A simple cost-effective remedy for habit-tic deformity

Dermoscopic Nail Disorders in School-Going Children.

Introduction The majority of nail diseases in children are comparable to those in adults, while there are some physiological changes that start to happen around this age and go away over a few years. These conditions could be symptoms of infections and systemic illnesses. Pediatric nail disorders are typically easy to diagnose clinically, although there are occasionally conditions that masquerade as juvenile nail problems. Dermoscopy has grown in favour as a rapid, easy, non-invasive clinical procedure for examining nail diseases. This study aims to assess dermoscopic findings of child nail diseases. Methods A prospective analysis was conducted for seven months between January and July, 2022, at PNS Shifa Hospital, Karachi, Pakistan. A total of180 patients who presented in the outpatient department with any dermatological complaint underwent thorough history and examination. Special emphasis was given to clinical and dermoscopic examinations of nails. Data analysis enclosed descriptive and inferential statistics. The quantitative data was presented with help of mean and standard deviation, while the qualitative data was presented with help of frequency and percentage tables. Chi-square test was applied to compare nail findings diagnosed on clinical examination and dermoscopic examination. Results The mean age of study subjects was 9.4±3.2 years and ranged between five years and 18 years. The most common nail changes were hand eczema (n=41, 23%) followed by nail changes due to nutritional disorders (n=38, 21%), anaemia (n=34, 19%) and habit tic deformity (n=31, 17.2%). Conclusions Clinical evaluation is crucial for nail disease diagnosis. Dermoscopy of nails can help with the final diagnosis of nail disease and confirm clinical diagnoses. Also, it aids in the management of nail illnesses by providing a clearer picture of pathology and nail structure.

Habit Tic Deformity of Bilateral Thumbnails

Nail examination is an important aspect of a wholistic dermatological examination, which when overlooked leads to missing out on important and interesting relevant findings. We observed one such typical interesting finding in a patient who presented to the out patient department with characteristic nail changes suggestive of habit tic type of onychotillomania as an incidental finding. Further, dermoscopy of the affected nails showed characteristic features of the same.

Median canaliform dystrophy of thumb and great toe nails in an 8 years old boy

Median canaliform dystrophy is a rare nail disorder characterized by midline longitudinal ridging or splitting with canal formation in the nail plate of one or both the nails of the thumbs. It is an acquired condition that mimicks habit-tic deformity, resulting in a temporary defect in the nail matrix and cuticle. Treatment is often prolonged and unsatisfactory. Some clinicians have used topical tacrolimus (0.1%) ointment and tazarotene (0.05%) ointment successfully. We report the case of an 8-year-old boy presenting with median canaliform dystrophy of Heller affecting nails of both the thumbs and great toes, respectively.

Nail tic disorders: Manifestations, pathogenesis and management.

Nail tic disorders are classic examples of overlap between the domains of dermatology and psychiatry. They are examples of body-focused repetitive behaviors in which there is an irresistible urge or impulse to perform a certain behavior. The behavior is reinforced as it results in some degree of relief and pleasure. Nail tic disorders are common, yet poorly studied and understood. The literature on nail tic disorders is relatively scarce. Common nail tics include nail biting or onychophagia, onychotillomania and the habit tic deformity. Some uncommon and rare nail tic disorders are onychoteiromania, onychotemnomania, onychodaknomania and bidet nails. Onychophagia is chronic nail biting behavior which usually starts during childhood. It is often regarded as a tension reducing measure. Onychotillomania is recurrent picking and manicuring of the fingernails and/or toenails. In severe cases, it may lead to onychoatrophy due to irreversible scarring of the nail matrix. Very often, they occur in psychologically normal children but may sometimes be associated with anxiety. In severe cases, onychotillomania may be an expression of obsessive-compulsive disorders. Management of nail tic disorders is challenging. Frequent applications of distasteful topical preparations on the nail and periungual skin can discourage patients from biting and chewing their fingernails. Habit-tic deformity can be helped by bandaging the digit daily with permeable adhesive tape. Fluoxetine in high doses can be helpful in interrupting these compulsive disorders in adults. For a complete diagnosis and accurate management, it is imperative to assess the patient's mental health and simultaneously treat the underlying psychiatric comorbidity, if any.

Median Canaliform Dystrophy of Heller occurring on thumb and great toe nails

Median Canaliform Dystrophy of Heller is a rare but morphologically striking habit tic deformity of thumb nails characterized by midline longitudinal furrow with multiple transverse parallel lines. The proposed etiopathogenesis is repetitive trauma to the nail plate and cuticle, however some case reports have suggested familial occurrence and use of oral retinoids in its causation. Treatment is often prolonged and unsatisfactory, though some topical agents have been used successfully. We report a case of a young male patient presenting with Median Dystrophy of Heller affecting both Great thumb and toe nails simultaneously.

The Habit Tic: Onychotillomania

A 67-year-old man presented to his primary care physician for routine care. The absence of cuticles was noted on his bilateral thumbnails, and a midline crease with horizontal ridges from the proximal nail fold to the distal nail (Fig. 1). The patient stated that these changes had been present for over a year. He admitted to frequent “picking” and biting of the cuticles and repeated hand washing, as evidenced by the xerosis of his fingers. Habit-tic deformity is a type of onychodystrophy that is classified in a group of disorders known as onychotillomania, which includes entities such as nail biting (onychophagia), nail picking, and finger sucking. The deformity is the result of repetitive nail bed trauma from the conscious or unconscious manipulation of the cuticle. The characteristic findings include a midline furrow in the nail plate, often yellow in color, running the length of the nail, with a series of transverse ridges. Treatment includes physically covering the nails with bandages or glue to prevent further trauma. In addition, behavioral therapy and SSRIs have been tried, given the obsessive-compulsive nature of the habit. Cessation of the habit leads to complete resolution of the nail changes.

To the Editor: Habit-tic deformity (HTD), a nail dystrophy caused by repeated, habitual trauma to the nail matrix, commonly occurs on the thumbnail and creates a series of transverse ridges with a central depression. Medial canaliform nail dystrophy (MCND) represents a group of uncommon nail disorders of unknown etiology that may be related to HTD. MCND usually affects the thumbnail and resembles a fir tree, with transverse ridges that radiate from a paramedian canal. This report describes 2 patients with no prior history of trauma who presented with nail changes resembling HTD and MCND that were treated with multivitamins (Dermavite; MediNiche, Saint Louis, Mo).

<h3>REPORT OF A CASE</h3> A 45-year-old white man with no significant medical or psychiatric history complained of a 20-year history of eczema on his fingers. Previous treatment included application of 1% ciclopirox olamine lotion twice daily. This resulted in no significant improvement. On physical examination, several of his fingernails showed loss of the cuticles, parallel transverse grooves with a central depression, and elongation of the lunulae. The proximal and lateral nail folds had edematous, erythematous plaques with linear, angulated erosions (<b>Figure 1</b>).
