Expert consensus on the diagnosis and treatment of immunoglobulin-G4 related ophthalmic disease (2024 edition)

Immunoglobulin-G4 related ophthalmic disease (IgG4-ROD) is an immune-mediated fibroin flammatory condition characterized by ocular mass lesions and elevated plasma IgG4 level. Since all the ocular tissues can be involved, and it can be accompanied by multiple organ and systemic diseases, the diagnosis and treatment require multidisciplinary collaboration. The Oculoplastic and Orbital Disease Group of Chinese Ophthalmological Society of Chinese Medical Association and the Chinese Rheumatology Association convened experts in ophthalmology, and rheumatology and immunology, and formulated "Expert consensus on the diagnosis and treatment of immunoglobulin-G4 related ophthalmic disease (2024 edition)" for the diagnosis and treatment of IgG4-ROD based on domestic and international studies and experience. The consensus mainly standardized the diagnostic criteria of IgG4-ROD from the aspects of ocular imaging, serum IgG4 level and ocular histopathology, and recommended the methods and indications of drug therapy (glucocorticoids, immunosuppressants and biologics) and surgical treatment.

Forecasting food trends using demographic pyramid, generational differentiation and SuperLearner

The objective of this paper is to predict food consumption patterns for future decades by different social groups while taking generational change into account in the modelling. Using over 20 million observations of households in Switzerland from 1990 to 2017, we develop and apply four forecasting techniques that shift from referenced linear forecasts to population-driven forecasts. Each method considers values of selected household characteristics to define a “social group”, derives the proportion of each social group in society for the years 1990–2050, forecasts the future consumption of 75 food items in each social group in its unique way, and weighs these consumption patterns to obtain a future consumption for the total population. Although the results vary for each of the 75 food items and each method, altogether and in general, they define a narrow interval of future consumption development until 2050. All aspects of the approaches and the comparison of the outcomes contribute to knowledge about possible and nontrivial forecasting techniques on big data and foresight about the future of home food consumption.

From agent-based simulations of disease dynamics to expert support on public health interventions

Abstract Agent-based epidemiological simulators have long been used to predict the dynamics of infectious diseases. They allow to combine compartment approaches with representing contact networks derived from real or synthetic populations. Often, scenario techniques are applied to model the impact of public health interventions. During the COVID-19 pandemics the agent-based simulator COVASIM has been established for modelling the spreading of SARS-CoV-2. It includes four different contact networks related to homes, schools, workplaces and environment. During the pandemic it became obvious that the current simulation techniques were not sufficient to cover important needs in public health administrations and society. While most simulations were based on scenarios predicting expected numbers for the infected, hospitalized or critically ill, these studies could not provide easily strategic options on a variety of equivalent public health interventions, which are all suitable to keep disease dynamics within acceptable bounds, set e.g. by hospital capacity. Knowing about a variety of equivalent options, however, is of crucial importance to enable a fair burden sharing throughout all groups of society. Therefore, we applied machine learning techniques to create an inverse model for epidemiological simulations based on COVASIM. For given transmission rates it outputs a range of quantitative interventions which are equivalently suitable to keep epidemiological dynamics within predefined bounds. Currently, it covers interventions like school and workplace closings and other mobility restrictions. Future work will be directed to include other interventions, like vaccination campaigns, and we will work to make the approach more generalizable for other infectious diseases. The tool may turn out helpful fur public health administrations and stimulate the public debate on the best way a society may take in a pandemic event or other situations where public health action has to be imposed. Key messages • Machine Learning allows to invert agent based simulators. • Instead of disease dynamics simulations a set of equivalent public health interventions consistent with pre-defined constraints can be calculated.

Inequalities in access to healthcare for older adults during COVID-19 pandemics in Lithuania

Abstract Background During COVID-19 pandemic, healthcare systems experienced unprecedented shock. Older adults were the most vulnerable group of Lithuanian society, facing distinctive health challenges and highest mortality rates. The aim of the study was to examine perceived inequalities and challenges in access to healthcare of Lithuanian population over 65 years old during COVID-19 pandemic. Methods Representative sample of Lithuanian population over 65 years old (1503 individuals) was involved in the anonymous questionnaire survey, performed in January, 2024. Results 22 % of respondents reported not using healthcare services during COVID-19 pandemics. Among them, 76% did not have any need for healthcare services, while 13.6% did not approach healthcare due to fear of getting infected by Covid-19, and the other 10.4% had various accessibility challenges. 15.8% reported frequent problems in registering with family doctors, and 20.0% - with the specialist appointment. There was a clear social gradient observed among those underserved: it was associated with educational level and place of residence. Those with lower education and living in smaller towns and rural areas reported more common healthcare accessibility problems. Higher education, income and better self-perceived health correlated with higher satisfaction with travel time reaching health care services and registering for the appointment (p < 0.05). Conclusions Health system resilience reflects the ability to continue service delivery in the face of extraordinary shocks. Limited health care accessibility and clear social gradient could increase vulnerability of older adults, which resulted in exceptionally high mortality rates during COVID-19 pandemics. Results of the survey will be used for further development of transformative capabilities of healthcare system, responding to public health disasters and protection of the vulnerable groups of the society. Hospital employees’ perceived racism, ethnically offensive behavior, and patient violence

Abstract Background Hospitals are sensitive environments with complex interactions between employees and patients. Israeli society is ethnically diverse, and Israeli hospitals reflect this diversity and may create opportunities for rapprochement between different ethnic groups or might raise conflicts between them. In radical situations, such encounters might lead to violence. The phenomenon of physical or verbal violence towards caregivers and hospital employees exists. However, the uniqueness of this study is in examining whether the origin of the violence stems from racist reactions and how offensive behavior on an ethnic background could develop into actual violence. Methods Data were collected through an online close-ended survey among 370 hospital employees. 83% were Jewish, 68% were female, and 38 years old on average, with a mean hospital tenure of 6.5 years. The survey explored respondents’ perceived racism toward them, their exposure to offensive patient behavior based on ethnicity, and their exposure to general violence. Data were analyzed by SPSS 28. Results Preliminary results suggest positive correlations between study variables (r=.42-.52, p< .001). Regression analysis revealed that perceived racism contributed to general violence (β=.43, p< .001; R²=.19). Next, ethnic-based offensive behavior was added to the model (β=.36, p< .001). The analysis shows that general violence can be explained by these variables [F(2, 359)= 70.42, p< .001; R²=.28]. The R² of the general model increased significantly by 9.6%. The weight of perceived racism dropped yet is still significant (β=.24, p< .001). These results indicate that patients’ ethnic-based offensive behavior partially mediates the relationship between employees’ perceived racism and general violence. Conclusions The results suggest that employees who suffer from racism experience patients’ offensive behaviors based on ethnicity, which finally results in higher exposure to general violence in the workplace. Key messages • Understanding the phenomenon of violence in hospitals and examining the relationship between it and racism may amend policy and improve the quality of care and working conditions in the health sector. • The hospital environment involves diverse groups of society. Hence, the awareness of offensive behavior on an ethnic background is a condition to reduce racism and violence proactively.

The history of the breeding of Clematis (Clematis L.) in the M. M. Gryshko National Botanical Garden, NAS of Ukraine

Purpose. Investigate the history of Clematis breeding in the M. M. Gryshko National Botanical Garden (NBG), NAS of Ukraine, determine the main directions of hybrid Clematis breeding work. Results. It was established that the result of more than 16 years of breeding Clematis in the NBG was the selection of the 61 perspective hybrid, 42 of which, the most perspective, were assigned names. Of these, large­flowered varieties are the most numerous according to the distribution of variety groups in the current Royal Horticultural Society of Great Britain classification. In particular, Late Large­flowered account for about 40% of the total and Early Large­flowered for about 29%. Small­flowered hybrids make up the smaller part, of which about 26% belong to the Viticella group and one hybrid (‘Sputnik’) to the Integrifolia. The known directions of distribution of hybrid Сlematis of the NBG breeding indicate the possibility of their successful cultivation both in continental and maritime areas with a high level of humidity in the temperate zone and in the dry Mediterranean climate of the subtropical climate zone, which reveals their significant adaptive potential. Conclusions. Obtaining varieties of hybrid Clematis of domestic breeding is important for the implementation of sustainable horticulture practices on the territory of Ukraine, since a significant part of the modern introduced assortment of Clematis raises doubts about their adaptability to the local climate, and requires additional research into their adaptive potential. A successful combination in vertical landscaping of NBG­bred Clematis belonging to different varietal groups can ensure continuous flowering from May to the end of the season. These unique characteristics of Clematis ensure the creation of not only environmentally sustainable, but also visually appealing spaces that will contribute to the overall health and beauty of the urban environment.

Factors associated with dementia-related stigma in British adolescents

BackgroundDementia-related stigma is a prominent barrier for people living with dementia, leading to poor well-being and social isolation. Adolescents are an under-researched group in society that may already have experience of dementia and are more susceptible to attitudinal change which makes them ideal targets for anti-stigma initiatives outlined by public health policy. For the development of evidence-based anti-stigma initiatives in adolescents, it is important to understand which socio-demographic groups are most likely to develop stigmatising attitudes and why. This study aims to identify factors of dementia-related stigma in adolescents.MethodsA total of 1,044 adolescents (aged 11–18 years) from across six regions of England were included in the analysis of this cross-sectional, survey-based study. Descriptive statistics and multiple regressions were employed to explore the association between demographic variables, modifiable factors of dementia-related stigma and the outcome of dementia-related stigma. A path analysis via a structural equation model was employed to test for direct and mediatory effects.ResultsMultiple regression models revealed that younger adolescents, those with higher levels of contact with dementia, higher levels of empathy, higher levels of dementia knowledge, and higher affinity to older adults, are associated with more positive dementia attitudes in adolescents (p < 0.05). Within the accepted structural equation model, empathy, level of contact and dementia knowledge were key mediators of dementia-related stigma (p < 0.05).ConclusionThis study highlights that modifiable factors such as level of contact, ageism, and empathy have a potentially important role in how dementia-related stigma may start to form in the adolescent years. Developing contact-based strategies that stimulate empathetic responses may be useful targets for stigma reduction initiatives for adolescents.

Urban services and the invisibility of local governments in Nigeria in a COVID-19 pandemic context

The Covid pandemic was expected to provide a catalyst for better service provision by governments in general and local governments in particular. However, the complexity of governance suggests that citizens’ expectations of performance of constitutionally assigned roles may not always meet the realities of state–state relations. This paper provides a nuanced example of the interplay between state–state relations, citizens’ expectations and state–society relations in a three-tiered multi-governance context. Using findings from a virtual focus group discussion and online survey conducted in the first-wave lockdown in Lagos, Nigeria, it is shown that existing state–state relations influenced the relegation of local governments in the Covid-19 response structure, while citizens’ expectations continually demanded their visibility in service provision, a role that community actors and civil society groups took up. It is argued that it is not enough for services (particularly in health emergencies) to be provided in local government areas and through local governments, but services provided by local governments provide the competency local governments need to partner with civil society and other actors, thereby strengthening their capacity to assume devolved roles.

Capacity and crisis: examining the state-level policy response to COVID-19 in Tamil Nadu, India.

The capacity of government agencies to develop effective policy responses to external shocks is an important area of focus for health policy processes, as illustrated by the COVID-19 pandemic. However, few empirical studies exploring sub-national capacity of governments and the influence of institutional, organizational and political factors in shaping the policy response to complex emergencies have been conducted. The purpose of this study is to examine the governance capacity to develop and implement a policy response to a major health emergency-COVID-19-in Tamil Nadu, India, and to understand the factors shaping governance capacity during the first and second waves (2020-2021). Tamil Nadu offers a useful case for exploring governance capacity due to its longstanding public health institutions and previous experiences with disaster and outbreak response. We utilized three sources of data: (1) a review of key policy documents (n = 164); (2) a review of English-language media articles in the Indian press (n = 336); and (3) in-depth interviews with senior decision-makers, technical experts and other stakeholders (n = 10). We present four key findings from this analysis. First, Tamil Nadu's institutional framework enabled state-level governance capacity during an emergency of massive complexity, allowing for flexibility and nimbleness to adapt to evolving dynamics of centralization and decentralization over the course of the pandemic. Second, the ability to integrate public health expertise was circumscribed at important phases. Third, while coordination with external experts was utilized extensively, engagement with civil society groups was perceived as limited. Fourth, the electoral cycle was perceived by some to have constrained governance capacity at a critical point in the pandemic. By analysing the dynamics of state-level capacity in Tamil Nadu during a complex emergency, this study provides important learnings for other contexts globally regarding the drivers shaping capacity to develop and implement policy responses to crises.

Can Data Empower Indigenous People? Unveiling an Innovative Dataset for Quantitative Analysis, Replication Modeling, and Economic Development

Abstract Indigenous peoples are among the most vulnerable, ignored, and marginalized groups in society. Poverty is the oldest social problem and difficult to counter. The Indigenous people with which the authors live and work, the Agta Tabangnon, suffer from poverty and multidimensional socioeconomic deprivations. Indigenous peoples’ studies are qualitative, while poverty studies are typically generic, exposed to large sampling errors, and intended for nationwide decisions. Therefore, measuring poverty for specific tribes through complete enumeration with multifaceted disaggregation is critical for economic development. There is no comprehensive census specifically designed for Indigenous peoples to encompass the multidimensional aspects of their way of life. Nonetheless, the authors are resourceful in generating useful datasets from their partners. The locale is situated in the poorest district of the poorest province in the poorest region of Luzon, Philippines. The datasets contain multidimensional poverty indicators that are readily usable, along with complementary analytics to visualize the data. They may serve to measure poverty in Indigenous communities across different regions and countries. By utilizing this data, further empirical analysis, regressions, machine learning, and econometric modeling can be conducted. It can be freely utilized to target policies that address the multifaceted poverty and promote economic development within tribal communities.

Unequally distributed education impacts of ecosystem degradation: Evidence from an invasive species

Ecosystem degradation can have substantial social and economic costs, which may vary across groups in society. In this paper, we leverage variation from the introduction of the emerald ash borer beetle to explore how invasive species-induced declines in environmental quality impact education outcomes in a metropolitan setting. Exploiting the idiosyncratic and staggered spread of the ash borer throughout the Chicago Metropolitan Region from 2006 to 2014, we show that infestation led to declines in tree cover and subsequently, education outcomes. Our findings indicate that ash borer infestation reduced canopy cover in affected areas, stemming from both increased tree cover loss and declines in tree cover gain. Further, the ash borer reduced standardized test performance at exposed schools. Infestation exposure led to an average of 1 percentage point (1.22%) fewer students that met or exceeded the state’s testing benchmark at the typical school. While exposure to ash borer infestation was lower around low-income schools, education impacts were concentrated almost entirely among low-income students. This work adds to our understanding of the environmental drivers of education outcomes and the unequally distributed impacts of human-induced environmental change.

Unpacking “the surprise chain”: the governance of food security during the COVID-19 pandemic in Melbourne, Australia

AbstractFood systems are being affected by multiple shocks related to climate change, the COVID-19 pandemic and geopolitical events. Food prices and food insecurity are rising globally as a result, raising questions about the effective governance of food security during shocks. This paper critically examines the governance of food security in Melbourne, Australia during a major food system shock, the COVID-19 pandemic. It draws on document analysis and 34 stakeholder interviews with 41 participants from government, industry and civil society between May 2020 and March 2021. The paper examines the role of these actors in governance mechanisms for two important aspects of food system governance during the COVID-19 pandemic, continuity of food supply and delivery of emergency food relief. Clark et al.’s (2021) “governance engagement continuum” is used to show how civil society groups were excluded from these formal governance mechanisms, instead establishing their own spaces of “self-governance”. The resilience aims associated with these governance mechanisms are also analyzed using Zurek et al.’s (2022) “three R’s” (robustness, recovery and reorientation) framework. Our study highlights the significant role of food industry actors in the continuity of food supply and delivery of emergency food relief, and it shows the vulnerabilities of emergency food relief that depends on industry food “surpluses” as a model for addressing food insecurity during food system shocks. We conclude that greater government leadership is needed in the governance of food security during food system shocks, and in implementation of legislative and policy approaches that are grounded in the human right to food.

Unveiling discrimination

This article examines the historical and legal marginalization of the hijra, a third-gender community in South Asia, with a particular focus on the Indian subcontinent. Utilizing a postcolonial theoretical framework, the article analyzes contemporary lawfare dynamics between hijra and transgender activists in India and Pakistan. The study integrates gender and class analysis to explore key legal developments, drawing on examples from both colonial and contemporary legislation in India and Pakistan. The article argues that despite increased legal recognition of transgender rights, hijra remain marginalized as transgender activists seek acceptance as members of the middle class, often at the expense of hijra communities. By exploring legislative interventions and the narratives around activist endeavors, lawfare is analyzed at two levels, legislative and social, demonstrating how transgender activism can perpetuate discrimination against hijras in both Pakistan and India. The analyses underscore the complexity of alliances within genderqueer groups in postcolonial South Asian societies, challenging the assumption that these groups are natural allies. Ultimately, the article reveals the complex intersection of gender, class, and legal strategies, critiquing the role of lawfare in perpetuating social divisions and calling for more inclusive approaches to gender rights.

Технологизация аграрного сектора как основа продовольственной безопасности страны

The article discusses concepts and approaches to the definition of the concept of "food security" in the scientific literature and documents of the Food and Agriculture Organization of the United Nations (FAO). Under the influence of these ideas, a research boom began in daily nutrition practices and the study of their role in social health and active longevity of people. The problem of youth nutrition culture as an innovative group of society that determines the future state of the world has become, according to the authors, a trending research area. The analysis of the effectiveness of the use of information technologies and artificial intelligence in agriculture as drivers of industry development and food security is actively studied by domestic and foreign scientists. The dialogue of the scientific community is focused around the most relevant research topics: the problems of changing the nature of agricultural labor in the context of digitalization, robotization and the introduction of artificial intelligence; the impact of the technologization of agricultural enterprises on the specific activities of farmers, the existing risks of unintended destruction of ecosystems as a result of intensification of agriculture. Successful practices of using artificial intelligence in crop production and animal husbandry, the possibility of preserving traditional forms of farming on earth and many other issues are being actively studied. It is concluded that it is important to study the problems of agricultural technologization as a way to improve the efficiency of the industry and a way to ensure the country's food security.

Since the 2010s, a populist turn has been taking place in Central and Eastern Europe, and consequently populist parties have assumed control over the respective governments. In the literature, populist rule has been characterised with a) an irresponsible fiscal policy which ignores the budget constraint; and b) a biased redistribution policy that deliberately divides society into two antagonistic groups. Our analysis, covering the period between 2010 and 2022, demonstrates that populist governments in the region – generally speaking – have not relied on the accumulation of public deficit and debt; they, have, in fact, not differed from their non-populist peers with regard to fiscal discipline. Nevertheless, incumbent populist parties have used a conscious redistribution policy in their spending activities by favouring certain groups in society. Yet the spending preferences of populist parties in the region do show discrepancies as well; therefore, no single type of fiscal programme can be identified in the studied period.

Effect of educational intervention based on the theory of planned behavior (TPB) on amount of salt intake in pregnant women with PreHypertension

BackgroundPrehypertension is one of the common disorders during pregnancy. Reducing salt intake is among the best cost-effective interventions to reduce the risk of hypertension. The present study aimed to explore the effect of an educational intervention based on the Theory of Planned Behavior (TPB) on amount of salt consumed by prehypertensive pregnant women.MethodsThe present quasi-experimental study was conducted in 2022 on 61 prehypertensive pregnant women visiting the Comprehensive Health Services Centers in Birjand, Iran. Pregnant women were selected through a convenience sampling method and randomly assigned to intervention and control groups. A blood pressure test and a 24-hour urine test were taken in 3 stages (before, immediately after, and one month after the intervention). A reliable and valid questionnaire based on the TPB was used to measure the cognitive variables. The intervention program included three training sessions based on the TPB held on an online platform (WhatsApp social network). Data were analyzed using repeated measures ANOVA and longitudinal marginal model with the GEE approach in SPSS19. The significance level for all statistical tests was set at p < 0 0.05.ResultsAfter intervention, the mean score of perceived behavioral control (p = 0.02), intention (p = 0.004), and salt consumption behavior (p = 0.03) increased significantly in the intervention group, and the mean score of systolic blood pressure (p < 0.001) and diastolic blood pressure (P < 0.01) decreased significantly in this group. In the control group, a statistically significant difference was observed in the systolic and diastolic blood pressure of the subjects (p < 0.01). However, the score of attitude, subjective norms, perceived behavioral control, intention, salt consumption behavior and the daily salt intake did not show a statistically significant difference over time (P < 0.05).ConclusionThe present findings showed that the perceived behavioral control, intention, behavior, systolic and diastolic blood pressure can be modifiable variables to improve the amount of salt consumed by pre-hypertensive pregnant women. Therefore, we recommend that the present model be used to develop interventions to improve health indicators in pregnant women as a highly susceptible group in society.

Public health, pluralism, and the telos of political virtue.

In the ethics of public health, questions of virtue, that is, of what it means for public health to act excellently, have received little attention. This omission needs remedy first because achieving improvements in population-wide health can be in tension with goals like respect for the liberty, self-determination, or non-oppression of various individuals or groups. A virtue-ethics approach is flexible and well-suited for the kind of deliberation required to resolve or mitigate such tension. Public health requires practically wise and careful thinking, which virtue ethics brings with it. Furthermore, too tight a focus on delivering outcomes in determining how public health should act has, in some cases, actually undermined its ability to achieve those consequences. However, the main concern about incorporating virtue into public health in a pluralistic society is likely to be that virtue is generally teleological, and we would surely need some widely agreed upon idea of something like flourishing or the common good for this to work. In this paper, I propose that for public health to express virtue in its work, it must express a commitment to justice as it goes about its business promoting and protecting the health of the population. Justice is both a contributor toward better health for groups in society, and a worthwhile goal in its own right. I will sketch an argument that justice as non-oppression - not merely health equity - is the right telos toward which excellent public health should aim in a pluralist society.

Sociodemographic and Mental Health Factors Associated with Obesity in Resettled Refugees in Syracuse, New York.

Resettled refugees have been exposed to stressful and life-threatening events preresettlement and are among the most marginalized and vulnerable groups in society. Postresettlement, they face challenges when assimilating to an unfamiliar host country, which renders them vulnerable to adverse health outcomes including obesity, a major public health burden. This study was conducted to examine the association of mental health and sociodemographic factors, including language proficiency and educational attainment, with obesity in first-generation resettled refugees. We used data from electronic health records from the Adult Ambulatory Medicine Clinic of the State University of New York Upstate, Syracuse, NY. The probability of being overweight and obese (class I and class II) relative to normal weight was estimated using fully adjusted multinomial logistic regression models with relative risk ratios (RRRs). Relative to male refugees, female refugees were more likely to have class I obesity (RRR=1.83; 95% confidence interval [CI]=1.19, 2.80) and class II obesity (RRR=4.07; 95% CI=2.41, 6.87). Limited English proficiency increased the risk of being overweight (RRR=2.02; 95% CI=1.29, 3.17) and having class II obesity (RRR=2.14, 95% CI=1.20, 3.81). A clinical mental health diagnosis increased the risk of class I (RRR=2.00; 95% CI=1.35, 2.96) and class II (RRR=1.76; 95% CI=1.15, 2.71) obesity. Having no formal education was associated with decreased risk of class II obesity (RRR=0.42; 95% CI=0.19, 0.90). Obesity prevalence and subsequent related morbidity continue to be major public health burdens in vulnerable, often underserved populations in the United States. Further investigation into social determinants of obesity in refugees in a community setting that captures the unique experiences of heterogenous refugee groups outside the clinical setting is warranted.

Sociolect occurs in various groups of society according to social class, age and occupation. Language differences between upper class and lower class society, teenagers and old people, doctors and lawyers, are examples of social dialect. This research aims to describe the use of sociolectic language variations in communication between the Bugis-Makassar community, Minasa Upa Village, Rappocini District, Makassar City. The data in this research are all everyday language utterances in communication that are identified as using sociolect. Meanwhile, the data source used is the Bugis-Makassar community who live in Minasa Upa Village, Rappocini District, Makassar City. Data collection techniques include; observation techniques, recording techniques and note-taking techniques. The data analysis technique is carried out in an interpretative descriptive manner, namely data that has been collected on a device, listened to and recorded, then identified and analyzed in depth by sorting and selecting information according to the research focus, namely the various forms of sociolect in communication among the Bugis-Makassar community. Based on the results of the research, the researchers found 16 data on sociolectic language variations, especially conversations in terms of age level, nobility level, and education level, with details of 8 data variations of colloquial language, 2 data variations of slang language, and 6 data variations of vulgar language.
