patient's scalp approximately 2 weeks after surgery. The operation was completed in less than 3 hours, without any fluctuations in the patient's blood pressure or any unusual blood loss. There were no other precipitating factors such as anemia or coagulopathies. The probable cause of this unexpected result was the patient's braided hair coil, which had not been noted before the operation. The patient habitually, and on the day of her operation, combed her hair into a braided coil, which placed extra pressure on the occipitoparietal region. The hair loss was temporary, and hair regrowth was complete within 2 months. This incident may have been avoided if the braided hair coil had been noted by nursing or other medical staff preoperatively. Repositioning the head every 30 minutes and providing adequate head padding during surgery are advised to protect the patient and prevent such incidents. 5.

Penile hair coil strangulation of the child

We report the case of a child with a delayed presentation of penile strangulation with a coil of hair that resulted in a complete transection of the urethra. Hair coil strangulation of the penis is uncommon. It is also known as penile Tourniquet syndrome. It has been reported with circumcised and uncircumcised penises and it can lead to serious complications like the amputation of the penis. Prompt diagnosis and treatment are necessary to prevent complications.

Urethral catheter was left for a mean of 5.5 (4-7) days. In the mean follow-up period, we had 4 complications in the form of 2 tiny urethrocutaneous fistulas and 2 anastomotic urethral strictures. The fistulae were closed surgically after the primary surgery by 1 year in the 2 cases, with no recurrence. Urethral strictures were managed by endoscopic visual urethrotomy, with no recurrence. Conclusion Penile tourniquet syndrome can cause serious penile complications. Awareness of this rare syndrome can help in preventing such complications. Being familiar with the surgical reconstruction guarantees high success rate.

Hair Coil Strangulation—An Uncommon Cause of Penile Edema

Penile strangulation by hair coil is an uncommon, under-recognized condition with a spectrum of potential complications including urethral transection and partial or complete penile amputation. Early recognition and treatment can greatly reduce the incidence of these devastating complications. We report a 5-year-old, circumcised boy with penile edema and erosions secondary to a hair wrapped around the coronal sulcus. Prompt diagnosis and removal of the hair led to rapid resolution without surgical repair. We also analyze the possible underlying causes which can lead to strangulation of the penis with a hair.

ORIGINAL RESEARCH—SURGERY: Construction of Neoglans Penis: A New Sculpturing Technique from Rectus Abdominis Myofascial Flap

ABSTRACT Introduction Construction of a neoglans penis may be required following glans amputation at circumcision, strangulation by a hair coil, or self-mutilation, among other indications. It may also be combined with phalloplasty to imitate the natural appearance and to support a penile prosthesis. Aim This is a report on a novel technique of neoglans construction for a patient with an amputated glans penis as a result of circumcision injury. Methods A rectus abdominis myofascial flap was used. The flap was deigned to be a 12 × 4 cm segment of the infraumbilical portion of the muscle, based on the inferior epigastric vessels. The flap was harvested through a paramedian incision. The penis was partially degloved through a circumferential incision 1 cm below its summit. The distal penile skin was utilized to elongate the urethra, so that the urethral meatus would be at the tip of the neoglans. The flap was reflected and tunneled underneath the mons veneris and alongside the penis, to emerge distal to the summit of the penis. The flap was fashioned into the shape of a glans and secured in place around the neourethra. The impression of a corona was achieved by tucking the proximal edge of the flap to its undersurface. Result Six months following surgery, the patient had a neoglans penis, a corona, and a urethral meatus at the very tip. The neoglans had similar consistency, color, and shape to the normal glans. Conclusion Construction of a neoglans penis is possible using the described sculpturing techniques, with satisfactory cosmetic results.

Progressive hair coil strangulation of penis.

Strangulation of penis by hair strands is a well-known, but uncommon clinical entity. We report two cases occurring in circumcised boys causing progressive painless disfigurement of penis with strands of hair coiled around the penile shafts, strangulating the penis. Primary repair was done in one case and delayed primary repair was done in the other, both with satisfactory results.

Penile strangulation secondary to a strand of hair acting as a tourniquet is an uncommon injury in young boys. Paraphimosis most commonly produces vascular congestion and edema without strangulation. However, serious penile and urethral injury can occur when a human hair becomes wrapped around a child's penis. We present a three-year-old, circumcised boy who presented with a swollen glans penis thought to be due to infection, and who ultimately was found to have a constricting hair coil at the coronal edge. The purpose of this report is to call attention to hair as a potential source of injury in the pediatric population and to stress the importance of early diagnosis and treatment, which are necessary to prevent a potentially devastating injury to the penis.

Strangulation of the penis caused by a coil of hair is uncommon. It can produce varying grades of injury that may result in a urethral fistula or even gangrene of the glans. Therefore it is important to be aware of this condition so that the strangulating hair can be removed and treatment instituted. The authors describe 15 boys who presented with hair coil strangulation of the penis. Their age range was 5 to 11 years. All were previously circumcised. Three boys had a superficial injury, and 11 had a more severe injury that resulted in a urethral fistula. One boy presented with gangrene of his glans penis, which resulted in autoamputation. One-stage repair was performed in 10 boys. The first three had recurrence of the fistula, and hence the technique was modified for the remaining seven patients. The latter technique proved simple and satisfactory.
